Endovascular stent graft treatment of a traumatic aortocaval fistula.

Aortocaval fistula (ACF) is an infrequently reported sequela of trauma. Most ACF have been repaired via an open approach. During the past 10 years, there has been one reported case of spontaneous ACF and two cases of traumatic ACF repaired using an endovascular technique. We present a third case of traumatic ACF repaired with an endovascular stent graft. A 40-year-old male sustained two gunshot wounds to the right chest and one to the right upper abdomen. He was taken from the emergency department directly to the operating room, where an exploratory laparotomy was performed. Through-and-through injuries to the stomach and transverse colon were repaired primarily. Subsequently, the patient developed abdominal compartment syndrome. An urgent exploratory laparotomy was performed, revealing a nonbleeding hematoma on the posterior lateral surface of the right lobe of the liver, which was left undisturbed. Open abdominal management was instituted with vacuum pack closure. On the nineteenth hospital day, the patient again had a significant decrease in hematocrit. An aortogram was performed in order to evaluate the patient for intrahepatic arterial bleeding amenable to transcatheter embolization. There was no evidence of hepatic arterial bleeding. However, a supraceliac ACF was identified. The patient was taken to the operating room, and an AneuRx aortic extension cuff was advanced under fluoroscopy and deployed to cover the fistula. Completion angiography revealed total obliteration of the ACF and appropriate placement of the stent graft. Postoperatively, the patient was returned to the intensive care unit, where his hospital course was complicated by ventilator-associated pneumonia and sepsis. Repeat computed tomographic scanning 6 months and 1 year following this repair demonstrated patency of the graft without evidence of graft migration or aortocaval communication. Further research and experience are necessary with this technique regarding long-term outcome and technical aspects. In particular, the sizing problems associated with repair of acute traumatic ACF in emergency situations should be addressed. The endovascular approach provides an attractive and exciting alternative to traditional methods for repair of ACF.

Reasons for requesting emergency contraception: a survey of 506 Italian women.

OBJECTIVE: To evaluate the reason for requesting emergency contraception (EC), previous use of contraceptive methods and provision route in a Family Planning Clinic in Italy. METHODS: Women requesting EC were interviewed, through a questionnaire containing questions on demographic characteristics, about their reasons for requesting EC, their prior contraceptive use, their reasons for not using an effective contraceptive method (or possible reasons for its failure) and specifically about the so-called 'provision route' (i.e. whether and where they had previously requested EC receiving a negative response). RESULTS: Almost 70% of all women requesting EC were aged between 18 and 25 years. Some 80% of all women were in a stable relationship with their partner, with fewer than 20% having had an occasional intercourse. The vast majority of women (83%) reported prior use of a modern contraceptive method, i.e. 64% with a condom, 27% for combined oral contraceptives and 1.1% for the intrauterine device (IUD). In addition, 15% of the women had used more than one method (oral pills and condoms). Concerning the reasons for requesting EC, condom breakage or slipping was the most frequently cited (64%), followed by totally unprotected intercourse (28%), failed withdrawal (5%) and forgetting one or more pill (only 1.1%). CONCLUSIONS: More than one-third of the women interviewed had previously used an emergency contraceptive modality; although no one did so more than four times. Therefore, it can be inferred that-at least in the present series-EC had not been used as a routine contraceptive method. Finally, it seems clear that in Italy, even in large cities, information about the availability, proper usage and mechanism of action is lacking. This seems due to information being spread by word of mouth between peers and friends, with more formal communication channels lagging behind.

A data-generated basis for medical ethics education: categorizing issues experienced by students during clinical training.

PURPOSE: To use issues identified by students in order to establish an experience- and evidence-based approach to medical ethics education. METHOD: A total of 628 sophomore and senior students at the State University of New York at Buffalo School of Medicine and Biomedical Sciences were asked to identify incidents during their clinical training that had raised ethical concerns. The sophomores were surveyed during two time periods: 1979-80, and 1991-92 and 1992-93; the seniors were surveyed in 1991-92 and 1992-93. Responses were analyzed and categorized through content analysis. RESULTS: In all, 249 students (45% of the sophomores and 20% of the seniors) responded. The categories of issues identified were professional norms, limits of intervention, defensive shielding of professional colleagues, respect toward patients, communication, and student boundaries (situations where the student feels uncomfortable). The most frequently reported incidents reflected the students' perceptions of lapses in level of care (under- or over-treatment), communication, respect toward patients, and maintenance of professional norms. The seniors and the 1979-80 sophomores reported respect toward patients as an issue less often than did the 1991-92 and 1992-93 sophomores. The seniors most often identified concerns raised over limits of intervention and resource allocation. CONCLUSION: The differences between the responses of the sophomores and seniors tend to support other research suggesting a retardation of moral sensitivity in the course of medical education. It may be that clinical teaching and faculty behavior model values at odds with what is taught in the classroom. Ethics education should focus on issues relevant to students' experience.

Discussions regarding reproductive health for young women with breast cancer undergoing chemotherapy.

PURPOSE: Young women who undergo chemotherapy for breast cancer face serious consequences to their reproductive health. Research in this area has previously focused on men, or on childhood cancer survivors. We sought to explore self-report of reproductive health counseling in young women undergoing chemotherapy for breast cancer. PATIENTS AND METHODS: A total of 166 premenopausal women aged < or = 50 years were recruited from oncology offices in academic and private practices in four northeastern states, as part of a randomized controlled clinical trial aimed at stress reduction. Women were asked a variety of questions regarding diagnosis and treatment, including whether they received any counseling regarding early menopause and fertility issues. RESULTS: Sixty-eight percent and 34% of women reported recalling a discussion with a physician regarding early menopause or fertility, respectively. In multivariate analysis, hormonal therapy and early stage of disease were associated with significantly increased odds of recall of a discussion regarding menopause. Difficulty communicating with medical team was associated with increased odds of recalling a discussion regarding fertility, whereas older age and anxiety in medical situations were associated with decreased odds. CONCLUSION: Many women fail to recall discussions regarding the reproductive health impact of chemotherapy. Demographic, psychological, and disease-related variables are related to recalling such discussions. Counseling about premature menopause and fertility changes is an overlooked aspect of preparation for adjuvant chemotherapy in young premenopausal women with breast cancer. Future research should explore this issue further.

[A correlation study of perceived importance of nursing interventions with performance frequency in psychiatric nursing units using the 3th NIC].

PURPOSE: The purpose of this study was to identify the perceived importance of nursing interventions of psychiatric nurses according to domains, classes and interventions using the 3th NIC. METHOD: In this study, a 435 of 486 nursing interventions were selected from 75% consent reached by experts. Data were collected from 9 hospitals and 141 nurses(return rates : 94.0%) in Seoul, Kyonggi, Chungnam, Gwang-ju and Chonnam region from January, 2002 to February, 2002 using 4 point Likert scale. RESULT: Total perceived importance score was 2.905+/-0.463 and total correlation score with performance frequency was r=0.295. The most important perceived domain and class was found to be 'safety' (3.217+/-0.465) and 'community health promotion' (3.285+/-0.866). The most correlated domain and class with performance frequency appeared to be 'behavioral' (r=0.431, p=0.000) and 'communication enhancement' (r=0.439, p=0.000). The most important perceived nursing intervention was found to be 'active listening' (3.652+/-0.549). CONCLUSION: In conclusion, nurses in clinical settings were found to perform less than perceive its importance. Therefore, further researches are needed to identify factors related to impede and develop strategies to improve the performance of nursing interventions.

Attitudes toward the elderly among nursing students in Korea.

PURPOSE: The primary purpose of this study was to reexamine underlying dimensions of attitudes toward the elderly held by undergraduate nursing students. A secondary purpose was to investigate characteristics of nursing students associated with attitudes toward elders. METHODS: A survey was performed using self-report questionnaire completed by nursing students from a total of 10 nursing schools or departments each selected randomly from one province of Korea. Students' responses (N=366) were analyzed using factor analysis, correlation coefficients, t-test, and ANOVA. RESULTS: Three attitudinal dimensions explaining 35.29% of the variance were extracted: vitality, generosity and flexibility. From a neutral point of 4.0 in the range of 1 to 7, only generosity (4.47) was evaluated positively, whereas vitality (3.31) and flexibility (2.91) were evaluated negatively. The mean scores of the vitality and generosity dimensions were significantly different by students' level of communication with the elderly, experience living with the elderly, and interest in elders or in issues related to elders. Interest in elders/elder issues was the only characteristic affecting significant mean score difference in flexibility. CONCLUSION: Educational process should increase students' exposure to healthy elderly to modify negative attitudes toward the elderly among nursing students.

Coronary heart disease complaints against health care personnel in Finland.

This study comprised all complaints submitted from 1980 to 1990 by coronary patients and their relatives to the Finnish National Board of Health (NBH). The purpose of this study was to determine the reasons for complaints and their consequences. The complaints were coded and the material was analysed using the statistical BMDP package. Only 59 of a total of 7,516 complaints dealt with coronary disease. The majority of complaints concerned professional technical competence. The NBH found negligence in ten cases and gave administrative counselling to the professionals at fault. The negligence was not deemed serious enough to warrant any penalties or the restriction of licences. Communication problems were difficult to prove and none of these complaints resulted in counselling. Treatment of coronary heart disease seldom results in a formal complaint and negligence is found only in a minority of cases. Patient dissatisfaction can well be studied by analysing complaints, but the general quality of coronary care has to be addressed with other methods.

Human listeriosis in Scotland 1967-1988.

In order to study the epidemiology of listeriosis from 1967-1988 in Scotland, various sources of data were examined. These included reports by laboratories, reference laboratory records, hospital death and discharge records, death certificates and hospital laboratory records. Cases were reported from 13 of Scotland's 15 Health Boards. Case ascertainment via laboratory reports to the Communicable Diseases (Scotland) Unit was validated in two Health Boards. A total of 198 cases was identified with an overall attack rate which increased from 0.5 per million in 1967-1971 to 7.0 per million in 1987-1988. Feto-maternal cases were the commonest (64%). Of all cases, 33% were neonates; 53% presented with bacteraemia and 41% with meningitis. The predominant serovar of Listeria monocytogenes was 4b.

Depression: treatment compliance in general practice.

A survey of over two hundred patients attending fourteen family doctors in five different practices investigates the treatment in general practice of patients suffering from depressive illness. Medication was the principal treatment offered, but was often inadequate either because of prescription variables or because patients failed to comply with the treatment prescribed. The reasons for patient non-compliance fell into three main categories (a) side effects, (b) attitudes to drugs, (c) patient--doctor communication failure. It is suggested that a more extensive discussion of mental illness and its treatment is required as part of the total treatment of the individual, that the family doctor should take a more active role in the supervision of the treatment regimen, and that drugs with fewer side effects, particularly anticholinergic effects, are all required as essential measures to improve patient compliance with treatment.

Double-blind, placebo-controlled study of L-carnosine supplementation in children with autistic spectrum disorders.

L-Carnosine, a dipeptide, can enhance frontal lobe function or be neuroprotective. It can also correlate with gamma-aminobutyric acid (GABA)-homocarnosine interaction, with possible anticonvulsive effects. We investigated 31 children with autistic spectrum disorders in an 8-week, double-blinded study to determine if 800 mg L-carnosine daily would result in observable changes versus placebo. Outcome measures were the Childhood Autism Rating Scale, the Gilliam Autism Rating Scale, the Expressive and Receptive One-Word Picture Vocabulary tests, and Clinical Global Impressions of Change. Children on placebo did not show statistically significant changes. After 8 weeks on L-carnosine, children showed statistically significant improvements on the Gilliam Autism Rating Scale (total score and the Behavior, Socialization, and Communication subscales) and the Receptive One-Word Picture Vocabulary test (all P < .05). Improved trends were noted on other outcome measures. Although the mechanism of action of L-carnosine is not well understood, it may enhance neurologic function, perhaps in the enterorhinal or temporal cortex.

A computer method for assessing satisfaction with hearing aids.

The purpose of this study was to evaluate a new clinical assessment, the Dynamic Assessment of Hearing Aids (DAHA), for a large clinical population. Unlike traditional questionnaire methods, the DAHA has patients use an intuitive graphical computer interface to record visual analogue ratings of satisfaction with various features of their hearing aids (e.g. clarity, cost, appearance). Data were collected from 191 participants.A subset of participants returned for retest. The DAHA items assess satisfaction with hearing aids within four domains: communication, physical features, sound quality, and personal reactions. The concurrent validity was determined by comparing DAHA results to those obtained with the satisfaction with amplification in daily life (SADL). Ratings for personal reactions to hearing aids indicate the most satisfaction, and ratings for communication (especially group conversations and phone use) indicate the least satisfaction. The DAHA total score was found to have good test/retest and high internal consistency. Concurrent validity was supported by a strong correlation between total scores on the DAHA and the SADL. Results suggest the DAHA maybe an effective tool for clinical use.

Teleconsultation with the mobile camera-phone in remote evaluation of replantation potential.

BACKGROUND: The purpose of the present study was to investigate the feasibility of teleconsultation with the mobile camera-phone to transfer clinical images and communicate on line for evaluation of replantation potential in completely amputated fingers. METHODS: Teleconsultations including clinical images of the amputated portion and stump as well as patient information were transmitted between the physicians in the emergency room and the consultant phlstic surgeon through Panasonic camera-phones, which had a built-in 110,000-pixel digital camera and a 65,536 colors display. The digital images displayed on the screen were further evaluated by three remote plastic surgeons individually and the evaluations were compared with the decision made according to onsite inspection by the consultant surgeon. RESULTS: The study population consisted of 35 patients with a total of 60 digital injures occurring between January to October 2003. The ability to identify the amputation location and status of amputation kwel from remote diagnosis was demonstrated by all three surgeons in 90% and 87% of these sixty amputated digits respectively. Of the 42 digits that were considered to have replantation potential during onsite evaluation, 38 (90%) digits were considered to be so by all three surgeons in group agreement during remote diagnosis. Of the 18 digits that were not considered to be replantable during onsite evaluation, 15 (83%) digits were also deemed without replantation potential, thus making the sensitivity and specificity of recognizing digital replantation potential, 90% and 83_'O re,_1pectively. CONCLUSIONS: The camera-phone is a feasible tool for remote evaluation regarding the replantation potential of completely amputated fingers and it holds significant promise in avoiding unnecessary patient transfer by providing useful information.

Cloning of the Gene Fragment Encoding the Coenzyme-binding Domain of Yeast ADH I and Its Fusion Expression in E. coli.

In this communication we report the cloning and the over-expression of the coenzyme-binding domain of yeast alcohol dehydrogenase. A gene fragment about 400 bp encoding the NAD binding domain of yeast S. cerevisiae alcohol dehydrogenase I was obtained by the PCR method. The domain was expressed as a fusion protein by a plasmid which was constructed by inserting the PCR-amplified gene fragment into the plasmid pWR590-1 which includes E. coli Lac promoter and a truncated Beta-galactosidase gene. The fusion protein was characterized by SDS-PAGE and Western blot analysis. The yield of the fusion protein was 20% of the total E. coli protein.

THE CDC-CDCR DOCUMENTATION PROJECT.

A progress report on an experiment in mechanized methods for the storage and retrieval of communicable diseases information. The project, supported by National Institutes of Health grants and begun in 1958, is being conducted by the Center for Documentation and Communication Research (CDCR), School of Library Science, Western Reserve University, Cleveland, Ohio, in cooperation with the Communicable Disease Center (CDC), United States Public Health Service, Atlanta, Georgia. Initial phases have been completed. Seventy-seven questions submitted by the CDC Documentation Committee have been searched by CDCR on their GE-225 computer. For seventy-three of the seventy-seven questions, the system responded with at least one appropriate answer. The combined pertinent and peripheral responses totaled 94.7 percent. The file of abstracted literature is being increased preparatory to large-scale testing. Further investigation and evaluation are necessary before the project can be fairly appraised.

[Attempt to analyze the extrahospital time in managing an acute myocardial infarct patient].

The time of prehospital attendance of patients with acute myocardial infarction is analyzed for the period 1970-1983 by an emergency team according to a scheme of the stages and time for transport, communication and therapy respectively. The duration of the separate stages is determined and a conclusion is drawn that the time for communication and transport is most protracted, and the time for the therapy of a non complicated infarction--is one fourth of the total attendance time. Potentialities are looked for in order to shorten the stages by the regulation of the team, including a stretcher-bearer and a nurse, the necessity of qualification of all teams. The statistical processing of the material is presented as well as a scheme with the outcome of 230 myocardial infarctions--survival, total mortality rate and lethality, 314 patients were followed up as regards hospitalization and a correlation is presented of the members of the team and attendance time with respresentative and chronometric methods.

Physicians' strategies for safeguarding confidentiality: the influence of community and practice characteristics.

BACKGROUND: Confidentiality is an issue of particular importance in rural communities where it seems everyone knows everyone else. Privacy may be compromised when professional relationships are interwoven with personal ones. We conducted a study to determine the influence of practice characteristics on safeguarding confidentiality. METHODS: A survey was mailed to all family and general practice physicians in Kansas. Differences in confidentiality strategies were studied to determine if they were related to characteristics of the respondents and their practice environments. RESULTS: A total of 510 physicians (55%) responded to the survey. Only in practices in which more than 5% of the patients were personal acquaintances was a strategy for preserving confidentiality affected: of seven specific strategies, only one, speaking with office personnel about confidentiality, was statistically associated with this practice characteristic. CONCLUSIONS: Physicians practicing in smaller communities and areas with more extensive social interconnectedness may need to address the issue of privileged communication to a greater extent. If other options are exhausted, physicians may need to send patients with particular needs for confidentiality outside their communities to obtain care.

Seamless service.

Describes the process used by the Mater Infirmorum Hospital in Belfast in 1992 to achieve high quality care (seamless service), motivate staff to deliver and measure performance. Aims of the project included focusing the organization on the customer, improving teamwork and motivation. Opinions of staff, patients and GPs were examined and key issues for improvements were identified. Interface from every single interaction between Mater and customers was measured, taking into account customer perception of service, and staff perceptions of service. Data collection methods included taped interviews, video vox pops and questionnaires. Concludes that a mismatch was found between service offered and service required. Workshops launched ongoing activities and a small steering group designed a series of management forums to gain future support.

Symmetry and quasi-symmetry: an example in modelling pairs of sounds from children's early speech.

The models of symmetry and quasi-symmetry may have use in the analysis of categorical data in square tables where the row and column classifications are the same. Under the model of symmetry, the probability that an observation falls in, say, the (i,j) cell of a square table is the same as the probability that it falls in the (j,i) cell. The model of symmetry requires that the expected marginal total for any one row of the table be the same as the expected marginal total for the corresponding column. The model of quasi-symmetry, which is a generalization of the model of symmetry, does not require this equality of expected row and column totals. In this paper, we illustrate the use of the models of symmetry and quasi-symmetry in the analysis of categorical data that summarize pairs of sounds in infants' and toddlers' early utterances. In the past, the complexity of infants' early vocalizations has been difficult to measure because of a lack of methods to quantify prelinguistic phonetic variables. We use the models of symmetry and quasi-symmetry as a first attempt to quantify the complexity of infants' early speech. We hope that the discovery of models appropriate for quantification of language development in normal children will have later use in identification of children at risk for communicative disorders.
