Sterile endophthalmitis in vitrectomised eyes due to suspected heat resistant endotoxins in the infusion fluid.

PURPOSE: To report on to the possibility of development of severe postoperative sterile endophthalmitis due to heat-resistant bacterial endotoxins in commercially available infusion fluids METHODS: A case study of 4 eyes that had previously undergone vitreoretinal surgery, which developed clinical endophthalmitis within 18 hours of surgery and two eyes that had vitreous surgery with intraocular gas and did not develop clinical endophthalmitis following intraocular surgery on three consecutive operative days RESULTS: The vitreous samples were sterile, both for bacteria and fungi. The only common supply in all cases was a new batch of Ringer's lactate infusion fluid. Though the Ringer's lactate solutions in the same batch were also sterile, the infusion fluids contained abnormally high levels of bacterial endotoxins detected by gel clot method. CONCLUSION: Commercially available infusion fluid may be sterile, yet contain endotoxins from killed bacteria. This could cause postoperative sterile endophthalmitis.

[The discovery of a novel flower color mutation in male sterile rapeseed ( Brassica napus L. )].

The author found a novel yellow-white flower color mutation in the male sterile progenies derived from a commercial Brassica.napus hybrid C022, which was produced by a male sterility line 9012A, whose sterility was controlled by interaction between two pairs recessive genes and a pair of epitatic genes. The mutant was named 991S. 991S had three morphologic characters:(1) The color in the middle of every petal was yellow, yet the both sides were white.(2)Every calyx might become striped albino (3) Only male sterile plants in different populations had mutative character. Except flower color, they had similar morphologic characters to the normal male sterile sib-plants. The plants were slender and little with little flowers that had flat petals, bended stigmas, degenerative stamen, and dry anthers. Primary analysis of its origin and inheritance showed that this mutative character was controlled by recessive partial albino gene.

Presumed sterile endophthalmitis following intravitreal triamcinolone acetonide injection.

BACKGROUND AND OBJECTIVE: To report acute postoperative, presumed sterile endophthalmitis following intravitreal injection of triamcinolone acetonide (IVTA). PATIENTS AND METHODS: Retrospective, interventional, multicenter study of patients with acute sterile endophthalmitis following IVTA injection. RESULTS: A total of 922 IVTA injections were performed. Eight eyes of 8 patients with presumed sterile endophthalmitis were identified. The incidence of endophthalmitis was 0.87% (95% confidence interval, 0.38% to 1.70%). Median time to presentation was 1.5 days (range, 1 to 7 days). Median presenting visual acuity was 20/563 (range, 20/80 to light perception). Initial treatment included vitreous tap and injection of antibiotics (n = 4), pars plana vitrectomy and injection of intravitreal antibiotics (n = 2), or systemic treatment alone with oral levofloxacin (n = 2). Six of 6 intraocular cultures were sterile. Median follow-up was 5.9 months (range, 4 to 9 months) with a median visual acuity at last follow-up of 20/75 (range, 20/40 to counting fingers). CONCLUSIONS: Acute presumed sterile endophthalmitis following IVTA injection presents early in the postoperative period. Visual outcomes are generally good.

[Study on mechanism of infertility or sterility caused by uterine septa and reproductive prognosis after hysteroscopic metroplasty].

OBJECTIVE: To study the mechanism of infertility/sterility caused by uterine septa and the reproductive prognosis after transcervical resection of septum (TCRS). METHODS: One hundred and eleven patients with uterine septum including 34 patients of infertility and 77 patients of sterility were performed on TCRS. The patients were followed up of the reproductive outcomes. The process of pregnancies and deliveries were compared with those of 1548 normal uterine pregnancies. We also studied and compared the pathomorphologic and the micro-structure features between the septum and uterine wall in 30 cases (8 cases with infertility and 22 cases with sterility). RESULTS: (1) The amount of the endometrium glands on septum was less than that on the uterine wall or endometrium un-synchronous growth between septum and uterine wall was shown in 7 cases of infertility and 13 cases of sterility. The positive index level of estrogen and progestogen receptors on septum was weaker than that on uterine wall. The densities of the smooth muscle cells were thicker and the collagenous fibers were sparser in the base and middle of septum compared with uterine wall (P < 0.05). The amounts of the small arteries in septum were rarer than in uterine wall (P < 0.05). Ultrastructure characters showed glandular epithelium cells distributed irregularly and poor pili on septum in the sterility patients. (2) Sixteen infertility patients became pregnant (47.1%, 16/34) postoperation, which was much higher than pregnancy rate of preoperation (8.8%, 3/34, P < 0.01). In sterility group, the spontaneous abortion rate decreased from 84.1% (180/214) to 29.0% (18/62, P < 0.01), and the mature delivery rate and live birth rate rose from 3.3% (7/214), 0.9% (2/214) to 56.4% (35/62) and 64.5% (40/62) respectively (P < 0.01). (3) There were no differences between TCRS patients and normal uterine pregnant women in complications, and the mortality of perinatal (P > 0.05). The rate of cesarean section in post-TCRS patients was higher than in normal control group (P < 0.01). CONCLUSIONS: The differences of pathomorphology and ultra-structure between septum and uterine wall are the causation of infertility/sterility in the uterine septum patients. The reproductive prognosis can be improved remarkably by performing TCRS, which does not increase pregnant complications or affect fetus growth. Cesarean section should not be a standard way for delivery.

[The ultrastructural observation of anthers of Chinese cabbage's mail-sterility].

The fertile and sterile anthers of a Chinese cabbage (Brassica campestris L. ssp. chinensis Makino) were observed using electron microscope to find the ultrastructural feature of sterile anthers. The earliest abnormal phenomenon in sterile anther was nucleolus of sporogenous cells locating in the edge of nucleus. During microspore mother cell development, callus wall surrounding the cell displayed uneven in the thick ness and was discontinuous, and the some cytoplasm leaked out of the cell from some rifts in the wall. After meiosis of microspore mother cells, the cells of tetrad were irregular and some of them contained several nuclei. The exine of pollen began to be formed in tetrad in this cabbage. The evident disorder during exine formation in the sterile pollen occurred during its primexine formation and then the sporopollenin was irregularly deposited to form a layer of uneven and discontinuous pollen exine. Cytoplasm of aborting microspores contracted and finally degenerated after them released from tetrad. The tapetal cells of fertile anther began to synthesis abundant lipid material during microspore development. However, the tapetal cells of sterile anther did not synthesis lipid material during microspore aborting. The microspore abortion was first and tapetal degeneration second. Therefore, aborting microspore induced the functional default of tapetal cells synthesizing lipid material. The ultrastructural results on this study further complete and correct our previous results obtained by light microscope.

[Correlation of Ureaplasma urealyticum and Chlamydia trachomatis infections with male sterility: a meta-analysis of randomized control trials].

OBJECTIVE: To evaluate the correlation of Ureaplasma urealyticum (Uu) and Chlamydia trachomatis (Ct) infections with male sterility. METHODS: Data from CBMA and CNKI were searched and studies were made for the correlation of Uu and Ct infections with male sterility by retrieval strategy worked out according to the Collaborative Review Group search strategy. The results were expressed as odds ratio (OR) and 95% confidence intervals (95% CI). The results of 28 clinical controlled trials from 1994 to 2005 were analyzed by software RevMan 4.2. Odds ratio (OR) was applied to the evaluation of the correlation between Uu infection and male sterility. RESULTS: Eighty-eight relative trials were retrieved, of which 28 were included in the Meta-analysis. The combined ORs of Uu and Ct infections to male sterility were OR(Uu): 4.73 (95% CI: 3.77-5.94) and OR(Ct): 4.59 (95% CI: 3.24-6.50). CONCLUSION: Uu and Ct infections are very important risk factors of sterility in Chinese men.

[The importance of laparoscopy in the diagnosis of tubal sterility].

Analyzing the findings in diagnostic laparoscopy the authors describe in details and interpret the results from the examined organs in the pelvis and abdominal cavity. Object of the study are 200 sterile women, admitted to the clinic of Sterility and Gynecological Endocrinology of Research Institute of Obstetrics and Gynecology for examination. The mean age of the patients is 29 years as 128 of the women suffer from primary sterility, but 72--from secondary sterility. The results are summarized and interpreted in 5 groups. Uterine body--form, size, motility, measurements, adhesions, fibrillary apparatus, patency; Ovaries--form, size, position, motility, surface, follicles; Adhesions--type, dimensions, vascularization, position (in the pelvis and abdominal cavity); Data for endometriosis--presence, size and localization of endometriomas, endometriotic cysts and outgrowths, an increased vascular line. The inference, which the authors make, is that laparoscopy is basic (and necessary) current diagnostic method for obtaining real notion for the state of internal genital organs (especially for tubal factor in female sterility) and furthermore also for evaluation of the necessity and possibility for performance of microsurgical reproductive operations.

[An overview of psychogenic sterility].

The authors remind of the difficulty in defining psychogenic sterilities and evaluating their exact frequency. Then, they attempt to offer a classification based on clinical examples. Secondary sterilities of psychogenic origin are only seen in women who have lost a child in the pre, per or post-partum period. This is even a characteristic example of non organic sterility. Primary sterilities may be psychological in origin, especially by absence of maturation of the child planning project (case of sterilities cured after adoption). They are seen more infrequently: in adult anorexia nervosa, psychological sexual differentiation disorders, masculine women. These difficulties are not easy to approach as much as psychological and organic factors may be entangled. In addition, one question is raised: who should take care of it? The psychiatrist or the gynecologist? Overall, the latter is preferable under the condition that he/she is properly trained. Anyway, it is ultimately the woman's choice.

[Role of Chlamydia trachomatis in tubal pathology (acute salpingitis and tubal sterility). Microbiological study of 175 samples of peritoneal fluid].

The study was carried out on 175 patients who underwent laparoscopy together with search for chlamydia in the peritoneal fluid when they were having investigations for sterility or for pelvic pain. These patients are classified into three groups according to the clinical and laparoscopic features: 50 cases of acute salpingitis. 104 cases of tubal sterility and 21 control cases who were normal on laparoscopy. The mean age was 25.7 years for acute salpingitis and 30.4 years for tubal sterility. The purpose of this study was to work out the role played by chlamydia trachomatis in tubal phatology and two techniques were used at the same time: The identification of chlamydia trachomatis in 175 samples of peritoneal fluid taken during a laparoscopy; Research for anti-chlamydia antibodies in serum using an indirect micro-immunofluorescent technique. Cell cultures were performed after the peritoneal fluid had been centrifuged. It had been in transport medium 2 SP and frozen. Our technical methods for isolating chlamydia (microplaque culture on Hela lines, incubation in the presence of cycloheximide and research for chlamydial inclusion bodies using direct immunofluorescence with monoclonal antibodies and 2 successive passages of the cells) have allowed us to confirm that chlamydia was present in 44% of cases of acute salpingitis and in 37% of cases of tubal sterility. The search for anti-chlamydial antibodies in the serum in indirect immunofluorescence with a single antigen (of L2 serotype) gave positive results which were comparable to those in other studies, i.e. 50% of positive serological results in acute salpingitis and 63% in tubal sterility.(ABSTRACT TRUNCATED AT 250 WORDS)

Effect of luteinizing hormone-releasing hormone on ovulation, mating and conception in androgen-sterilized rats.

Attempts were made to induce pregnancy in androgen-sterilized rats. Because it is known that the preovulatory LH surge is absent in androgen-sterilized rats, a classical approach was taken to circumvent the probable deficit in cyclic release of LH by giving a subcutaneous injection of LH-RH (400 ng) every four days. Cyclic injection of LH-RH induced a regular estrus cycle in the greater part of treated females, whereas a saline-treated control group showed a persistent estrous smear. The number of eggs recovered and the mating rate increased with subsequent treatment with LH-RH, indicating a priming effect by the initial injection. However, when mated animals were autopsied on day 10 (sperm = day 1), no pregnancies had occurred. When a pituitary gland was transplanted from a normal rat into the kidney capsule of an androgen-sterilized rat in order to maintain functional corpora lutea, implantation occurred in 38% of the group of females treated with five separate LH-RH injections. Endometrial trauma in androgen-sterilized rats elicited a better response in the group which received multiple LH-RH injections than the group which received a single injection. These results indicate that the cyclic LH surge may be a necessary factor for induction of pregnancy in androgen-sterilized rats.

Infection rates in sterile males of morsitans, palpalis and fusca groups Glossina for pathogenic Trypanosoma species from East and West Africa.

Infection rates in sterile male Glossina morsitans centralis, G. austeni, G. palpalis palpalis, G.p. gambiensis, G. fuscipes fuscipes, G. tachinoides and G. brevipalpis for Trypanosoma vivax, T. congolense and T. brucei isolated from East and West Africa, were studied. Five groups of the sterile males, together with the five groups of sexually fertile males, of each of the respective species and subspecies were allowed to feed for 24 days on a Boran calf or goats infected with T. vivax or T. congolense, or with T. brucei for 34 days, after which they were dissected. The results showed that the infection of the pathogenic Trypanosoma species became better established in some tsetse species than in others. Also, the infection rates of T. vivax, T. congolense and T. brucei for sterile and sexually fertile males of any of the above Glossina did not differ significantly. These results indicate that releases of sterile male tsetse in the tsetse control programme will potentially increase the risk of trypanosomiasis during the period of tsetse releases in the affected areas, unless in the areas with low tsetse density the sterile male tsetse are rendered refractory to trypanosome infection prior to their releases while in the areas with medium to high tsetse densities, the resident tsetse populations are initially reduced with insecticides, traps and/or targets.

[Epidemiology of tubal sterility in Morocco. Apropos of 386 cases].

In Morocco 11.7% of couples are sterile. Even after 3 years the figure is as high as 6%. We have studied in this work 386 cases of tubal sterility which constitute 58.22% of 663 couples who were fully investigated. The mean age of the patients was 29 years. Patients come for assessment late-75% of them after 5 years of sterility. Most of these (54.15%) suffer from primary sterility due to infections acquired before marriage. One-third of all patients have been married more than once, having been divorced because of their sterility. Although tuberculosis is still very frequent in Morocco, not many of our cases were suffering from pelvic tuberculosis. 11% of the patients had had pelvic surgery.

[Incidence and pregnancy rates in various causes of sterility].

A precise diagnostic procedure could be done in 1,450 sterile couples in the years 1975 to 1982. The prevailing cause of sterility was female in 67 per cent, male in 22 per cent and both female and male in 11 per cent. Pregnancy rate over all was 35 per cent and after therapy 45 per cent. There was a distinct correlation between pregnancy rate, age of the patient and duration of desire for child. 73 per cent of all conceptions occurred in female sterility; 77 per cent of it in ovarian sterility. Simple therapeutic methods like gestagens or clomiphene and conversational therapy achieved the highest pregnancy rate. Only a little proportion of the pregnancy rate could be obtained by highly specialized measurements like FSH/HCG (7 per cent) or operative interventions (8 per cent). In cases of male reasons only 8 per cent of all pregnancies could be attained. 62 per cent of all pregnancies supervised were live births. Because highly specialized methods are not able to influence the rate of conceptions over all embryo transfer will be more and more the alternative method in cases of sterile couples treated without success till now.

[Hysterosalpingography in sterility].

From 1967 to 1975 the authors performed 500 hysterosalpingographies in 851 women examined for sterility. Hysterosalpingography was carried out under the diascopic control of an electronic amplifier and a tele-monitor. As a contrast medium only the water-soluble Joduron U-S (Cilag) was used. In 286 women (57%) primary sterility and 214 (43%) secondary sterility were established. Radiographic analysis revealed 209 anomalies of the uterus, 114 of which related to hypoplastic uterus. In 286 women with primary sterility there was occlusion of one or both tubes in 62 cases (21.67%) and the sactosalpinx of one or both tubes in 48 cases (16.78%). In 214 women with secondary sterility, there was occlusion of one or both tubes in 66 cases (31.30%) and the sactosalpinx of one or both tubes in 48 cases (22.42%). In 500 hysterosalpingographies the intravasation of the contrast medium into the venous system occurred in 23 cases (4.60%) but there was not a single case of unatoward side-effects. Sixty-six women became gravid 1--6 months after hysterosalpingography, without the use any other kind of therapy.

Chemosterilization of Culex pipiens fatigans Wiedemann by exposure of aquatic stages. 2. Sterilization potential of certain phosphoramides and s-triazines.

Many aziridinyl compounds are known to induce high sterility in Culex pipiens fatigans Wiedemann, but as the practical application of these chemosterilants is quite hazardous, compounds such as phosphoramides and s-triazines have been tried against this species. These compounds are relatively less reactive, thermally more stable and less toxic to mammals. The phosphoramides included hempa, N,N,N',N'-tetramethyl-P-piperidino-phosphonic diamide (ENT-51007) and a compound, ENT-60210, whose structure is not known to the authors. The s-triazines employed were hemel, 2,4-diamino-6-morpholino-s-triazine hydrochloride (ENT-51143), 2-amino-4,6-bis (dimethylamino)-s-triazine hydrochloride (ENT-51146), and a compound ENT-60433, whose structure is not known to the authors. The triazines were more toxic than the phosphoramides in both larval and pupal treatments. Among the phosphoramides, hempa and ENT-51007 were quite promising for larval treatment and resulted in 80% and 83% control of reproduction respectively at non-toxic doses. Hempa at a toxic dose induced complete sterility. ENT-60210 was least toxic and least effective. Among the triazines, hexa-substituted hemel was better than tetra-substituted ENT-51146 and cyclic-substituted ENT-51143 in inducing sterility. ENT-51143 was most toxic and least effective. For larval treatment, hempa was better than hemel in inducing sterility while for pupal treatment the latter was better than hempa. Both the compounds produced more sterility in treated females than in treated males. Oviposition was significantly lowered in treated females.

Blood group incompatibility and cervical hostility in relation to sterility.

Fifty-two sterile couples, characterized by persistent and unexplained incompatibility between spermatozoa and cervical mucus, without any other evident factor to explain their sterility, were investigated for the possible role of blood group incompatibility as a cause of their sterility. After a study of the distribution of blood groups in sterile and fertile couples, the distribution of blood group substances in pairs of husbands and wives, and the effect of blood group antibodies on sperm migration, it seems highly unlikely that blood group incompatibility contributes significantly to cervical hostility and the resulting sterility.

[A microbiological study of swabs taken laparoscopically in cases of salpingitis and tubal sterility. Research for Chlamydia trachomatis and for mycoplasmas (author's transl)].

Research was made for chlamydia trachomatis and ureaplasma urealyticum in the peritoneum and the tubes of 99 women divided into 4 groups: 17 of them were being investigated because of acute salpingitis (Group A), 17 were being investigated for tubal sterility with chronic inflammation diagnosed laparoscopically (Group B), 29 were being investigated for tubal sterility without any laparoscopic evidence of inflammation (Group C) and 36 women had absolutely normal pelves and were being investigated for sterility. These were the control group (D). Swabs were also taken from the lower genital tracts as well as serological tests for chlamydia trachomatis and cytological samplings of the fluid from the Pouch of Douglas and the histology of the tubes. In the 17 women who had acute salpingitis the swabs 4 cases of C.T. and 4 of U.U. In the 46 women who had tubal sterility the laparoscopic swabs showed cases of C.T. and 7 of U.U. The swabs were most often positive in Group B. This group is characterised by a special appearence of the inflammation, with fluid present and viscous adhesions as well as peritoneal inflammatory cysts. These altogether help to make a presumptive diagnosis of C.T. infection on laparoscopy. In the control group of 36 cases there was no sign of C.T. in any case, although 2 swabs from the peritoneum showed U.U. So there is a statistically significant difference between the groups that were suspicious and the control group whether the results were obtained by cultures or by serological diagnosis. On the other hand there is no definitive difference as far as U.U. is concerned. These observations, which are similar to those published by other authors, lead us to think that micro-organisms and especially chlamydia trachomatis could be the bacteriological agent responsible for chronic inflammatory states found so frequently in women with tubal sterility.

Stability of sterile dexamethasone acetate suspensions and dexamethasone sodium phosphate injections submitted by U.S. hospitals.

The stability of sterile dexamethasone acetate suspensions and dexamethasone sodium phosphate injections that had been stored in hospital pharmacies across the United States was studied. Through a voluntary FDA drug stability program, all hospital pharmacies in the United States were asked in October 1981 to complete a response card indicating information about the sterile dexamethasone acetate suspensions and dexamethasone sodium phosphate injections they had in stock. Based on the responses, FDA selected 21 samples of sterile dexamethasone acetate suspensions (representing two manufacturers) and 114 samples of dexamethasone sodium phosphate injection (representing 11 manufacturers). These samples were analyzed for identification, pH, and strength. All samples of sterile dexamethasone acetate suspension met USP requirements. Eleven samples of dexamethasone sodium phosphate injection representing 10 lots from three manufacturers failed USP assay requirements for strength. All samples that failed to meet strength requirements showed evidence of degradation by oxidation. Sterile dexamethasone acetate suspensions appear to be stable when stored under actual marketplace conditions, but there is a problem with the shelf-life stability of dexamethasone sodium phosphate injections made by some manufacturers.
