[Effects of islanding on plant species diversity in Thousand-island Lake region].

In this paper, the typical area of fragmentation in Thousand-island Lake region was selected to study the effects of islanding on plant species diversity after reservoir built. 26 quadrates were installed on 18 small, medium and large islands to investigate the species and number of trees and shrubs, with inland as the control One-way variance analysis (ANOVA) showed that the species richness of trees was significantly different (F = 13.055, P = 0.000) among all kinds of islands, which was significantly higher on large islands than on small and medium ones, but not significantly different from that on inland. The species richness of shrubs was not significantly different among all kinds of islands and inland. Spearman correlation analysis showed that the species richness of trees was significantly positively correlated with island area, while the correlation between specie richness of shrubs and island area was not significant. Shannon-Wiener Index (H) analysis suggested that the diversity of both trees and shrubs on large islands was the highest, followed by on inland, but the diversity of shrubs was larger on small than on medium islands. The analysis of Simpson index and Pielou index showed that the species evenness of trees was the highest, but the dominance was the lowest on medium and large islands, while the species evenness of shrubs was the highest on medium and small islands, but the dominance was the lowest on small islands.

[Prevention and control of pain in (adeno)tonsillectomy].

Pain prevention and analgesia in (adeno)tonsillectomy OBJECTIVE. To study the efficacy of three analgesia protocols in children subjected to (adeno)tonsillectomy. SETTING. Twenteborg District Hospital, Almelo, the Netherlands. PATIENTS AND METHODS. In 64 children with body weights ranging from 15 to 25 kg (mean 20.4 kg) subjected to (adeno) tonsillectomy according to the procedure most current in the Netherlands (according to Sluder with administration of a volatile anaesthetic without intubation in day treatment), three protocols for preventing and treating postoperative pain were applied, with the analgetics most current in the Netherlands. Permission from the Medical Ethical Committee and written informed consent from the parents were obtained. According to protocol I 240 mg paracetamol was given, according to protocol II, 500 mg paracetamol and according to protocol III, 500 mg paracetamol plus 10 mg codeine, in suppository form, every 4 hours always starting 60 minutes before operation. Initially, protocol I was carried out, subsequently randomization of protocols II and III. After a pilot study use of a control group was omitted for ethical reasons because of the severe pain in a placebo group. MEASUREMENTS. The efficacy of the analgesia was evaluated by means of pain assessments by the children themselves using the Oucher test, 1, 3 and 6 hours after operation. The statistical processing of the children's pain scores was performed primarily in a descriptive sense, with simultaneous comparison of the three groups using a single variance analysis for comparison of several groups. Multiple comparisons were carried out according to Ducans' multiple range test. The level of significance was put at p less than or equal to 0.05. RESULTS. One hour postoperatively there was no significant difference among the mean pain scores (0: no pain and 10: very bad pain) of the three groups, viz. 5.2 (SD 3.0), 4.7 (SD 2.9) and 4.7 (SD 2.3), respectively; three hours postoperatively there was a significant difference between the mean pain scores of group I (4.5; SD 2.4) and group II (2.7; SD 2.4) but not between group I and group III (3.3; SD 1.5); six hours postoperatively there was a significant difference between the mean pain scores of group I (4.1; SD 2.1) on the one hand and of groups II (2.1; SD 1.5) and III (2.6; SD 1.6) on the other. CONCLUSION. The findings indicate existence of violent pain immediately after recovery from anaesthesia after (adeno)tonsillectomy as shown by the pain assessments by the children themselves one hour postoperatively, in spite of preventive administration of a high dose of a 'minor' analgetic according to each of the three protocols. Modification of the anaesthetic technique should be considered to effect adequate prevention or suppression of pain.

[Depression evaluation in lung cancer patients and in healthy subjects in dependence on their attachment styles].

THE PURPOSE: of this study was to look for effects of attachment styles for emotional reacting with depression in 117 lung cancer patients awaiting surgery and in 112 healthy subjects. MATERIAL AND METHODS: All subjects were classified according to their attachment style (secure, avoidant, ambivalent) assessed by means of the instrument based on Hazan and Shaver's (1987) descriptions of how people typically feel in close relationships. The Beck Depression Inventory was used to measure depression. Clinical group differed neither in age t(227)=0,32 p>0,7 (mean for controls 58,59; mean for clinicals 58,97) nor in sex chi2 (1)= 0,248 p>0,6 (29 females in clinicals and 31 in controls; 88 males in clinicals and 81 in controls). The groups did not differ in education years t(225)=0,947 p>0,3. The groups did not differ in the attachment styles chi2 (2)= 4,20 p>0,7. Manova variance analysis revealed significant main effect of group factor F(1,217)= 60,5 p< 0,005, higher depression in cancer patients (mean= 15,37) than in controls (mean 6,50); significant main effect of attachment style factor F(1,217) = 7,14 p< 0,002, higher depression in the anxious-ambivalent subjects (mean= 13,65) than in the secure ones (mean= 8,15); significant interaction between two factors F(1,217)= 5,23 p< 0,007, higher depression in the attachment styles in lung cancer patients (secure 10,76; avoidant 14,27; ambivalent 21,04) than in controls (secure 5,5; avoidant 7,75; ambivalent 6,27). Correlational analysis indicated that the secure style is favourable for weaker depression of patients (Pearson's r= -0,27 p< 0,01), but the anxious-ambiwalent style for stronger depression (Pearson's r= 0,43 p< 0,01). The avoidant style did not correlate significantly with depression neither in healthy (Pearson's r= 0,07 p> 0,1) nor affected by the disease subjects (Pearson's r= 0,01 p> 0,1). CONCLUSIONS: the secure or the avoidant style may protect lung cancer subjects before operation from excessive reacting with depression but the anxious-ambivalent style may be a risk factor for maladaptive emotional reacting with depression.

[Adult attachment styles and depression in lung cancer patients undergoing surgery during and after hospitalization].

The purpose of this study was to look for effects of attachment styles for emotional reacting with depression in 117 lung cancer patients who were undergoing surgery during hospitalization and six weeks later. All subjects were classified according to their attachment style (secure, avoidant, ambivalent) assessed by means of the instrument based on Hazan and Shaver's (1987) descriptions of how people typically feel in close relationships. The Beck Depression Inventory was used to measure depression. Anova variance analysis revealed higher level of depression in anxious-ambiwalent style than in the secure style or the avoidant style during hospitalization. Six weeks later higher level of depression was in the anxious-ambivalent subjects than in the secure. The avoidant style took the middle position and it did not differ with the level of depression from others. The results indicate that the secure or the avoidant style may protect lung cancer subjects from overreacting with depression during hospitalization, but the anxious-ambivalent style may increase the depression level. After the return home the insecure styles may react with stronger depression and the secure style with weaker depression.

Gel vehicles are not inherently more irritating than creams.

BACKGROUND: It has been thought that topical gels are inherently more irritating than topical creams. Nevertheless, the irritancies of topical products are potentially quite variable, and a priori assumptions about relative irritancy of gels versus creams may not be accurate. PURPOSE: To determine whether a metronidazole gel formulation is more or less irritating to the skin compared to metronidazole creams. METHODS: A total of 32 normal, healthy volunteers were tested using irritancy patches with 0.75% metronidazole gel and cream, 1% metronidazole cream, and petrolatum (used as the "negative control"). Blinded observers evaluated the application sites for signs of irritancy. A numerical score was assigned to these irritancy patch sites each day for 21 days, or until significant irritation developed, and cumulative irritancy scores were calculated for the study period. A mixed model of variance analysis was performed. RESULTS: After 21 days of evaluation, analysis of the mean cumulative irritancy scores for each of the agents used showed there to be no statistical difference in irritancy potential between the metronidazole gel and the metronidazole creams. However, the 1% metronidazole cream was significantly more irritating than petrolatum. CONCLUSION: There was no significant difference in the cumulative irritancy potential of cream and gel preparations of metronidazole. The irritancy of topical formulations for treating rosacea should be considered on a case by case basis.

Changes in bone resorption and vascular endothelial growth factor after a single zoledronic acid infusion in cancer patients with bone metastases from solid tumours.

Zoledronic acid (Zometa, ZOL) is increasingly used to treat tumour-induced bone disease, and is also reported to have antiangiogenic properties in vivo. In this study, we have investigated the correlations between changes in the proangiogenic cytokine, vascular endothelial growth factor (VEGF), and markers of bone resorption in a cohort of patients with metastatic bone disease, following a single infusion of ZOL. Twenty-four consecutive selected cancer patients with scintigraphic and radiographic evidence of bone metastases were treated for the first time with a single infusion of 4 mg ZOL. Patients were considered ineligible if they had received any steroid therapy, radiotherapy, chemotherapy, immunotherapy or haemopoietic growth factors in the 4 weeks before or during the study period. Circulating levels of VEGF and beta crosslinked type I collagen C-telopeptide (betaCTX) were measured at base-line and at 1, 2, 7 and 21 days following ZOL infusion. The majority of our patients (23/24) developed a significant reduction in circulating levels of betaCTX at just 1 day after the single zoledronic acid infusion, median percentage decrease 67.05% (95% CI, 52.39%; 76.27%). This reduction persisted at all following time points in almost all subjects in our patient population (day 2, 95.8%; day 7, 100%; day 21, 91.7%). The median decrease at day 2 was 85.67% (95% CI, 78.23%; 90.16%); at day 7, 67.38% (95% CI, 67.38%; 86.98); and at day 21, 76.89% (95% CI, 35.00%; 83.16%). Moreover, a linear regression model with variance analysis demonstrated a statistically significant correlation between median VEGF and betaCTX circulating levels at each of the time points (1, 2, 7 and 21 days after ZOL infusion). The present work demonstrates that a single infusion of ZOL was able to induce a rapid and long lasting decrease of betaCTX plasma levels in the majority (23/24) of the included cancer patients. Furthermore, we found that there is a correlation between the levels of VEGF and betaCTX following ZOL treatment. Future clinical trials should be designed to prospectively evaluate the prognostic role of reduction of betaCTX and VEGF in response to ZOL to predict clinical and skeletal outcome.

[The hemodialysed patient and his/her family caregiver. Comparison of perceptions of the chronic illness].

BACKGROUND: According to health psychology, the family caregiver (fc), i.e. the person who takes care of a hemodialysed patient, plays a pivotal role in coping with dialysis. This study explored and compared the lifestyle and the main needs of a cohort of hemodialysis patients, with reduced personal autonomy, to their fc, evaluating some psychological functional parameters, such as the perception of familial and social support, the psychological quality of life, the disability due to chronic illness, and the communication style. METHODS: An anonymous multiple versions questionnaire, administered according to the caregiver's family relationship, was given for self assessment to 54 couples of patients and related fc (spouse, son/daughter and brother/sister), mean age 66 and 60, respectively; mean dialytic patients' age: 8 years and 6 months. The questionnaire consisted of three different sections, demographics, renal disease and psychological evaluation, with 4 standard scales (Social Support Satisfaction, Marital Communication, Psychological General Well-Being Index and Evaluation of Needs). A multivariate variance analysis (MANOVA) was subsequently performed. RESULTS: Women have a higher perception of their lifestyle change after dialysis, and, in general, patients communicate more easily with their fc than vice versa. Communication problems are more common in patients with a recent diagnosis. Patients and fc mostly need a better dialogue with their nephrologists and urge some psychological help. CONCLUSIONS: The quality of the relationship between physicians, patients and their families is a key element in the process of healing.

[Accuracy of weight prediction of full term newborns in ultrasonography].

OBJECTIVES: The estimation of prognostic value of ultrasonography in fetal weight assessment. DESIGN: Assessment of practical value of ultrasonography in full term newborns. MATERIAL AND METHODS: We have retrospectively analyzed the outcomes of 611 ultrasonographic studies. It was divided into three groups: K, B, BD in relation to weight of newborns. According to that K group involved 302 newborns weighing between 2500-4000 g, B group 262 of newborns weighing more than 4000 g, BD group (47 cases) with weight exceeding 4500 g. Expected fetal weight were determined by using Shepard's algorithm. The single factor variance analysis with multiple coparison test was applied for assessment of mean errors of predicting fetal weight. RESULTS: The highest absolute value of error of expected weught of newborns occured in the group of the newborns wieghing more than 4500g in comparison with its real weight stated just after delivery (p<0,05). The underestimation of fetal weight appeared with incresing newborn weight (beta, 1 BD groups). The overestimation of fetal weight was found in the group of the newborns between 2500-4000 g (p<0,05). CONCLUSIONS: The prognostic value of Shepard algorithm in predicting fetal weight is the most accurate for fetuses weighing between 2500-4000 g. The result of our study indicates U/S was the most useless for the newborns over 4500 g. Obstetricans should take into account other factors affecting fetal growth (maternal, paternal, environmental), especially with suspicion of fetal weight over 4500 g.

Colocalization of ion channels involved in frequency selectivity and synaptic transmission at presynaptic active zones of hair cells.

Calcium ions serve as intracellular messengers in 2 activities of hair cells: in conjunction with Ca2(+)-activated K+ channels, they produce the electrical resonance that tunes each cell to a specific frequency of stimulation, and they trigger the release of a chemical synaptic transmitter. Our experiments indicate that both of these functions are conducted within a region that extends a few hundred nanometers around each presynaptic active zone. In focal electrical recordings from the plasma membranes of isolated anuran hair cells, we found nearly all of a cell's Ca2+ channels and Ca2(+)-activated K+ channels clumped at a fixed ratio in an average of 20 clusters on the basolateral membrane surface. Because serial-section electron microscopy indicated that each hair cell has approximately 19 afferent synaptic contacts with a similar distribution upon its basolateral surface, we conclude that the channel clusters coincide with synaptic active zones. Ensemble-variance analysis of current fluctuations indicated that each cell has a total of approximately 1800 Ca2+ channels and approximately 700 Ca2(+)-activated K+ channels; if these are uniformly divided, we estimate that each channel cluster contains approximately 90 Ca2+ and approximately 40 Ca2(+)-activated K+ channels. Freeze-fracture electron microscopy demonstrated an average of 133 large intramembrane particles in the presynaptic membrane at each active zone, an observation that suggests that the particles are the clustered channels. We used the K+ channel's sensitivity to intracellular Ca2+ to assay the concentration of free Ca2+ in the presynaptic cytoplasm, which we found to vary between 10 microM and 1 mM over the physiological range of membrane potentials. The inferred concentrations agreed with the values predicted for free diffusion of Ca2+ away from Ca2+ channels scattered randomly within a 300-nm-diameter synaptic active zone. The close association among Ca2+ channels, Ca2(+)-activated K+ channels, and synaptic active zones is necessary both for the rapid activation of K+ currents required in electrical resonance and for the transmission at afferent synapses of information about the phases of high-frequency stimuli.

The investigation of solubility values, water sorption and bond strength of auto-polymerising and heat-polymerising acrylic resin materials.

In this study the solubility, water sorption and the bond strength of auto-polymerising and heat-polymerising acrylic resins were investigated. Two auto-polymerising and five heat polymerising acrylic resin materials were used for this study. Thirty-five specimens were prepared (50 mm in diameter and 0.5 +/- 0.01 in thickness) for water sorption and solubility tests. In order to examine bond strength, specimens were prepared in 2.5 x 10 x 64 mm dimensions. These specimens were repaired with QC and major repair acrylic. Some specimens were kept in distilled water for 50 hours, and others for 30 days. The strength of the bond was measured by a Pull-Compress device. As a result of the variance analysis; water sorption, solubility and the bond strength between acrylic resins were identified as statistically significant (P < 0.001). The water sorption of the major acrylic resin specimens were lower than the other materials (P < 0.001).

Functional results of vertebral augmentation techniques in pathological vertebral fractures of myelomatous patients.

INTRODUCTION: This is a retrospective study to determine the effects of vertebroplasty and kyphoplasty on quality of life in multiple myeloma patients with spinal compression fractures. MATERIAL AND METHODS: Thirty-four patients with primary multiple myeloma were treated for symptomatic compression fractures between June 2003 and June 2005. Kyphoplasty was applied to 22 levels in 18 and vertebroplasty to 28 levels in 16 patients. The pain-related disability was evaluated for every single daily living activity using visual analog scale (VAS) over 10 points. (pain at rest, walking, sitting-standing, taking a shower and wearing clothes). (This evaluation is performed to every patient with degenerative disorders of the spine upon admission to our clinic.) Overall VAS scores were evaluated over 50 points (0 minimum, 50 maximum) preoperatively, at postoperative six weeks, six months and at one year prior to taking analgesics. The amount of analgesic use was recorded. Data was analyzed statistically using variance analysis, Friedman's multiple comparison test and Student's t test. RESULTS: The mean overall pain score in the kyphoplasty group decreased from a preoperative value of 36 to 12.13 at the sixth postoperative week, to 8.63 at the sixth month and to 9.72 at one year. (p<0.001). The mean overall pain score in the vertebroplasty group decreased from a preoperative value of 37.83 to 15.33 at the sixth postoperative week, to 12.17 at sixth months and to 13.47 at one year. (p<0.001). Student's t test was used to analyze the percentage of differences in overall pain score. Difference between groups was not statistically significant at the sixth week (p=0.106) but was statistically significant both at the sixth month (p=0.024) and at one year (p=0.027) in favor of kyphoplasty group. No secondary collapse was observed in adjacent levels in both groups. There were no intrapostoperative neurologic/pulmonary complications in both groups. Analgesics usage significantly decreased in both groups. CONCLUSION: In multiple myeloma, when pathological spinal compression fractures cause intractable pain and are unresponsive to conservative treatment, both vertebroplasty and kyphoplasty are effective in increasing quality of life and decreasing pain.

Natural history of benign prostate hyperplasia.

BACKGROUND: Benign prostate hyperplasia is one of the most common diseases affecting the health of the aging males. Watchful waiting is an acceptable management strategy for benign prostate hyperplasia in which the patient is monitored by the physician but receives no active intervention. The epidemiological data on this are lacking in China. Our study was designed to evaluate the changes of signs and symptoms of patients with benign prostate hyperplasia during management by watchful waiting in China. METHODS: One hundred and forty-five patients with benign prostate hyperplasia aged > 50 years were enrolled in management by watchful waiting. All the patients were visited every 6 months and were given an International Prostate Symptom Score and Quality of Life questionnaire to complete. They also had uroflowmetry and were assessed using ultrasonography to get the volume of prostate, transition zone and amount of residual urine. The Student's t test, the Chi-square test, and variance analysis were used in the statistical analysis. RESULTS: All patients were visited after 6 months, the mean volume of transitional zone was found to have increased by 1.6 ml (P < 0.01), International Prostate Symptom Score was increased by 0.8 (P < 0.01) and Quality of Life was increased by 0.2 (P < 0.01), and there was no statistical change in other data. Among these patients, 17.9% (26/145) visited again after 12 months when the data failed to show a statistically significant difference among the three groups (0, 6, and 12 months). CONCLUSIONS: After one year's follow-up, the progression of benign prostate hyperplasia was slow and the clinical data did not undergo much change.

[Rehabilitation of patients with unilateral congenital microphthalmos].

The purpose of the present study was to develop a surgical treatment for congenital microphthalmos, by using the biomaterial Alloplant that causes an increase in eyeball sizes. There were 48 cases of unilateral congenital microphthalmos prior to surgery. The patients' age ranged from 2 to 14 years; the duration of follow-ups was 1 to 6 years. Data processing by parametric variance analysis indicated that there was an increase in the mean sizes of the anteroposterior and transverse axes (APA and TA) of the eye 2-3 years following surgery. With the use of the authors' method, the conjunctival cavity is not deformed and, on the contrary, after surgery it acquires a regular slit-like shape with vaults of sufficient depth, which upgrades the quality of prosthetic repair in future. The increases in the sizes (APA and TA) and volume of the eyeball not only just after surgery, but also in the late periods (up to 2-3 years) suggest that the developed operation using the biomaterial Alloplant triggers the mechanism of coordinated eyeball growth.

[Evaluating a new, fully automated blood separator with continuous blood flow].

The AS 104 separator provides for platelet yields in agreement with given standards. The number of undesired cells (leukocytes, erythrocytes) compares favorably with other technical separation systems. Concentrate volumes, total amount of ACD-A used, extracorporal blood volume and the reduction of donor blood cells are no challenge for the donor. The number of technical problems is not unusual for a new separation system entering into the first clinical evaluation. These data, as well as the deviation of yields, recommend further optimation. The technical optimation achieved so far and an expanded evaluation using statistical procedures permit an optimal, donor-independent (using variance analysis) or individual (using regression analysis) equilibration of the machine and permit the recognition of potential differences between different separators, centers or software versions, thus allowing the calculation of the number of runs needed to recognize differences from optimal standards.

[Rehabilitation after lumbar intervertebral disk operation].

122 patients were admitted 3 months after a lumbar disc operation to a rehabilitation clinic. Conservative treatment during the rehabilitation induced a decrease of low back pain (70 out of 107 patients), of paresis (30/51 patients), and of paresthesia (51/77 patients). More than 20 pre- and post-operative variables were tested with a rank-variance analysis regarding a possible influence on efficacy of the rehabilitation treatment. The success of the conservative treatment measured by improvement of paresis, paresthesia, pain and mobility of lumbar spine was influenced favourably by preoperative paresis (p less than 0.03). Women showed more often than men an improvement of paresis (p = 0.006) immediately after surgery. Patients with a preoperative paresis had a shorter history of radicular symptoms (p = 0.002), an acute onset was seen more often in patients with persistent paresis (p = 0.019). Paresthesia was found more frequently before surgery (p = 0.010) and at begin of rehabilitation (p = 0.006) in patients with paresis compared to patients without paresis. A statistically significant association was also evaluated between decreased lumbar mobility and laminectomy (p = 0.007). Patients with L5/S1 disc operation had a longer duration of radicular symptoms (p = 0.012), a decreased frequency of paresis (p = 0.040), but more often paresthesia (p = 0.001) compared with L4/5 operation.
