[The comparison of biological characteristics between tumor-infiltrating lymphocytes and tumor-associated lymphocytes in ascites of epithelial ovarian carcinoma].

OBJECTIVE: To compare biological charicteristics between tumor-infiltrating lymphocytes (TIL) and tumor-associated lymphocytes (TAL) in epithelial ovarian carcinomas. METHODS: The surgical samples of four ovarian specimens and eight cancer ascites from epithelial ovarian carcinoma were collected, and ascites were divided into bloody and non-bloody groups. The cancer tissue was dissected to yield single cells suspensions. TIL and TAL were grown in coculture with autologous tumor cells and 500 U/ml recombinant interleukin-2 (rIL-2). Phenotype was studied using flow cytometry for a varity of human immunocompetent cell surface markers (CD3, CD4, CD8, CD25). Cytotoxicity was investigated using 4-hr 51Cr-release assays with primary ovarian carcinoma cell cultures and the CAOV3 cell line and the K562 cell line as target cells in cultured period. RESULTS: (1) There was no significant difference in CD3, CD4, CD8 and CD25 expression among the three groups (P > 0.05). CD4 to CD8 (CD4/CD8) ratio was consistently more than 1 in cultured period. (2) In all three groups, Natural killer (NK) acitivity was high and no difference among them (P > 0.05). (3) In all twelve cases, TIL and TAL in the non-bloody ascites possess higher potential to lyse the autologous tumor target than TAL in the bloody ascites (P < 0.01) when effector cell to target cell (E:T) was 50 to 1 (50:1). Lytic activity was 30.2%-51.0% and 10.8%-15.8% respectively. CONCLUSIONS: TIL and TAL in non-bloody ascites in epithelial ovarian cancer have the same biological characteristics.

Direct infusion of ascites into the blood circuit during hemodiafiltration in uremic patients with cirrhosis.

Two chronic dialysis patients with massive ascites caused by cirrhosis were treated by infusion of their ascites directly into the blood circuit. This stabilized their hemodynamics during dialysis, facilitating the control of weight gain and ascites, and thus markedly improving their general condition. Long-term use of this therapy was able to prevent the accumulation of ascitic fluid. Interestingly, fever occurred when this therapy was performed with hemodialysis, but not with hemofiltration or hemodiafiltration, suggesting that a pyrogen in the ascites was removed by filtration.

[Experimental study on CPT-11 intraperitoneal chemotherapy--metabolism of CPT-11 in malignant ascites].

The metabolism of CPT-11 in malignant ascites of gastric cancer patients with peritoneal seedings was studied in advance of the intraperitoneal chemotherapy of CPT-11 in humans. Malignant ascites and blood were drawn from gastric cancer patients. CPT-11 solution (20 mg/ml; 0.2 ml) was added into 3.8 ml ascites or plasma under 37 degrees C and CPT-11, SN-38 and SN-38GLU concentrations were measured with HPLC at times of 5, 30 and 60 minutes after addition of CPT-11. The change from CPT-11 to SN-38 was minimal not only in plasma, but also in malignant ascites. SN-38 GLU concentration was below the limit of measurement. This study showed that in malignant ascites, the enzymes such as carboxyesterase that convert CPT-11 to SN-38 were not present or minimal.

Albumin gradient and portal vein velocity in severe viral hepatitis patients complicated with ascites.

BACKGROUND/AIMS: To ascertain the mechanism of development of ascites and the hemodynamic change of portal system, 28 patients who had an episode of severe viral hepatitis with and without spontaneous bacterial peritonitis were studied. METHODOLOGY: By means of a non-invasive duplex system, the relationships between the albumin gradient of serum-ascites and the portal vein velocity were assessed in 10 patients with spontaneous bacterial peritonitis and 18 patients without spontaneous bacterial peritonitis. RESULTS: In the group of patients with severe form of hepatitis but without spontaneous bacterial peritonitis, there is a significant positive linear correlation between the level of serum albumin and the albumin gradient of serum-ascites (P < 0.001). However, a receded linear correlation was found between the serum albumin and the portal vein velocity (P < 0.02), and between the albumin gradient of serum-ascites and the portal vein velocity (P < 0.027) with Spearman linear correlation analysis. CONCLUSIONS: The present study suggests that hypoalbuminemia may be related to impairment of synthesis function in severe hepatitis itself, but not to the intraabdominal shifting of protein. Decreased portal vein velocity may be the first presenting sign in patients with severe form hepatitis complicating simple ascites. Yet this relationship will be masked and complicated once spontaneous bacterial peritonitis occurs.

Ascites caused by brucellosis: a report of two cases.

Brucellosis is a zoonosis still endemic in developing areas of the world. Focal complications of Brucellosis are well known and often troubling in the differential diagnosis. Gastrointestinal complications of Brucellosis are randomly reported, ascites being particularly rare, and usually in the context of a predisposing condition such as cirrhosis. We describe two cases of ascites attributed to Brucellosis, one in a previously healthy patient with an accompanying clinical picture of acute Brucellosis, in whom ascites reflected the response of the peritoneal mononuclear phagocytic system, and one in a HBsAg-positive, but not cirrhotic, patient, in whom Brucellosis seemed to act as a trigger effect for the development of ascites. Both cases of ascites were of an effusion type, with a predominantly lymphocytic cell count, and exhibited an excellent response to treatment with tetracycline and rifampicin.

[Crohn's disease and ascites].

Crohn's disease is not usually included among the causes of ascites. Although cases of Crohn's disease presenting as ascites have occasionally been described, the appearance of ascites during follow-up, with no relationship with other complications of the disease, such as thrombosis of the suprahepatic veins, has not been previously described. We present the case of a 36-year-old female patient with Crohn's disease who presented ascites during evolution. Evacuative paracenteses were required. No causes apart from Crohn's disease were found. Ascites followed a parallel course to acute episodes of this disease.

The effects of dietary flax oil and antioxidants on ascites and pulmonary hypertension in broilers using a low temperature model.

1. Three experiments were conducted using a low temperature model to induce pulmonary hypertension (PH) and ascites in broiler chickens. Diets containing 25 g or 50 g flax oil/kg food and control diets with an equivalent amount of animal/vegetable (A/V) blend oil, with and without supplemental antioxidants (vitamin C and vitamin E) were used. The amount of PH was assessed by the ratio of right ventricle weight to total ventricle weight (RV/TV ratio). Birds were considered to suffer from pulmonary hypertension syndrome (PHS) if the RV/TV ratio was greater than 0.299. 2. In experiment 1, the test diets contained 50 g oil/kg food and were given during the grower period only. Birds fed on the flax oil diet tended to have a lower incidence of PHS, ascites and lower RV/TV ratios than birds fed on the control diet. However, when the flax oil diet was supplemented with antioxidants, the incidence of ascites, PHS, haematocrit and whole blood and plasma viscosity increased compared with birds fed on the flax oil diet without antioxidants. These effects were not seen in experiment 2, when the test diets containing 30 g oil/kg food (25 g flax oil plus 5 g A/V blend oil/kg food compared to 30 g A/V blend oil/kg food) were given during the grower period. However, in experiment 3, when the test diets containing 30 g oil/kg food were given from day 1 to week 8, birds fed on the control diet supplemented with antioxidants had a higher incidence of PHS than those fed on the control diet alone. 3. In all 3 experiments, there was no significant effect of dietary fat source or supplemental antioxidants on total food intake or food conversion. 4. We conclude that diets containing 50 g flax oil/kg food tend to reduce the incidence of PHS and ascites in broilers using a low temperature model but the results were not statistically significant. In some cases, supplementing diets with a combination of vitamin E and vitamin C increased the incidence of ascites and PHS.

Isolated fetal and neonatal ascites: report of two cases.

Neonatal ascites is an uncommon problem that may be caused by a number of etiologies including diseases of genitourinary system and gastrointestinal system, cardiac disease, hepatic disease, systemic infection such as TORCH or parvovirus, chylous, ovarian cause, inborn error of metabolism and idiopathic. We reported two cases of neonatal ascites, one was caused by cytomegalovirus and no obvious causes could be detected in the second one. The ascites were diagnosed by prenatal ultrasound at the gestational age of 25 weeks and 37 weeks respectively and were resolved spontaneously after birth. One-year follow-up of both cases revealed normal growth and development. No recurrent ascites could be detected by abdominal sonography except for evidence of mild hepatomegaly that was noted in case 1. Thus, isolated fetal and neonatal ascites without other concomitant abnormalities were diagnosed, a separate entity from non-immune hydrops fetalis with excellent prognosis.

Recurrent ascites and pleural effusions after surgery for early-stage endometrial adenocarcinoma.

A case of massive postoperative ascites in a woman treated for endometrial cancer is reported. A workup for typical causes of ascites yielded negative results, prompting a more detailed analysis of the patient's condition. Hypothyroidism was discovered. After correction of the hypothyroidism, the ascites slowly resolved. Since myxedema is an uncommon cause of ascites, this is usually a diagnosis of exclusion. However, hypothyroidism must be ruled out to prevent unnecessary and possibly inappropriate treatments for ascites.

The effect of diuretic treatment on red cell and plasma sodium concentration in patients with hepatic cirrhosis and ascites.

The sodium content of red cells and plasma in some cirrhotic patients with ascites has been measured, before, during and after diuretic treatment. The results were compared with those found in 37 normal subjects and 42 untreated cirrhotic patients with ascites. In the 10 cirrhotic patients with ascites studied, the mean sodium content in erythrocytes before diuretic treatment was 9.95 (+1- SD 3.27) mEQ/kg, 27.15 (+/- SD 5.60) mEq/kg of dry weight and 15.02 (+/- SD 2.70) mEq/kg of red cell water. During diuretic treatment, the mean sodium content in red cells was significantly decreased to 6.27 (+/- SD 1.46) mEq/kg; 18.85 (+/- SD 4.36) mEq/kg of dry weight and 10.15 (+/- SD 2.41) mEq/kg of red blood cell water (P less than 0.01, P less than 0.01, and P less than 0.02 respectively). The mean plasma sodium concentration which was 135.95 (+/- SD 4.14) mEq/kg before diuretic treatment was significantly decreased during treatment to 129.19 (+/- SD 2.77) mEq/kg, P less than 0.001. Once the ascites had disappeared and the diuretic treatment had been discontinued the sodium concentration in both plasma and red cells became normal. The radioactive 22Na exchange "in vitro" between plasma and red cells of cirrhotic patients with ascites remained unmodified during diuretic treatment.

Soluble FLT-1 expression suppresses carcinomatous ascites in nude mice bearing ovarian cancer.

Vascular endothelial growth factor (VEGF), a bifunctional protein enhancing vascular permeability and stimulating endothelial growth, is thought to be responsible for fluid accumulation and angiogenesis in ascites tumors. To investigate the effects of stable expression of the soluble form of Flt-1 VEGF receptor (sFlt-1), a known endogenous inhibitor of VEGF, on the malignant ascites tumors, we cotransduced RMG-1 human ovarian cancer cells with adeno-associated virus vectors carrying the sFlt-1 cDNA and Neo gene or Neo gene alone and isolated both the sFlt-1-expressing clone and the Neo-expressing clone. In vitro growth characteristics were essentially the same. As expected, conditioned medium collected from the sFlt-1-expressing cells significantly inhibited the human umbilical vein endothelial cell proliferation in the presence of recombinant VEGF. Expression of sFlt-1 significantly suppressed RMG-1 cell-induced angiogenesis in vivo in the mouse dorsal air sac assay model. We then inoculated sFlt-1- or Neo alone-expressing cells i.p. into female BALB/c nude mice. The average volume of ascites fluid, number of leaked RBCs, and number of cancer cells were significantly lower in mice injected with sFlt-1-expressing cells than in the controls. Survival time was significantly prolonged in mice injected with sFlt-1-expressing cells. These results suggest that inhibition of VEGF activity by sFlt-1 expression may provide a means to control carcinomatous ascites and angiogenesis of malignant ascites tumors.

[Intrarenal arterial doppler ultrasonography in cirrhotic patients with ascites, with and without hepatorenal syndrome].

BACKGROUND: The pathophysiological hallmark of the hepatorenal syndrome (HRS) is renal vasoconstriction. Doppler ultrasonography can be used to assess the vascular resistance in small renal intraparenchymal vessels through analysis of the Doppler waveform by a parameter termed Resistive Index (RI). We postulated that the RI could be important for the diagnosis and prognosis of HRS. AIMS: To assess the RI in cirrhotic patients with ascites, with and without HRS. PATIENTS AND METHODS: We studied 48 cirrhotics with ascites, of whom 12 were with and 36 without HRS and other 23 were normal subjects. We measured the intrarenal arterial RI (Resistive index = Peak systolic velocity - Minimum diastolic velocity/Peak systolic velocity) with color Doppler ultrasonography after visualization of interlobular or arcuate arteries. It was considered abnormal when higher than 0.70. RESULTS: The RI values, mean and SD) were: normal subjects: 0.58 +/- 0.05, cirrhotics with ascites: 0.65 +/- 0.05 and cirrhotics with ascites and HRS: 0.78 +/- 0.11. Patients with HRS had significantly higher values than those without HRS (p < 0.001). The Relative Risk of developing the HRS in patients with a RI > or = 0.70 were 3.32 (CI 95% = 1.79-6.2) CONCLUSIONS: The RI was useful in patients with cirrhosis and ascites for the prognosis of HRS and could suggest diagnosis of HRS with values of 0.78 or higher, if other clinical conditions that produce renal vasoconstriction are excluded.

Malignant ascites: demographics, therapeutic efficacy and predictors of survival.

Demographics, treatment patterns, treatment efficacy and clinical predictors of survival were studied in 76 consecutive patients with malignant ascites. Sixty-four percent of patients were female, and mean age was 63 years. The most common primary malignancies were ovarian cancer, carcinoma of unknown primary, breast cancer, colorectal carcinoma and lymphoma. Ascites was present at the time of diagnosis of malignancy in 39%. Diuretics were administered in 22% of patients with a 22% response in ascites; all responding patients had hepatic metastases. Systemic anticancer therapy was administered in 38%, with a 38% objective response in ascites. Five peritoneovenous shunts were placed, with one shunt functional at 2 weeks. Paracentesis was performed in 71% of patients with complications potentially due to paracentesis occurring in 24% of these patients. Median survival was 78 days from the clinical diagnosis of ascites. Multivariate analysis revealed significantly shortened survival in patients with liver metastases and elevated serum bilirubin, while ovarian cancer was a significant independent predictor of prolonged survival.

Detection of soluble tumor necrosis factor-p55 levels in the serum and ascitic fluid of patients with hepatocellular carcinoma.

OBJECTIVE: To examine soluble tumor necrosis factor receptor-p55 (sTNFR-p55) levels in the serum and ascitic fluid and investigate the significance of this examination in assessment of the clinical status of patients with primary hepatocellular carcinoma (HCC). METHODS: Enzyme-linked immunosorbent assay (ELISA) was used to examine sTNFR-p55 levels in the serum and ascitic fluid in 25 patients with HCC and 25 with liver cirrhosis (LC). RESULTS: sTNFR-p55 levels in the serum and ascitic fluid in patients with HCC were significantly higher than those in patients with LC and controls (P=0.001). No significant difference was found between LC and the control in terms of serum sTNFR-p55 levels (P=0.19). Positive correlation was observed between sTNFR-p55 levels in the serum and in ascitic fluid of patients with HCC and LC (r=1.000, P<0.001). Logistic regression revealed that in patients with HCC, serum sTNFR-p55 levels were positively correlated with TBil and AFP in the peripheral blood (r=0.524, P=0.01 and r=0.234, P=0.03, respectively). CONCLUSIONS: Increased sTNFRs-p55 levels in the serum and ascitic fluid reflect abnormal immune status of the patients with HCC and help predict the development of tumor.

Transjugular intrahepatic portosystemic shunt for intractable posthepatectomy ascites.

We report two cases of transjugular intrahepatic portosystemic shunt for control of intractable ascites after resection of cirrhotic livers. The first case was a 46-year-old male who had undergone right lobectomy of the liver for a small hepatocellular carcinoma. His liver function had recovered within a week after the operation, but ascites drainage of 1-4 L/day persisted for more than a month despite vigorous medical therapy. We performed transjugular intrahepatic portosystemic shunt on the 49th postoperative day and the pressure gradient between the right atrium and the left portal vein was reduced to from 21 mmHg to 6 mmHg. Thereafter, ascites became responsive to diuretic therapy and was well controlled without complication. Second case of a 54-year-old male patient who had undergone left lateral segmentectomy due to a small hepatocellular carcinoma presented intractable ascites of 1-3 L/day, which was also effectively controlled after transjugular intrahepatic portosystemic shunt performed on the 34th postoperative day, though there was an episode of hepatic encephalopathy stage 1. Based on our limited experience, hepatectomized patients suffering from prolonged intractable ascites despite a favorable profile of liver function may be candidates for transjugular intrahepatic portosystemic shunt with an acceptable risk of hepatic failure and procedure-related complication.

[Clinical significance of detecting telomerase activity in ascites cells from patients with ovarian carcinoma].

OBJECTIVE: The purpose was to investigate the clinical significance of telomerase activity for tumor diagnosis in ascites cells from patients with ovarian carcinoma. METHODS: Thirty ascites specimens from patients with ovarian tumor were examined for telomerase activity by PCR-based TRAP (telomeric repeat amplification protocol) method, and the result of detection was compared with the cytological diagnosis. RESULTS: Telomerase activity was detected in ascites cells from patients with ovarian tumor in 19 of the 20 cytologic-possitive specimens and in 3 of the 7 cytologic-negative specimens and in all the 3 cytologic-suspicious specimens. The positive rate in 30 ascites samples by cytological examination and by the detection of telomerase activity was 66.67% (20/30), and 83.33% (25/30), respectively. CONCLUSIONS: The result suggested that measuring telomerase activity in ascites cells would be helpful to cytological examination in diagnosis of patients with ovarian carcinoma.

Preperitoneal collection after endoscopic extraperitoneal inguinal hernioplasty in a patient with malignant ascites.

Painful preperitoneal collection is a rare complication following endoscopic totally extraperitoneal inguinal hemioplasty. Here we present the case of a woman who underwent endoscopic extraperitoneal inguinal hernioplasty for a left inguinal hernia. Her past health was good. During the dissection of the extraperitoneal space, clear ascitic fluid was noted upon breaching the peritoneum near the round ligament. Endoscopic stapling was used to close the peritoneal tear, and the procedure was completed uneventfully. The patient complained of left iliac pain after the operation. A physical examination showed no swelling over the left iliac fossa. Contrast computed tomography of the abdomen revealed preperitoneal fluid collection over the hernioplasty site and a small amount of ascites. Expectant treatment with pain control by oral analgesics was adopted. A follow-up CT scan 4 months after the operation showed resolution of the preperitoneal fluid collection but with increased ascites. Abdominal paracentesis with peritoneal fluid for cytology analysis found adenocarcinoma cells. The patient succumbed to a terminal malignancy a year after surgery. Conversion of endoscopic extraperitoneal inguinal hernioplasty to open repair should be considered upon intraoperative discovery of ascites. Painful preperitoneal collection is a possible sequela following endoscopic extraperitoneal hernioplasty in patients with malignant ascites.

[Liver cirrhosis: pathogenesis of ascites and disturbance of renal function].

The accumulation of ascites in cirrhosis has been classically attributed to the transudation of fluid from the splanchnic vascular bed to the peritoneal cavity due to the presence of portal hypertension. This transudation of fluid would induce hypovolemia responsible for the renal abnormalities frequently observed in these patients. Recent studies, however, indicate that the pathophysiology of ascites in cirrhosis is far more complex. Cirrhotic patients with ascites usually have profound disturbances in systemic hemodynamics and renal function and an activation of the endogenous vasoactive systems that contribute to the formation of ascites. In the present article the pathogenesis of these disturbances and their possible contribution to the formation of ascites are discussed.
