Preliminary studies on the relationship between Ixodes ricinus activity and tick-borne infection among occupationally-exposed inhabitants of eastern Poland.

Relative density (activity) of the tick Ixodes ricinus was determined in five districts of the Lublin region (eastern Poland) by vegetation flagging. Tick activity values were compared to the determined by ELISA tests seroprevalence to tick-borne encephalitis virus (TBEV) and Borrelia burgdorferi in forestry workers and farmers from the same areas. A significant correlation between tick activity and seroprevalence to both TBEV and B. burgdorferi antigen was found in farmers and in the total examined population.

Energy and nutrients intake of elderly people living in the Warsaw region, Poland.

The dietary intake of energy, macronutrients, minerals and vitamins was studied in a group of elderly living in urban, suburban, and rural areas of Warsaw region, Poland. The study was conducted in spring 1999 and 298 persons, born between 1919-1924, randomly selected with criteria of age and place of living, were included. Dietary data were based on the 3-day food records. The energy and nutrients intake was calculated on the basis of tables of nutritive value of food products and compared to the Polish RDA's. Daily food rations of the subjects (except for men living in Warsaw) had an incorrect structure as well as insufficient energy content. The highest (i.e. almost 60% or higher) prevalence of intake lower than 2/3 of Polish RDA's was observed for calcium, magnesium, copper, vitamin B2 and vitamin C as well as for iron and vitamin B1 among women. The average intake of fat, cholesterol, and phosphorus was above recommended level. The differences related to area of living were less pronounced for men than for women. Women from suburban area were the most vulnerable group in respect to nutritional inadequacy.

[An epidemiologic survey of multiple sclerosis in the Szczecin province in Poland].

The possibility of a focus of the province of Szczecin (Northwestern part of Poland) was first noticed in our survey covering the period of 1960-86. Starting from 1986 the incidence of MS in this region was closely followed. Seven communes with an area of 1202 km2 and 45,231 inhabitants, had 50 MS patients on Dec. 31st, 1995 and a prevalence rate of 110,54/100,000. This is twice the prevalence more than in surrounding area (52,89/100,000). New cases appeared irregularly, with four peaks in 1960, 1967-8, 1976-7 and 1981-2. The age at onset, duration of disease, life expectancy and natural history of the disease in the focus and rest of Szczecin region were similar. We conclude, that clustering seems to be due to the influence of environmental factors, such as peat soil, coniferous forest, quality of drinking water and nearby industrial plants.

Spatial variability and trends in ambient air concentrations of metals in Cracow, Poland.

Even though the air quality has been improving since the beginning of the 1980s, Cracow still belongs to the most polluted cities in Poland. The air pollution originates mainly from industry, small-size emission sources and transport. Metals in ambient air have been monitored since 1992 by 4 stations located within the city. The aim of the study was to determine the city areas where the national limits of heavy metals in suspended particles are exceeded and to assess the trends for the years 1992-1999. The monthly mean and maximum values of lead, zinc, chromium, copper, cadmium, nickel and iron were used in the analysis. Between 1992 and 1999, the level of most monitored metals in suspended particles was much below the national standards. Only the concentration of lead exceeded the limits by 50% in the area with the station monitoring traffic air pollution. However, the substantial variability in concentrations of monitored metals observed within the city was most pronounced around the metallurgical plant. Nowadays a new factor prevails: heavy traffic has resulted in a substantially enhanced concentration of lead across the city.

[Professor Jan Witold Moll (1912-1990): a creator of Poznań thoracosurgery, a pioneer of cardiosurgery in Poland, on the 10th anniversary of his death].

The theme of his work concerns studies on the professional career of Professor Moll and on his pioneering achievement in the fields of thoracosurgery and cardiosurgery. He worked in the Surgical Ward of the Joseph Struś Town Hospital 8/12 Szkolna St. in Poznań. During the period 1945-1958 he performed, in addition to other operations, 804 resections of lung tissue. He was also the author of the first Polish edition of "The techniques of lung tissue resection" (1957). His experience in this field lead him to cardiosurgery and on the 1st of June 1958 he took up the position of Head of the II Clinic of the Medical Academy in Lódź. In 1975 he became Head of the Cardiosurgery Clinic and it was there that he carried out the first heart transplantation in Poland, on the 4th of January 1969, 13 months after Christian Barnard.

[Public health care in Poland in the Prussian Sector].

Modernization processes in modern European countries, characterised by the development of contemporary organisational structures, have led to a wider range of public health and hygiene activities. New, developing ideas of public health care demonstrated the need for competent institutions, operating within national government and administration structures, capable of preventing mass epidemics and of eliminating factors from the external environment which area danger to health. These attempts were fulfilled in the beginning of the 19th century. The Prussian state and then united German empire was, at that time, a classical example for the effective organisation of self-governed public health care organisations. In the Prussian sector, public health care matters were governed by the boards of provincial, regional and district authorities. In this sector public health care was also the responsibility of sanitary commissions, which were a continuation of the earlier, so called, medical police forces. The studies devoted to the issue formulated in the title show that within the three-sector scale in partitioned Poland, the Prussian sector could serve as an example of optimal systemic solutions, not only for self-governed health care, but also for a health insurance system. These concepts were the basis for the model of public health care after 1918, in the Public state liberated from its oppressors.

Epidemiological studies on Creutzfeldt-Jakob disease in Poland.

The epidemiological, clinical and neuropathological study of Creutzfeldt-Jakob disease (CJD) in Poland was established in 1996. It was preceded by wide, repeated informative action among neurologists and psychiatrists in the whole country. The investigations were partially sponsored by the European Commission (E.C.) as part of the programme Biomed 1. The results obtained by us during the first three years of the study are presented in this paper. In 1996-1998 over 60 probable or possible cases of CJD (or information about them) were referred to our Institute. Neuropathologically typical changes for spongiform encephalopathy were found in 28 cases (among them four cases in laboratories of Medical Schools in Szczecin and Poznań). Neuropathological evaluation was based on paraffin slices stained by H-E, PAS, Bielschowsky, Kanzler-Arendt and Klüver-Barrera methods. In certain cases antibody 3F4 was used. In three patients only clinically probable CJD were diagnosed, since neuropathology was not done. In twenty-five persons, a detailed inquiry form was filled in after the model indicated by the E.C. As the result of processing the whole material, we diagnosed in twenty-four patients a sporadic form of CJD. The remaining case belonged to a family with Gerstmann-Sträussler-Scheinker syndrome. In sporadic CJD cases examined and observed by us no exogenic risk factors for the disease could be detected.

[Detection of anti-Borna disease virus antibodies in patients hospitalized in psychiatric hospitals located in the mid-Western region of Poland].

Borna Disease Virus (BDV) is single stranded RNA virus, which may infect a wide range of animal species. Manifestations of the experimental BDV infection show some resemblance to psychopathological symptoms of mental disorders in humans. Several reports suggest the higher prevalence of anti-BDV antibodies in psychiatric patients than in healthy controls. However, the seroprevalence of anti-BDV antibodies varied due to the different serological methods used in the previous studies. Electrochemiluminescence Immunoassay (ECLIA) is a recently developed, highly specific method of detecting antibodies directed toward two BDV proteins: p24 and p40. We used the ECLIA method for the assessment of seropositivity in 946 psychiatric patients hospitalized in the psychiatric hospitals in the western part of Poland. All patients were clinically diagnosed with ICD-10 criteria. Anti-p40 antibodies have not been found in the studied sample. We found anti p-24 antibodies in 23 cases, which give the seroprevalence rate of 2.4%. This result is consistent with the outcome of Japanese population assessment, done with the same methodology. The seropositive cases did not show diagnostic specificity. We did not find statistically significant gender differences in rate of seropositivity. The seroprevalence of anti-BDV antibodies was not significantly different in patients of urban and rural residence, and in patients of different age groups. This is the first demonstration of anti-BDV antibodies in the Polish population of patients hospitalized in psychiatric hospitals.

[Associate professor Oscar Liszka, pioneer of surgery in Poland in the 40th anniversary of the first operation].

Oscar Liszka was the first in Poland who performed operations on human brain with the use of stereotactic apparatus. It took place in 1961 in the department of Neurosurgery in Cracow. He introduced Guiot-Gultingham method, which was modified in 1964 by Wicentowicz, Goscinski and Kunicki. Since 1964 stereotactic neurosurgical operations were carried out in the Department of Experimental and Clinical Medicine, Polish Academy of Sciences in Warsaw by E. Mempel. He introduced the original Talairach method and Coopers in Parkinson syndrome. Several years later, J. Bidzinski used the apparatus of Leksell at the Department of Neurosurgery, Medical Academy in Warsaw, stereotactic procedures were also performed by Mert and Bromowicz in Lodz, by Trojanowski and Turowski in Lublin, by Slosarek in Szczecin, by Sloniewski in Gdansk and by Harat in Bydgoszcz, as well Koszewski and Zabek in Warsaw. The treatments aimed at compensating for the profound loss of dopamine in the substantia nigra substantially limited the number of stereotactic operations from 1970 to 1980. Recent advancements in Computerized Axial Tomography and Magnetic Resonance imaging as well as in the histological techniques have led to new opportunities for a small stereo-static biopsy for histological diagnosis of profound brain tumors.

[A feeling of coherence and delayed sequelae of post-traumatic stress in persons persecuted for political reasons in Poland in the years 1944-56].

The paper presents the results of research of 100 persons persecuted for political reasons in Poland in the years 1944-56. The research aims to study the relations between the level of Sense of Coherence and delayed effects of post-traumatic stress disorder in personal functioning (which is manifested by anxiety, depression, lack of meaning of life, and low well-being), and in interpersonal functioning. Introduction of this article presents the problems connected with delayed effects of post-traumatic stress disorder, and the general principle of Antonovsky's salutogenic concept also characterizes the sense of coherence category (SOC). SOC is a generalized, long-lasting feeling of confidence that the world is comprehensible (internal and external environments are structured, predictable, and explicable). People with a high level of SOC see their life as having some purpose and that their life is worthy of personal investments, and see themselves as having the personal resources to cope with the difficulties. The data was collected with using: The Sense of Coherence Questionnaire, PTSD-Interview, Beck Depression Inventory, State-Trait Anxiety Inventory, Purpose in Life Test, The Psychological General Well-Being (PGWB) Index, and The Scale of Interpersonal Relationships. The results indicate that the strength of the sense of coherence is negatively correlated with level of anxiety, depression, enmity syndrome, and positive with meaning of life, psychological well-being, and pro-social relationships. The research confirmed the important role of a sense of coherence (SOC) in modifying experience of delayed consequences of post-traumatic stress in victims of political persecution.

[The dispute over eugenics in interwar Poland].

The eugenic problem seems not as well known as it was in the period between the world wars. The word "eugenics" was introduced into the scientific language by the British biologist, Francis Galton at the end of the XIX century. In general, it means the study of the methods of protecting and improving the quality of the human race by selective breeding. Galton's remarks initiated a wider (social and political) movement, which was active in some countries. The eugenic tendency was visible in American immigration policy before the Great War - making it difficult for people suffering from particular diseases to enter the USA. After the war, the eugenic movement became much stronger. In some countries (e.g. Sweden, Norway, Finland, Denmark, Switzerland, Germany, and over half of the states of the USA) some components of the eugenic programme were introduced into legislation. The eugenic movement also appeared in reconstructed Poland. In 1918 it published it own magazine "Zagadnienia Rasy" (Problems of Human Race), later "Eugenika Polska" (Polish Eugenics). This did not imply however, that interest in the eugenic programme was generally very strong. In fact, it was limited by the influence of the Catholic Church. Another significant factor was lost Polish - German hostility. In the late thirties eugenic slogans lost popularity because of their use in Germany racist policy.

[The prevalence of type II diabetes mellitus in rural urban population over 35 years of age in Lublin region (Eastern Poland)].

The prevalence of type 2 diabetes mellitus (DM 2) has increased dramatically in the last decade. Data relating to the number of undetected cases of diabetes are underestimated. The aim of the study was to evaluate the prevalence of DM 2, obesity, hypertension, and lipid disturbances in a representative group of urban and rural population in the Lublin region (Eastern Poland). The study was performed in 1998-2001. A two-layer draw was applied: two groups of 3000 people were drawn from the population of Lublin town and from the rural areas each comprising 100,000 inhabitants. In all subjects physical examination was performed and body weight, height, and blood pressure measurements were obtained. Blood samples were taken from the basilic vein to estimate: blood glucose, lipids and insulin concentration. Venous blood glucose concentration was measured using a Glucotrend glucometer. Oral glucose tolerance test (OGTT) after a 75 g-glucose load was performed in subjects without previously diagnosed diabetes mellitus and when the fasting blood glucose was < 8.0 mmol/l (144 mg/l). The LDL-cholesterol level was calculated according to Friedewald formula. DM 2 was identified according to the WHO criteria from 1985. Obesity and hypertension were diagnosed according to the new WHO criteria (Body Mass Index > or = 30 kg/m2, blood pressure > or = 140/90 mm Hg). 3782 subjects: 1809 in the rural area and 1973 in Lublin town were examined. The response rate among rural and urban population was 60.3% and 65.8% respectively. The prevalence of DM 2 was assessed in 17.6% of rural and in 14.1% of urban population. 75% of diabetics in the rural areas and 56% in the town were the newly diagnosed cases. We found impaired glucose tolerance in 30.3% of rural and in 21.6% of urban population, BMI > or = 30 kg/m2 in 30.8% and 30.1%, hypertension in 69.4% (29.2% newly diagnosed) and 68.6% (27.7% newly diagnosed), hypercholesterolaemia (total cholesterol > or = 5.2 mmol/l (200 mg/dl)) in 66.4% and 60%, hyper-LDL-cholesterolaemia (> or = 3.5 mmol/l (135 mg/dl)) in 57.3% and 52.6%, hypo-HDL--cholesterolaemia in 21.7% and 31.4%, hypertriglyceridemia (> or = 2.3 mmol/l (200 mg/dl)) in 15.1% and 22% respectively. This finding indicates the urgent need for introducing a national program for early diagnosis and prevention of DM 2 and concomitant metabolic disturbances.

Experiences with computer simulation at two large wastewater treatment plants in northern Poland.

Mathematical modelling and computer simulation have became a useful tool in evaluating the operation of wastewater treatment plants (WWTPs) in terms of nutrient removal capability. In this study, steady-state simulation results for two large biological nutrient removal WWTPs are presented. The plants are located in two neighbouring cities Gdansk and Gdynia in northern Poland. Simulations were performed using a pre-compiled model and layouts (MUCT and Johannesburg processes) implemented in the GPS-X simulation package. The monthly average values of conventional parameters, such as COD, Total Suspended Solids, total N, N-NH4+, P-PO4- were used as input data. The measured effluent concentrations of COD, N-NH4+, N-NO3- and P-PO4- as well as reactor MLSS were compared with model predictions. During calibration, performed from the process engineering perspective, default values of only five model parameters were changed. The opportunities for further applications of such models in municipal WWTPs are discussed.

p53 codon 72 polymorphism in cervical cancer patients and healthy women from Poland.

A polymorphism at codon 72 of gene p53 results in the presence of either arginine or proline at this position. We investigated the distribution of p53 codon 72 polymorphism in cervical cancer patients and a control group of healthy women from Poland. Our results do not confirm the hypothesis that the p53 codon polymorphism could play a role as a factor for squamous carcinoma of the cervix.

[Surgery of the hand in Poland--a controversial issue].

Since hand surgery separated from orthopedic and general surgery as an independent subspeciality, the problem of training new surgery came into life. Many studies indicate that a country with Poland's population should have about 250-300 trained hand surgeons. Current estimates show that there are three to four times less hand surgeons than necessary. The establishment of the Section of Hand Surgery by the Polish Orthopedic Society in 1965 and the founding of the Polish Society for Surgery of the Hand (1998) made formal ground for taking up both training in hand surgery and assenting it as a separate clinical sub-category. No official statement has been made in this matter neither by the Ministry of Health or by any scientific society. The authors suggest the need for organizing a system for training and achieving certified qualifications in hand surgery, for example like the FESSH European diploma in hand surgery. It seems that this matter should be discussed broadly among both general and orthopedic surgeons in order to create a separate sub-category for hand surgery.

Use of the Internet as a research method in a study of the emotional effects of tooth loss in Poland.

The study explored the use of the Internet to investigate the emotional effects of tooth loss in a Polish population. The Internet was used as a novel method of recruiting study participants. The study was announced on Internet pages in the Polish language inviting anyone who had lost some natural teeth to respond. Respondents were sent a questionnaire by e-mail, which they completed and returned by the same means. Twenty nine completed questionnaires were received in a twelve month period. The respondents were predominantly males under the age of 50 years reflecting the profile of Internet users in Poland. Despite the sample bias, the results showed a trend similar to the findings of earlier studies into the emotional effects of tooth loss with many of the respondents reporting difficulty in coming to terms with the loss of some or all their natural teeth.

Coexistence DNA of Borrelia burgdorferi sensu lato and Babesia microti in Ixodes ricinus ticks from north-western Poland.

The tick Ixodes ricinus may carry microorganisms which cause serious human and animal diseases, i.a., the Lyme disease (borreliosis), caused by the spirochaete Borrelia burgdorferi sensu lato and babesiosis, induced by the protozoan Babesia microti. Both microbe species may co-occur in the same and other species of the genus tick and produce a mixed infection in humans and animals. The major objective of the study was to identify DNA of B. burgdorferi and B. microti in the I. ricinus ticks collected in spring and autumn 1999 from 6 sites in north-western Poland. The microbial DNA was identified with polymerase chain reaction (PCR). The marker used to detect the B. burgdorferi s.l. DNA was a fragment of the fla gene encoding the protein flagellin, while the B. microti DNA was detected with a fragment of the gene encoding 16S rRNA. A total of 550, 1,160, and 385 tick adults, nymphs, and larvae, respectively, were examined. Among the 155 (7.4%) B. burgdorferi- infected ticks and the 130 (6.2%) infected with B. microti, mixed infection was detected in 0.6% of individuals. The prevalence of coinfection differed between the tick developmental stages. Coinfection was most prevalent (3.1%) in females, males and nymphs being less affected (0.4 and 0.2%, respectively). No coinfection was revealed in the tick larvae. The study described was the first of its kind to be conducted in the former District of Szczecin. For the phenomenon of microbial co-occurrence and related mixed infections to be properly evaluated, the research will be continued.
