Reliability of the Roland - Morris Disability Questionnaire (Thai version) for the evaluation of low back pain patients.

OBJECTIVE: To determine the reliability of the Thai version of the Roland - Morris disability questionnaire in low back pain patients. MATERIAL AND METHOD: To develop Thai version of the Roland - Morris disability questionnaire and test it in 120 low back pain patients. Reliability of the Thai version of the Roland - Morris disability questionnaire was assessed by internal consistency using Cronbach's alpha coefficient. RESULTS: The overall Cronbach's alpha coefficient of the scale was 0.83. The Cronbach's alpha coefficient of each question in the Thai version of the Roland - Morris disability questionnaire exceeded 0.7 (range, 0.71- 0.93). The Cronbach's alpha coefficient tested in acute or chronic low back pain patients whether they have back pain only or back pain with radiculopathy which also exceeded 0.7 (range, 0.83-0.87). CONCLUSION: The Thai version of the Roland - Morris disability questionnaire is a reliable tool for assessing functional disability of low back pain in Thai patients.

[Validation of the SCOFF questionnaire for screening of eating disorders in university women].

BACKGROUND: The prevalence of eating disorders has been steadily increasing in recent years. This has led to a growing sense that instruments are necessary for early detection in high risk populations. OBJECTIVE: Herein the SCOFF (sick, control, one, fat, food) questionnaire was examined for its internal consistency, reliability, and criterion validity in screening eating disorders among university women. MATERIALS AND METHODS: A sample of 385 students was selected at random from six schools in the Universidad Autonóma de Bucaramanga, Colombia, and were evaluated independently with the SCOFF questionnaire and a CIDI (Composite International Diagnostic Interview). RESULTS: A total of 149 (38.7%) students were positive for eating disorder syndromes according to the SCOFF questionnaire, whereas 82 (21.2%) students met eating disorder using the CIDI. SCOFF questionnaire sensitivity was 78.4% (95%CI 64-88%), specificity 75.8% (95%CI 69-82%), ROC curve area 0.823 (95%CI 0.76-0.89), Cronbach's alpha 0.480, Cohen's kappa 0.433 (95%CI 0.32-0.55), and test-retest concordance 91.6% (95%CI 86-96). CONCLUSION: These data indicate the SCOFF questionnaire to be a good scale in screening for eating disorders among university women. Its added advantages are that the test is simple and brief.

Chronic venous disease: health status of a population and care impact on this health status through quality of life questionnaires.

AIM: There is growing interest for patient-reported outcomes (PROs) in clinical research. Application of quality of life (QoL) questionnaires to patients suffering from chronic venous disease (CVD) is thought to be an important improvement in the assessment of results in these complex disorders. The aims of the study were to describe the health status of female patients suffering from CVD, and to assess the care impact through QoL questionnaires. METHODS: Symptomatic female patients suffering from CVD (all CEAP clinical classes), aged over 18, newly treated by their general practitioner with a phlebotropic drug were enrolled in the study. Every patient had to complete a self-questionnaire including the SF-12, the CES-D, the Epworth and the CIVIQ scales at day 0 (D0), day 3 (D3) and day 7 (D7). The CIVIQ scale assesses pain and the psychological, physical and social consequences of CVD. The SF-12 is a generic measure of health status. The CES-D scale studies presence of depressive symptoms (DS) in the general population. The Epworth scale measures daytime sleepiness. RESULTS: A total of 765 physicians took part in the survey from April to August 2003. The results of the study concern 1 045 patients. Analyses are realized on the 399 patients who filled out all 3 questionnaires. Mean age was 45 years (SD=11), patients average height was 164.39 cm (SD=5.99) for an average weight of 65.2 kg (SD=12.5). Concerning change in QoL between inclusion and Day 7, the mean CIVIQ total scores at inclusion were 26.7 in the CEAP class 0-2 group and 36.3 in the CEAP class 3-6 group. The mean CIVIQ total scores at D7 were 18.5 in the CEAP class 0-2 group and 27.3 in the CEAP class 3-6 group. The mean CIVIQ total scores demonstrated that QoL is more impaired for patients in the CEAP class 3-6 group. In both groups, an improvement in patients' QoL was observed following patient management (P<0.001). At inclusion time, MCS-12 and PCS-12 were respectively 44.7 (SD=10.6) and 46.4 (SD=8.4): at day 3 and day 7, these dimensions were respectively: D3: 46.5 (SD=10.2) and 46.2 (SD=8) D7: 48 (SD=10.3) and 46.2 (SD=7.8). For the mental dimension, the difference was statistically significant (P=0.0001). In our study a possible DS (score ?17) was reported for 36.3%, 32.3% and 29% respectively at day 0, D3 and D7 (P<0.01, n=328). Concerning daytime sleepiness the Epworth mean total score showed a decrease in daytime sleepiness for patients with severe CVD (C3-C6) between day 0 and D 7 (P=0.02). CONCLUSIONS: Our study demonstrates the high impact of CVD on health status and the validity of QoL questionnaires in this pathology. They show a significant improvement of the QoL after one week of medical management.

[Questionnaire for the mobbing risk: CDL2.0].

The aim of the study is to develop and validate a questionnaire able to evaluate the risk of mobbing at the workplace. A multiple-choice questionnaire has been developed which contains, among the different items, only one revealing a mobbing situation. The questionnaire has been administered to two groups (group A--243 subjects in a mobbing situation and group B--63 subjects without exposure to mobbing) and the differences in the scores obtained have been analysed. The questionnaire has proved to be valid and reliable. The results show that the presence of five mobbing actions is sufficient to define the workplace situation as potentially at risk for mobbing. The study reveals some limits in the selection of the two samples thus needing some adjustment. However, the questionnaire, also in the present form, can be considered a tool able to detect the mobbing situations.

Self-report health questionnaire: a necessary and reliable tool in dentistry.

This study was designed to assess whether the number and type of medical conditions elicited by self-report on a questionnaire were equivalent to those obtained through verbal inquiry by blinded trained dentists. The study sample consisted of 100 adult patients who were seeking treatment from the University of Kentucky Orofacial Pain Center. Evaluations occurred between September 2003 and January 2004. Patients completed a standardized medical health questionnaire containing 92 questions. Subsequently, the patients were questioned verbally about their medical history by uniformly trained dentists, using a systems review approach as part of routine clinical protocol. The medical histories obtained by both methods were statistically analyzed using Kappa values, paired and independent sample t-tests. The nature of the questionnaire did not distinguish between past and present conditions, and did not give information regarding onset, severity, duration, and impact of the marked conditions. It was concluded that even though a self-reported health questionnaire is precise and consistent, more detailed information can be obtained by verifying marked medical conditions through a verbal inquiry. Every history and examination should include a combination of both a self-reported medical health questionnaire and a verbal inquiry to aid in diagnosis and treatment.

Testing of the questionnaire on dental care satisfaction in a sample of adult patients visiting dental clinics at Faculty of Odontology, Kaunas University of Medicine. A pilot study.

UNLABELLED: A pilot study was conducted with the aim to test and develop a questionnaire which covers dental patients attendance, satisfaction with dental care, oral health self-evaluation, attitudes and knowledge about oral health, before applying it to a broad population. A total of 53 adult subjects (mean age 40.1) visiting Kaunas University of Medicine dental clinics, filled in the questionnaire presented by the researchers. Statement choices to define dental care satisfaction were selected from previously used instruments for similar studies in other countries. RESULTS: Major difficulties in answering questions were related to using the Likert scale response alternatives. All statements concerning dental care satisfaction with the mean Likert score value below 2.0 were decided to be indicative for the importance of characteristics. According to factor analysis and distribution of the mean scores of dental care satisfaction characteristics it was decided to include no more than 8 questions in each dimension to the final version of questionnaire. In case of equal loadings of several statements in the factor analysis, the statement with lower mean score was used. CONCLUSIONS: The use of professional terminology in questionnaire survey should be tested before applying to a broad population. The final instrument for measuring dental care satisfaction should be formed on the basis of statistical evaluation of relative importance and comparative load of different questions/statements to be included in the questionnaire.

Critical comparison of nine different self-administered questionnaires for the evaluation of disability caused by low back pain.

AIM: The aim of this study was to critically compare 9 self-administered questionnaires designed to evaluate disability caused by back pain. METHODS: The questionnaires were analyzed considering and comparing their psychometric characteristics (reliability, validity, responsiveness, minimal clinically relevant difference), together with other practical and technical aspects (number of items, number and kind of domains, scaling of items, scoring, time to complete, validated translations, etc.). Data were obtained from scientific literature. RESULTS: Only 3 out of 9 analyzed instruments (i.e., the Oswestry Low Back Pain Disability Questionnaire, the Quebec Back Pain Disability Scale, and the Roland-Morris Low Back Pain Disability Questionnaire) appear fully validated from a psychometric standpoint. CONCLUSIONS: On the basis of psychometric evaluations as well as feasibility considerations, the authors suggest using either the Roland-Morris or Oswestry questionnaire as the best assessment of the level of disability caused by back pain.

Adolescent health screening questionnaire.

Adolescent screening questionnaire is a set of questions made on determinants of adolescent health and used to screen adolescents exposed to health risk environment to offer preventive and promotional health care . Most evidence based questionnaire (GAPS) was developed by American Medical Association. Subsequently American Academy of Pediatrics (AAP) and Bright Future (BF) developed questionnaire. All of them were developed to serve American and Western society needs. Format of Adolescent Preventive Services (FAPS) questionnaire was prepared by Adolescent Health Clinic of Calcutta Medical College to suit Indian adolescents, having a different cultural background, with technical support from WHO. National consultations on this questionnaire was supported by WHO and GOI. It was field tested and its validity and reliability were assessed among adolescents of six school students. Use of this tool helps identification of adolescents exposed to high-risk environments and use of necessary early intervention to prevent morbidity and mortality in adolescents. The available software version of FAPS can be used in the computer class to screen all school adolescents regularly.

[Effect of social desirability on dietary intake estimated from a food questionnaire].

INTRODUCTION: Self-report of dietary intake could be biased by social thus affecting risk estimates in epidemiological studies. The objective of study was to assess the effect of social desirability on dietary intake from a food frequency questionnaire (FFQ). METHODS: A convenience sample of 483 Portuguese university students was recruited. Subjects were invited to complete a two-part self-administered questionnaire: the first part included the Marlowe-Crowne Social Desirability Scale (M-CSDS), a physical activity questionnaire and self-reported height and weight; the second part, included a semi-quantitative FFQ validated for Portuguese adults, that should be returned after fulfillment. All subjects completed the first part of the questionnaire and 40.4% returned the FFQ fairly completed. RESULTS: In multiple regression analysis, after adjustment for energy and confounders, social desirability produced a significant positive effect in the estimates of dietary fibre, vitamin C, vitamin E, magnesium and potassium, in both genders. In multiple regression, after adjustment for energy and confounders, social desirability had a significant positive effect in the estimates of vegetable consumption, for both genders, and a negative effect in white bread and beer, for women. CONCLUSION: Social desirability affected nutritional and food intake estimated from a food frequency questionnaire.

[A burden questionnaire for caregivers of peritoneal dialysis patients].

INTRODUCTION: Despite the interest generated by the increasing number of studies that measure Quality of Life among patients and caregivers, few of these studies measure the caregivers burden in Peritoneal Dialysis (PD). OBJECTIVE: The main target of this study was to create a burden measure questionnaire applicable amongst caregivers of PD patients. METHODOLOGY INCLUSION CRITERIA: 1) Patients had to be in PD treatment for more than 3 months; 2) Patients had to receive help with the PD treatment from a caregiver. The study was divided into 3 phases: 1st) design and use of the initial questionnaire; 2nd) a test-retest on a modified scale; and 3rd) to provide the questionnaire-3 to two collaborative centres with similar PD programs. Four groups of caregivers were established: A1:23, A2:17, B:7 and C:16 caregivers. We applied 5 scales (5): 1--Patient Dependence on caregiver, from caregivers' view (D); 2--Complete caregiver burden (CB), including 12 items which measure the caregivers' subjective burden, 3--Reduced caregiver burden (RB), as the one before but with only 8 items, 4--Repercussions on the caregiver (R), which measures objective burden; 5--Specific PD tasks (ST), a scale that measures the effort the tasks implied in the PD treatment represent for the caregiver. RESULTS: We studied 63 caregivers (table I): mean age: 53.43 (SD = 12.3); Sex: Females: 86.4%, Males: 13.6%, corresponding to 63 patients: mean age: 59.79 (SD = 15.9); Sex: Males: 80.3%, Females: 19.7%. Valuable results for reliability, unidimensionality, and discrimination were obtained in the 1st and 2nd phases, except for burden scale which was compound of two factors; then one of those factors was suppressed. In the 3rd phase, ANOVA did not show any differences between centres (table II). Consequently, all caregivers could be analysed together. Reliability results for each one of the third phase scales (table III) were: D: Cronbach alpha = 0,886; CB: alpha = 0,894; RB: alpha = 0,857; R: alpha = 0,892; ST: alpha = 0,62. Although the ST scale obtained an acceptable reliability, it was suppressed in the 3rd phase due to the low correlation with other scales and the fact that it was not applicable to all caregivers. Finally, a direct correlation was found between third phase scales (table IV): D-RB: r = 0.502, p < or = 0.001; D-R: r = 0.599, p < or = 0.001; RB-R: r = 0.775, p < or = 0.001. We must headlight that both Burden scales, and the Repercussion scale, obtained a direct correlation with the Dependency scale. CONCLUSION: A questionnaire has been created to measure burden and repercussions on caregivers of peritoneal dialysis patients. It can already be applied, as requirements of both reliability and validity are fulfilled. This questionnaire can be a useful tool to prevent caregivers' burnout.

Validation of the Thai version of a screening questionnaire for detection of systemic lupus erythematosus.

OBJECTIVE: The present study was performed to assess the sensitivity and specificity of the translation questionnaire for Systemic Lupus Erythematosus (SLE) in Thai speaking populations. MATERIAL AND METHOD: The 10- item questionnaire was applied to out-patients at the rheumatology clinic of the Chiang Mai University Hospital. One hundred and thirty-nine SLE, 109 Rheumatoid Arthritis (RA), and 35 Scleroderma (Scl) patients, as well as 88 Healthy Controls (HC) were enrolled into the present study. RESULTS: All subjects completed the questionnaire within 2 minutes. A positive response to three or more questions of the questionnaire gave a sensitivity and specificity of 92.81% and 76. 39%, respectively, and was comparable to the original version. CONCLUSION: This Thai-version of the screening questionnaire should be applied in the general population to determine the prevalence of SLE.

[Study of the kid-kINDL questionnaire scores for children with developmental disorders in normal classes and their parents].

The KINDL-questionnaire is able to measure degrees of health and adaptability in relation to quality of life (QOL) in children. The questionnaire can be completed by children, adolescents, and their parents (parent-version). We had translated Kid-KINDL questionnaire into Japanese as Questionnaire for Measuring QOL in Japanese Elementary School Children, and reported their reliability and validity. This study investigates Japanese elementary school version of the Kid-KINDL questionnaire scores consisting of 6 dimensions (4 items each;total score, 100) for children with developmental disorders without mental retardation including high-functioned pervasive developmental disorder, attention deficit/hyperactivity disorder and learning disorder, and the parent-version scores for their mothers. Twenty individuals in normal class and their mothers participated after informed consents were obtained. Their total QOL scores were significantly lower than those in control group. Four of 6 dimensions consisting of emotional well-being, self-esteem, family, friends and school had significantly lower points. Their mothers' points in total QOL scores and all dimensions except for family dimension were significantly lower than those in control group. In comparison of scores between children with mild developmental disorders and their mothers, children estimated lower in self-esteem and family dimensions instead mother estimated lower in health, emotional well-being and school dimensions. There was no difference in total scores between them. Children with mild developmental disorders and their mothers estimate lower points of QOL scores than those of the normal control group. There are different perceptions in 5 of 6 dimensions between the children and their mothers.

[Dépistage de l'épilepsie en zones tropicales: validation d'un questionnaire en Mauritanie].

Prevalence of epilepsy in developing countries is considered to be 2 to 5 times superior to the industrialized countries. In tropical areas, the diagnosis of epilepsy is still reliant on the clinic and requires a precise semiological description. A good epidemiological and clinical research requires standardized and validated screening tools. The objective of this work was to evaluate the validity of the screening module of the investigation questionnaire on epilepsy in tropical areas, in Nouakchott, Mauritania. All the patients included in the study were examined at first by a neurologist, then directed towards an investigator who asked the 5 screening items of the investigation questionnaire. Any person answering "yes" at least one question was regarded as "suspected of epilepsy" by the investigator. Secondly the neurologist re-examined the subjects and confirmed the diagnosis. The sensitivity and the specificity of the module of screening were calculated. On the whole, 236 patients were included in the study 131 had answered "yes" at least one of the questions of the module of screening and was regarded as "suspected" of epilepsy (55.5%) by the questionnaire. The diagnosis of epilepsy was made for 82 subjects by the neurologist. The sensitivity of the module of screening of the epilepsy was 95.1% (CI 95%: 87.3-98.4) and specificity was 65.6% (CI 95%: 57.5-72.9). The screening module of the investigation questionnaire of epilepsy in tropical zones, made up only by 5 questions, has diagnosis qualities acceptable and comparable with those reported in the literature. The use of this tool integrated in a more general questionnaire of investigation of the. epilepsy proves to be an essential instrument in conducting epidemiologic studies in epileptology' in tropical zones.

[Development and testing of a semi-quantitative food frequency questionnaire for Bolivian adolescents].

INTRODUCTION: Since previous studies suggest that the dietary patterns and lifestyle in adolescence have an impact in health later in life, it is pertinent to monitor and document their dietary patterns. OBJECTIVE: To develop and test the reliability and validity of a semi-quantitative food frequency questionnaire for Bolivian adolescents. SETTING: La Paz, September - October 2004. METHODOLOGY: A convenience sample of 82 students from a secondary participated in the study. A semi quantitative food frequency questionnaire was applied twice with one month interval (Q1 and Q2). A 3 days dietary record (3R) was obtained from 32 of the Students as standard. RESULTS: Most of the questions (87%) reporting intake of each individual food item agreed moderately to substantially while only 2 individual food items fell into the category of slight agreement and 11 into the category of fair agreement using the kappa statistic. Quantities obtained from both applications of the questionnaire yielded similar results (P > 0.05). Nutrient estimates obtained from the Q1 and Q2 and the 3R were not statistically different (P > 0.05). CONCLUSION: The semi-quantitative food frequency questionnaire is apparently reliable and valid for the measurement of absolute food intake in Bolivian adolescents. Care will be taken in the future to prevent overestimation of intake measured by this particular questionnaire.

[Evaluation of a questionnaire for measuring individual environmental exposure to disinfection byproducts found in drinking water].

Epidemiological studies to evaluate the association between environmental exposure to risk factors and negative health effects often use population level aggregated data to measure exposure, but do not consider personal characteristics that may affect the degree of exposure at the individual level. The aim of this study was to evaluate the validity and reproducibility of a questionnaire specifically designed to measure individual oral, inhalation and transdermal exposure to disinfection byproducts in drinking water. Reproducibility of the questionnaire was evaluated by administering the questionnaire twice, in different time periods, to the same subjects. On the other hand, validity was tested by comparing responses to the questions with information contained in diaries filled out by the subjects daily over a period of one week (gold standard). Questionnaire reproducibilty and validity were both found to be very good, as shown by high Spearman's correlaton coefficients, Intraclass correlation coefficients (ICC), kappa concordance coefficient, sensibility and specificity. The use of a questionnaire such as the one evaluated in this study may be a valuable aid when conductin epidemiological studies to assess individual environmental exposure to risk factors found in drinking water and to explain the effect of such factors on human health.

Evaluation of a questionnaire used in dermatological epidemiology. Discrepancy between self-reported symptoms and objective signs.

A questionnaire about facial skin problems was answered by 3745 office employees with the aim of discovering whether VDU work causes skin disease. From this group, 809 randomly selected persons were examined and interviewed by a dermatologist. There was a relatively good correspondence (87%) between the results from the interviews and the questionnaires, but the questionnaire results corresponded to current status in only 46% of the cases. No important difference was seen between those who were informed and those who were not informed about the purpose of the investigation. There was a relatively wide variation in the correlation between two responses to the same questions. In conclusion, the self-administered questionnaire seems to give reasonably adequate answers regarding the occurrence of more clear-cut symptoms over a period of time, rather than answers regarding the presence of signs at a particular point in time. However, less marked changes might not be picked up particularly well and the use of questionnaires alone cannot be recommended when studying mild skin problems due to a particular exposure in a population.

Low back pain--correspondence between questionnaire, interview and clinical examination.

The aim of this study was to analyse the correspondence between answers to a questionnaire about trouble from the musculoskeletal system, answers in a personal interview and clinical findings indicating low back disorder. The questionnaire was answered by 1,773 construction workers. Out of those, 206 workers underwent interview and clinical examination. Among those who reported no lifetime LBT in the questionnaire 63% gave the same report in the personal interview and those were all assessed negative at a blind clinical examination. Of those reporting current LBT in the interview 80% (47/59) were clinically positive. The clinical criteria used in the examination seemed to indicate lumbar painful structures. Answers to a question about functional impairment were in conformity with clinical findings. As regards answers to a question about frequency of pain and a question, in the specific Nordic questionnaire for the low back, concerning inability to do normal work the correlation to clinical findings was less apparent. Reported inability to do normal work in the questionnaire corresponded only to 43% with reported sick-leave in the interview.

[Assessment of type A personality by using a Polish questionnaire].

Friedman's and Rosenman's Structured Interview and Jenkins' questionnaire were not suitable for the studied population and it is the reason of the preparation of the Polish Questionnaire for the Assessment Type A Behavior Pattern for adults (ATAB Questionnaire, Form B). Content and formal aspects of Type A behavior pattern (TABP) were taking into consideration at the development of ATAB Questionnaire. It contains 22 items, which have a good discriminative power. Reliability of ATAB Questionnaire was assessed on two ways: by absolute stability and internal consistency. Validity was assessed by concurrent validity. The results of reliability and validity studies are satisfactory. Percentile ranks are prepared on the basis of 1438 females and 1211 males scores.
