[Pregnancy in adolescents. Complications, birth weight, somatometry of the newborn and Apgar score, comparison with the general population].

The objective of this study was to compare pregnancy evolution, newborn's weight, its somatometry and Apgar's qualification of teenagers and the general population in a community where the socio-economic factor is similar among those two groups. Three hundred and two pregnancies in teenagers were analyzed, 69 of 12-15 years and 233 of 16-17 years at the time of pregnancy, against a control group of 304 pregnant patients of 18-39 years. It was a prospective, comparative, longitudinal study. Average age at the first delivery of the 606 patients was 18.1 years. There were no significant difference among the groups as to occupation. There was not significant difference among the three groups as to pre-natal control, weeks of gestation incidence of threatening abortion, toxemia, bleeding of the second half of pregnancy, premature rupture of membranes, proportion of deliveries and cesarean sections, as well as forceps application and newborns' sex. The most common indications of cesarean section in teenagers were CPD and acute fetal suffering, in the control group were CPD and pelvic presentation. There was significant difference comparing the weight, Apgar qualification at one minute and at three minutes in the 16-17 years group, and older of 18 years being greatest in the last group. There was significant difference in size, cephalic perimeter, thoracic perimeter and PC/pa ratio among the three groups.(ABSTRACT TRUNCATED AT 250 WORDS)

[Static and dynamic contrast sensitivity of myopic eyes before and after laser in situ keratomileusis].

OBJECTIVE: To evaluate the static and dynamic contrast sensitivity changes in myopic patients before and after laser in situ keratomileusis (LASIK). METHODS: Seventy-three eyes in 37 patients with myopia (with or without astigmatism) who received LASIK were tested for static and dynamic contrast sensitivities using the METRO VISION MON ELEC I system at 0.7, 1.4, 2.7, 5.5, 11, and 22 cpd and cps prior to LASIK, and at one-, three-, and six-month intervals after LASIK. RESULTS: All eyes gained naked visual acuity of more than 0.5 after LASIK. The contrast sensitivity was depressed at all frequencies 1 month after LASIK, as compared to one week prior to LASIK. The depression at 2.7, 5.5, 11 (P < 0.01) and 22 cpd (P < 0.05) was statistically significant for static contrast sensitivity, and also at 5.5 (P < 0.01) and 11 cps (P < 0.05) for dynamic contrast sensitivity. Myopic eyes between 6.25 D and 14.0 D, and astigmatic eyes 2 DC and more, suffered more static and dynamic contrast sensitivity depression than the myopic eyes between 1.25 D and 6.00 D and astigmatic eyes less than 2 DC. Contrast sensitivities were improved and exceeded preoperative levels 3 months after LASIK, and improved even more 6 months after LASIK. All sequences were statistically significant for static contrast sensitivity (P < 0.01), while only 2.7, 5.5, and 11 cps were statistically significant for dynamic contrast sensitivity (P < 0.01). The astigmatic eyes 2 DC and more showed less improvement, even below the preoperative level at 1.4 cps of dynamic contrast sensitivity. CONCLUSIONS: While temporary depression of contrast sensitivity for myopic eyes after LASIK was seen, contrast sensitivity soon returned to exceed preoperative levels at 3 months after LASIK, while improving even more 6 months after LASIK.

Fibrin glue is useful in preventing early dialysate leakage in children on chronic peritoneal dialysis.

OBJECTIVE: To assess if application of fibrin glue sealant to the peritoneal cuff suture is useful in the prevention of early dialysate leakage in children with end-stage renal disease on chronic peritoneal dialysis (CPD). DESIGN: Single-center, open-label, prospective randomized study. SETTING: University Pediatric Hospital. METHODS: 52 catheters were implanted in 45 children (mean age 6.2 +/- 4.5 years). Catheters were randomly assigned to either the control group or the sealant group. In the latter group, 1 mL of fibrin glue sealant was applied to the peritoneal cuff suture. 18 catheters were used for the first time within 5 days after implantation (early-used catheters). Leakage, exit-site or tunnel infection, peritonitis, and adverse secondary effects were evaluated during the initial 60 days after implantation. RESULTS: No adverse secondary effects were seen after the application of the fibrin glue sealant. The incidence of exit/tunnel infection and peritonitis was similar in the two groups. The incidence of leakage was significantly lower in the sealant group (p < 0.02). In the early-used catheters, leakage was detected in 9% of the catheters in the sealant group and in 57% of the control group (p < 0.05). CONCLUSIONS: The application of 1 mL of fibrin glue to the peritoneal cuff suture prevented early dialysate leakage without secondary adverse effects in children on CPD.

Acute systemic inflammation is associated with an increase in peritoneal solute transport rate in chronic peritoneal dialysis patients.

BACKGROUND: This study was performed to evaluate the effects of acute systemic inflammation on peritoneal solute transport rate (PSTR) in chronic peritoneal dialysis (CPD) patients. METHODS: A baseline standard peritoneal equilibration test (PET) was performed on each patient every 6 months, and blood concentration of high-sensitivity C-reactive protein (hs-CRP) was assayed every 2 months in our peritoneal dialysis clinic. Acute systemic inflammation was defined as a greater than 10-fold increase in hs-CRP concentration compared with baseline value, in the absence of peritonitis, and returning to baseline level in 2 months. In patients with acute systemic inflammation, PET and hs-CRP concentration assays were performed during inflammation and after recovery. Ten patients with acute systemic inflammation were enrolled in the inflammation group and 42 other patients served as controls. RESULTS: There were no significant changes in hs-CRP and dialysate-to-plasma ratio of creatinine (D/Pcreat) in the control group during the study period. In the inflammation group, median hs-CRP levels at baseline, during acute inflammation, and at recovery were 2.3 mg/L (range 0.3 - 4.5 mg/L), 39.2 mg/L (range 15.1 - 117.4 mg/L), and 3.7 mg/L (range 0.9 - 8.9 mg/L), respectively. Median D/Pcreat increased significantly from baseline (0.64; range 0.55 - 0.98) to time of acute inflammation (0.72; range 0.60 - 0.96) (p < 0.05). The D/Pcreat at recovery was 0.67 (range 0.52 - 0.94), which decreased significantly from time of acute inflammation (p < 0.05). There was no correlation between changes in log (hs-CRP) and changes in D/Pcreat. CONCLUSION: We have shown here that acute systemic inflammation is associated with a temporary increase in PSTR in CPD patients.

Cloning and molecular characterisation of a gene encoding cytochrome P450 C-23 steroid hydroxylase (CYP90) from aspen.

Gene CPD (constitutive photomorphogenesis and dwarf) encodes a cytochrome P450 steroid side-chain hydroxylase (CYP90) involved in biosynthesis of brassinosteroids (BRs) in Arabidopsis thanalia. To dissect the molecular mechanism underlying the biosynthesis and action of the new phytohormone in perennial woody plants, a 1442 bp cDNA and a 1900 bp genomic DNA fragments whose 3'- half was well overlapped with the 5'-half of the former were obtained from a hybrid aspen (Populus tremula x tremudelois) by screening of the cDNA and genomic DNA libraries of this tree using a cDNA fragment from the Arabidopsis CPD gene in the present study. Sequence analysis revealed that the open reading frame comprising the cDNA and genomic DNA fragments encoded a protein of 476 amino acids with a molecular weight of 63 kD. This protein is four amino acid residues longer than Arabidopsis CYP90, and 78.32% identical to the latter. Also it shared high homologies with the Arabidopsis CYP90 in all known functional domains, including steroid-binding region closely related to BR biosynthesis, indicating a high conservatism of the putative CDP gene between annual herb and perennial woody plant species.

A clinical study of caesarean section at the University Teaching Hospital (C.H.U.) Yaounde (1982-1989).

During the period extending from January 1982 to December 1989, we delivered 9,637 women at the University Teaching Hospital (C.H.U.), Yaounde, Cameroon. Seven hundred and forty-one (7.69 pc) of these patients underwent caesarean section (CS) Most of them were in the 20-30 year age group and of low parity (0-4). Foetal distress, cephalopelvic disproportion (CPD), and malpresentation were the commonest indications for abdominal delivery. Elective repeat sections were usually done for CPD. Most of the operations were carried out by qualified staff and maternal and foetal outcome were satisfactory especially for a Third World institution dealing mainly with higher risk pregnancies.

Telangiectatic osteogenic sarcoma of the extremities. Results in 17 patients treated with neoadjuvant chemotherapy.

Seventeen patients with telangiectatic osteogenic sarcoma (TOS) of the extremities were treated with neoadjuvant chemotherapy, according to two different protocols. Preoperatively, the patients received high-dose methotrexate (HD-MTX) and cisplatinum (CPD) (HD-MTX/CPD) and doxorubicin. CPD was delivered intraarterially, the other drugs intravenously. Limb-salvage surgery was performed in 12 instances, and five patients were treated with amputation. Postoperative chemotherapy was tailored according to the grade of chemotherapy-induced necrosis. In 13 cases (86%), the resultant grade of necrosis was greater than 90% (good responders). The mean follow-up interval was 3.5 years with a range of 18 to 78 months. Fourteen patients (82%) remained continuously disease-free, while three patients developed lung metastases. Two of these died because of uncontrolled disease, whereas the third patient is alive and disease-free after metastasectomy. No local recurrences were observed. These results are better than those observed in 215 contemporary cases of conventional osteosarcoma treated with the same protocols. This study confirms that TOS is not a uniformly lethal tumor as suggested by prior reports. By employing neoadjuvant chemotherapy, a high percentage of patients with TOS can be cured, and in most of them, a limb-sparing surgery is possible and safe.

X-ray pelvimetry-reappraisal.

Antenatal erect lateral X-ray pelvimetry was performed for 116 primigravidas and 53 multigravidas because of suspected cephalopelvic disproportion (CPD). Multigravidas were further subgrouped into 4 gravidas and greater than 4 gravidas. In the primigravidas, there was no statistical difference in the mean value of the anteroposterior diameter of the pelvic inlet (APD) of the elective (10.4 cm, +/- 0.6 SD) and the emergency (10.5 cm, +/- 0.9 SD) caesarean section group. In the multigravidas, the mean value of the APD showed a statistically significant decrease with increased parity. In this group, the chances of delivery by caesarean section were 74%, when the APD was less than 10.5 cm, and 12% if the APD was greater than 11.5 cm. It is concluded that cephalopelvic disproportion in primigravidas should only be diagnosed after adequate trial of labour with adequate uterine contractions. In multiparous patients, especially grand multiparas, X-ray pelvimetry is recommended in cases of suspected CPD before a trial of vaginal delivery is conducted, since the mode of delivery seems to depend primarily on the pelvic capacity.

Progressive vs single-stage treadmill tests for evaluation of claudication.

The reliability of claudication pain and the metabolic and hemodynamic measurements of the lower limbs of patients with stable peripheral vascular occlusive disease (PVOD) were compared during and following single-stage (S) and progressive (P) treadmill tests. Ten patients (69.8 +/- 1.8 yr; X +/- SE) walked to maximal claudication pain twice a month for 4 months. Patients walked at 1.5 mph up a 7.5% grade (S test) and at 2 mph on a 0% grade, increasing by 2% every 2 min (P test). Distance walked to the onset of claudication pain (CPD) and maximal walking distance (MWD) were recorded. Foot transcutaneous oxygen tension (TcPO2) was measured before, during, and after exercise, while ankle systolic blood pressure (SBP) and the ankle-to-brachial SBP index (ABI) were measured before and after exercise. Intraclass correlation coefficients (R) of CPD and MWD during S tests were R = 0.53 and R = 0.55, respectively. In contrast, the respective R values during P tests were R = 0.89 and R = 0.93. Higher R values of foot TcPO2 were also obtained during and following P tests, while ankle SBP and ABI were highly reliable following both tests. It is concluded that the severity of PVOD is better assessed by P treadmill tests because clinical measurements are more reliable during exercise and recovery.

[Normal and defective development of visual stimulus processing: psychophysical and electrophysiologic test results].

Two methods were developed for the assessment of normal and disturbed visual development in infancy and early childhood: the preferential looking procedure (PL) as a psychophysical method and measurement of visual evoked potentials (VEP) as an electrophysiological principle. Both methods test visual functions, but on different levels of the visual system. Proper interpretation of the methods requires direct comparison. This was accomplished over a 12-months period in infants and young children with normal and disturbed visual development in the first year of life. For normal infants older than 4 weeks we found accurately configurated VEP responses and rapidly increasing visual accuity, ranging from 0.4 cpd (age 2.5 weeks) to 10 cpd (36 weeks) and better. There was a close agreement between the VEP and PL results, possibly due to our conservative interpretation of VEP acuity, which was not extrapolated from the VEP amplitude versus spatial frequency function, but defined by the relationship to the simultaneously registered noise level of the responses. The last response that could be differentiated from this noise level was defined as VEP acuity in analogy to the acuity definition of the Teller acuity cards. In visually impaired children there was a delay in visual development. Visual deficits in congenital cataract were much more marked than in congenital squint. Infants with congenital cataract showed a nearly newborn stage when tested shortly after lensectomy. With our preliminary results is was not possible to determine any statistical relationship among the psychophysical, electrophysiological and clinical findings.(ABSTRACT TRUNCATED AT 250 WORDS)

Use of plasma with high levels of ionised calcium in the production of model scale coagulation factor concentrates.

We have attempted to exploit the Ca2(+)-dependent stability of factor VIII in producing factor VIII concentrates of higher yield. Plasma levels of ionised calcium were increased in two ways: (a) whole blood collection into half-strength citrate CPD anticoagulant, leading to free Ca2+ levels of ca 120 microM and (b) apheresis collection of plasma which was then recalcified to free Ca2+ levels of ca 300 microM under heparin cover. Coagulation factor concentrates were prepared using model versions of our industrial scale manufacturing methods. Factor VIII yield was increased through low citrate collection. This did not compromise factor IX yield or thrombogenic potential. Use of recalcified heparinised plasma did not lead to any improvement in factor VIII yield and resulted in a marked drop in factor IX recovery, possibly from interference by heparin of factor IX binding in ion-exchange chromatography. The benefits accruable through the use of half-strength citrate CPD anticoagulant support the continued evaluation of this preservative in large scale blood collection and fractionation. The deleterious effects of heparin in charge-mediated plasma fractionations may pose serious difficulties in harvesting vitamin K dependent factors.

Pharmacological properties of new neuroleptic compounds.

RMI 61 140, RMI 61 144 and RMI 61 280 are newly synthetized N-[8-R-dibenzo(b,f)oxepin-10-yl]-N'-methyl-piperazine-maleates which show interesting psychopharmacologic effects. This work contains the results of a study performed with these three compounds, in order to demonstrate their neuropsycholeptic activity in comparison with chloropromazine (CPZ) and chlordiazepoxide (CPD). The inhibition of motility observed in mice shows that the compounds reduce the normal spontaneous motility as well as the muscle tone. The central-depressant activity is evidenced by increased barbiturate-induced sleep and a remarkable eyelid ptosis can also be observed. Our compounds do not show any activity on electroshock just as do CPZ and CPD. As to the antipsychotic outline, our compounds show strong reduction of lethality due to amphetamine in grouped mice and a strong antiapomorphine activity. They show also an antiaggressive effect and an inhibitory activity on avoidance behaviour much stronger than CPZ. We have also found extrapyramidal effects, as catalepsy, common to many tranquillizers of the kind of the standards used by us. As for vegetative phenomena, the compounds show hypotensive dose related action ranging from moderate to strong, probably due to an a-receptor inhibition. Adrenolytic activity against lethal doses of adrenaline, antiserotonin and antihistaminic effects, as well as other actions (hypothermia, analgesia, etc.) confirm that RMI 61 140, RMI 61 144 and RMI 61 280 are endowed with pharmacologic properties similar and more potent than those of CPZ. Studies on the metabolism of brain catecholamines show that they are similar to CPZ, although with less effect on dopamine level.

Lipids changes in rat tissues in experimental urolithiasis.

Total lipids, free and ester cholesterol, triglyceride and phospholipids were determined in plasma, liver, kidney and intestine in control and calculi producing diet (CPD) fed rats. Cholesterol, phospholipids and triglycerides were increased in plasma while they were decreased in all the three tissues of CPD fed rats, compared to that of control. Distribution studies of phospholipids in the tissues of treated rats showed marked decrease in the concentration of the major lipids, i.e., PC, PE, PI and SPH. However, significant increase in absolute concentration as well as percent distribution of phosphatidic acid in kidney of treated rats was observed.

Gastroesophageal reflux and recurrent/chronic pulmonary disease in infants and children.

It is now widely recognized that gastroesophageal reflux (GER) causes many symptoms in infants and children; however, the implication of a causal relationship between GER and pulmonary disease still raises skepticism in many clinicians. Recurrent or chronic pulmonary disease (CPD) is a significant cause for morbidity and mortality in infants and children. There is no single, reliable, diagnostic test that consistently demonstrates that pulmonary symptoms are caused directly by GER. This contributes to lack of firm association between GER and CPD. Is GER a contributory factor to the severity of the pulmonary disease, as in chronic asthma? Is it the primary cause of all the respiratory pathophysiology? Or, is GER the result of altered pulmonary mechanics? These are questions that remain unanswered. The purpose of this article is to review the literature raising skepticism among clinicians, paving the way for future directions of research.

[Comparative studies of rheologic properties of erythrocyte concentrates in relation to a 6-week storage period in various preservative solutions].

Buffycoat free red cell concentrates were stored at 4 degrees C for 42 days in CPD-A1 and two new red cell preservation solutions, PAGGS-Sorbitol and SAG-Mannitol. The rheological behaviour of the red cells was measured by viscometry of red cell suspensions (hematocrit 45%) at steady flow at various intervals during the storage period. The viscoelastic properties of the red cells were studied at oscillatory flow using an oscillation rheometer. The deformability of the red cells during storage was determined by measurement of the viscosity of hard packed cells at steady flow. The results show better red cell deformability during the entire storage period using the new solutions PAGGS-Sorbitol or SAG-Mannitol compared to the standard blood bank preservation solution CPD-A1.

Effect of oxalate and malonate on red cell metabolism.

The addition of oxalate to blood stored in Citrate-phosphate-dextrose (CPD) produces a marked improvement in 2,3-diphosphoglycerate (2,3-DPG) preservation; an increase in 2,3-DPG levels can also be documented in short-term incubation studies. Oxalate is a potent in vitro inhibitor of red cell lactate dehydrogenase, monophosphoglycerate mutase, and pyruvate kinase (PK). In the presence of fructose 1,6-diphosphate the latter inhibitory effect is competitive with phospho(enol)pyruvate (PEP). Determination of the levels of intermediate compounds in red cells incubated with oxalate suggest the presence of inhibition at the PK step, indicating that this is the site of oxalate action. Apparent inhibition at the glyceraldehyde phosphate dehydrogenase step is apparently due to an increase in the NADH/NAD ratio. Oxalate had no effect on the in vivo viability of rabbit red cells stored in CPD preservatives for 21 days. Greater understanding of the toxicity of oxalate is required before it can be considered suitable as a component of preservative media, but appreciation of the mechanism by which it affects 2,3-DPG levels may be important in design of other blood additives. Malonate, the 3-carbon dicarboxylic acid analogue of oxalate late did not inhibit pyruvate kinase nor affect 2,3-DPG levels.

Serial determinations of PF4 and beta TG: comparisons between multiple venipunctures vs a catheter infusion system.

Platelet factor 4 (PF4) and beta thromboglobulin (beta TG) are platelet-specific proteins which are released upon platelet aggregation and which can be accurately measured by radioimmunoassay. We devised a catheter-infusion system that enables serial determinations of these proteins. In 20 subjects (10 healthy volunteers and 10 patients with stable coronary artery disease), we compared samples collected by individual venipunctures with those simultaneously obtained by means of a simple catheter-infusion system. At least 5 samples were obtained over a period of time which was as long as 60 min, and at least 30 min. Subjects with stable coronary artery disease were selected so that they would be expected to have stable and normal PF4 and beta TG levels. Thus, elevations of either PF4 or beta TG would represent artifacts secondary to sampling technique. Analysis of the results demonstrated that the catheter-infusion system was equivalent to individual venipunctures for determination of PF4 and beta TG. 16.8% of samples obtained via the catheter and 17.2% of those obtained by individual venipunctures were spuriously elevated. A second series of studies were performed to refine the technique further by examining the impact of infusion rate and the addition of citrate phosphate dextrose (CPD) to the infusate. Ten additional subjects had catheter systems utilized in both arms simultaneously. The addition of CPD resulted in significantly less abnormal values at slower infusion rates (1 and 2.5 cc/min). At 5 cc/min D5/w or saline alone are suitable. These investigations confirm that this simple catheter system is equivalent to individual venipunctures for determination of PF4 and beta TG while avoiding patient discomfort.(ABSTRACT TRUNCATED AT 250 WORDS)

Changes in serum corticosterone and testosterone during induced maternal behavior in rats.

Serum corticosterone (Cpd B) and testosterone (T) concentrations were studied during the avoidance phase of artificially induced parental behavior in male rats. Adult rats were exposed to the presence of standard size foster pups for 60 min daily. On day 1 the avoidance behavior characterized by typical burying reaction was accompanied by an elevation of Cpd B and T. The behavioral responses and hormonal changes diminished during the days of repeated exposure. It was found that neither pup-killing nor spontaneous retrieval were dependent on circulating hormones in the male rat. Subcutaneous injection of ACTH 4-10 inhibited extinction of the avoidance and the humoral responses in both female and male animals. Oxytocin failed to exert any influence on the behavioral and hormonal components of the aversive reaction.
