Abnormalities of gastric emptying in portal hypertension.

OBJECTIVES: To investigate: 1) the rate of gastric emptying of portal hypertensive patients and 2) whether alterations in gastric emptying play any role in the development of portal hypertensive gastropathy. METHODS: Fifty patients (37 with esophageal varices) with cirrhosis and seven with extrahepatic portal hypertension underwent upper GI endoscopy followed by radionuclide gastric emptying studies using a semi-solid meal. Twenty-six patients also under went corrected wedged hepatic venous pressure measurement. Sixteen normal subjects underwent gastric emptying studies only. RESULTS: Varices were completely obliterated by sclerotherapy in 17 patients and were patent in 27. Thirty-seven patients had portal hypertensive gastropathy, 25 of whom had mild changes and 12 severe. No significant difference in gastric emptying was observed between patients with mild and severe portal hypertensive gastropathy and between those with portal hypertensive gastropathy and a normal gastric mucosa. There was no significant difference in gastric emptying between normal subjects and portal hypertensive patients although the latter group showed a tendency for faster gastric emptying. No difference in the rate of gastric emptying was observed between portal hypertensive patients with intrahepatic and extrahepatic pathology. However, patients with esophageal varices (patent and obliterated) emptied their stomachs significantly faster than those without (p = 0.01). There was no correlation between the rate of gastric emptying and corrected wedged hepatic venous pressure. CONCLUSIONS: It would appear that, although alterations in gastric emptying are common in portal hypertension, gastric emptying does not appear to play a causative role in the mucosal changes characteristic of portal hypertensive gastropathy.

The effects of Helicobacter pylori colonization on gastric function and the incidence of portal hypertensive gastropathy in patients with cirrhosis of the liver.

OBJECTIVES: To investigate the prevalence of Helicobacter pylori infection in 50 cirrhotic patients with biopsy-proven cirrhosis with and without portal hypertensive gastropathy and to study whether or not the effects of H. pylori colonization of the stomach on gastric acid and pepsin secretion, serum gastrin and pepsinogen I levels, gastric mucus, and gastric emptying contributed to the development of portal hypertensive gastropathy in cirrhotics. METHODS: All patients underwent an upper GI endoscopy followed by determination of basal and pentagastrin and insulin-stimulated gastric acid and pepsin secretion and serum gastrin and pepsinogen I levels. The gastric biopsies were stained to detect H. pylori infection, portal hypertensive gastropathy, and gastritis. The amount of gastric mucus was estimated by a microanalytical technique. The rate of gastric emptying was assessed by the radionuclide method using a semi-solid meal. RESULTS: Thirty-three (66%) patients had endoscopic evidence of portal hypertensive gastropathy, 10 with the severe (20%) and 23 with mild form (46%). Twenty (40%) patients had histological evidence of H. pylori infection. Eleven out of 33 (33%) patients with endoscopic portal hypertensive gastropathy had microscopic evidence of H. pylori infection. Eighteen out of 20 (90%) patients with chronic active gastritis had concomitant H. pylori colonization. In contrast, the gastric mucosa was histologically normal in 21 of the 30 patients (70%) not infected with H. pylori. Marked hypochlorhydria and reduced pepsin secretion associated with a tendency to hypergastrinemia were observed in cirrhotic patients colonized with H. pylori compared with those without. However, there was no significant difference in serum pepsinogen I concentrations, the ratio of polymeric to degraded gastric mucus, or the rate of gastric emptying between cirrhotics with and without H. pylori colonization of the stomach. Furthermore, these parameters were not significantly different in patients with portal hypertensive gastropathy with and without H. pylori infection. CONCLUSION: These observations suggest that H. pylori infection is unlikely to be involved in the pathogenesis of portal hypertensive gastropathy.

Chlamydia trachomatis antibodies in serum and ejaculate of male patients without acute urethritis.

In a prospective study, the prevalence of specific antibodies against Chlamydia trachomatis in serum and seminal plasma evaluated by a genus specific immunofluorescence test in 101 men without acute urethritis was investigated. The results were compared to the clinical diagnosis, cell culture of urethral swabs, demonstration of DNA particles by Polymerase Chain Reaction (PCR) in the ejaculate and signs of genital inflammation by counting peroxidase-positive leukocytes and elastase level in semen. The objective was to evaluate the significance of chlamydial antibodies in genital infection. Serum specific IgG and IgA antibodies were found in 26% and 15%, respectively; seminal IgG and IgA antibodies were present in 6% and 7%, respectively. Serum specific antibodies were not associated with the clinical diagnosis of infection nor with C. trachomatis cell culture, PCR findings, peroxidase positive leukocytes or PMN-elastase level. It is concluded that serum antibodies are not useful in detecting a chlamydial infection. Seminal plasma antibodies were not correlated with the clinical diagnosis of infection, positive cell culture, PMN-elastase levels and leukocytes in semen. However, a significant correlation was found for positive PCR in the ejaculate (p < 0.001 for IgG, p < 0.05 for IgA, p < 0.001 when combined). Though seminal antibodies may be more useful in detecting ascended or occult chlamydial infection, their significance remains unclear, their absence does not exclude chlamydial infection. In particular, the biological significance of locally derived IgA needs further evaluation.

Hepatobiliary disease in inflammatory bowel disease.

PSC is the most common and most important hepatobiliary disease seen in association with IBD. Approximately 5% of all patients with CUC have PSC, and most patients with PSC ultimately develop IBD, usually CUC. PSC and CUC appear to be associated diseases-one does not cause the other, but common pathogenic mechanisms are likely involved. PSC alone does not differ from PSC with IBD with regard to clinical, biochemical, cholangiographic, and hepatic histologic features. There is an overlap syndrome of CAH and PSC in patients with CUC suggesting that patients with CAH and CUC should have a cholangiogram. Colectomy in patients with PSC and CUC does not influence the PSC and, if done for colitic indications, should be accompanied by an ileal pouch-anal anastomosis. Serologic markers are being identified, which are frequently found in PSC with or without CUC, including markers for the dreaded complication of cholangiocarcinoma. Unfortunately, patients with PSC and CUC are doubly at risk for malignancies of the colon and biliary system. Medical therapies are being assessed that may beneficially affect both PSC and CUC, and liver transplantation is life-saving for patients with advanced PSC. Although CAH and gallstones are also found in association with IBD, they are much less common and of considerably less clinical importance than PSC associated with IBD.

The replacement of water with deuterium oxide significantly improves the thermal stability of the oral poliovirus vaccine.

The poliomyelitis eradication programme relies largely on the massive administration of the oral poliovirus vaccine (OPV). The major difficulty in assuring good vaccine coverage, especially in hot climates, is the thermostability of the vaccine. Several attempts have been made to stabilize the OPV with limited benefits. In this report, we describe a heavy water based stabilization procedure, which has been shown to increase the thermostability of the vaccine, notably at temperatures which are commonly encountered during usual transportation in conditions of cold chain failure. Safety considerations regarding the human use of heavy water containing bioproducts are discussed.

Estrogen receptor, growth factor receptor and protooncogene protein activities and possible signal transduction crosstalk in estrogen dependent and independent breast cancer cell lines.

Binding of estrogen to its receptor (ER) activates early genes that drive responsive cells through the proliferative phase. Earlier studies to evaluate the expression of protooncogenes, growth factors, growth factor receptor and steroid hormone receptor gene activities in the rat uterine system indicated complex pathways that involve significant 'crosstalk' between ER-systems and signal transduction pathways (Bhattacharyya et al., 1994). To analyze the interactions between these factors, we examined two well characterized estrogen dependent (MCF-7) and estrogen independent (MDA-MB-231) human breast cancer cell lines. Antibodies to estrogen receptor, epidermal growth factor receptor, c-Fos, c-Jun, and Ras proteins, protein kinases involved in receptor tyrosine kinase signal transduction pathway, MEK1 and phosphotyrosine were utilized in immunocytochemical localization experiments to evaluate temporal expression of these factors in response to estrogen treatment. ER, which was diminished in MCF-7 cells grown in estrogen-stripped medium, increased 9-fold in estrogen-reconstituted medium by 120 min. Fos and Jun appeared at nuclear and perinuclear cytoplasmic sites within 60 min after estrogen treatment in MCF-7 cells. Fos/Jun proteins were prominent in MDA-MB-231 cells, especially in association with actin filaments. Immunolabeling studies revealed no EGF-r in MCF-7 cells, while MDA-MB-231 cells contained intense EGF-r labeling in the plasma membrane. Ras protein was prominent in the cytoplasm and at the cell surface within 60 min after treatment of MCF-7 cells with estrogen. Ras was intense in MDA cells. Similarly, MCF-7 and MDA cells contained high concentrations of MEK1 and phosphotyrosine (pTyr) containing proteins in their cytoplasm and immunolabeling remained high as long as MCF-7 cells were grown in medium containing estrogen. It is speculated that MEK1 (cytoplasmic) functioning through Fos/Jun or Myc/Max (nuclear) may regulate the activity of AP-1 transcription factor. In all cases however, MEK1 and pTyr protein labeling was more intense in the highly metastatic and hormone independent MDA-MB-231 breast cancer cells. Results revealed signal transduction pathway proteins in ER+ estrogen dependent cells suggesting possible crosstalk between both receptor pathways during the proliferative phase of MCF-7 cells.

[The evolution of the incidence of glial brain tumors in Moldova in 1981-1991: the effect of excessive accidental hyper-radioactivity].

The authors discussed the effects of nuclear accident from Chernobyl (1986) upon the histotypes of cerebral glial tumors, first diagnosed during 1981-1991 at the Department of Pathology, Neurosurgical Hospital, Iaşi. It's microscopic diagnosis was determined according to the World Health Organization criterion (1979). The investigations established a tendency to diminish the incidence of astrocytomas and to increase the oligo-astrocytic tumors number, after 1986. Malignant astrocytomas did not presented any modifications with statistical significance during the eleven studied years.

Histopathological and enzymological liver alterations in rat liver in the napthol intoxication.

The study was performed on Wistar adult rats divided in 4 experimental and 2 control groups. In the 4 experimental groups, we used the oral naphtol administration in two doses: 15 mg/kg body-weight and 1.5 mg/kg body-weight. The rats were sacrificed 3 months, respectively 10 months later. Some liver parenchyma dystrophic lesions were noticed. In the subacute test group and using high drug doses the lesions were more severe, especially in the female rats. In the chronic test groups, the lesion area was decreased and in the female rats the liver showed normal histological patterns; in this test groups we also noticed a marked hepatocyte regenerative tendency. Histoenzymatically, a decrease of SDH and ATP-ase activity was recorded. The marked decrease of the hepatocyte glycogen content correspond to an increase of the G-6-P activity.

Multi-facial anomalies in mandibulofacial dysostosis. A report of three cases.

Mandibulofacial dysostosis is readily recognized on the basis of a characteristic facial appearance caused by hard and soft tissue abnormalities of the face, including malformations of the ear. Generally, the abnormality is symmetrical. The psychological and social stigma associated with severe facial deformity makes this syndrome one of the most challenging reconstructive problems presented to the craniomaxillofacial surgeon.
