Nursing implications of interventional device placement in pediatric cardiology and pediatric cardiac surgery.

The use of interventional devices to complete or augment repair of congenital heart lesions capitalizes on the collaborative skills of the cardiovascular team: cardiology, cardiovascular surgery, and nursing. Placement of devices, eliminating the requirement for surgical intervention, places new demands on nursing staff regarding patient and family preparation. Supporting families, if the inability to place a device occurs, is a critical function of the nursing staff. The experimental nature of the devices and the complexity of the disease places additional stress on the family and increases the demands of postimplantation nursing care.

[The results of treatment by the Madrid Pediatric Oncology Group according to the BFM protocols in non-Hodgkin's B-cell lymphoma and acute B-cell lymphoblastic leukemia in pediatric patients].

The objective of this report is to present the results of the BFM group in the treatment of 41 children with non-Hodgkin's B cell lymphoma and acute B cell lymphoblastic leukemia according to the BFM 86 and 90 protocols. Forty-one children, between 2 and 16 years of age, were treated from November 1987 to October 1993. Of these, 25 were treated with the BFM 86 protocol (18 non-Hodgkin's B cell lymphomas and 7 acute B cell lymphoblastic leukemias) and the rest with the BFM 90 protocol (15 non-Hodgkin's B cell lymphomas and 1 acute B cell lymphoblastic leukemia). Complete remission was achieved in 97.5% of the patients. A relapse occurred in 12.5% of the cases. Currently, 80.4% remain in continuous complete remission and 17% have died. The 5 year actuarial survival rate of those treated with the BFM 86 and 90 protocols was 79% and 87%, respectively, and event free survival in the same period was 76% and 87%, respectively. There was no statistically significant difference in the results obtained with the two treatment protocols.

[Progress and perspectives in pediatric oncology: 30-year experience of the clinic of pediatric oncology].

Seventy percent of cases of Hodgkin's disease and 65% of those with kidney malignancies can be cured due to application of modern procedures of oncopediatrics. More intensive modalities have to be used for treatment of generalized forms of non-Hodgkin's lymphoma and sarcoma of Ewing. The rates of 3-year survival have risen to 70% in patients suffering acute leukemia following introduction of the new Protocol procedures. Infantile mortality from malignancies has been halved for the past 5 years (from 8.2/100,000 to 4.2/100,000) thanks to the taking of said measures and restructuring the oncopediatric service.

[Head injuries in the pediatric emergency department: a 5-year experience at the Pediatric Clinic of Modena].

We re-evaluated the cases of 436 children with minor head injury admitted in the paediatric department of Modena University Hospital in the years 1991-1995 in order to revise diagnostic and therapeutic protocols. The external lesions, the presence of loss consciousness, vomiting, vegetative reactions, late complications, skull X ray, TC scan and EEG were considered. Signs and symptoms at admission have been faced with instrumental findings and eventual late complications to evaluate the prognostic significance. External lesions and vomiting did not correspond to TC scan and EEG positivity or significant sequelae. A more close connection to these parameters was found when an history of immediate loss of consciousness after trauma, or presence of vegetative reactions. Skull X ray appeared useless both to judge the seriousness of the lesion and to formulate a prognosis. EEG appeared a sensible tool to evaluate the gravity of the trauma and its late consequences. TC has confirmed to be the unique instrument able to solve any diagnostic or prognostic doubt. When symptoms suggest a bad prognosis, diagnostic procedures should be limited to TC and EEG. By our point of view, patients that at admission have a normal neurological examination at the Glasgow Coma Scale, no significant symptoms and signs, an history of a minor impact dynamic and no signs of child abuse, can be discharged from the emergency department; in this case parents should be instructed how to do an observation at home in order to evaluate eventual complications.

Human immunodeficiency virus/acquired immunodeficiency syndrome education in schools. American Academy of Pediatrics. Committee on Pediatric AIDS.

The human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS) epidemic has grown during the past 15 years. Education remains a critical component of our efforts to prevent HIV infection/AIDS in school children and young adults. To accomplish this goal, school personnel should receive updated information about HIV infection/AIDS so that accurate teaching on this topic can be included in the K-12 health education curriculum. Informed pediatricians and nurses can serve as important resources for school health services and administration to provide current information for the curriculum. Each community should have a school health advisory committee that enlists community support and provides input to health education programs in schools.
