Positive youth development linked with prevention in a Vietnamese American community: successes, challenges, and lessons learned.

PURPOSE: To describe a positive youth development program that was successfully designed and implemented in partnership with a Vietnamese American community and to provide insight into the types of outcomes that can result from such programs in similar communities. METHODS: In-depth interviews were conducted with Vietnamese community leaders (n = 8) and youth (n = 10) who were involved in a positive youth development program in Oklahoma City. Interviews were tape recorded, transcribed verbatim, and coded by two independent analysts. Data matrices were used to help identify major themes. RESULTS: Participants felt they had strengthened numerous assets as a result of involvement in the youth development program, including communication with parents or another adult, leadership skills, constructive use of free time, and positive peer influences. Some of the participants also reported establishment of a relationship with a nonparent adult, inclusion of community service in their aspirations for the future, and avoidance of high-risk behaviors owing to their involvement in the program. Both adults and youth felt that the Vietnamese community had changed its perception of youth as a result of increased involvement of young people in community service and cultural activities. CONCLUSION: Positive youth development is a promising prevention strategy that can be adapted to Vietnamese communities in the United States.

Relating landscape characteristics to non-point source pollution in mine waste-located watersheds using geospatial techniques.

Landscape characteristics of a watershed are important variables that influence surface water quality. Understanding the relationship between these variables and surface water quality is critical in predicting pollution potential and developing watershed management practices to eliminate or reduce pollution risk. To understand the impacts of landscape characteristics on water quality in mine waste-located watersheds, we conducted a case study in the Tri-State Mining District which is located in the conjunction of three states (Missouri, Kansas and Oklahoma). Severe heavy metal pollution exists in that area resulting from historical mining activities. We characterized land use/land cover over the last three decades by classifying historical multi-temporal Landsat imagery. Landscape metrics such as proportion, edge density and contagion were calculated based on the classified imagery. In-stream water quality data over three decades were collected, including lead, zinc, iron, cadmium, aluminum and conductivity which were used as key water quality indicators. Statistical analyses were performed to quantify the relationship between landscape metrics and surface water quality. Results showed that landscape characteristics in mine waste-located watersheds could account for as much as 77% of the variation of water quality indicators. A single landscape metric alone, such as proportion of mine waste area, could be used to predict surface water quality; but its predicting power is limited, usually accounting for less than 60% of the variance of water quality indicators.

Factors affecting detoxification readmission: analysis of public sector data from three states.

The objective of this study was to understand the rate of detoxification readmissions and the factors associated with readmission within a public sector population. The study sample was drawn from an integrated database that includes Medicaid and state mental health and substance abuse agency data from three states (Delaware, Oklahoma, and Washington) for 1996-1998. Clients with at least one state agency-sponsored detoxification event in 1996 or 1997 were included in the study. Twenty-seven percent of the sample was readmitted for detoxification within 1 year of their index detoxification. Clients who received two or more substance-abuse-related services within 30 days of their index detoxification were less likely to be readmitted and had a longer time until their second detoxification admission. Detoxification readmission is common in the public sector. Engaging patients in treatment following detoxification may reduce readmission rates and time to readmission.

Impact of geochemical stressors on shallow groundwater quality.

Groundwater monitoring wells (about 70 wells) were extensively installed in 28 sites surrounding Lake Texoma, located on the border of Oklahoma and Texas, to assess the impact of geochemical stressors to shallow groundwater quality. The monitoring wells were classified into three groups (residential area, agricultural area, and oil field area) depending on their land uses. During a 2-year period from 1999 to 2001 the monitoring wells were sampled every 3 months on a seasonal basis. Water quality assay consisted of 25 parameters including field parameters, nutrients, major ions, and trace elements. Occurrence and level of inorganics in groundwater samples were related to the land use and temporal change. Groundwater of the agricultural area showed lower levels of ferrous iron and nitrate than the residential area. The summer season data revealed more distinct differences in inorganic profiles of the two land use groundwater samples. There is a possible trend that nitrate concentrations in groundwater increased as the proportions of cultivated area increased. Water-soluble ferrous iron occurred primarily in water samples with a low dissolved oxygen concentration and/or a negative redox potential. The presence of brine waste in shallow groundwater was detected by chloride and conductivity in oil field area. Dissolved trace metals and volatile organic carbons were not in a form of concentration to be stressors. This study showed that the quality of shallow ground water could be related to regional geochemical stressors surrounding the lake.

Understanding childhood lead poisoning preventive behaviors: the roles of self-efficacy, subjective norms, and perceived benefits.

INTRODUCTION: Understanding individual and social influences on behaviors commonly recommended to prevent lead poisoning in children can guide more effective educational interventions. METHODS: In-person interviews were conducted with primary caregivers (n = 380) of American Indian and White children aged 1 to 6 living in or near the Tar Creek Superfund site in northeastern Oklahoma. Caregivers' perceived health benefits, self-efficacy, and subjective norms were assessed for four lead poisoning prevention behaviors (i.e., annual blood lead testing, playing in safe areas, washing hands before eating, and dusting with a damp cloth). RESULTS: Caregivers spoke with their own mothers, spouses, and other female family members most often when they had concerns or worries about taking care of their children. In multivariate models, subjective norms, perceived benefits, and self-efficacy were positively associated with the hand-washing and damp-dusting behaviors, while only self-efficacy was associated with playing in safe areas. None of the variables were found to have significant influence on the blood lead testing behavior. CONCLUSIONS: Education programs should address individual level factors such as self-efficacy and perceived health benefits but also consider new strategies that incorporate a normative dimension to lead poisoning prevention.

Factors contributing to the amount of vehicular damage resulting from collisions between four-wheel drive vehicles and passenger cars.

This study examines the vehicular damage resulting from motor vehicle crashes involving four-wheel drive vehicles and passenger cars in the state of Oklahoma. In particular, the focus of the analysis is on differences in vehicular damage to passenger cars and four-wheel drive vehicles. Results indicate that passenger cars sustain significantly greater vehicular damage than four-wheel drive vehicles. In addition, several other factors significantly influence the level of damage resulting from collisions between four-wheel drive vehicles and passenger cars. Driver behavior or unsafe acts, represented by the rate of travel prior to the collision, failure to yield, failure to obey a stoplight or a stop sign, the consumption of alcohol and the use of drugs, also contributed to the amount of vehicular damage. In addition, results also indicate that the level of damage was influenced significantly by environmental factors, represented by a reduced intensity of light, wet or slippery roadways and the type of collision. The findings reinforce the importance of a number of policy initiatives that may reduce the vehicular damage resulting from collisions involving four-wheel drive vehicles and passenger cars. For example, the study indicates a need to initiate legislation that lowers the speed limit during dark and twilight hours, commits additional resources to road maintenance to reduce unsafe road conditions, and stimulates improvements in automotive design that provide better lateral protection to vehicles.

Factors associated with the likelihood of injury resulting from collisions between four-wheel drive vehicles and passenger cars.

The specific effects of vehicular type on the likelihood of an injury occurring are relatively unexplored. This study sought to assess the relative risk of injury to occupants of four-wheel drive vehicles and their counterparts in passenger cars. Data for 1143 occupants from all of the 454 crashes in Oklahoma, in 1995 that involved a four-wheel drive vehicle were used. Multiple logistic regression analysis determined the association between potential predictive factors and vehicular injury. Odds ratios revealed occupancy in a passenger car to be a major predictor of the likelihood of injury. Other factors include the driver being female, driving too fast, travel on curved or level roadways, and being hit laterally or from the rear.

There are at least three genetically distinct small piroplasms from dogs.

The 18S nuclear subunit ribosomal RNA (18S rRNA) gene of small piroplasms isolated from dogs from Okinawa (Japan), Oklahoma, North Carolina, Indiana, Missouri, and Alabama, was isolated and sequenced. Phylogenetic analysis of these sequences and comparisons with sequences from other Babesia, Cytauxzoon, and Theileria species revealed that all canine small babesial isolates, with the exception of isolates from California and Spain, were placed in a group containing the Babesia spp. sensu stricto. Within the Babesia spp. sensu stricto, there was support for separating the small canine piroplasms from the large canine piroplasm, Babesia canis. The isolate from California was in a distinct phylogenetic clade, closely related to babesial isolates from wildlife and humans from the Western US. The canine isolate from Spain was closely related to Babesia microti. These results suggest that there are multiple small piroplasm species in dogs. The isolates from the Midwestern and Eastern US and the one from Japan probably represent a single species with wide geographic distribution.

Therapeutic choices made by patients with end-stage cancer.

The Oklahoma Children's Memorial Hospital Oncology Service has developed a new approach for its patients with end-stage cancer. The emphasis is on the patients' acknowledgement of the progression of the disease and the imminence of death, and informed choice between the use of research drugs and no further chemotherapy. Forty-three patients between 6 and 20 years of age participated in a conference of this type. The results demonstrated that they understood that their health was declining. They decided about further therapy autonomously or together with their family. The majority of children who chose supportive care without chemotherapy and lived for a period of time, participated in activities at home as far as their state of health permitted. Severe depression and severe behavioral problems occurred rarely. There was evidence that this open approach enabled patients and family members to communicate openly with each other.

Quality and cost control of home care services through coordinated funding.

The growing cost and demand for home care has resulted in increased concern over the quality and cost-effectiveness of care. In Tulsa, Oklahoma, local home care funders reduced the cost and enhanced the quality of home care through a system coordinating the funding and assessment of care. The foundation of this system was the funders' agreement on common definitions of services and service standards and their adoption of a uniform unit-cost reimbursement rate. Another aspect of the coordination was a joint Request for Proposals process that set out specified criteria for home care agencies seeking funds. A home care assessment team was established by Tulsa's local funders to perform ongoing monitoring and evaluation of the home care agency selected to receive funds. As a result of the coordinated funding and assessment system, the quality and efficiency of home care improved measurably, and community access to care increased.

Bilateral root or root canal aberrations in a dental school patient population.

Clinicians have often noted that aberrant root morphology in a given tooth is also observed with varying degrees of frequency in the corresponding contralateral tooth. The purpose of this study was to determine the proportion of bilateral morphological root aberrations in a random sample of adult human dentition. Five hundred one dental records were selected from the retired record section at the University of Oklahoma College of Dentistry and their full-mouth radiographs were reviewed for aberrant root canal morphology. Bifurcation of the canal in mandibular first premolars was the most common finding (22.8% of patients), with 60% of these being bilateral. Maxillary anterior teeth had the least aberrations. It was observed that unusual root morphology is bilateral approximately 60% of the time. Therefore, the incidence of root or root canal abnormalities reported by the percentage of patients involved will always exceed the incidence of abnormalities reported by type of tooth (e.g. mandibular or canine) involved except for abnormalities which are found bilaterally in 100% of the patients studied. Radiographic interpretation appears to result in a lower incidence of anatomical aberrations than direct identification. The more rare the aberration, the more likely it is to be bilateral in occurrence.

The diagnosis of diabetic retinopathy. Ophthalmoscopy versus fundus photography.

PURPOSE: To compare fundus photography with ophthalmoscopy in the detection of diabetic retinopathy. METHODS: Ophthalmoscopy and fundus photographs with a nonmydriatic camera, both performed through dilated pupils, were compared to diagnose retinopathy in a cohort of 410 Oklahoma Indians with noninsulin-dependent diabetes mellitus. A total of 795 eyes were examined using both methods. The mean age of participants was 60.3 years, with a mean duration of diabetes of 17.3 years. RESULTS: An overall agreement of 86.3% with a kappa statistic kappa of 0.74 was found between ophthalmoscopy and fundus photography with a nonmydriatic camera. For the diagnosis of proliferative diabetic retinopathy, kappa = 0.84 with an agreement of 98.1%. With a total of 61 cases of proliferative retinopathy diagnosed by either method in our study, ophthalmoscopy alone detected 88.5% and fundus photography, 78.7%. When compared on a lesion-by-lesion basis, agreement between the two diagnostic methods was highest for nonproliferative retinopathy, as well as fibrous proliferation. CONCLUSION: The fundus photography with a nonmydriatic camera, performed with mydriasis, is comparable to ophthalmoscopy for the detection of retinopathy. It may prove to be a suitable, cost-effective method for routine screening in diabetes clinics, provided ophthalmologic referral is ensured for those with a diagnosis of any form of retinopathy, questionable retinopathy, nondiabetic retinopathy, those with poor quality photographs, as well as those with acute changes in visual acuity.

Unmarried black adolescent fathers' attitudes toward abortion, contraception, and sexuality: a preliminary report.

The attitudes of a limited population of unmarried black adolescent fathers toward abortion, contraception, and sexuality were assessed using standardized data collected in Tulsa, Oklahoma, Chicago, Illinois, and in Columbus, Ohio. A majority of the surveyed fathers in each city surveyed, were opposed to abortion. Two-thirds indicated a willingness to share contraceptive responsibility with the female. These data suggest that if family planning agencies offered sex education programs for adolescent fathers, particularly in relation to contraceptive responsibility, the incidence of repeat unwed adolescent fatherhood might be reduced.

Immunotoxicity risks associated with land-treatment of petrochemical wastes revealed using an in situ rodent model.

Land-treatment of petrochemical wastes is a widely used method to dispose of hazardous and non-hazardous waste by biodegradation. However, no comprehensive assessment of the impact of such disposal techniques on terrestrial ecosystems has been conducted. Despite the presence of suspected immunotoxicants in the soil, wild rodents frequently reside on these waste sites after closure or abandonment. We explored the seasonal sensitivity of the immune system of the hispid cotton rat (Sigmodon hispidus) to in situ exposures on sites land-treated with petrochemical wastes. Animals were monitored on five contaminated land-treatment sites and five ecologically matched-reference sites in Oklahoma, USA, over two seasons (summer and winter). Most hematological parameters were not adversely affected by land-treatment; however, platelet counts were 26% greater in cotton rats from land-treatment sites compared to reference sites in winter. Significant treatment-related differences were observed in total serum protein concentrations, organ mass and organ cellularity, but these differences were not consistent across the five land-treatment units. Lymphoproliferative responses of cotton rat splenocytes stimulated in vitro were elevated for a T-cell mitogen and depressed for a B-cell mitogen in animals from land-treatment compared to reference sites. The ability of splenocytes to proliferate in response to interleukin-2 receptor-binding was not influenced by treatment. Total yields of peritoneal cells, yield of peritoneal macrophages, and yield of peritoneal lymphocytes were influenced to varying degrees by land-treatment. Functionally, in vitro metabolic activity of peritoneal macrophages was 114% greater in cotton rats from land-treatment sites compared to reference sites during summer. These results indicate that petrochemical wastes applied to soils on these five land-treatment sites had variable immunomodulatory effects in resident cotton rats. Immune alterations for some assays were indicative of enhancement on some land-treatment sites while suppressive on other land-treatment sites, which could have been a function of type and concentration of immunotoxicants present on each site and highlights the uniqueness of each land-treatment site.

Water quality at five marinas in Lake Texoma as related to methyl tert-butyl ether (MTBE).

Water quality in five marinas on Lake Texoma, located on the Oklahoma and Texas border, was monitored between June 1999 and November 2000. Focus was to evaluate lake water associated with marinas for methyl tert-butyl ether (MTBE). Lake water was collected at locations identified as marina entrance, gasoline filling station, and boat dock. Occurrence of MTBE showed a direct seasonal trend with recreational boating activity at marina areas. There was a positive correlation with powerboat usage ratio, which was directly related to the gallons of gasoline sold. Sampling before and after the high boat use holiday weekends determined the apparent influence of powerboat activity on MTBE contamination. Boat dock locations were the most sensitive sites to MTBE contamination, possibly due to gasoline spillage during engine startup. The most common compound of the BTEX series found with MTBE was toluene and co-occurrence was most frequent at gasoline filling stations.

Groundwater quality surrounding Lake Texoma during short-term drought conditions.

Water quality data from 55 monitoring wells during drought conditions surrounding Lake Texoma, located on the border of Oklahoma and Texas, was compared to assess the influence of drought on groundwater quality. During the drought month of October, water table levels were three feet (0.9 m) lower compared with several months earlier under predrought climate conditions. Detection frequencies of nitrate (> 0.1 mg/l), orthophosphates (> 0.1 mg/l), chlorides (> MCL), and sulfates (> MCL) all increased during drought. Orthophosphate level was higher during drought. Largest increases in concentration were nitrate under both agriculture lands and in septic tank areas. An increase in ammonium-nitrogen was only detected in the septic tank area. The study showed that stressors such as nitrate and total salts could potentially become a health or environmental problem during drought.

The development and use of a computerized database for the evaluation of facial fractures incorporating aspects of the AAOMS Parameters of Care.

PURPOSE: This article discusses the development and use of a computerized database to evaluate facial fracture patients. Examples of epidemiologic and treatment outcome analyses that can be performed are also discussed. MATERIALS AND METHODS: FileMaker Pro 2.1 and 3.0 (Claris Corporation, Santa Clara, CA) for the Macintosh (Apple Computer, Inc, Cupertino, CA) was used for the development of the database. The database contained information on the facial fracture patients treated at The University of Oklahoma Health Sciences Center by the Oral and Maxillofacial Surgery service between January 1, 1994 and December 31, 1996. Eight evaluation forms were used: general information, and mandibular, maxillary, zygomatic, nasal, naso-orbital-ethmoid, orbital, and frontal sinus fractures. Indications for therapy and postoperative complications from the AAOMS Parameters of Care, Section on Trauma Surgery, were also included. RESULTS: This database allowed collection of a vast amount of data on 265 patients. Some of the analyses done on patients with mandibular fractures are described. CONCLUSION: This computerized database provides a quick and systematic method of obtaining and retrieving information on facial fracture patients. Numerous epidemiologic and treatment outcome analyses can be performed. Overall complication rates based on the AAOMS Parameters of Care are higher than previously published rates because of the longer list of complications being evaluated.

Apolipoprotein E4 and tau allele frequencies among Choctaw Indians.

Apolipoprotein genotyping and tau haplotyping were carried out on a series of cases with dementia and controls from the Choctaw Nation of Oklahoma. Both the Apolipoprotein E4 allele frequency and the tau H2 haplotype frequency were low in the Choctaw compared with Caucasians and there was the possibility that the association between dementia and the E4 allele was weaker than in Caucasians.
