The study aimed to describe self-assessed disability, using the Nottingham Health Profile (NHP), in patients with arthrosis of the hip joint, to test the reliability of the Swedish version of the NHP, and to compare disability perceived by the patient with the Charnley/d'Aubignet score. The NHP was administered twice to 73 patients, waiting for total hip replacement. Pain, energy, sleep, and mobility were severely affected. The correlations between subjective and objective scores were, apart from pain, moderate. The two test administrations correlated highly. In conclusion, the NHP is a reliable instrument, which may provide valuable supplementary information in the assessment of disability.
Insufficient understanding of tribological behaviour in total joint arthroplasty is considered as one of the reasons for prosthesis failure. Contrary to the continuous motion input profiles of hip simulators, human locomotion contains motion interruptions. These occurring resting periods can cause stick phenomena in metal-on-metal hip joints. The aim of the present study was to investigate the tribological sensitivity of all-metal bearings to motion interruptions on in vitro test specimens and retrieved implants. Friction and wear with and without resting periods were quantified. Unlike the metal-on-polyethylene joints, the static friction of metal-on-metal joints increased up to micros = 0.3 with rest, while wear appeared to be unaffected. This effect is caused by the interlocking of firmly adhered carbon layers, which were generated from the protein-containing lubricant through tribochemical reactions. Since more than 80 per cent of the retrieved implants exhibited macroscopically visible carbon layers, the increase in friction presumably also occurs under physiological conditions, which is then transferred to the bone-implant interface. These recurrent tangential stress peaks should be considered for the design features of the cup-bone interface, in particular when larger-sized implant heads are used.
The postoperative x-rays of 34 patients with rheumatoid arthritis or spondylitis who had received a cement-free fixed total hip endoprosthesis were examined. The postoperative period ranged from 3 months to four years. In the region of the acetabular component an increasing rate of osseous integration in relation to the postoperative time was observed. Adaptive modeling changes in the area of the femoral stem were found as radiolucent lines parallel to the implant. Also a densification of cancellous bone and cortical hypertrophy as well as cortical rarefaction was noticeable. The bone in rheumatoid arthritis or spondylitis reacts to the implantation of a cementless fixed total hip prosthesis similar as the bone in osteoarthritis.
PURPOSE OF THE STUDY: The authors deal with the issues of loosening of Walter cementless total hip replacement. The intention was to develop the curve of survival of components of the implant and analyze the causes of their failure. MATERIAL: On the group of 127 patients on between 1988 and 1991 with the average follow-up of 9.4 years the authors operated analyze results of this type of implant which was applied in the whole main spectre of orthopaedic indications. METHODS: Based on the results survival curves were drawn by the method after Kaplan-Meier and they were compared from different views. RESULTS: Loosening requiring revision was recorded in 33 cases, a marked decline of the curve was evident after the fifth year. Ten-year survival was recorded in 66% of cups. The comparison of the implant survival diagram in patients operated on by different surgeons revealed the substantial impact of the surgical technique on the implant survival. Loosening of the stem was recorded in 16 cases. More than 80% of stems have survived ten years. DISCUSSION: As potential causes of the failure of the cup the authors consider errors in the surgical technique, excessive pitch of cup thread and material composition of the used alloy from which the primary series of cups was manufactured. The error was also caused by the indication of a conical cup in cases of oval defects solved by the combination of the cup and a solid homogenous bone graft (21% of loosened cups). In stem loosening the cause again consists in incorrect surgical technical and material composition of the used alloy. A marked improvement of the survival curve may be achieved by the stem surface finish in the form of a combined plasmatic hydroxyapatite coating. CONCLUSION: Results obtained from the preceding model of the Walter cup led to the change of the design of both the cup and the stem. The original titanium alloy VT6 was replaced by Tl6A14V ELI (Extra Low Interstitial) according to a more strict ASTM F136 standard. Osteointegration of both components has been improved by porous titanic surface combined with hydroxyapatite.
Based on his own experimental findings, the author provides strong evidence for negative consequences of the deranged vascular network of haversian systems while screwing the screw pedicles of hip joint endoprostheses into the endostalic margin of the cortical layer of the diaphysis. It is shown that it is not advisable to use a endoprosthetic assembly unit in the intertrochanteric area for biomechanical reasons. The latest endoprosthesis of the author's own design, which has a monolithic transition of the neck into the pedicle, is described, with emphasis on driving the endoprosthetic screw pedicle into the diaphysis rather than on its screwing. The prosthesis has a pedicle with a specific trapezoidal thread and a knock-down mobility unit.
1. The activity of slowly adapting joint receptors was recorded from fibers of the posterior articular nerve of the hip in deeply anesthetized cats. The static stimulus was any position of the femur maintained at least 2 min after passive displacement. Most of the fibers are active in all the positions where the femur may be placed. Modulation in the discharge frequency occurs in any axis of displacement, and higher activity is recorded at extreme positions. The maximal discharge may be reached in several positions of the femur far away from one another. 2. Adaptation to static stimuli occurs in all the receptors. Joint receptors fire at very regular, steady rates; the mean coefficient of variation was 0.079. 3. In most of the receptors, when the femur is displaced along a single axis, the frequency of discharge varies as a monotonic function of joint position. This relationship could be described either by linear and logarithmic or by linear and power functions, according to the criteria adopted to indicate the intensity of the stimulus. 4. Repetition of identical static stimuli results in different responses. 5. The static properties of the receptors are not modified by the section of periarticular muscles. 6. Hip joint receptors are able to code hip joint position.

In the years 1978-1986 the authors implanted 207 total prostheses in patients with the fracture of the femoral neck or after the fractures in the area of the hip joint. 50 patients were operated on for an acute fracture of the neck. The surgical technique in these indications does not differ on principle from the surgical technique of coxarthrosis. They point out a higher number of some peroperative complications. They checked 141 patients with and average follow-up of 47 months. In 4.2 per cent of late complications they reported roentgenological symptoms of loosening. Reoperation because of the loosening of the acetabular component was inevitable in 2.9 per cent of patients. All these cases fall into the group of patients in which the total hip arthroplasty was performed after the fractures of the acetabulum. In no patient it was necessary to reoperate the femoral component. Within the check up the patients underwent roentgenological examination and the result was assessed by the method according to Harris. In total, excellent result was reported in 43.2 per cent of patients, good result in 29.8 per cent, fair result in 14.2 per cent and poor result in 12.8 per cent of patients. With regard to a low number of complications and very good results of the evaluation according to Harris the authors consider total hip arthroplasty in the treatment of post-traumatic conditions for fully indicated. Total hip arthroplasty is indicated also in acute fractures of the femoral neck in which it brings in a short time interval good functional results with minimum complications. A group at risk is represented by patients operated on after the fractures of acetabulum.
An evaluation of the treatment results of Salter osteotomy in 21 patients aged 3.7 years with unilateral congenital subluxation of the hip joint was made. After one-year-follow-up, the treatment result was excellent in all the cases. An analysis of the X-ray pictures revealed that the peripheral fragment of the osteotomized bone was displaced 4 mm laterally, acetabular roof inclination (acetabular index) decreased about 4 mm on the average. In consequence, the length of the acetabular roof increased about 11 mm. The authors think that Salter osteotomy in children aged over 3 years is recommended if the acetabular roof inclination angle/does not exceed 45 degrees; over this value and especially when the roof is short, Dega transiliac osteotomy should be performed.
