Quality control for parasitologic data.

Accuracy of data is of paramount concern for all research. The task of providing objective assurances of accuracy of parasitologic data for a large, multi-center epidemiologic research project in Egypt (Epidemiology 1, 2, 3 [EPI 1, 2, 3]) presented a unique set of challenges undertaken jointly by the Ministry of Health's Qalyub Center for Field and Applied Research with technical assistance from Tulane University (New Orleans, LA). The EPI 1, 2, 3 project was part of large bilateral research program, the Schistosomiasis Research Project, undertaken jointly by the governments of Egypt and the United States. This paper describes the nature of the quality control system developed to accomplish this task, presents results and discusses the findings.

Trieste PACS project: a functional assessment.

A picture archiving and communication (PAC) system (CommView by AT&T and Philips) has recently been installed in the Radiology Department of the University of Trieste, Italy, and is currently under experimental evaluation. The current status of the system and the planned developments are reported. A major project at present is the evaluation of the impact of the PAC system on the radiology system on a functional basis, by applying in the medical field a modelling methodology developed to analyse manufacturing systems. Other research in progress is devoted to technical, clinical and ergonomic aspects, on the basis of a comprehensive assessment project.

[[On the parameter estimation problems in the multiregional demographic growth model]].

"In this paper, we critically prove the formulas of Ledent-Rogers-Willekens.... Our procedure does not depend on multi-regional life table technique but on direct discretization of the continuous-time model. Here our main purpose is not to provide new results but to clear the conditions under which these methods for estimating the parameters can be applied. Finally, we give another method of calculation of parameters, which makes it possible to do a consistent disaggregated projection when we have already had an aggregated population projection." Development of an instrument to measure health center (HC) personnel's computer use, knowledge and functionality demand for HC computerized information system in Thailand.

Knowledge about socio-technical aspects of information technology (IT) is vital for the success of health IT projects. The Thailand health administration anticipates using health IT to support the recently implemented national universal health care system. However, the national knowledge associate with the socio-technical aspects of health IT has not been studied in Thailand. A survey instrument measuring Thai health center (HC) personnel's computer use, basic IT knowledge and HC computerized information system functionality needs was developed. The instrument reveals acceptable test-retest reliability and reasonable internal consistency of the measures. The future nation-wide demonstration study will benefit from this study.

[A report on medical assistance for patients with cleft lip and palate and technical transfer in Vietnam].

We had participated in the charitable operation for the patients with cleft lip and palate and technical suidance in Vietnam in 1998 for 2 weeks. The project was a part of the activities of the Japanese Cleft Palate Foundation, which were performed as part of the Official Development Assistance (ODA) of Japan, and was aided by state subsidy of the Ministry of Posts and Telecommunications, the Ministry of Foreign Affairs and the Ministry of Education and so on. Our team consisted of members of Tokyo Dental College, Iwate Medical University, Dalhousie University, and our university. The hospital we visited was Odonto-Maxillo-Facial Center in Ho Chi Min City, which had 7 clinical departments, 36 dental units, 60 beds in ward, and 75 doctors in total. This hospital dealt with all kinds of disease in connection with oral and maxillo-facial area including dental caries, periodontal disease, maxillo-facial injuries, and maxillo-facial anomalies. Forty-five patients with cleft lip and palate were operated on during our stay in Vietnam. The age of these patients was higher than that in medical advanced countries because of economic problems and insufficient medical institutions. In some cases, surgery was performed in collaboration with local doctors, which means technical suidance leads to more effective treatment.

Value of CT in determining the need for angiography when findings of mediastinal hemorrhage on chest radiographs are equivocal.

The role of CT in determining the need for angiography in patients with possible thoracic vascular injury resulting from blunt trauma is controversial. During a 24-month period, we prospectively evaluated the results of CT to screen 90 patients with a history of decelerating thoracic trauma for evidence of mediastinal hemorrhage or great vessel abnormality. All patients either had equivocally abnormal mediastinal contours on chest radiographs (64%) or had technically suboptimal chest radiographs owing to body habitus or restriction to the supine projection (36%). Patients with unequivocal signs of mediastinal hemorrhage on chest radiographs underwent immediate arteriography without prior CT. Thoracic CT was interpreted as normal in 63 (77%) patients and no further imaging was performed. Five patients had technically suboptimal CT studies, and CT scans were interpreted as equivocal in six. These 11 patients had normal arteriograms. Sixteen CT scans (18%) demonstrated evidence of mediastinal hemorrhage and/or great vessel contour abnormality. Four (27%) of 15 patients who underwent arteriography had injury to the great vessels. One patient refused to undergo angiography. In 11 patients with CT evidence of mediastinal hemorrhage, major vascular injury was not seen on arteriography. These results suggest a valuable role for CT in determining the need for arteriography to detect potential great vessel injury in patients with blunt decelerating thoracic trauma and equivocally abnormal mediastinal contours on chest radiographs.

A model for integrating technology into a multi-agency community service delivery system.

This article presents the results of a 4-year project to develop and test a model for integrating technology into a multi-agency community human services delivery system. "Technology" in this sense refers to both information technology and assistive technology. The project focused on systems change rather than on the creation of comprehensive assistive technology services. Several strategies of the model were developed and tested by the project. The project developed an information/communication tier that lay on top of the existing service delivery system. The information/communication tier was provided to agencies through an electronic network and searchable database, assistive technology advice-giving software, a public awareness campaign, and professional training. The results of these strategies are used to illustrate the Community Assistive Technology Services Network (CATSN) model. The model consists of five modules: 1) client assessment and technology examination, 2) professional and client training and technical assistance, 3) information and referral and needs analysis, 4) special projects, and 5) central coordination and facilitation--the hub. This model is important given recent nationwide initiatives for using assistive technology to provide people with disabilities opportunities for maximum independence, productivity, and integration.

Are mortality projections always more pessimistic when disaggregated by cause of death?

"It is often observed that mortality projections are more pessimistic when disaggregated by cause of death. This article explores the generality and strength of this relationship under a variety of forecasting models. First, a simple measure of the pessimism inherent in cause-based mortality forecasts is derived. Second, it is shown that the pessimism of cause-based forecasts can be approximated using only data on the distribution of deaths by cause in two pervious time periods. Third, using Japanese mortality data during 1951-1990, the analysis demonstrates that the pessimism of cause-based forecasts can be attributed mainly to observed trends in mortality due to cancer and heart disease, with smaller contribution due to trends in stroke (women only), pneumonia/bronchitis, accidents, and suicide. The last point requires the important qualification, however, that observed trends in cancer and heart disease may be severely biased due to changes in diagnostic practice." (SUMMARY IN FRE)
