OR satellite pharmacies: demographics, services, and implementation.

OR satellite pharmacies are a relatively new addition to a system of pharmacy-coordinated drug distribution. The institution of a pharmacy branch to service the operating room is hypothesized to have a number of advantages over a centralized pharmacy system including improved service, reduced inventory, increased charge capture, better narcotic control, and more efficient drug delivery. Hospital Pharmacy and the OR Satellite Pharmacy Bulletin published a questionnaire to obtain information on OR satellite pharmacies from those actively involved in their planning, implementation, and operation. The questionnaire covered three major areas: demographics, functions and services, and implementation. This article summarizes the results.

The effect of pharmacy intervention on aminoglycoside costs.

Antibiotics constitute a large percentage of every hospital's drug budget. In an effort to control the escalating costs of antimicrobial therapy, we focused on the usage of aminoglycosides at our institution. The aminoglycosides, gentamicin and tobramycin, are similar in terms of antimicrobial spectra and toxicities. Since gentamicin is much less expensive, it was felt that significant cost savings would be realized if gentamicin were to be used preferentially over tobramycin. Specific criteria for the use of tobramycin were developed and approved by the Pharmacy and Therapeutics Committee. All patients prescribed parenteral tobramycin during the five week data collection period were entered into the study. We chose to use direct verbal intervention as our method for altering physician's prescribing patterns. An educational program of intervention was set up such that when a physician prescribed tobramycin, the patient's chart was immediately reviewed. If tobramycin was prescribed for a purpose other than those in the approved criteria for tobramycin use, the physician was contacted personally to discuss the cost-effectiveness of gentamicin use. Utilization figures from the previous six months showed that our interventions would save approximately $32,000 over a one year period. Our interventions on aminoglycoside prescribing represents a highly successful, cost-effective and educational method for altering physician's prescribing patterns.

Non-accidental injury--the adult problem.

In November 1986, 141 people attended the Accident and Emergency (A and E) Department of the Cardiff Royal Infirmary as a result of deliberate violence (assault). This represented 2.3% of new attendances at an estimated cost of pounds 10,591 for investigation and management. The workloads imposed on A and E Departments and the financial burden on the NHS are considerable.

Quality assurance of outcome in mental health care: a model for routine use in clinical settings.

A problem-oriented method of quality assurance of clinical outcome was used in 1,384 patients who were assessed and treated over 8 years by 41 nurse therapist trainees. The clinical audit suggested that for most diagnoses patients' improvement after treatment was usually satisfactory. Though the system was developed to monitor the progress of patients with anxiety, sexual and habit disorders having behavioural psychotherapy, it could also be applied to other disorders like depression and to other treatments. The method is simple enough for routine use, and computerisation would put it within the grasp of most clinics and training programs for all mental health care professionals.

Pricing in health care organizations. A key component of the marketing mix.

Pricing is one of the key components of a successful marketing mix. Pricing objectives, strategies, and tactics cannot stand alone, however. To be effective, price must work in harmony with other marketing and management activities. Despite its importance, use of pricing as a management tool is limited in health care compared to other industries. Many factors contribute to this situation, including the structure of the health-care exchange process, limited consumer knowledge, and a limited ability to measure costs. I will provide an overview of pricing information, both within and outside health care. Specifically, we will explore the definition of pricing, nonmonetary pricing, price elasticity, classical pricing theory, and the role of pricing in a health-care setting.

Hospitals cannot afford to be without access to MRI.

Although practicing state-of-the-art diagnostic medicine is impossible without access to magnetic resonance imaging, this high-ticket technology is a mixed blessing in an era of cost containment. The good news is that MRI's diagnostic capabilities continue to grow, and the prices for scanners are dropping. However, MRI services remain expensive, and reimbursement is declining. To get the maximum benefit from MRI, physicians must stay up to date with the latest advances, a task that hospitals can facilitate through educational programs. Hospitals need to plan carefully for the introduction or expansion of MRI services, but the end result will be better patient care.
