[Diagnosis and treatment of liver injuries and their sequelae].

The authors studied 222 cases of closed and open injuries to the liver and, in addition, had 5 patients who were treated for sequelae of injury (hemobilia, abscesses, biliary fistula of the liver). The diagnosis of open injuries to the liver is established without great difficulties; laparocentesis and laparoscopy are indicated in closed injury. The volume of the operation is determined by the character of the damage: closure of the hepatic wound proves sufficient in most patients; surgical debridement is necessary in deep extensive wounds; resection of the liver is resorted to in disturbed blood supply. The high postoperative mortality rate is due to the fact that 71.8% of patients had concurrent injuries of the liver, other abdominal organs, organs of other cavities, central nervous system, and the locomotor apparatus.

Psychological sequelae of war. A 3-year prospective study of Israeli combat stress reaction casualties.

The long-term psychological sequelae of combat were assessed employing the Impact of Event Scale (IES) 1, 2, and 3 years after the 1982 Lebanon War. The following groups of Israeli veterans participated: combat stress reaction casualties (N = 213) and comparable controls (N = 116). For the purpose of the study, the subjects in each group were screened and further divided according to whether they did or did not suffer from posttraumatic stress disorder (PTSD). Results showed that elevated rates of distress were reported by both combat stress reaction and PTSD casualties at all three points in time. In both study groups the level of distress declined with time. Theoretical and methodological implications were discussed.

[Tuberculosis and its sequelae as a cause of death of patients in a multi-profile hospital].

The intravital diagnosis was established only in 20 patients (19.2 percent) out of 104 persons who died from tuberculosis and its sequelae over the last 20 years at the general hospital. The diagnostic errors were mainly accounted for by the difficulties in the recognition of the acute and generalized patterns of tuberculosis, by grave condition of the patients admitted to the hospital, by predominance among the patients of elderly persons with a number of concomitant diseases, by inadequate examination of the patients (the lack of x-ray, bacteriological and instrumental studies) as well as by the lack of attention on the part of internists to the clinical manifestations of tuberculosis today.

[Mass screening and prognosis of sequelae of closed cranio-cerebral trauma in children].

A prognostic method is offered in order to improve the dispensary supervision system for children after closed head trauma. For this purpose, a program of mass screening was specially designed and applied for long-term follow-up in child population in the framework of yearly medical checkups. This provided the information on 210 patients' pretrauma state. The patients were divided into groups according to severity of the posttrauma disorders of the following 2 years that allowed regarding any index in relation to its prognostic value. Computer data processing yielded prognostic criteria for the course of sequelae of the closed head injury.

[Prosthetic rehabilitation in the sequelae of labiopalatoschisis].

Labiopalatoschisis patients present functional and aesthetic alterations in the maxillomandibular area that often require prosthetic correction. Given the features of these malformations therapy and rehabilitation should be programmed as soon as possible after birth and the plastic surgeon should be informed of the factors that cause the malformations and influence the growth of such patients. The present paper briefly outlines the pathogenesis of L.P.S. and proposes a classification system for L.P.S. since both are considered essential for the understanding and assessment of its sequelae. The prosthetic-reconstructive problems in cases of cheilognathopalatoschisis subjected to orthodontic and surgical treatment are described since this is considered the most difficult form of rehabilitation the prosthesis is faced with.

[Mental disorders as late sequelae of closed cranio-cerebral trauma].

In a total of 308 patients with late sequelae of closed craniocerebral trauma inflicted during war or peace time studied in hospital conditions were the peculiarities of the course of trauma-related disease as related to various additional factors. The course of several syndromes was followed with special reference to the nature, severity and repetitiveness of the trauma, the age of affliction, and such additional determinants as alcoholism and vascular pathology. The risk factors of vascular disorders and alcoholism were determined and measures designed for their prevention and treatment.

Low frequency of adverse sequelae of obesity in South African rural black women.

In a series of 210 black women aged 25-45 years in a village in Eastern Transvaal, South Africa, 40 (19.0%) were obese, having a Body Mass Index (kg/m2) of 30 or more. Mean triceps skinfold was 23.5 mm; in 10 (25%) it was 25 mm or more) Hypertension (greater than or equal to 160/95 mm Hg) was present in 3 women (7.5%), hyperglycaemia (greater than or equal to 7.8 mmol/l) in 1 (2.5%), hypercholesterolaemia (greater than or equal to 5.2 mmol/l) in 3 (7.5%), and hypertriglyceridaemia (greater than or equal to 1.8 mmol/l) in 3 (7.5%). Dietarily, mean intake of energy was 2260 kcals, protein 73 g (12.9% energy), fat 55 g (20.8% energy), and carbohydrate 369 g (65.3%). In this context, of low fat, low animal food intakes, together with high physical activity, one or more of the adverse sequelae enumerated were present in 5 women (12.5%). Accordingly, in 35 women (87.5%), their obesity could be regarded as "healthy" obesity.

[Surgical treatment of sequelae in injuries of main peripheral blood vessels].

The authors examined 46 patients with sequelae of injuries to the peripheral vessels: pulsatile hematoma (in 12), traumatic aneurysm (in 21), arteriovenous aneurysm (in 5), and disease of a ligated vessel (in 8). Operations were carried out on 45 patients. Favourable results were produced in 92.7% of 41 patients who were treated by restorative operations. Replacement of the involved vascular segment by an autologous vein was the operation of choice in this case.

Behavior management of aggressive sequela after Reye's syndrome.

This study describes the use of behavior management procedures to reduce the occurrence of severe behavioral sequela in a 14-year-old, post-Reye's syndrome male. Prior to intervention the client exhibited severe self-injurious, hyperaggressive, and food refusal behaviors that precluded any opportunities for rehabilitation, placed him at severe medical risk, and made him a candidate for institutional placement. With contingent restraint, in vivo desensitization, and reinforcement contingencies it was possible to control these behaviors and begin intensive rehabilitation. Subsequently, the client returned to his family and community day school for the severely handicapped where he remains five years later.

[Digestive intolerance in newborn infants with extensive intestinal resections. Nutritional problems and long-term sequelae].

The experience of two neonates with extensive intestinal resection and severe difficulties in the introduction of oral nutrients in presented. The pathophysiology and treatment of short bowel syndrome is reviewed, emphasizing the importance of maintenance of nutritional support as well as the factors that contribute to the readaptation mechanism. Mention is made of the complications and long-term sequelae encountered in these cases, whose body weights and growth were normal at the age of 5.

The late sequelae of poliomyelitis.

Progressive functional deterioration following poliomyelitis has been reported after a prolonged period of stability. We present follow up data on 209 patients; the period from the original illness to December 1985 or death was between two and 73 years (mean 33.9). One hundred and sixty-three (78 per cent) patients developed late functional deterioration. This was due to purely respiratory factors in 99 cases, new neurological signs in 20 cases, and orthopaedic problems in 17 cases; 31 patients deteriorated due to a combination of factors. The commonest cause of respiratory deterioration was the development of nocturnal alveolar hypoventilation, sometimes associated with late progressive scoliosis. Eighty-six patients needed respiratory support beginning between one year and 66 years (mean 28.5) after the acute illness. New neurological signs had a clearly defined cause in each of 20 patients. No cases of motor neurone disease or post-poliomyelitis muscular atrophy (PPMA) were identified. This series shows that late sequelae following poliomyelitis are common. There were no patients with functional deterioration after poliomyelitis in whom a clear underlying cause could not be shown. The major causes of deterioration may be treatable or avoidable, and in particular, those who develop chest infections or new respiratory symptoms should be treated with caution as respiratory failure may rapidly supervene.

Pneumolabyrinth as a late sequela of temporal bone fracture.

Pneumolabyrinth, air in the labyrinthine system, is an uncommon entity. We report one case, adding to previous reports of two such cases. The diagnosis was made radiologically, which has become possible only recently with the advent of high-resolution computed tomography (CT) scanners; we suspect it will be used more often in the future. To our knowledge, this is the first case in which pneumolabyrinth has been identified as a delayed sequela of a temporal bone fracture. Furthermore, this is the first description of a pneumolabyrinth occurring in a transverse temporal bone fracture.

[Bacterial infantile coxitis and its sequelae].

Septic arthritis constitutes a serious threat for the infant hip. Literature data mention a favourable outcome only after surgical drainage within 3-4 days after the onset of symptoms. Clinical observations impose the idea that recognition of this disease often occurs too late, which brings about a subsequent serious damage to the hips involved. In order to verify this idea and to demonstrate the grave sequela of late recognition an investigation was carried out among 18 patients known as having suffered from this disease, collected from 2 children's hospitals and one university clinic. Thereby literature data were confirmed. The variety of hip joint damage is described.

Nasal septal abscess: unusual causes, complications, treatment, and sequelae.

Abscesses of the nasal septum are most likely to occur after trauma. Proper management of a nasal septal abscess requires prompt diagnosis, adequate surgical drainage, and parenteral antibiotics to prevent the potentially dangerous spread of infection and the development of severe functional and cosmetic sequelae. Without a history of prior trauma, physicians should be alert to other, less obvious causes for nasal septal abscesses. A rare case of an isolated sphenoid sinus infection leading to a nasal septal abscess illustrates the surgical treatment of these entities.

The physical and psychological sequelae of torture. Symptomatology and diagnosis.

We present a review of the international literature on the medical and psychological effects of torture. Our review reveals that certain tortures and their physical and emotional sequelae are more prevalent than previously appreciated. They include the common occurrence of sexual violence during the torture of women and female adolescents and the high frequency of head injury and associated neuropsychiatric consequences. We recommend the use of standardized diagnostic criteria in the evaluation of patients who have survived torture; this will facilitate patient care and the documentation of human rights violations.
