[Tumor of the omentum in 2 patients with a large, mobile mass in the upper abdomen].

A mobile mass in the right upper abdomen was felt in an 81-year-old woman with nausea and a 45-year-old man with exertional dyspnoea. Radiological investigations did not reveal sufficient information, whereupon laparotomies were carried out. In both cases, a tumour was found at the exit of the greater omentum. Following resection, the tumour recurred in the woman five months later, and she died as a result; the man made a rapid recovery and remained tumour-free two years later. Tumours originating in the omentum majus are rare. The presentation of these tumours is late, due to late presentation of complaints. These tumours are usually only detected once they reach a diameter of more than 10 centimetres. In general, it is difficult to establish whether the tumour is benign or malignant either pre- or peri-operatively. Surgery is the only curative option. A wide resection is preferable, with resection of any (parts of) organs which may be involved.

Cocoon abdomen in a liver transplant patient.

Cocoon abdomen is a rare cause of intestinal obstruction and its pathogenesis remains unclear. This is probably the first report of the development of such a condition in a liver transplant recipient. The management of this patient is discussed and the current literature is reviewed

[Examination of the organs in the upper abdomen with ultra sound (author's transl)].

Ultrasonography is a harmless reproducibel technique with its own specific indications, an established part of clinical diagnosis in obstetrics, gynecology, ophthalmology, neurology and cardiology. Investigations on the organs of the upper abdomen have greatly extended the indications. Interpreting the sonograms requires experience in providing and evaluating the picture. In part I of this paper normal ultrasonic findings on liver, pancreas, spleen and kidneys are shown and explained on the basis of the physical characteristics of ultrasound techniques and the various ultrasoundtopographic data. This echographic normal anatomy of the organs mentioned enables us to understand pathologic changes shown in part II. An attempt is also made to allocate and evaluate ultrasonography among other diagnostic techniques.

[Impact of blood loss on lipid peroxidation and on the antioxidative system in victims with isolated injury to the chest and abdomen].

The paper deals with the impact of the volume of blood loss on the time course of changes in the parameters of lipid peroxidation (LPO) and the antioxidative system (AOS) in the sera of 22 and 31 victims with isolated chest and abdominal injuries, respectively, on days 1, 3, 7, and 14 days after injury. All the victims were found to have enhanced LPO processes and altered AOS. The severity of disorders in the LPO-AOS system depends on the size of blood loss, the disorders manifest themselves to a greater extent with over 2-liter brisk bleeding and enhance on days 7 and 14 after injury. In mild blood loss equal to as low as 1.0 liters, there was a mobilization of AOS components, such as (-tocopherol and ceruloplasmin, to the blood bed in the victims with both isolated chest and abdominal injury. AOS failure remaining during all periods of the study was detected in victims with abdominal organs and over 2-liter blood loss. Disorders in the LPO-AOS system were pronounced in abdominal injury. Multimodality treatment in victims with isolated injury of the chest and abdomen does not normalize impaired LPO processes and AOS, which makes it necessary to include into antioxidative therapy for brisk bleeding.

[Spiral computed tomography in the diagnosis of the abdomen gunshot wounds and their complications].

The results of studies by spiral computed tomography (SCT) in 62 patients with abdominal wounds were analyzed at the Main Military Clinical Hospital of Internal Forces, Ministry of Internal Affairs of Russia. The main contingent of the wounded included active servicemen who had been wounded in the areas of local conflicts, military men. The studies were conducted on days 2 to 15 of their receiving a wound. The potentialities of the technique in evaluating damages to abdominal organs, retroabdominal space, bony structures, in assessing the topography of foreign bodies are described. SCT was compared with classical X-ray and ultrasound studies, laparocentesis which are traditionally used in the diagnosis of gunshot injury. In the authors' opinion, SCT has become the method of choice in the diagnosis of gunshot injury to the abdomen and its complications today.

[Evaluation of volume and degree of blood loss in trauma of the chest and the abdomen].

A mathematical method based on a complex of capillary blood examinations was proposed and tested for detection of volume and degree of blood loss in trauma of the chest and the abdomen. This method may be used for optimization of intraoperative fluid management. It is very important when bleeding is not only into the serous cavities (blood loss is easy to evaluate during surgery) but also into the intermuscular and interstitial spaces, and also in internal hemorrhage.

[Ultrasonics in diagnosis of the upper abdomen].

On the basis of seven cases of suspected liver pathology, an albeit modest contribution has been made to the significance and diagnostic possibilities of ultrasonographic investigation of the upper abdomen. In four cases, ultrasound findings were confirmed by scintigraphy and in three cases by necropsy and histological reports.

Pain in abdomen--do not forget lead poisoning.

Lead toxicity has been recognized for thousands of years, and is still around. We encountered 11 patients with lead toxicity in the last two years. All patients had presented with diffuse pain in the abdomen, anemia and mild derangements of liver biochemistry. History of intake of indigenous or herbal medicine for diabetes mellitus or psychosexual disorders was present in eight patients. All of them had elevated blood lead levels. Abdominal pain responded promptly to treatment with chelating agents.

Abnormal 67Ga-citrate scan of the abdomen in tuberculous peritonitis: case report.

Tuberculous peritonitis in a 34-year-old alcoholic man was associated with an abnormal 67Ga-citrate scan of the abdomen. Repeated studies after thorough bowel cleansing revealed no change in the site and shape of the abnormality for 2--5 days after injection of the tracer. The inflammatory process may have been responsible for the abnormal scan.

Heat penetration in locally applied hyperthermia in the abdomen during intra-operative hyperthermic intraperitoneal chemotherapy.

BACKGROUND: Hyperthermic intraperitoneal chemotherapy (HIPEC) with mitomycin C has been applied following cytoreductive surgery for various peritoneal surface malignancies. The aim of this study was to evaluate heat penetration in the abdomen during 10 HIPEC procedures. MATERIALS AND METHODS: Temperature measurements were taken at 5 levels through the abdominal wall. Core temperature and room temperature were also measured. The relationships between the temperature gradient and room or core temperature were studied. RESULTS: At the start of perfusion, the temperature was estimated on average to be 40.6 degrees C at the first level, then it decreased by 1.7 degrees C (SD 1.0 degree C, p = 0.0001) in the first mm. In outward direction, it decreases by a further 1.5 degrees C per cm (SD 0.3 degree C/cm, p < 0.0001). The core temperature influenced the temperature gradient; the room temperature was not found to be a significant factor. At the end of perfusion, the temperature is estimated on average to be 40.1 degrees C at the first level, then it decreased by 0.8 degree C (SD 0.7 degree C, p = 0.011) in the first mm. In an outward direction, it decreased by a further 1.7 degrees C per cm (SD 0.4 degree C/cm, p = 0.0001). No evidence of an association between the temperature gradient and the room temperature or the core temperature was observed. CONCLUSION: Hyperthermia used during HIPEC procedures has a limited penetration depth. The slope in temperature seems to be related to the core temperature.

[Gunshot wounds of the chest and abdomen in peacetime].

320 gunshot wounds of the trunk in peacetime are analyzed. Wounds from hunting rifle (71.2%) dominated, penetration into pleural or abdominal cavity was revealed in 89.4% cases. Thoracoabdominal and combined wounds of the chest and abdomen reached 23.8%. Significant destruction of the soft tissues and bones, multiple damages of organs were seen often. Causes of infectious complications and lethal outcomes were analyzed. Improving of care quality has reduced lethality from 21.8 to 15.6%.

[Functional analysis of the muscles of the anterolateral wall of the abdomen using electromyocartography].

The purpose of this study was to analyze the function of the superficial muscles of the anterolateral wall of the abdomen in dorsal decubitus by an original technique: electromyocartography. This technique attempts to monitor the individual or inter-subjective variations that complicate quantitative analysis of the overall electromyogram with integration of action potentials. It is shown that relative quantification of the areas explored with respect to the most active area considerably improves the consistency of the results. The most dependable electrode locations for the analysis of the abdominal muscles are determined. From a functional point of view: The rectus abdominis is active in flexion of the cranial portion of the trunk, either alone or in combination with right or left rotation of the trunk. Its activity is less, and predominates in the distal portion of the muscle, in flexion of the caudal portion of the trunk in combination with a 15 degree flexion of the hips. The obliquus internus abdominis, which can be explored only in a single area, is especially active in rotation. The obliquus externus abdominis plays a major role in stabilizing the pelvis when the lower limbs are involved in the movement of the trunk.

[Laparoscopy in penetrating wounds of the abdomen].

Within the recent 5 years, the authors observed 103 sufferers with penetrating injury to the abdomen. Operated on were 79 patients. Of them, 39 underwent explorative laparotomy. Laparoscopy was performed in obscure diagnosis in sufferers with associated trauma who were unconscious, in those with the signs of alcohol intoxication, in drug addicts etc. The advantages of the use of laparoscopy in the diagnosis, and in some patients--in the treatment of injuries were noted.

Acute intermittent [corrected] porphyria. An often forgotten diagnosis in acute abdomen.

Three cases of Acute Intermittent Porphyria (AIP) are described. All presented with acute intermittent abdominal pains. One had grand-mal epilepsy as well. Two were diagnosed by chance. In the third case the diagnosis was thought of. It is suggested that AIP should always be considered as one of the differential diagnosis in Acute Abdomen in West Africa.

[Significance of the autoimmune component in the genesis of inflammatory "tumors" of the abdomen].

Data concerning the studies on autoimmune processes in 110 cases of inflammatory "tumors" of the abdomen and its anterior wall are set forth in this article. With this goal, pathomorphological immunomorphological studies of inflammatory "tumors" specimens, a dynamic study of the reaction intensity of the complement fixation (RFC) with the tissue antigen and also the immunoglobuline serum level together with eosinophylic reaction of the organism were carried out. The dynamic study of the fixation of the complement reaction, immunoglobuline level and eosinophylic reaction of the body permits to make a more complete judgement on the degree of the pathological process activity and on the postoperative clinical course.
