[Update in medical informatics in oncology].

Through the implementation of the French reform of resources allocation, according to the French DRG system (PMSI), all the medical records of French hospitals are currently indexed according to the same rules and classifications. As a by-product, these discharge summaries are merged into huge administrative databases covering both public and private settings. Statistical analysis of these data have begun, and seems to be worthwhile in oncology, e.g. for assessing the burden of cancer treatments in hospitals, for revealing the regional variations in practice or for estimating the incidence of melanoma. However, fully anonymous data leading to double counts, questionable quality of summaries and lack of complete covering in radiotherapy centres, still impede from using the data for epidemiological purpose. Short term improvements are under way: changes in quality assessment and control, possibility of linking successive stays of a patient while respecting privacy, implementation of new classification of procedures and new descriptions of "casemix". The French DRG could then have a place in the monitoring of cancer both at national and regional levels.

Nursing informatics: a universal nursing language.

Knowledge is necessary to use information to make decisions in nursing situations. Nursing data must be grouped in a logical way so the interrelationship between the classification systems and specific concrete phenomena in individual nursing situations is useful. The processing and management of information for decision making is the focus in nursing and health care. The development and use of language that represents the domain of nursing is an essential dimension in an integrated and coordinated health care delivery system. The design and acceptance of a universal nursing language system is critical to collect appropriate data to document nursing's role, functions, and effectiveness in health care by the year 2000. This is an urgent message for nurses to be involved in the technology and nursing care documentation to demonstrate quality care that is cost effective. In the next century, nurses must speak a common nursing language that has the same meaning for all nurses.

Consumer informatics: helping patients to access health information via the Internet.

Now that many patients independently access health information on the World Wide Web (WWW), healthcare professionals are becoming concerned with control and quality of information available there. The technology has the potential to help patients to become more self-sufficient in managing their own health care and outcomes. This paper examines the importance of developing mechanisms to assess the quality and content of health information websites.

[Applications of informatics in anesthesiology: anesthesia graphics].

Computerization has brought radical changes to anesthesiology. Quality of care, management, cost control, training, research, safety and privacy have all improved. The anesthesiologist has been freed from repetitive clerical tasks and is able to make better use of time. A graphic display of anesthesia is only one of the many computer applications available as a consequence of links created among monitoring, continuous infusion and intelligent alarm systems, automatic data collection, network monitoring and the availability of bibliographic information (through Internet connection). The computer graphic display of anesthesia is more precise, legible, complete and reliable (during critical events, in substitutions of anesthesiologists or for research) than the traditional graph. One of the greatest problems of computer graphing today--besides start-up costs--is that of inserting comments on monitoring artifacts, given that the graph is a legally valid medical document.

An academic-business partnership for advancing clinical informatics.

A jointly funded partnership between the school of nursing at a large midwestern university and a premier health care information technology supplier represents a pioneering event for education and for the health care information technology industry. The impetus for this partnership arose from Institute of Medicine reports published in late 1999 and early 2001 addressing the quality, error, and waste in the health care system in the United States. The Simulated E-hEalth Delivery System (SEEDS) provides opportunities based on best practices in education to learn and practice clinical skills in a state-of-the-art environment using a live-production, clinical information system designed for care delivery. A pilot project that began with a small cohort of baccalaureate nursing students has been implemented and extended. SEEDS will also be extended to other health professional programs.
