[Epidemiology of the climacteric period in a large city].

Epidemiological screening of 1,939 females working in Moscow showed the incidence of the climacteric syndrome to be as high as 42.4%. The mean age at the onset of the pre-menopause was 48 +/- 0.4, its duration 12 to 17 months, mean age at menopause onset 49 +/- 0.5. A close correlation between the development of the climacteric syndrome and arterial hypertension was detected.

Dorsal relieving incision for the non-retractile foreskin.

The operation of dorsal relieving incision (DRI) for non-retractibility of the foreskin is described and the results of a retrospective review of 67 boys so treated are presented. We conclude that DRI is a simple, safe and effective procedure for the non-retractile foreskin not affected by fibrotic scarring.

Plasma-cell gingivitis in children and adults. A clinical and histological description.

Plasma-cell gingivitis (PCG) is a rare, non-symptomatic lesion found in the attached gingiva. In the county of Södermanland 14 cases of PCG were identified during the last 13 years. Clinically, the PCG is a sharply demarcated red area within the attached gingiva. Abundant plasma cells are found in the connective tissue beneath a thin epithelium. Lesions with similar clinical appearance were also found in the genital mucosa of five of the patients with a PCG. An extensive bacterial plaque was frequently found on the gingiva and teeth in the affected areas. Although an improved oral or genital hygiene did not heal the lesions, an allergic reaction to the bacterial plaque may be the cause.

Genomic modifications in naturally occurring neurovirulent revertants of Sabin 1 polioviruses.

Five type 1 Sabin-like poliovirus strains were isolated from paralytic cases of poliomyelitis. Their Sabin origin was confirmed by antigenic analysis with monoclonal antibodies and restriction fragment length polymorphism (RFLP) assay. Several nucleotide positions known to play a role in the reversion towards neurovirulence were examined: viz 480, 2741, 6203, 7441. Analysis was performed by enzymatic restriction and partial sequencing of PCR-amplified genomic segments. All the strains bore the reversion G > A at residue 480; in four of the five viruses, only this reversion was found. The fifth strain presented four additional reversions and several new mutations. These results indicate that, during multiplication in the human gut, Sabin 1 poliovirus carrying one or several mutations related to reversion towards neurovirulence can be selected.

Histological evidence for recurrence of primary biliary cirrhosis after liver transplantation.

Whether primary biliary cirrhosis recurs after orthotopic liver transplantation remains a controversial issue. Sixty consecutive patients with primary biliary cirrhosis with at least 1 yr of follow-up after liver transplantation were studied. All patients were treated with triple-drug immunosuppression (cyclosporine, prednisone, azathioprine). Hepatic biochemical parameters and protocol liver biopsy specimens were evaluated 1 wk, 3 wk, 4 mo and yearly after orthotopic liver transplantation and at times of liver dysfunction. Antimitochondrial antibody titers and IgM levels were determined at 4 mo and yearly. At the time of last follow-up, all patients had marked symptomatic improvement compared with their pretransplant condition, and 91% of the patients had normal hepatic biochemical parameters, including serum levels of alkaline phosphatase, bilirubin, ALT and IgM. In addition, all patients had significant decreases in antimitochondrial antibody titer (p = 0.0001) and significant decreases in serum levels of IgM (p = 0.0001). Forty-one of the 60 patients had near-normal liver histological appearance. Of those with abnormal histological appearance, five patients, 2 to 6 yr after orthotopic liver transplantation, had histological features typical of a florid duct lesion, suggesting recurrent primary biliary cirrhosis. All five patients with portal granulomas had normal hepatic biochemical values and were clinically asymptomatic. Two of the five patients had persistent antimitochondrial antibody titers. We consider the documented histological changes highly suggestive of recurrence of primary biliary cirrhosis after liver transplantation but, so far, have no evidence that the condition in these patients is progressive.

[The psychiatric aspect of the climacteric syndrome: its clinical picture and treatment].

A total of 130 patients with the climacteric syndrome associated with mental disorders were examined by psychiatrists. Two groups of patients were distinguished. In Group 1 specific climacteric disorders ('flushes', excessive sweating, etc.) were the major components, whereas mental disorders (asthenic, phobic, hysterical, depressive) were just concomitant. In Group 2 the climacteric syndrome resulted in development of various mental diseases, mostly neuroses, depressions of various origins, superworship ideas. Recommendations on psychotropic therapy of these disorders were developed and contraindications against the use of hormonal therapy formulated.

[The pathogenetic mechanisms of hyperthermia in patients with the premenstrual syndrome].

Functional status of the central and autonomic nervous system was analyzed in 28 patients with the premenstrual syndrome one of whose manifestations was disordered thermoregulation. The examinations were carried out during both cycle phases. Analysis of the EEG data and clinical findings has shown different patterns of autonomic thermoregulation disturbances in three groups of patients and permitted the authors to suggest that changed activity of certain neurotransmitter systems connected with hormonal secretion over the course of the cycle was responsible for these disturbances.

The identification of Mycoplasma mycoides mycoides LC as the aetiological agent of balanoposthitis and vulvovaginitis in sheep in South Africa.

Clinical ulcerative balanoposthitis and vulvovaginitis was experimentally reproduced in 14 sheep infected with a Mycoplasma mycoides mycoides LC field strain, isolated from the Straussheim Dorper stud. The study encompassed a series of field observations, a therapeutic trial and experimental investigations. A wide range of bacteria and various mycoplasma spp., but no viruses, were isolated from a large number of infected animals.
