The aim of the present study was to investigate, using laser Doppler (LD) flowmetry, the phases immediately prior to and following experimental, neurosurgical laser therapy in animals. Two statistical models obtained on the basis of discriminant analysis are described. These models should enable the user to classify the sets of parameters calculated from the LD signals. With both models which contain 3 and 5 parameters, respectively. 100% discrimination of the two phases was attained. The usefulness of the models could be validated by results obtained with other models.

PURPOSE: To compare the efficacy of full-dose local intrapulmonary thrombolysis (LIT) versus AngioJet rheolytic thrombectomy (ART) in the treatment of massive pulmonary embolism. METHODS: A retrospective review was conducted of 8 consecutive patients (5 women; mean age 66.0+/-5.9 years, range 56-74) who underwent LIT with high-dose intrapulmonary urokinase (4400 IU/kg over 10 minutes followed by a 2000-IU/kg/h infusion) and a subsequent 6 consecutive patients (4 men; mean age of 59.2+/-17.0 years, range 26-69) who underwent ART plus adjunctive low-dose urokinase infusions (100,000 IU) until hemodynamic recovery was achieved. Pre and postprocedural Miller scores were calculated, and relative Miller score improvement, total urokinase doses, and duration of therapy were compared. RESULTS: Hemodynamic stability was restored in all 8 LIT patients and in 5 (83%) of the 6 ART patients; 1 (16.7%) patient died during the ART procedure due to recurrent MPE. In the LIT group, the mean Miller score prior to intervention was 17.38+/-2.67, which was reduced to 6.13+/-1.46 after the intervention (p<0.0001) compared to scores of 18.83+/-2.86 and 6.83+/-2.79, respectively, in the ART group (p<0.0001). The mean urokinase dose was 2.07+/-0.44 million IU in the LIT group versus 0.70+/-0.36 million IU in the ART group (p<0.0001). The mean duration of therapy was 11.45+/-2.94 hours in the LIT group versus 3.37+/-1.41 hours in the ART group (p<0.0001). No significant difference in relative Miller score improvement was observed. CONCLUSION: By accelerating the fragmentation of thrombus, ART plus adjunctive low-dose urokinase seems to be more rapidly effective compared to LIT. ART achieves both rapid cardiovascular relief and reduces the dose of thrombolytic agent necessary in patients with massive pulmonary embolism.

OBJECTIVE: This study sought to investigate the influence of Diabetes Mellitus (DM) on immediate results after coronary stenting implantation (CSI) according to clinical presentation. METHODS: Between January, 1997 and December, 2003, 11,874 diabetic patients underwent CSI, as recorded by CENIC database: 7,386 (62.3%) had chronic coronary disease (CCD); 3,142 (26.4%) acute coronary syndrome with non-ST segment elevation (ACSNST); and 1,346 (11.3%), reported acute myocardial infarction (AMI), with ST Segment elevation. Those groups were compared with 48,103 non-diabetics: 30,980 (64.5%) with CCD; 10,938 (22.7%), with non-elevated ST segments and unstable angina; and 6,185 (12.8%), with AMI. RESULTS: Diabetic patients presented worse clinical and angiographic characteristics. Diabetics with CCD showed similar incidence of MACE as compared to non-diabetics (0.98% x 0.91%, p = 0.5971); however, diabetics with ACSNST and AMI reported higher incidence of events: 2.76% x 1.46% (p < 0.0001) and 7.87% x 4.1% (p < 0.0001), respectively. Multivariate analysis showed DM to act as independent risk predictor for larger adverse events under non-elevated ST segment and unstable angina (ACSNST) (OR: 1.92 CI: 1.46-2.52 p < 0.0001) and with AMI (OR: 2.0 CI: 1.57-2.54 p < or = 0.0001) and no influence for CCD (OR: 1.08 CI: 0.83-1.42 p = 0.5470. CONCLUSION: Diabetic patients with CCD reported similar outcome as compared to the non-diabetics; however, those with ACSNST and AMI presented higher incidence of major adverse cardiac events during hospital stay.
Analyses of the correlation between hyperspectral reflectance and pigment content including chlorophyll-a, chlorophyll-b and carotenoid of leaves in different sites of rice were reported in this paper. The hyperspectral reflectance of late rice during the whole growing season was measured using a Spectroradiometer with spectral range of 350-1050 nm and resolution of 3 nm. The chlorophyll-a, chlorophyll-b and carotenoid contents in rice leaves in rice fields to which different levels of nitrogen were applied were measured. The chlorophyll-a content of upper leaves was well correlated with the spectral variables. However, the correlation between both chlorophyll-b and caroteniod and the spectral variables was far from that of chlorophyll-a. The potential of hyperspectral reflectance measurement for estimating chlorophyll-a of upper leaves was evaluated using univariate correlation and multivariate regression analysis methods with different types of predictors. This study showed that the most suitable estimated model of chlorophyll-a of upper leaves was obtained by using some hyperspectral variables such as SD(r), SD(b) and their integration.
The National Institutes of Health (NIH) includes visual impairment in the 10 most prevalent causes of disability in America. As rehabilitation programs have the potential to restore independence and improve the quality of life for affected persons, NIH research priorities include evaluating their effectiveness. This paper demonstrates a clinical perspective on the use of the Rasch person-item map to evaluate the range and precision of a new vision function questionnaire in early analysis (prior to full sample). A self-report questionnaire was developed to measure the difficulty that persons with different levels of vision loss have performing daily activities. This 48-item Veterans Affairs Low-Vision Visual Functioning Questionnaire (VA LV VFQ-48) was administered to 117 low-vision patients. Preliminary analysis indicates that the questionnaire items are applicable to persons of differing abilities. The Rasch person-item map demonstrates that the field-test version of the VA LV VFQ-48 has good range and is well centered with respect to the person measure distribution. Construct validity and reliability are also demonstrated.
The determination of stoichiometries and stability constants of complexes by means of UV-visible spectrophotometry applying traditional methods does not evaluate the quality of the values obtained, since the classic application of these methods does not provide coverage intervals. However, the use of chemometric techniques in different steps of the application of these methods makes it possible to obtain not only a real value of the characteristics of the complexes but a validation of such a value. In this paper a methodology is proposed that combines some traditional methods, three different regression models (LMSR, LSR and LSPR) and a small number of mathematical algorithms. This methodology is able to estimate, in a simple and rigorous way, the stoichiometry and stability constant of a complex and its corresponding uncertainties.
A micro-miniature array of sensing capsules for optical monitoring of pH, potassium and glucose is described. Optode technology translates the respective ionic levels into variable colors of ionophore/dye/polymeric liquid micro-beads stuffed into individual capsules. Glucose is monitored indirectly, by coupling through glucose oxidase (GOX) immobilized in cellulose acetate phthalate (CAP) based microscopic beads that are stuffed into another microcapsule together with pH sensitive optical microscopic beads. The electrolyte and glucose sensing capsules are embedded in a transparent cellulose acetate bar 300-500 microm wide and 2-2.5 mm long called the sliver sensor that includes also a white capsule made of micro-beads without dye for optical reference. By adding further capsules custom combinations of analytes can be monitored in biomedical and non-biological contexts. In this work, as an example, design, fabrication and testing of a sliver sensor that could be developed for in vivo use are described.

The widespread use of actigraphy has led to the recognition that a number of methodological issues have to be addressed to facilitate an increased acceptability of this relatively new method. These methodological issues include actigraph placement, reliability, and sensitivity, and the phenomenon known as the "first night effect." Our findings have demonstrated that actigraphy is a reliable instrument for assessment of sleep and wakefulness. In addition, actigraph placement and reliability do not constitute a significant methodological problem as no differences were found in all of these studies. We have also observed no first-night effects associated with sleep-wake monitoring with actigraphy.
For the purposes of studying the phenomenon of evoked oto-acoustic emissions (EOAEs) in very-low-birth-weight (VLBW) infants, and the conditions affecting the utility of EOAE ear screening in this population, click EOAEs were repeatedly recorded in ears of 144 VLBW infants, at different postconceptional ages of the infants and at two different test sites, i.e. in the neonatal high-care unit (ward), or at the neonatal outpatient clinic. The postconceptional age of the infants examined in the ward was 30-49 weeks and 37-66 weeks for the infants examined at the outpatient clinic. Overall 840 recording attempts were done. In the ward 86% of these attempts (388) were successful against 60% (of 452 attempts) at the outpatient clinic. In the latter group of infants the success rate of recording was only 33% at the corrected age of 6 months, which is significantly less than the 66% until the corrected age of 3 months. For a cross-sectional analysis of age effects one ear of each successfully recorded infant was selected. Analysis of the 127 successful recordings revealed that the EOAE prevalence was 71% in the ward (54% for infants receiving extra oxygen per naso) and 91% at the outpatient clinic. Compared with healthy newborns, VLBW infants are much more difficult to test, especially at the outpatient clinic. However, the EOAE prevalence at this test site is the highest and approaches that in healthy newborns. At the outpatient clinic response levels of EOAEs recorded approach levels found in healthy newborns. The higher success rate of recording in the ward and the lower EOAE prevalence are two counteracting factors as to the utility of EOAE-based ear screening of VLBW infants.
