[The efficacy of a formula for estimating premorbid intelligence in the Spanish population].

INTRODUCTION: An important aim of neuropsychological assessment is to identify impairment in higher cognitive or intellectual, functioning. In order to achieve this task the assessment of premorbid intellectual ability becomes crucial. The estimation of premorbid cognitive functioning will allow us to assess level of cognitive deterioration but also best cognitive functioning that can be expected in rehabilitation. Estimating premorbid cognitive ability is also crucial in forensic practice and for research purposes. Methods that allow the estimation of premorbid intelligence have been developed. Estimates derived from these methods are typically based on one or both of the following sources of information: (a). life history variables (b). measures of current cognitive functioning that are highly correlated with intellectual functioning and that are relatively unaffected by brain damage. OBJECTIVE: To present a method of estimating premorbid intelligence in Spanish population. SUBJECTS AND METHODS: The method we present has been developed applying linear regression models to the standardization sample of the WAIS III in Spanish population. The model is based exclusively on demographic and socio educational variables, allowing estimation free of the influence of neurological lesions squeal. CONCLUSIONS: The efficiency of the method we present is similar to that of other methods widely used in other countries.

[Postoperative pain and emotional intelligence].

OBJECTIVES: To examine the relation between emotional intelligence (the ability to understand and regulate feelings) and postoperative pain in patients who had undergone abdominal surgery. METHODS: In a prospective intra-subject design we studied 32 patients in a natural context in which they underwent digestive and urological surgery. We used a structured interview, psychometric tests, and two quantitative scales for evaluating pain. RESULTS: Results indicated a significant negative correlation between the variable repair of feelings and pain scores on both verbal and visual analog scales (p < 0.05). The pain scores on each scale were highly correlated (p < 0.01). Likewise, the score on the visual analog scale on the first postoperative day and the overall score correlated (p < 0.01). Regression analysis indicated a significant effect of clarity and repair of feelings on the perception of postoperative pain. CONCLUSION: Patients showing higher emotional intelligence scores (clarity and repair) assessed their postoperative pain as less intense.

[Factors influencing intelligence in elderly population in a community-based study in Taiyuan city].

OBJECTIVE: To explore the factors which influencing the intelligence in elderly in a community, so as to provide reference on primary prevention of dementia. METHODS: A door to door survey was conducted. A total number of 830 elderly were assessed using WAIS-RC, H-NTLA and data collected through questionnaires on lifestyles, family and social activity were analysed, using the factor analysis, stepwise regression and canonical correlation analysis. RESULTS: The main variables related to the verbal were eggs diet, blood pressure, fruit, visit their children, alcohol intake (standard coefficients of regression are -0.118, -0.079, -0.060, -0.036, -0.117, respectively). The main variables related to cognition were recreation, sports and hereditary history of mental disorders (standard coefficients of regression were 0.035, -0.127, respectively). The main variables related to memory were contact to chemical materials, age of their parents during delivery, alcohol intake and the relationship between husband and wife (standard coefficients of regression were -0.063, 0.055, -0.030, -0.037, respectively). The variables related to canonical variable V(1) would include education and occupation (canonical correlation = 0.5993, P = 0.0001) while V(2) would include cerebrovascular accident (canonical correlation = 0.3925, P = 0.0005). CONCLUSION: Intelligent work, family harmony, prevention of cerebrovascular diseases were the main areas to prevent intellectual disability in the elderly.

[Relation between comfortable loudness and maximal speech intelligibility in children with sensorineural hearing loss].

The levels of comfortable loudness and those of maximal intelligibility were evaluated in 88 schoolchildren with dull hearing (20-90 dB reduction). All the examinees had the levels of comfortable loudness in the range of the maximal intelligibility, this permitting it to be a criterion in determination of optimal amplification in hearing aids. A significant correlation exists between comfortable loudness and hearing threshold at 500 Hz frequency, mean hearing threshold in the range of the principal speech frequencies 500-2000 Hz and in the range of the safest speech frequencies 250-1000 Hz. However, the range of the individual values of comfortable speech intensity in the same hearing thresholds proved to be 15-25 dB for one-syllable words and 20-40 dB for the standard speech tables making it impossible to recommend the threshold parameters for calculation of optimal intensity of the speech to receive. The most reliable way to do this is the adjustment of the speech loudness level by subjective sensation of the child.

Effect of different iodine intake on schoolchildren's thyroid diseases and intelligence in rural areas.

BACKGROUND: Reports are increasingly appearing on the side effects caused by excessive iodine intake. Our objective was to find out whether iodine excess would impair the thyroid function and intelligence of schoolchildren in rural areas of China. METHODS: A comparative epidemiological study was made on thyroid function and intelligence of the schoolchildren in the areas of low, moderate or excessive intake of iodine. In the area of low intake of iodine (Panshan, Liaoning province, median urinary iodine (MUI) was 99 microg/L), of moderate intake of iodine (Zhangwu, Liaoning Province, MUI was 338 microg/L) and of excessive intake of iodine (Huanghua, Hebei Province, MUI was 631 microg/L). The numbers of schoolchildren from each area selected to take part in a Chinese version of Raven's Test were 190, 236 and 313, respectively, and then 116, 110 and 112 of them were tested for thyroid function, thyroid autoantibody (TAA) and urinary iodine (UI). RESULTS: There were no significant differences in the incidences of overt hyperthyroidism, subclinical hyperthyroidism and overt hypothyroidism in Panshan, Zhangwu and Huanghua. But significant differences were found in the incidences of subclinical hypothyroidism (P = 0.001) in these three areas. The incidences of subclinical hypothyroidism in Huanghua and Zhangwu were 4.76 and 3.37 times higher than that in Panshan. TAA were negative in all the schoolchildren with subclinical hypothyroidism except for one. No significant difference was found among the rates of thyroid peroxidase antibody (TPOAb) and thyroglobulin antibody (TGAb) in these three areas. Mean serum thyroglobulin (TG) value of Huanghua was markedly higher than those of the other two (P = 0.02). Mean serum TG value of Zhangwu was higher than that of Panshan but the difference was not significant. Mean IQ value of the schoolchildren in Huanghua was markedly higher than that for Zhangwu (P = 0.001). Mean IQ value of the schoolchildren in Panshan was lower than that of Huanghua and higher than that of Zhangwu but, again, the differences were not significant. CONCLUSIONS: The increase of iodine intake may increase the risk for schoolchildren of subclinical hypothyroidism. In the area of iodine excess, most of the subclinical hypothyroidism cases are not of autoimmune origin. No obvious effect of excess iodine was found on mental development of schoolchildren.

The multi-trellis software architecture and the intelligent cardiovascular monitor.

A real-time, intelligent cardiovascular monitor is complex. It must process multiple waveforms, recognize artifacts, extract pertinent parameters, recognize a patient's clinical state, analyze the problem and formulate a response. This paper presents the multi-trellis (a collection of process trellises), a software architecture for building such a monitor. A process trellis is a uniform hierarchical framework for heterogeneous program modules. The multi-trellis extension allows one to compile several process trellis programs with widely varying run-time requirements into a single executable program that it is efficient, predictable and usable. Our prototype consists of two process trellises. The lower trellis contains processes to analyze three different analog signals: the blood pressure from a non-invasive monitor and an arterial catheter, and the ECG. The upper trellis contains processes to help detect evolving hemodynamic trends, identify abnormalities, and present a succinct summary to the clinician. Our prototype shows that the multi-trellis is a demonstrably useful software architecture for building these real-time, intelligent monitors.

New paradigms in telemedicine: ambient intelligence, wearable, pervasive and personalized.

After decades of development of information systems dedicated to health professionals, there is an increasing demand for personalized and non-hospital based care. An especially critical domain is cardiology: almost two third of cardiac deaths occur out of hospital, and victims do not survive long enough to benefit from in-hospital treatments. We need to reduce the time before treatment. But symptoms are often interpreted wrongly. The only immediate diagnostic tool to assess the possibility of a cardiac event is the electrocardiogram (ECG). Event and transtelephonic ECG recorders are used to improve decision making but require setting up new infrastructures. The European EPI-MEDICS project has developed an intelligent Personal ECG Monitor (PEM) for the early detection of cardiac events. The PEM embeds advanced decision making techniques, generates different alarm levels and forwards alarm messages to the relevant care providers by means of new generation wireless communication. It is cost saving, involving care provider only if necessary and requiring no specific infrastructure. This solution is a typical example of pervasive computing and ambient intelligence that demonstrates how personalized, wearable, ubiquitous devices could improve healthcare.

[Elemental research on intelligent non-invasive temporary pacemakers].

Some research on intelligent non-invasive temporary pacemakers is introduced in this paper. An industrial computer, some IC chips and other elements are used to construct its hardware, and its software is in C++ language. The experimental device has some intelligent functions of recognizing some arrhythmia. The system has a pacemaker module and an ECG monitor module. Its software includes a main program, a RS-232C communication program, a printer VxD, a pacing control VxD and ECG signal pretreatment and recognizing program and so on. The pacing-generating circuit is employed to make the precision control of pacing current. The communication between industrial-computer system and ECG module is completed through the DLL. The real time processing of ECG signals is based on filter method for a higher recognizing ratio. The system calculates several parameters to recognize certain arrhythmia and uses MIT/BIH database to validate the reliability of ECG recognition.

Freeband: the research program for ambient intelligent communication.

People today are surrounded by communication means. The last few decades have lead to an explosion of different means of communication. We are on the edge of a new 'paradigm change' which will move the centre of information control to the individual. He will become surrounded by "intelligent" electronic equipment that can provide almost all of their information and communication needs on demand: an ambient intelligent environment.

[Development of an intelligent control system for radiography].

An intelligent control system has been designed using the single chip and the related circuit, and with the assemble language. It is connected with the common X-ray units to control the exposure dose. The result shows that three parameters for radiography are well controlled by the intelligent control system, and auto-radiography is realized.

Analysis of intelligence quotient in patients with homozygous beta-thalassemia.

OBJECTIVE: To compare the intelligence quotient (IQ) of patients with thalassemia major (TM) to that of normal children. METHODS: We conducted the study in April and May 2002 on 294 homozygote beta-thalassemia patients, (157 male and 137 female, mean age of 13.2 years; range, 9-18 years). These 294 patients were randomly selected from the 984 TM patients who routinely refer to Shiraz Cooley's Medical Center in Dastgheyb Hospital, Iran for blood transfusion. Another 294 subjects age and gender matched control group were studied. Intelligence quotients were computed using the Ravin test. RESULTS: The mean IQ score +/- standard deviation (SD) in the thalassemia group was 109.83 +/- 15.94. This score revealed no statistically significant difference with the control group's score (p<=0.079). A correlation existed between the thalassemia patients' IQ and their level of education (p<0.049). CONCLUSION: The IQ of TM patients does not differ significantly from the normal population.

Speech discrimination and intelligibility: outcome of deaf children fitted with hearing aids or cochlear implants.

We retrospectively studied outcome of a sample of 36 congenitally deaf children who were fitted with either a conventional hearing aid (HA, N = 14) or a cochlear implant (CI, N = 22), and who received many years of audio-phonatory training by our multi-disciplinary team. In order to rate speech discrimination perception, we developed a new metric of speech discrimination performance based on several existing discrimination tests for the French language. Speech perception skills were measured with and without lip-reading cues. We also studied the evolution of auditory capacity in real-life situations, and the evolution of the intelligibility of spoken language during a five year period. To rate auditory capacity and speech intelligibility, we used commonly used metrics: CAP and SIR. During the five successive years of observation, CAP and SIR scores improve for both the HA and CI children. Improvement, however, is more pronounced for the CI children. At the end of the 5-years period, CAP and SIR scores of CI children are significantly higher than those of HA children. Our study is one of few that evaluates outcome of HA and CI fitting with age matched congenitally deaf children anno 2000.

Energizing the nursing lecture: Application of the Theory of Multiple Intelligence Learning.

Nurse educators struggle to find ways to create learning opportunities that are interactive and appeal to the needs of various students. The key to energizing the nursing lecture is to create an environment that encourages students to be active participants. It is essential to use creativity to design cognitive strategies that appeal to students' learning preferences. This article discusses the methods one educator has used to implement the Theory of Multiple Intelligence Learning in the classroom. Specific cognitive strategies that address the learning preferences of each intelligence are discussed.

Intelligent management of epidemiologic data.

In the lifecycle of epidemiologic data three steps can be identified: production, interpretation and exploitation for decision. Computerized support can be precious, if not indispensable, at any of the three levels, therefore several epidemiologic data management systems were developed. In this paper we focus on intelligent management of epidemiologic data, where intelligence is needed in order to analyze trends or to compare observed with reference value and possibly detect abnormalities. After having outlined the problems involved in such a task, we show the features of ADAMS, a system realized to manage aggregated data and implemented in a personal computer environment.

[The Dutch Reading Test for Adults: a measure of premorbid intelligence level].

The construction of the Dutch Adult Reading Test (DART) is described. The DART is the Dutch version of the National Adult Reading Test. Both tests consist of a series of words with an irregular pronunciation. The score on the test is a predictor of premorbid intelligence of brain damaged patients. Furthermore, results of reliability and validation studies with the DART are reported. The main findings, which were obtained with the NART, were replicated by the DART. These findings consist of a high correlation (.85) with verbal intelligence in healthy controls (n = 22) and insensitivity to cerebral deterioration in brain damaged and demented patients (n = 53). The test also appeared to be insensitive to cognitive deterioration in a group of psychotic patients (n = 43).

Evaluation of adult aphasics with the Pediatric Speech Intelligibility test.

Results of conventional adult speech audiometry may be compromised by the presence of speech/language disorders, such as aphasia. The purpose of this project was to determine the efficacy of the speech intelligibility materials and techniques developed for young children in evaluating central auditory function in aphasic adults. Eight adult aphasics were evaluated with the Pediatric Speech Intelligibility (PSI) test, a picture-pointing approach that was carefully developed to be relatively insensitive to linguistic-cognitive skills and relatively sensitive to auditory-perceptual function. Results on message-to-competition ratio (MCR) functions or performance-intensity (PI) functions were abnormal in all subjects. Most subjects served as their own controls, showing normal performance on one ear coupled with abnormal performance on the other ear. The patterns of abnormalities were consistent with the patterns seen (1) on conventional speech audiometry in brain-lesioned adults without aphasia and (2) on the PSI test in brain-lesioned children without aphasia. An exception to this general observation was an atypical pattern of abnormality on PI-function testing in the subgroup of nonfluent aphasics. The nonfluent subjects showed substantially poorer word-max scores than sentence-max scores, a pattern seen previously in only one other patient group, namely young children with recurrent otitis media. The unusually depressed word-max abnormality was not meaningfully related to clinical diagnostic data regarding the degree of hearing loss and the location and severity of the lesions or to experimental data regarding the integrity of phonologic processing abilities. The observations of ear-specific and condition-specific abnormalities suggest that the linguistically- and cognitively-simplified PSI test may be useful in the evaluation of auditory-specific deficits in the aphasic adult.

Laboratory robotics and artificial intelligence.

Intelligent robots, which incorporate artificial intelligence in their controlling software, are the next step in bringing the laboratory robot to its full potential. The areas currently under study in our laboratory are improved user interfaces for laboratory robotics, the integration of object-oriented databases into robot control programs, and strategies to optimize multi-step procedures. The ultimate goal of this work is the Standard Robotics Method. The Standard Robotics Method we envision would allow a robotic method to be transferred from one laboratory to another.

Spectral enhancement to improve the intelligibility of speech in noise for hearing-impaired listeners.

At speech-to-noise ratios between -3 and 6 dB, many hearing-impaired listeners have difficulty in understanding speech, but spectrograms reveal that the formant peaks of voiced speech and some of the spectral peaks associated with unvoiced speech stand out against the background noise. Our speech-enhancement process is based on the assumption that increasing spectral contrast will result in improved intelligibility. The enhancement involves calculating an auditory excitation pattern from the magnitude spectrum of overlapping short segments of the speech signal. This pattern is convolved with a difference-of-Gaussians function whose bandwidth varies with frequency in the same way as the auditory filter bandwidth. Magnitude values from this enhanced pattern are combined with the unchanged phase spectrum from the original signal to produce the enhanced speech. The processing was used to enhance Boothroyd and Bench-Kowal-Bamford Audiometric lists which had been digitally combined with speech-shaped noise at speech-to-noise ratios between -3 and 6 dB. The subjects had moderate to severe sensorineural hearing losses. The processing produced small but significant improvements in intelligibility for the hearing-impaired listeners tested. Possibilities for improving the processing are discussed.
