[The effect of ETA receptor antagonist WS009A on the pathogenesis of hypoxic pulmonary hypertension in rats].

OBJECTIVE: To investigate the effect of ETA receptor antagonist, WS009A, on the pathogenesis of hypoxic pulmonary hypertension in rats. METHODS: In the course of chronic hypoxia, rats were treated with WS009A (10 mg/kg) by a siliastic catheter inserted in the left external jugular vein. RESULTS: The measured values of mean pulmonary arterial pressure (mPAP) were 2.7 +/- 0.33, 3.97 +/- 0.47 [symbol: see text] 2.39 +/- 0.27 kPa in the control, the hypoxic and the WS009A treated groups, respectively. The percentages of vascular wall area/total vascular area (MA%) were 0.26 +/- 0.03, 0.52 +/- 0.04, 0.32 +/- 0.07, respectively. The percentages of vascular wall thickness/vascular external diameter(MT%) were 0.14 +/- 0.02, 0.31 +/- 0.03, 0.18 +/- 0.05, respectively. The ratios of right ventricular wall to left ventricle plus septum[RV/(LV + S)] were 0.24 +/- 0.02, 0.35 +/- 0.03, 0.26 +/- 0.03, respectively. The variance analysis revealed there were decreases in mPAP and the ratio of RV/(LV + S) in WS009A group (P < 0.01, compared with those in hypoxic group). There were no haemodynamic and right ventricular hypertrophy differences between WS009A group and control group. The MT% and MA% in WS009A groups were higher than those in controls, but there were significantly decreased MT% and MA% in WS009A group as compared with those in hypoxic group (P < 0.01). CONCLUSION: The ETA receptor antagonist, WS009A, attenuates the pulmonary vascular wall remodelling and the pulmonary hypertension induced by chronic hypoxia.

Outcomes of touch therapies during bone marrow transplant.

CONTEXT: The integration of complementary modalities into mainstream healthcare is gaining increasing emphasis. It is important, therefore, to document the effects of these interventions on patient outcomes. OBJECTIVE: To investigate the effects of Therapeutic Touch and massage therapy on the outcomes of engraftment time, complications, and perceived benefits of therapy during bone marrow transplant. DESIGN: Randomized clinical trial. PARTICIPANTS/SETTING: Subjects were adult patients on the bone marrow transplant unit of a large urban tertiary care center. INTERVENTIONS: Subjects were randomly assigned to 1 of 3 treatment groups: Therapeutic Touch (TT), massage therapy (MT), and a control group called the friendly visit (FV). Subjects (N = 88) were stratified by type of transplant (allogeneic or autologous). Twenty-seven subjects received MT; 31 received TT; and 30 received FV. Nurses with expertise in the 2 touch therapies administered them. The interventions of MT, TT, and FV were administered according to standarized protocols every third day beginning the day chemotherapy began until discharge from the program. OUTCOME MEASURES: Time for engraftment, complications, and patient perceptions of benefits of therapy were the main outcome measures. Analysis of variance and analysis of covariance were used to determine significant differences among the 3 groups with respect to time of engraftment. RESULTS: A significantly lower score for central nervous system or neurological complications was noted for subjects who received MT comppared with the control group; however, no differences were found among the 3 groups with respect to the other 10 complication categories or in the total mean score for complications. Patients' perception of the benefits of therapy (total score) was significantly higher for those who received MT compared with the FV control group. The mean scores on the comfort subscale were significantly higher for patients receiving both MT and TT compared with the FV control group. CONCLUSIONS: Massage therapy may be effective in altering the psychological and neurological complications associated with chemotherapy during bone marrow transplant. Both massage and Therapeutic Touch provide comfort to patients undergoing this challenging process.

[Pharmacokinetics and relative bioavailability of probucol inclusion complex capsule in healthy dogs].

AIM: To study the pharmacokinetics and relative bioavailability of probucol inclusion complex capsule. METHODS: Following oral administration of a single dose of 250 mg of conventional tablet (formulation A, purchased from the market) and probucol inclusion complex capsule (formulation B, a new formulation for preclinical trial) to each of 6 healthy dogs in a randomized crossover design, the plasma levels of the active drug at different time points were determined by HPLC and the plasma concentration-time profiles of formulation A and B were obtained. The pharmacokinetic parameters as well as relative bioavailability were analyzed. RESULTS: The concentration-time curves of formulation A and formulation B were found to fit a two-compartment open model. The Tmax values of formulation A and formulation B were (9.3 +/- 2.1) h and (9.3 +/- 2.1) h, the Cmax values were (1.5 +/- 1.0) microgram.mL-1 and (2.3 +/- 0.9) microgram.mL-1 and the AUC0-240 values were (85 +/- 56) microgram.h.mL-1 and (134 +/- 55) microgram.h.mL-1, respectively. The relative bioavailability of formulation B was found to be (198 +/- 90)% compared with formulation A. The results of variance analysis and two one-side t-test showed that there was significant difference between the two formulations in the AUC0-240. CONCLUSION: The high bioavailability by the inclusion of formulation B is attributed to the improvement of its water-solubility by the inclusion process and this is supposed to be a key factor for improving drug bioavailability.

[Obesity--a risk factor].

The nutritional status of a selected population--859 male members of the Fire Rescue Service from 12 districts in the Czech Republic--was assessed in 1997 and 1998. The study provides extensive information on selected anthropometrical and biochemical parameters, especially on those, which are generally used as risk indices for the origin and development of cardiovascular disease (CVD): body mass index (BMI), body fat percentage (b. fat), waist circumference (waist), serum concentrations of total cholesterol (Tchol), HDL- and LDL-cholesterol (HDL-chol, LDL-chol), triacylglyceroles (TAG) and the atherogenic index (AI). As far as lipid parameters were concerned, increased serum levels were estimated in 30.4% (TAG), 54% (Tchol) and 60.9% (LDL-chol) of volunteers. Decreased serum levels of HDL-chol were found in 38.2% of volunteers. An AI higher than 3.5 a.u. was calculated for 79.9% of all subjects in study while an AI higher than 5.0 a.u. was calculated for 45.3% of all subjects. An AI higher than 3.5 a.u. was found in 62% of males with normal weight, in 85% of overweight males and in 92% of obese males. The results of the study proved the considerable prevalence of the overweightness and obesity in the male population group that was followed: according to the calculated BMI, 49.5% of volunteers were estimated to be overweight and 16.3% to be obese. The study concept made it possible to reveal the relationships among the anthropometrical and biochemical parameters followed. The correlation matrix documents a statistically significant dependence among the BMI or waist values and the serum concentrations of Tchol, LDL-chol, TAG and AI. A significant negative correlation was found between the HDL-chol serum concentration and the BMI or waist values. The variance analysis results (the estimated lipid parameter values were divided into groups according to BMI and waist categories) document a statistically significant increase in serum Tchol, LDL-chol, TAG and AI in age categories over 35 years in comparison with the category of men under 25 years of age. For Tchol and LDL-chol a statistically significant increase had already been found in the 25-35 year age category. In comparison with the normal weight category, Tchol serum levels and AI values were statistically significantly higher in both the overweight and obese categories. Statistically significant increase was proven for the TAG and LDL-chol serum levels in both the overweight II. gr. and obese categories. A statistically significant decrease in comparison with the normal weight category was found in the HDL-chol serum levels of both the overweight and obesity categories. In the same way, an increase in Tchol and LDL-chol, TAG serum concentrations and AI values in higher and high risk CVD categories (according to the waist circumference value) was found in comparison with the low CVD risk category.

Acne: evolution of the clinical practice and therapeutic management of acne between 1996 and 2000.

According to epidemiological studies, acne is a common condition affecting 80% of young people between 12 and 18 years of age. It is also the most common reason for consultation in private dermatology practices. Yet, paradoxically, very few studies have been concerned with the profile and therapeutic treatment of acne patients. The aim of this study was to compare both the evolution of the profile of acne patients and the therapies used by dermatologists between 1996 and 2000 through a survey. Two questionnaires had to be filled in by the dermatologists for all patients consulting for acne, the first in 1996 and the second in 2000. The purpose of these questions was to obtain information about the demographic characteristics of the patients, a clinical evaluation of the patients' acne, their quality of life and finally their treatment. The statistical tests used Chi2 tests and variance analysis. The alpha risk was fixed at 0.05. Two thousand two hundred and eighty four patients were included in the survey in 1996 and 2363 in 2000. The mean age was 21 7.6 in 1996 and 20.5 7.7 in 2000, the majority of whom were female (64.4% in 1996 and 63.6% in 2000). In 1996 37.4% were students, and 52.4% in 2000. For 81.1% of the patients, the consultations were spontaneous on their own initiative both in 1996 and 2000. As regards topical treatment, a decrease of antibiotic prescriptions was noted between 1996 and 2000 associated with a decrease of benzoyl peroxyde use. Concerning the systemic treatments, an increase of doxycyclin and isotretinoin prescriptions was reported together with a decrease of minocycline treatment. Prescription of zinc salts and hormonal treatments were stable. The frequency of biological investigations remained similar and low. The repercussions of acne on the patient's quality of life were more numerous in 1996 compared to 2000 but they remained equally intense for those who reported them. The profile of acne patients evolved slightly between 1996 and 2000 and interestingly the treatments were modified to take into account the information in the literature such as bacterial resistance or severe side effects.

[Distribution of L-type calcium channels on outer hair cells among various turns of guinea pig cochlea].

OBJECTIVE: To probe the relationship between the density of L-type calcium channels on outer hair cells(OHCs) among various turns and the tonotopic map in the guinea pig cochlea. METHODS: The L-type calcium channels in outer hair cells of guinea pigs were labeled with DM-BODIPY DHP and RH414 and monitored using a laser scanning confocal microscope. The fluorescence ratio of DM-BODIPY DHP/RH414 revealed the distribution of L-type calcium channels on the cellular membrane. RESULTS: The results showed that the cellular membrane had lesser DM-BODIPY DHP staining when pretreated with nifedipine, indicating that calcium channels on OHCs had the specificity of a L-type calcium channel. The density of L-type calcium channels had a tendency of increasing gradually from low frequency areas to high frequency areas. Variance analysis showed that the difference of the density of L-type calcium channels on OHCs between the fourth turn and the second turn was significant. There were no apparent differences in the distribution of L-type calcium channels among various regions on the cellular membrane of OHCs. CONCLUSIONS: The descripency of the distribution of L-type calcium channels on OHCs among various turns of the guinea pig cochlea may be one of the molecular mechanisms of the tonotopic map of the cochlea.

[Percentage of soil absolute moisture content at different succession stages of Castanopsis platyacantha-Schima sinensis formation in central subtropical zone of Sichuan Province].

The temporal-spatial characteristics of the percentage of soil absolute moisture content (PSAMC, defined as the percentage of soil water to dried soil) at different succession stages of C. platyacantha-S. sinensis formation such as brush, 10-, 40-, and 200-years old C. platyacantha-S. sinensis communities were studied, using the methods of orientation observation and variance analysis under the similar conditions including climates, topographies, landforms, and soils. The results indicated that the relationship between soil depth and PSAMC of all communities except for 10-years old community were significant. The effects of season on PSAMC at different stages of all C. platyacantha-S. sinensis communities was not significant. Similarly, aspect had no notable effects on PSAMC of all communities but 10-years old community. Different succession phases had significant effects on PSAMC (0-60 cm depth). The water retention ability of soil was restored up to 73% and 85% of primary C. platyacantha-S. sinensis forests when C. platyacantha-S. sinensis communities were 10- and 40-years old. PSAMC of different succession phases of C. platyacantha-S. sinensis had obvious characteristics of fluctuation and chaos.

[The evaluation of the mutagenic hazard of atmospheric air pollution for an urban population].

The authors describe the results of a genetic monitoring made in three towns of the Ukraine, which differ in the levels of polluted atmospheric air. Spontaneous abortions (SA) were selected as an index for genetic health of the population. The total number of CA, which were detected by primary documentation in the studied towns, the number of SA in early periods of pregnancy, and the incidence of SA, probably, of mutagenic origin, which is determined by multi-variance analysis, are greater in the contaminated towns than in comparatively pure towns. The findings suggest that the achieved level of atmospheric air pollution in some towns of the Ukraine is genetically significant.

[Respiratory inductive plethysmography in the functional evaluation of patients undergoing coronary by-pass surgery].

The functional respiratory changes following coronary by-pass surgery were previously evaluated with the traditional spirometric method. Aim of this work is to introduce the respiratory inductive plethismography (RIP), a well known non-invasive technique, into the study of those volumetric and ventilatory modifications. Eight cardioischemic patients were selected: all but one were free from pulmonary disorders. The tidal volume, the respiratory rate, the minute ventilation, the vital capacity and the relation between the abdominal and the thoracic part of the ventilatory act, were measured in the pre-intervention session and in the 1st, 3rd and 7th day after the operation of coronary by-pass. In the same sessions, except the 1st day after surgery, the maximal respiratory pressures were evaluated with traditional manometers. The functional changes of the examined parameters, statistically studied with the variance analysis, closely resemble the spirometric modifications registered by other authors. Furthermore the relative contributions of the thoracic and abdominal compartments of the pulmonary ventilation were evaluated, thanks to the non-invasive plethysmographic method. Rip makes it possible to measure the ventilatory pattern and some pulmonary volumes in the very early phases after thoracic surgery, without any trouble for the patient and in a non-collaborative state of consciousness, too.

Zoledronic acid induces significant and long-lasting modifications of circulating angiogenic factors in cancer patients.

PURPOSE: The commercial availability of zoledronic acid, a third generation bisphosphonate, prompted us to evaluate the modifications in angiogenic cytokines levels after a single i.v. infusion of this drug. EXPERIMENTAL DESIGN: Thirty consecutive cancer patients with scintigraphic and radiographic evidence of bone metastases were treated with a single infusion of 4 mg of zoledronic acid before any chemotherapy. The patients were prospectively evaluated for circulating levels of vascular endothelial growth factor (VEGF) and platelet-derived growth factor (PDGF) just before and at 1, 2, 7, and 21 days after zoledronic acid infusion. RESULTS: Basal serum VEGF median levels were significantly decreased at days 2 (-23%), 7 (-28%), and 21 (-34%) after zoledronic acid infusion (P = 0.0498, 0.0090, and 0.0011, respectively). Serum PDGF levels were significantly decreased by 25% 1 day after zoledronic acid infusion (P = 0.0032). This effect on circulating PDGF levels persisted for 2 days after bisphosphonate infusion (P = 0.0050). PDGF levels had returned to values similar to the median basal value at 7 and 21 days. Moreover, a linear regression model with variance analysis showed a significant positive correlation between basal VEGF and PDGF values but not at the following time points. No significant differences were recorded in platelet levels, WBC count, or hemoglobin concentration before and after zoledronic acid single infusion. CONCLUSIONS: This study confirms that zoledronic acid could have an in vivo antiangiogenic property through a significant and long-lasting reduction in serum VEGF levels.

[Composition and stability of fatty acids from deboned cachama and sardine meat during freezer storage].

Free fatty acids from total lipids and phospholipids in minced fish flesh from three sizes of Cachama (Colossoma macropomum) and Sardine (Sardinella anchovia) were evaluated. Cachama's most abundant unsaturated fatty acids from the total lipid fraction were: C18:1 (31-38%); C18:2, n-6 (13-15%); and C20:4, n-6 (3-5%), while in sardine were the follows: C18:1; C20:5 n-3 (12-24%) and C22: 6,n-3 (7-24%). C20:5, n-3 increased with the size and C22:6,n-3 decreased. The variance analysis indicated that percentage of polyunsaturated fatty acids of n-6 series in the total lipids of cachama and n-3 series in sardine were affected by the storage temperature from -10C to -20 degrees C (p < 0.01). Storage time had effect on the stability of sardine fatty acids (p-0.01) and cachama (p < 0.05). Sardine minced flesh presented the highest value of free fatty acids at-10 degrees C during the second month of storage (620 mg%) while cachama at the fourth month (230 mg%). TBA- value in sardine increased at -10 degrees C from 5.5 to 23 mg of malonaldehyde in fourth months while in cachama these value were almost the same (2-6 mg-kg) during the storage period at -10 degrees C and -20 degrees C.

CLINICAL STUDIES OF TOLAZAMIDE AND TOLBUTAMIDE: COMPARATIVE EFFECTIVENESS OF CONTROL OF DIABETES MELLITUS.

Tolazamide, a new oral hypoglycemic agent, was compared with tolbutamide, a related chemical compound, for stability of control of 12 patients suffering from maturity-onset diabetes mellitus. A short 12-week study was conducted which incorporated a cross-over design and the results were examined by variance analysis after dosage was individualized to the patient's requirements. Greater stability of fasting blood sugar was found on tolazamide; patients also had less glycosuria and lower fasting blood sugar on tolazamide. Tolazamide appeared to be between five and six times as potent as tolbutamide, mg. for mg.No hepatic, renal, hematologic or symptomatic toxic reactions were observed during the total of 72 person-weeks of tolazamide therapy.

[The effect of high-dose ultraviolet irradiation on sodium, calcium and aldosterone in the blood of calves].

Five Holstein-Friesian calves, from one sire, with prevalent black hair coat pigmentation were used in the experiment. The mean age was 33 days and the mean live weight 51 kg. The animals were exposed free running without interruption for 12 hours to an artificial ultraviolet light in the range of 280-320 nm. The mean doses of radiation was 179.10(-10) J/h/m. One-spot high-pressure mercury discharge lamps Tesla RVK 400 W were used as a radiation source. The dose rate was estimated from measurements by a spectral photometer with filter UG 2 for absorbtion of visible light located at the height of the back of standing calf. Blood samples were collected immediately before the beginning of treatment and after 5, 12, 24, 48 and 72 hours. The blood plasma aldosterone was measured by radioimmunoassays, the levels of sodium, potassium and calcium in blood plasma by flame spectrophotometry. Double classification variance analysis and evaluation according to the Snedecor F-test, the contrast effect test according to Duncan and regression analysis were used for statistical evaluation. Compared to the first sampling, sodium increased significantly after 5 and 12 hours of exposure (Tab. I) to 138.1 and 138.3 mmol/l, respectively. In the subsequent samplings this trend continued up to 72 hours from the beginning of irradiation (140.5 mmol/l). The potassium level did not change statistically significantly. Owing to an excessive irradiation, the calcium concentration increased significantly. The greatest increase occurred after 12 hours of irradiation (from 2.29 mmol/l to 2.61 mmol/l) and after 36 hours from the end of irradiation (2.70 mmol/l).(ABSTRACT TRUNCATED AT 250 WORDS)

Intrinsic radiosensitivity of normal human fibroblasts and lymphocytes after high- and low-dose-rate irradiation.

The existence of heritable radiosensitivity syndromes and clinical observations in radiotherapy patients suggests that human cellular radiosensitivity differs among individuals. We report here an in vitro study of radiosensitivity in 30 fibroblast and 29 lymphocyte cultures obtained from cancer patients and controls. In 25 cases, both fibroblasts and lymphocytes were obtained from the same donors. Fibroblasts were cultured from skin biopsy samples, and peripheral T-cell lymphocytes were cultured from blood. Clonogenic survival assays were performed by using high- and low-dose-rate irradiation; lymphocytes were in G0 phase and fibroblasts in confluent plateau phase. Various end points were calculated and compared (i.e., surviving fraction at 2 Gy, initial slope of the survival curve, and doses resulting in 10 and 1% survival, respectively). Depending on the end point, the coefficient of variation of the survival parameters ranged from 31 to 68% for lymphocytes and 21 to 41% for fibroblasts following high-dose-rate irradiation. Similar ranges were obtained after low-dose-rate irradiation. Variance analysis performed on replicate assays in cultures derived from the same patient showed that variation due to technical or sampling errors was significantly lower than variation between individuals (P = 0.00034 and 0.014 for fibroblasts and lymphocytes, respectively). No correlation was observed between the radiosensitivity of lymphocyte and fibroblast cultures derived from the same donors. We conclude that there is significant variation in normal cell radiosensitivity among individuals. On the other hand, comparisons of lymphocyte and fibroblast radiosensitivities suggest that tissue-specific characteristics, such as differentiation status, may variably modulate radiosensitivity.

[Determination of finasteride in human plasma by HPLC-MS].

AIM: To develop an HPLC-MS assay for determination of finasteride in human plasma and to investigate the bioequivalence in healthy volunteers. METHODS: After alkalization with sodium hydroxide, plasma was extracted with ethyl acetate and separated using a C18 column with a mobile phase of methanol-water (85:15). LC-ESI-MS was performed in the selected ion monitoring (SIM) mode using target ions at m/z 395 for finasteride and m/z 407 for the IS. The fragmentor voltage was 120 V. A randomized crossover design was performed in 20 healthy volunteers. In the two study periods, a single 10 mg dose of each tablet was administered to each volunteer. RESULTS: Calibration curves were linear over the range 1-200 micrograms.L-1 (r = 0.9986). The limit of determination for finasteride in plasma was 0.05 microgram.L-1. The recovery of finasteride from plasma was in the range of 85.9%-98.7%. The results of variance analysis and two one-side t-test showed that there was no significant difference between the two formulations in the AUC and Cmax. CONCLUSION: The assay was proved to be sensitive, accurate and convenient. The two formulations were bioequivalent.

[Comparison of subjective and objective quality of life in a group of schizophrenic patients].

AIM: The aim of this study is a comparison of subjective and objective quality of life in schizophrenic patients. METHODS: The research was carried out on 120 in- and out-patients (from 19 to 65 years) who fulfilled ICD-10 and DSM-IV criteria for schizophrenia. The quality of life was assessed by means of two instruments: Quality of Life Scale (QLS, Heinrichs et al. 1984), Self-Report Quality of Life Measure for People with Schizophrenia (SQLS, Wilkinson et al. 2000). The basic statistical methods used were: correlation analysis between measurable variables with Pearson's index (P), t-Student test, analysis of variance, factor analysis. RESULTS: In the investigated group there is no correlation between subjective and objective quality of life. Sex does not influence a correlation between these variables. Subjective and objective quality of life correlate with each other only in out-patients' group (P = -0.386). The factor analysis of SQLS distinguished 7 factors: mood; everyday activities, side effects connected with movement; side effects- others; support; interpersonal contacts; others; this division differs completely from a division on subscales. The factor analysis of QLS distinguished 3 factors: intrapsychic functioning; social functioning; functioning in roles; this division is similar to a division on subscales. CONCLUSIONS: It seems that in the schizophrenic patients' group using subjective and objective ways to assess the quality of life determines a difference of the obtained results. Only the subjective measurement fulfills the assumption of quality of life definition, that is why the usefulness of QLS in assessing quality of life is rather restricted.

[Comparative study of bilateral lesions in the entorhinal cortex and in the fimbria fornix].

INTRODUCTION: Numerous reports show that lesions to hippocampus afferents, such as the entorhinal cortex (EC) and the fimbria fornix (FF), exert an effect on memory in rodents. There are, however, no long term comparative studies that show which of these lesions could be most useful as a model for studies into neuroplasticity. MATERIAL AND METHODS: Young male Sprague Dawley rats were used. Bilateral electrolytic lesion was caused to the EC or the FF was damaged by transection. One, four or 12 weeks later the animals were evaluated in a Morris water maze, first with an invisible platform and then with the platform within view. The results from the two groups were compared to each other and to those obtained from healthy controls and subjects with false lesions by means of a variance analysis. RESULTS: In the test with an invisible platform, both types of lesion gave rise to serious, irreparable involvement of the spatial memory of the animals, at least up to 12 weeks after the lesion. The test with the visible platform revealed significant differences between animals with lesion to the EC evaluated at 12 weeks, which suggests the development of some visual or motor deterioration in these animals. CONCLUSIONS: Although both lesions gave rise to behavioural deterioration that was irreversible in the long term in rodents, the lesion to the FF seems to be a better model for evaluating specific effects on learning and memory, since the lesion to the EC apparently triggers additional sensory and motor involvement.
