[The five-word test in memory deficiency].

The 5-Word test is a bedside memory test with free and cued selective recollection. Here, we evaluated its reliability Belgian French speakers. Five groups were studied : normal subjects, depressive patients, patients with AD, patients with vascular dementia and 47 patients for a validation of a logistic model. The results of the 5-Word test with the MMSE, both values were included as variables in a logistic regression model. The probability of diagnosis of AD and dementia can be calculated and used prospectively for assessment of new cases in a validation study. The importance of the study of free and cued response during immediate and delayed recall was assessed by variance analysis in 4 groups. A third order interaction (p < 0.001) was observed between the normal and AD groups, the normal and vascular dementia (VD) groups and between AD and VD groups. With this initial model, 98% of the 62 normal subjects and the 57 patients with AD were correctly classified (p < 0.001). For AD diagnosis at the onset, the sensitivity of the MMSE is increased when it is associated with the 5-Word. This low-tech bedside method associating the 5-Word test and MMSE has a good specificity for the diagnosis of AD. It is very useful for screening a population of patients at the onset of cognitive and memory deficiency and for selecting the patients requiring an examination at a memory clinic.

Physique of patients with carcinoma of the female genital tract.

The authors carried out an investigation with a detailed anthropometric programme on 135 women suffering from different kinds of cancer: ovarian n = 35, endometrial n = 22, cervical n = 54, and vulvar/vagina n = 24. All patients were Hungarian and belonged to European ethnic groups. Their age varied between 25.6 and 85.0 years. Somatotype of the patients was estimated with the Heath-Carter anthropometric somatotyping method. Somatotype (endomorphy, mesomorphy, ectomorphy) of the patients with ovarian cancer was respectively: 6.8-5.3-1.0, patients with endometrial cancer 7.9-5.8-0.9, patients with cervical cancer 6.8-5.3-1.3, and patients with vulvar cancer 7.5-5.9-0.9. Based on variance analysis, there was no significant difference among subgroups at the p < 0.05 level. The patients in all four groups--in the overwhelming majority of cases--showed mesomorphic-endomorph forms, i.e., endomorphic elements dominated in their physique and mesomorphy (robusticity) was greater than ectomorphy (linearity).

[Doppler parameters of systolic function and heart rate].

This prospective study had two aims, to study the Doppler parameters of left ventricular systolic function with respect to heart rate, and to determine the influence of ischaemic heart disease on these variations. The Doppler indices (velocity time integral, maximum velocity and average acceleration of systolic flow in the left ventricular outflow tract) were measured and averaged over 3 beats after digitization: the measurements were repeated in 30 patients under basal conditions and after 2 minutes transoesophageal atrial pacing at 150 beats/min. These 30 patients were divided into 3 groups: group 1 control subjects with normal coronary arteries, n = 13, EF = 71 +/- 8.9%; group 2 coronary patients without myocardial infarction (greater than 70% stenosis on coronary angiography), n = 9, EF = 64.3 +/- 10.3%; group 3, coronary patients with previous infarction, n = 8, EF = 51.8 +/- 10.9% (p less than 0.0006). Variance analysis for repeated measurements showed significant decreases in velocity time integrals and maximum velocities after pacing (11.8% +/- 2.2 and 0.86 +/- 0.1 versus 18.3 +/- 2.2 and 0.91 +/- 0.1, p less than 0.0001 and p less than 0.05 respectively). This decrease was identical in the three groups. The variations observed were therefore related to the increase in heart rate and not to coronary status or left ventricular function.

Quantification of the retinal nerve fibre layer thickness in normal Indian eyes with optical coherence tomography.

PURPOSE: To quantitatively assess the normative values for peripapillary retinal nerve fibre layer (RNFL) thickness with Optical Coherence Tomography (OCT 3) in Indian subjects. METHODS: The peripapillary retinal nerve fibre layer of 146 normal subjects was imaged on OCT 3 in this cross-sectional study. Thickness of the RNFL around the disc was determined with three 3.4 mm diameter circle OCT scan. The RNFL thickness was measured in four quadrants; superior, nasal, inferior and temporal. The data was analysed using SAS commercial statistical software. Influence of age and gender was evaluated on various measured parameters using unpaired t test, one-way analysis variance (ANOVA) and Pearson's correlation coefficient. RESULTS: One hundred and forty six eyes of 146 patients, 84 males and 62 females were studied. The average RNFL thickness in the sample population under study was 104.27 +/- 8.51 (95% CI 87.25-121). The RNFL was thickest in the inferior quadrant, followed by the superior quadrant, and progressively less in nasal and temporal quadrant. The difference between inferior and superior quadrants was not statistically significant. Age had a significant negative correlation with average RNFL thickness (r = -0.321, P = 0.000) and with average superior (r = -0.233, P = 0.005) and average inferior RNFL thickness (r = -0.234, P = 0.004). There was no effect of gender on various RNFL thickness parameters. CONCLUSIONS: RNFL thickness is significantly correlated with age, but not with gender. This normative database of RNFL thickness with OCT in Indian eyes is similar to previously reported values in normal Asian eyes.

[Selection of TLC solvent system for separating substituted ferrocyclopentadienyl borane using orthogonal array method].

A series of substituted ferro-cyclopentadienyl borane compounds were separated by using thin-layer chromatographic technique. The optimum mobile phase was acetone-cyclohexane-tetraethyl ammonium halide (0.59:8.24:1.18). The orthogonal array method was used to get the optimum chromatographic condition. The types and the concentrations of different kinds of ion-pair reagents, the contents of acetone and cyclohexane were considered as factors which affected the results of separation. The operating conditions for each experiment was adopted based on the L9 (3(4)) design. A statistical method of variance analysis was used to evaluate the overall degree of separation (D(S)), a criterion which reflects the degree of optimization. As a result, we found the most important factors that affected the separation and the optimum chromatographic condition for 8 compounds of this sort.

Increased CD34+ cell peripheral traffic appears to be an unique feature of the allergic inflammation.

Evidence has been cumulated during the last years concerning the immaturity of the cells involved in the local and systemic aspects of allergic inflammation. Hematopoietic precursors (HPC) are mobilized from the bone matrix as multipotent cells or, more often, as progenitors that, after the initial white-lineage commitment reach through the peripheral blood (PB) their final destinations constituted by the target organs of allergy. Although several studies have investigated the CD34+ cells traffic and location at the level of the inflamed peripheral mucosae in allergic populations, limited information is available on their behaviour on the time-course of infectious diseases. The current study thus was designed to asses the peripheral traffic of CD34+ HPC during the infectious inflammation. To this end CD34+ HPCs have been enumerated, by flow-cytometric techniques, in PB of 24 adult healthy beings (Group A), 24 adult subjects with symptomatic extrinsic allergy (Group B) and in PB of 24 adult patients hospitalised for febrile infectious pathology (Group C). CD34+ cell values ranged 0.01-0.08% with a median of 0.03 in Group A. In Group B values ranged 0.17-0.75% with a median of 0.28 and in Group C values ranged 0.00-0.12% with a median of 0.07. Variance analysis test among the three groups was statistically significant (p<0.001) supporting the conclusion that CD34+ HPC mobilizing and increased peripheral traffic is an unique feature of the allergic inflammation.

Robotic systems and surgical education.

This experimental study aimed at evaluating the efficiency of robots in the learning of surgical techniques. We recruited 40 surgeons, divided them into 2 groups of 20, each of which used the robotic system. The first group consisted of experienced physicians, and the second group comprised physicians in training. Each surgeon was allowed to use the da Vinci robotic system for 30 minutes twice in the span of 24 hours. The practice time period was divided into 15 minutes for tying and placement of sutures and 15 minutes for incisions and vascular suturing. We recorded the times required for the performances, and a statistically significant outcome was obtained. With variance analysis (ANOVA), it has been shown that the time needed to perform the exercises depends in a statistically significant way on the kind of test to be performed (P<0.01), the experience of the surgeon (P<0.001), and the kind of operation (P<0.025). Robotic systems can be an optimal tool both for residents and experienced surgeons, for learning of basic surgical tasks and for perfection of clinical skills. The use of the system has great potential in surgical training, offering a reduction in the learning period, enabling checking for errors, and allowing an evaluation of the capabilities obtained. Final goals are a drastic reduction in the learning curve, a better technique, with a significant reduction in surgical errors and complications, with greater safety for the patient.

[Evaluation of the presence of bacteria in food and environment of an Oncological Service of a National Hospital, San José, Costa Rica].

The microbiological quality of 92 samples from the Oncological Service of a National Hospital, San José, Costa Rica was evaluated during the months of February to July, 2002. Of these samples, 48 were from salads, refreshments, and fruits and 44 from surfaces, air and hands of the medical personnel. The presence of total and fecal coliforms was determined by the total plate count technique, using Violet Red Bile Agar for the isolation of total and fecal coliforms, Baird-Parker Agar for Staphylococcus aureus, Cetrimide Agar for Pseudomonas and Listeria was enriched in University of Vermont Broth, Fraizer Broth and isolated in Oxford Agar. From the food samples analyzed, 77% were positive for at least one of the parameters studied; the fruits presented the greater contamination (94%) and the cooked salads the minor, even though Listeria was isolated in 13% of them. All tested salads (100%) were positive for fecal coliforms. Pseudomonas and S. aureus were isolated from fruits, raw salads, and refreshments. All the analyzed surfaces, with an exception, were negative for the parameters evaluated. 75% of the hands of the medical personnel were positive for at least one of the parameters evaluated; 25% had total coliforms (2.8x10(3) UFC/hand), 13% fecal coliforms (2.4x10(3) UFC/ hand), 69% S. aureus (1.3x10(3) UFC/hand) and 19% Pseudomonas sp. 115 isolates were randomly selected, and identified using miniaturized tests (API). When comparing these identifications with the 166 isolations done at the Clinical Laboratory of the Hospital from the infected patients of oncology, by means of variance analysis, it was determined that both bacterial populations were statistically similar.

[Clinical-epidemiological study of toxocariasis in a pediatric population].

OBJECTIVE: The variety of toxocariasis clinic manifestations and its relationship with asthma motivated this study. The aim was to study T.canis seropositivity at a public pediatric service and its association with laboratory, epidemiological and clinical factors. METHODS: This study was cross-sectional and controlled. Two hundred and eight children, from 1 to 14 years old and treated at the University of Santo Amaro Pediatric Department s Immunology and Pneumology clinic between January 2000 and January 2001, underwent serology testing. Antibodies were detected by ELISA testing for the larval excretory-secretory antigen of T. canis. We used the chi-square test for T.canis seropositivity (titers > or = 1:320) associations with: puppies at home, contact with soil, geophagia, onicophagia, mother's educational level, asthma, chronic cough, repetitive pneumonia, skin manifestations, rhinitis, hepatomegaly, splenomegaly, abdominal pain, anemia, eosinophilia, immunoglobulins, parasitosis and stunted growth. The Kruskal-Wallis variance analysis method was used for mean comparisons between seropositive and seronegative groups. Significance was set at p < or = 0.05. RESULTS: Seroprevalence was 54.8%, with a mean age of 6.5 years while for seronegative children the mean age was 5.8 years, showing no significant difference. There was also no difference between sexes. Seropositivity was significantly associated with puppies at home, contact with soil, hepatomegaly, asthma, eosinophilia, increased serum immunoglobulin (Ig) E levels and stunted growth. CONCLUSION: Seroprevalence was elevated in this study . T.canis infection must be considered in at-risk children, such as those with puppies at home, who have had contact with soil, who have hepatomegaly and/or asthma with eosinophilia and increased serum IgE.

Reaction exothermia of 2 relining resins for temporary crowns.

AIM: The aim of the study was to evaluate the reaction exothermia of 2 relining resins for temporary crowns. The resins used were Duralay, a methacrylate-based resin, and Protemp II, a bisacrylic resin. METHODS: The coronal parts of an adult molar and of an impacted one were used. Abutments were obtained from the crowns, while the radicular part was sectioned in order to insert a thermocouple into the pulpal chamber. During the experiment the tooth with the polymerising resin was put into an oral cavity simulator to keep a constant temperature. The thermal increase during polymerisation of the 2 resins was recorded for 6 minutes at 5 seconds intervals and with 1 and 2.5 mm dentin thickness (n = 10). Data were statistically analysed by variance analysis with Scheffe test. RESULTS: The intrapulpal thermal increase was significantly higher in relation to the type of resin (Duralay p < 0.0001), thickness (1 mm p < 0.0001) and type of dentin (young dentin p < 00001). CONCLUSIONS: In order to protect the pulp, during the direct relining of a temporary crown, it is recommendable to choose the best combination among resin, type of dentin and its residual thickness, besides adequate cooling techniques.

[Scorpion poisonous stings in the population of Khouribga (Morocco)].

Poisonous stings caused by scorpion constitute a public health problem in Morocco because of their frequency severity and socio-economic consequences that they generate. The province of Khouribga surveyed for this study is situated in a zone of high incidence and high lethality caused by scorpion stings in Morocco. In order to analyze the epidemiological profile and the poisoning risk factors, we are presenting the results of a past study conducted from April to December 2001. It consisted in making an exhaustive follow up from admission to release of all patients stung by scorpion. These patients consulted a sanitary structure (dispensary health centre or hospital) in the province of Khouribga, Morocco. Within a total of 1212 cases of stings, 63% of them occurred at night (between 06 pm and 06 am) particularly during July and August. Youngsters under fifteen represented 36% of these cases. 3.4% of the people who got stung presented typical poisoning and envenomation. The delay expected from sting to transfer to a medical structure was 1.85 +/- 0.11 hours for the patients who recovered against 2.67 +/- 0.11 for those who did not recover. To be noticed as well that 1.3% of the subjects who got stung die. Variance analysis indicates that the type of sanitary structures receiving patients, admission classes, and age of patients influence significantly the recovery rate of people who are stung.

[Isolation and purification of total tubeimosides from Bolbostemma paniculatum by macroporous resin].

OBJECTIVE: To select the optimum purification process of all tubeimosides from Bolbostemma paniculatum by macroporous resin. METHOD: Static absorption test and deabsorption test were carried out to screen the best macroporous resin for all tubeimosides. The single factor test, orthogonal experimental designs and variance analysis were applied to optimize the manipulation parameters of macroporous resin. RESULT: The macroporous resin NKA possessed the strongest absorption ability to all tubeimosides among the 9 resins studied, in addition to an easy de-absorption property. The optimum absorption conditions were A2B3C1, namely concentration of feed 30 mg x mL(-1), diameter: height of the chromatography column 1:7, the weight ratio of raw material and resins 1:2. CONCLUSION: The above experimental results can provide experimental basis for large-scale purification process of all tubeimosides.

[Estimation of reproducibility and repeatability in microbiological ring trials--robust versus conservative methods].

During the last years there was a lot of progress to be seen in the development of standardized methods for microbiological ring trials. The statistical analyzing strategies, in particular the calculation of estimations for the parameters repeatability and reproducibility, will be considered in this paper. Apart from the conservative method of the variance analysis robust methods are increasingly discussed. We will compare and discuss these methods using data of recently realized ring trials. If we can assume a normal distribution of our data, then all estimations are theoretically precise and efficient. But up to now, we know very little about the character of the robust estimations, if the normal distribution cannot be assumed. In addition to this, we have to mention once more, that the use of robust estimators is unreasonable without taking a critical look on the data themselves. Thus, we will show the possibilities of graphical presentation of all data to identify laboratories with critical results.

Changes of dehydrogenases activity in soils polluted with diesel fuel.

Activity of the enzymes can give information on the kind and duration of the effects of pollutants on the metabolic activity of soil. Results of studies on the dehydrogenase activity in soils experimentally contaminated with diesel fuel, with and without modification used in investigation over biodegradation process (including fertilization, stirring and inoculation) were presented in paper. Studies with two rates of pollution were carried out for soils of various organic matter content and different soil particle size grading. Dehydrogenase activity was determined with TTC (2,3,5-triphenyltetrazolium chloride) as a substrate. Increase of dehydrogenase activity was found in the soils that were treated with diesel fuel. Regardless of pollution rate, usually higher activity was recorded in sandy soil. Variance analysis confirmed the highly significant influence of applied modifications on increase of the enzymatic activity in relation to not modified object.

Effects of Nd: YAG laser irradiation on the adaptation of composite resins to root dentin.

One of the most important goals of the restoration of endodontically treated teeth with a composite resin post system is to achieve good adaptation of the composite material to dentin walls to prevent microleakage apically and coronally. In post core systems, to avoid microleakage between the dentin wall and resin material, laser irradiation may seem to be an alternative therapy for increasing adaptation quality. The aim of this study was to investigate the adaptation of a packable composite resin to lased root canal dentin when it was used as post material. Forty-five freshly extracted human teeth were selected and post spaces were prepared. The teeth were divided into three groups. In group 1 the dentin of the post spaces were etched with 35% phosphoric acid, in group 2 the dentin surfaces were irradiated with an Nd: YAG laser, and in group 3 the dentin surfaces were initially treated with the laser and then etched. The teeth of all groups were obturated with a bonding agent and composite. The groups were divided into two subgroups: a group for the scanning electron microscope (SEM) study and a group for the microleakage study. The lased group showed poor adaptation and more microleakage was detected (p<0.05; variance analysis). The laser beam and acid-etching showed no advantage compared to the acid-etching alone (p>0.05). The Nd: YAG laser irradiation adversely affects adhesion to dentin for using composite resin as a post material and does not constitute an alternative to acid-etching.

[The monitoring of cholinesterases in farm workers and tradesmen exposed to phosphoric esters and carbamates].

Cholinesterase (ChE) levels (Ellman method) were monitored in 90 subjects (69 males and 21 females) exposed to carbamate and organophosphate pesticides (78 agricultural workers and 12 pesticide vendors). Pre-exposure baseline values of plasma and red blood cell cholinesterase activities were defined for each subject with two blood samples (23 workers) or three blood samples (59 workers) taken almost thirty days after the last exposure. After control of intra-individual variation, 8 subjects with only one pre-exposure value and 13 with a coefficient of variation above 30% were excluded. For the other 59 subjects, the intra-individual variation of erythrocyte ChE (16%) was similar to the inter-individual one (15%), whereas the inter-individual variation of plasma ChE (21%) was higher than the intra-individual one (14%). Laboratory variation for plasma ChE measurements was 8%. Baseline values were analyzed (ANOVA) for sex, age, task and hour and season of sampling. Both erythrocyte and plasma enzymes, corrected for hematocrit, were lower in females. Plasma cholinesterase activity was lower in "re-entry" agricultural workers and in pesticide vendors. Post-exposure cholinesterase activity was measured in 54 workers within a few (1-21) days after last handling. Average relative reduction was 15.2% (95% C.I. = 4.9%-25.5%) in erythrocyte cholinesterase activity and 29.1% (95% C.I. = 18.2%-40.1%) in plasma cholinesterase activity. The one-way variance analysis showed marked plasma ChE reduction in mixers, loaders and appliers (36%, 95% C.I. = 24%-48%) and in parathion handlers (35%, 95% C.I. = 21%-49%. No significant reduction in blood cell cholinesterase activity in relation to task and to pesticide handled was observed. We conclude that the intra-individual variations of the baseline values were higher for three repetitions (88% and 84% of the population were within a variability of less than 30%, for AChE and for ChE respectively) than for two repetitions (91% and 88% of the population were within 30% of variability for AChE and for ChE respectively). The figures show a greater sensitivity of plasma ChE activity in acute exposure, probably due to a poor reliability in detection of erythrocyte ChE by local laboratories. The maximum reduction (38%, 95% C.I. = 22%-53%) in plasma ChE activity was observed within six days of the last exposure in loaders and appliers.

[Rate of serological markers of hepatitis B and C viruses in first-time users of the hemodialysis program at Hospital Nacional Cayetano Heredia (HNCH)].

PURPOSE: Determine the rate of markers of hepatitis B (HBV) and C (HCV) viruses in patients with terminal chronic kidney failure (TCKF) prior to the start of a chronic hemodialysis program (CHD), and assess the time relation between infection and epidemiological history. MATERIALS AND METHODS: This was a prospective and analytical study. The study population was composed of the entire group of TCKF patients using the CHD program for the first time at the HNCH, Lima, from June 2002 to September 2003. HbsAg and HBV Anti-HBc, and HCV anti-VHC markers were assessed. RESULTS: 86 patients were studied, 45 female (52.3%) and 41 male (47.7%). During the study, the total rate of HBV markers was 20.9%, HBsAg(+) was 2.3%, and anti-HBcT(+) was only 18.7%. Anti-HCV rate was 4.65%. Related factors for potential carriers of only anti-HBcT(+) were: elder age, sexual intercourse with prostitutes OR=6.1 (1.5-25.3), food consumption at restaurants OR=5.2 (1.6-16.4), or being born in the jungle area OR=6.7 (1.5-30.5). Multi-variance analysis showed that only elder age OR=1.03 (1.00-1.06), being born in the jungle area OR=13.1 (1.8-91.1), and food consumption in restaurants OR=5.0 (1.4-18.0) were related to total anti-HBc count. CONCLUSIONS: The study results suggest a low rate of serological markers of HBV and HCV in TCKF patients using chronic hemodialysis treatment for the first time at HNCH.
