High rates of alcohol problems and history of physical and sexual abuse among women inpatients.

A total of 88 consecutive new women patients were surveyed on an adult psychiatric inpatient unit which did not have a specific program for the treatment of alcoholics. Those with a self-reported history of physical and/or sexual abuse had significantly higher scores on the Michigan Alcoholism Screening Test (MAST) than those with no such history. Former drinkers and teetotalers were more likely to have been both physically and sexually abused than the others. Thirty-three patients (38%) reported a history of alcohol problems measured by scores of seven or more on the MAST, but only 20 had a diagnosis of alcohol abuse or alcohol dependence made by a psychiatrist.

The relationship between physical and sexual abuse and illicit drug use: a replication among a new sample of youths entering a juvenile detention center.

The relationship between child physical and sexual abuse and illicit drug use are little understood and underinvestigated. Our previous study of this association found that a sample of detained youths' physical and sexual abuse experiences were related significantly and positively to their use of illicit drugs. Data gathered from a new sample of detained juveniles, interviewed 3 years after our first study, relating to this issue were examined and our previous findings were replicated.

Differential drug use patterns among sexually abused adolescent girls in treatment for chemical dependency.

A sample of 444 girls admitted to adolescent chemical dependency treatment was divided into four groups based on sexual abuse experiences. Girls who reported intrafamilial abuse, extrafamilial abuse, or both, were compared with nonvictims in terms of alcohol and drug use histories. Prevalence and frequency of alcohol, marijuana, and cocaine did not differ significantly among the groups. However, all sexual abuse victims were significantly more likely to regularly use stimulants, sedatives, tranquilizers, and hallucinogens. Sexual abuse victims also reported earlier onset of alcohol and drug use, more self-medication, and more use to escape family problems.

Psychological evaluation and support in the pediatric intensive care unit.

This article has discussed the role of the psychological consultant in the PICU. New advances in critical care treatment have created the need for a greater awareness of psychological issues that affect the medical management of children, and the emotional impact on their families. Psychological consultation can assist in clarifying and addressing these issues by identifying and evaluating the stressors impinging on the patient, and implementing or directing appropriate interventions. This intervention may also entail assistance to the family and care providers in coping with intense and emotionally sensitive issues.

Social isolation and sexual abuse among women who smoke crack.

The purpose of this study was to explore the prevalence of social isolation and its relationship to sexual trauma in a sample of Black women who smoke crack cocaine. Using a convenience sample of 115 Black women with a history of smoking crack cocaine, participants were interviewed for 2 to 4 hours and asked a variety of questions about their health, relationships, sexuality, and drug use. Bivariate and multivariate logistical regressions were used to predict whether the women reported being socially isolated. While social isolation was not necessarily a common experience among the sample, it was found that women who had been sexually abused were three times more likely to report being socially isolated than women who had not been sexually abused. In addition, social isolation was more common among women who had been abused by a family member, who had been abused when they were young, and who had been abused for a long period of time. However, multivariate analyses revealed that the age at which the sexual trauma occurred was the most salient predictor of social isolation in adulthood. Implications for drug treatment are discussed.

The wider spectrum of child abuse.

Child abuse encompasses many facets aside from the acts of physical violence which have generally attracted attention. Sexual misuse of children often goes unrecognized, yet its effect on the mental health of the victim may be considerable; it may or may not be accompanied by violence, and it generally occurs within a setting of serious disturbance in the family. Clinical details of nine sexually abused children are described, and they serve to illustrate how this form of abuse may prevent. The management of these cases is discussed. Sexual abuse of children is a family problem and therefore must be the concern of the family doctor.

Living beside traumatic experience.

This article examines the limitations of the medical model's formulation of survivors of sexual abuse. It argues that therapeutic goals framed within the medical model reflect the outcomes of mastery and rational control in liberal individualistic conceptions of the self. Through a critical analysis of autobiographical vignettes of a survivor's experiences, the article proposes that a post-structural understanding of living beside traumatic experience is helpful in fully recognizing the issues faced by survivors of sexual abuse. It outlines 6 contributions that the formulation of living beside traumatic experience offers to current understandings of survivors.
