Study of plasma aldosterone in normal pregnancy, in pre-eclamptic women and in cord plasma of newborns.

A radioimmunoassay without chromatography was used for the determination of plasma aldosterone in pregnancy. The mean values (+/- S.D.) of aldosterone concentration increased consistently from 23.2 +/- 5.3 ng/100 ml (n = 14) during the first trimester to 37.2 +/- 10.6 ng/100 ml (n = 17) during the second trimester and 64.0 +/- 18.8 ng/100 ml (n = 29) during the third trimester of pregnancy. The highest values were found at delivery (71.9 +/- 14.2 ng/100 ml; n = 21) and in the cord plasma of newborns (83.4 +/- 14.9 ng/100 ml; n = 21). Significantly lower plasma aldosterone values were found in the plasma of pre-eclamptic women during the third trimester of pregnancy (41.9 +/- 21.3 ng/100 ml; n = 11).

[Pathogenesis of essential hypertension. Plasma noradrenaline, plasma renin and pressor effects of noradrenaline and angiotensin in normotensive patients and patients with essential hypertension].

Plasma noradrenaline, plasma renin and pressor action of exogenous noradrenaline and angiotensin in normotensive subjects and patients with essential hypertension. In normotensive subjects an inverse correlation was observed between the index of sympathetic nervous activity, the plasma noradrenaline concentration during physical exercise, and reactivity to exogenous noradrenaline. The relationship between the index of sympathetic nervous activity and reactivity to noradrenaline was invariably disturbed in age-matched patients with essential hypertension. A multiple regression analysis revealed a highly significant correlation between the combination of both factors and the height of mean arterial blood pressure (r = 0.91). The data suggest that both factors combined, sympathetic nervous activity and pressor response to noradrenaline, are an important determinant of the arterial blood pressure level. An inverse relationship could also be demonstrated between plasma renin concentration and pressor response to angiotensin II in normotensives and hypertensives. However, this relationship was unaltered in the hypertensives. Therefore angiotensin II does not appear to contribute directly to the high blood pressure.

[Effect of the external use of triamcinolone acetonide under foil occlussive dressings on the plasma cortisol level and on the diurnal rhythm of plasma cortisol].

Plasma cortisole concentration was measured in 44 patients with chronic dermatitis before, during, and after occlusive treatment with corticosteroid-containing ointment (triamcinolone-acetonide). In all patients plasma cortisole concentration decreased (p less than 0.001) and the day-night-rhythm disappeared. Five patients in whom less than 20% of the body-surface was treated, showed the same but less significant changes in the plasma cortisole concentration. In some patients the plasma coritsole concentrations increased three days after therapy, and their day-night-rhythm returned. This "systemic effect" of topical treatment with triamcinolone-acetonide is not the reason for the therpeutic efficacy, but rather the local concentration of steroids in the skin.

Kininogen, esterase and kininase activities in human peripheral venous plasma and in plasma from the intervillous space of human placenta, before and during labor.

Kininogen, esterase and kininase activity were measured in the peripheral venous and intervillous placental plasma of women submitted to cesarean section and of women who delivered normally. Kininogen consumption occurred in the intervillous placental plasma during labor. Kininase activity increased in the intervillous placental plasma during labor and was considerably higher than in peripheral venous plasma. Esterase activity was high during labor and decreased after delivery.

[Daily variations of plasma enzymes, plasma albumin and liver glycogen in 8- and 28-month-old rats].

Male Sprague-Dawley rats aged 8 and 28 months (each group 36 animals) were used in our study of the variations during the day in plasma glucose, total protein and albumin, the activities of plasma ALD, LDH, GOT, GPT and the glycogen content of the liver. The variations in these enzymes shown in rats of different ages and at different times of the day were interpreted as expressions of different metabolic conditions of the liver because when changes in the membrane permeability occur or liver cells die changes in plasma enzyme activity can be expected to follow the same direction. Lowered GOT and GPT levels in the old rats and the same glycogen content of the liver as that of the young rats indicated a decreased glucose metabolism in the old animals. The LDH activity did not follow any time pattern. Daily variations in GPT and liver glycogen (the maximum of both parameters was between 4.00 and 8.00) were shown more distinctly in the old rats, whereas the daily variations in the plasma total protein and albumin showed minima between 8.00 and 12.00 in the young animals. The authors concluded that daily rhythms, essential for the function of the organism as a whole, are accentuated with ageing. On the other hand, rhythms of minor importance are desynchronized.

[Variations in blood aldosterone, plasma renin activity and plasma levels of converting enzyme during bicarbonate dialysis in chronic kidney failure].

The changes of plasma renin activity, plasma aldosterone and serum angiotensin converting enzyme activity were studied in 12 patients with chronic renal failure during haemodialysis using a bath containing 30 mEq of bicarbonate and 1.5 mEq of potassium. There is no noticeable relationship between the change in plasma aldosterone and the change of the factors increasing or decreasing its secretion. However, hypokalemia causes regularly a decrease in plasma aldosterone.

Radioimmunoassayable plasma motilin in man. Secretagogues, insulin-induced suppression, renal removal, and plasma components.

In normal humans, significant motilin increases were found after meal ingestion, intraduodenal infusion of fat, and intraduodenal infusion of physiological HCl doses. Only a non-significant plasma motilin increase was found in response to intraduodenal infusion of cattle bile. Plasma motilin decreased significantly after an intravenous insulin injection. During routine cardiac catheterization in a group of 10 patients plasma motilin was significantly lower in the renal vein than in the femoral vein, femoral artery, right atrium, and hepatic vein, suggesting that the kidneys participate in the removal of motilin from the circulation. Fasting and oral fat-stimulated plasma motilin immunoreactivity eluted in two peaks on a Sephadex G-50 Fine column. The two peaks behaved identically with porcine motilin in dilution series.

Degradation of apolipoprotein B-100 of human plasma low density lipoproteins by tissue and plasma kallikreins.

Human plasma low density lipoproteins (LDL) contain one major apoprotein of apparent Mr = 550,000 designated apolipoprotein B-100 (apo-B-100) and in some LDL preparations, minor components termed apo-B-74 (Mr = 410,000) and apo-B-26 (Mr = 145,000). The structural and metabolic relationships among these LDL apoproteins remain obscure. In the present study, we show that the mixing of proteolytic inhibitors with blood at the moment of collection prevents the appearance of apo-B-74 and -26 in plasma LDL indicating that these peptides are derived by proteolytic degradation of apo-B-100. In order to simulate the degradation in vitro, LDL were digested with plasmin, trypsin, chymotrypsin, thrombin, and tissue and plasma kallikreins and the degradation products analyzed by polyacrylamide gradient gel electrophoresis. While plasmin, trypsin, and chymotrypsin caused extensive degradation of apo-B-100, thrombin, and tissue and plasma kallikreins generated limited cleavage patterns. LDL digested with thrombin contained stoichiometric amounts of two peptides with apparent Mr = 385,000 and 170,000. Mixing experiments showed that the thrombin-derived peptides of apo-B-100 did not co-migrate with apo-B-74 and B-26 during electrophoresis indicating that these peptides were different. In contrast, LDL digested with kallikrein contained stoichiometric amounts of two peptides with apparent molecular weights identical to apo-B-74 and -26. Together, the above results indicate that apo-B-74 and -26 are degradation products of apo-B-100 and are not produced by the action of thrombin. Whether the expression of a kallikrein-like activity in vivo accounts for the specific degradation of LDL B-100 to yield LDL B-74 and -26 remains to be determined.

Carbohydrate structure of hamster plasma fibronectin. Evidence for chemical diversity between cellular and plasma fibronectins.

The structure of the carbohydrate moiety of hamster plasma fibronectin was determined. The glycopeptides and oligosaccharides released by hydrazinolysis (Fukuda, M., Kondo, T., and Osawa, T. (1976) J. Biochem. (Tokyo) 80, 1223-1232) were analyzed by methylation and exoglycosidase digestion and the major oligosaccharide unit of hamster plasma fibronectin was shown to be (formula, see text) The structure of carbohydrate moiety of plasma fibronectin shown above (NeuGly, N-glycolyl-neuraminic acid) and that of cellular fibronectin (Fukuda, M., and Hakomori, S. (1979) J. Biol. Chem. 254, 5451-5457) are distinctly different with respect to the linkage of sialic acid, the degree of sialylation and the absence or presence of fucose. These results, therefore, provide the evidence for chemical diversity between cellular (fibroblast) and plasma fibronectins.

Studies of the effect of mode of delivery: obstetric analgesia and anaesthesia on maternal Plasma Renin Concentration (PRC) and Plasma Renin Activity (PRA) in the normotensive and hypertensive primigravidae.

This study evaluated, the influence of mode of delivery, obstetric analgesia and anaesthesia on the maternal plasma renin angiotension system (RAS) in the normotensive primigravidae (n-10) and those with Pregnancy Induced Hypertension (PIH) (n-18). A total of 56 plasma samples from these subjects were assayed for Plasma Renin Concentration (PRC) and Plasma Renin Activity (PRA), using the radio-immunoassay technique. The normotensive subjects had normal delivery, while their hypertensive counterparts were delivered vaginally under lumbar epidural analgesia (n = 10) and with Caesarean Section (n = 8). The blood sample for the study were taken pre-labour, and immediately after delivery. The mean pre-labour PRC and PRA levels for the normotensive subjects were 5.73 +/- 0.25, and 3.56 +/- 0.13 ngml.1hr-1; and the post-delivery PRC and PRA values were 4.43 +/- 0.18 and 2.1 +/- 0.05ngml-1hr-1 respectively. The mean pre-labour PRC and PRA levels for the hypertensive subjects, who were delivered under epidural analgesia were 6.38 +/- 0.52 and 3.64 +/- 0.09 ngml-1hr-1 and the post-delivery values for this group were 5.04 +/- 0.21 and 2.34 +/- 0.07 ngml-1hr-1 respectively. The mean pre-labour PRC and PRA levels for the hypertensive subjects who were delivered by Caesarean Section were 5.87 +/- 0.36 and 3.83 +/- 0.36 ngml-1hr-1 and their post-delivery PRC and PRA values were 4.55 +/- 0.30 and 2.30 +/- 0.09 respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

Elevated plasma lipid peroxide content correlates with rapid plasma clearance of all-trans-retinoic acid in patients with advanced cancer.

The addition of lipid hydroperoxides greatly accelerates the rate of oxidative catabolism of all-trans-retinoic acid (RA) in human cell microsomes; hydroperoxy metabolites of the arachidonate cascade are particularly active in the microsomal system. We have measured the plasma content of lipid peroxides in cancer patients during the course of therapy with RA, seeking to assess whether a correlation exists between the rate of oxidative catabolism of exogenously administered RA and whole body lipid peroxide levels. The assay used for plasma lipid peroxides is the capacity to react with thiobarbituric acid under specified conditions; the result is expressed as TBARS (thiobarbituric acid reactive substances). RA administration produced its own accelerated clearance RA within 72 h. Patients were considered to have "normal" or "rapid" baseline catabolism of RA if their Day 1 area under RA concentration over time curve was greater or less than 300 ng.h/ml, respectively. The mean plasma TBARS levels were: 12 normal volunteers = 0.14 microM; 19 "normal" RA catabolizers = 0.25 microM; and 14 "rapid" catabolizers = 0.82 microM. P = 0.008 (rapid catabolizers versus normal volunteers) and 0.05 (rapid catabolizers versus normal catabolizers). Repeat TBARS determinations were made during the course of therapy in 17 patients, all of whom converted to "rapid" RA catabolism on therapy. An increase in plasma TBARS levels > or = 20% of baseline was observed in 5 of 5 prostate cancer patients and 8 of 12 lung cancer patients treated with continuous RA therapy for 2 and 4 weeks, respectively. These observations support the hypothesis that high levels of lipid peroxides and rapid oxidative catabolism of RA are positively correlated.

[Value of oxypurines and uric acid in plasma, renal excretion of oxypurines and uric acid as well as plasma adenosine deaminase and AMP deaminase activity in patients with essential hypertension].

Serum uric acid and oxypurines (hypoxanthine and xanthine) renal excretion of uric acid and oxypurines as well as plasma adenosine deaminase activity and AMP deaminase activity were studied in 18 patients with essential hypertension and in 17 healthy subjects. The aim of the study was to evaluate uric acid production rate in essential hypertension. Serum uric acid was significantly higher (7.04 +/- 2.03 mg% = 370.5 +/- 106 mumol/l; p < 0.01) in essential hypertension in comparison with control group (5.2 +/- 1.0 mg% = 275.0 +/- 51.9 mumol/l) and plasma oxypurines were increased insignificantly. Impairment of fractional excretion of uric acid (p < 0.05) was found in patients with essential hypertension. Plasma adenosine deaminase activity and plasma AMP deaminase activity did not differ in the studied groups. Increased production of uric acid does not contribute the incidence of hyperuricemia in essential hypertension. The results suggest that tubular defect of oxypurines excretion similar to that of uric acid exists in patients with essential hypertension.

Increased plasma corticosterone and decreased plasma thyroid-stimulating hormone levels in rats treated with vincristine.

A single i.p. injection of vincristine (1000 microgram/kg) into rats increased the plasma concentration of corticosterone after a latent period of 4 hr; the effect lasted for 48 hr. The response was dose related with the threshold dose being 100 microgram/kg and the maximal effect occurring after 250 microgram/kg. Vincristine also increased plasma corticosterone levels in hypophysectomized rats, suggesting that the drug may have a direct action on the adrenal gland. The injection of 500 or 1000 microgram/kg also reduced the plasma concentration of thyroid-stimulating hormone. Twelve and 18 hr after the injection of vincristine (1000 microgram/kg), the plasma concentration of thyroid-stimulating hormone was reduced, whereas the hypothalamic content of norepinephrine, a neurotransmitter involved in the regulation of thyrotropin-releasing hormone-thyroid-stimulating hormone secretion, remained unchanged. The adrenocortical stimulation produced by vincristine may play some role in the antineoplastic effects of this drug.

Prebeta-1 HDL in plasma of normolipidemic individuals: influences of plasma lipoproteins, age, and gender.

Prebeta-1 HDL is a molecular species of plasma HDL of approximately 67 kDa mass that contains apolipoprotein A-I, phospholipids, and unesterified cholesterol. It participates in a cyclic process involved in the retrieval of cholesterol from peripheral tissues. In this cycle, unesterified cholesterol from cells is incorporated into prebeta-1 HDL, providing a substrate for esterification of cholesterol by lecithin:cholesterol acyltransferase. Prebeta-1 HDL then becomes incorporated into larger HDL species of alpha mobility as esterification proceeds and is regenerated during the transfer of cholesteryl esters from alpha HDL particles to acceptor lipoproteins. Thus the steady state level of prebeta-1 HDL in plasma reflects the relative efficiencies of the major metabolic processes involved in its generation and removal. We have used an isotope dilution technique to measure prebeta-1 HDL levels in the plasmas of 136 normolipidemic individuals (46 M, 90 F). The mean absolute concentration of prebeta-1 HDL as apolipoprotein A-I was 68 +/- 40 microg/ml for women, and 84 +/- 49 m/ml for men. Prebeta-1 HDL represented 5.5 +/- 3.3% of total apolipoprotein A-I in women, and 7.2 +/- 4.0% in men. The distributions of both absolute and percent prebeta-1 HDL are highly asymmetric, with skew toward higher values. However, the skew appears not to be attributable to either plasma cholesterol or triglyceride levels which are also skewed in population samples. The percent prebeta-1 HDL was negatively correlated with HDL cholesterol levels (P < 0.0001), whereas absolute levels of prebeta-1 HDL were positively correlated with apolipoprotein A-I and negatively correlated with HDL cholesterol (P, for both, < 0.0001). Multiple linear regression analysis revealed effects of age and gender, but no association with lipoprotein fractions other than HDL. Lower levels of prebeta-1 HDL were associated with female gender in all models.

Gas-liquid chromatographic determination of azinphos ethyl in human plasma and in mouse plasma, tissue, and fat.

A new method for the determination of azinphos ethyl (O,O-diethyl-S-(4-oxo-1,2,3-benzotriazin-3(4H)-ylmethyl) phosphorodithioate) in human plasma and in mouse plasma, tissue, and fat has been developed. The method is based on extraction with benzene or hexane and cleanup of fat and tissue samples by a minicolumn containing Florisil and sodium sulfate. Azinphos ethyl is eluted from the column with 10% acetonitrile in benzene and is concentrated to an appropriate volume for gas-liquid chromatographic analysis, using a 63Ni electron capture detector and a glass column containing 3% OV-1 on Gas-Chrom Q. The method is sensitive to 0.005 ppm in human plasma, 0.01 ppm in mouse plasma, 0.08 ppm in mouse liver, 0.05 ppm in mouse brain, and 0.10 ppm in mouse fat. The limit of detection is 2 pg; mean recoveries ranged from 96 to 98%.

Clonal plasma cells from monoclonal gammopathy of undetermined significance, multiple myeloma and plasma cell leukemia show different expression profiles of molecules involved in the interaction with the immunological bone marrow microenvironment.

The immunological bone marrow (BM) microenvironment plays a major role in controlling growth and survival of clonal plasma cells (PC); this might translate into different patterns of expression of molecules involved in immune responses on PC from different types of monoclonal gammopathies (MG). We have studied the expression of a group of nine such molecules on both BMPC and the plasma of 61 newly diagnosed MG patients (30 MG of undetermined significance (MGUS), 27 multiple myeloma (MM) and four plasma cell leukemia (PCL)) and five normal individuals. Clonal PC from all MG displayed significantly increased levels of CD56, CD86 and CD126, and decreased amounts of CD38 (P<0.001). Additionally, HLA-I and beta2-microglobulin were abnormally highly expressed in MGUS, while CD40 expression was decreased in MM and PCL (P<0.05). Interestingly, a progressive increase in the soluble levels of beta2-microglobulin was found from MGUS to MM and PCL patients (P=0.03). In contrast, all groups showed similar surface and soluble amounts of CD126, CD130 and CD95, except for increased soluble levels of CD95 observed in PCL. Overall, those phenotypic differences are consistent with increased antigen presentation and costimulatory capacities in MGUS, which progressively deteriorate in malignant MG (MM and PCL).

Filtration of fresh frozen plasma used as substitution fluid in plasma exchange in order to remove microaggregates.

Freshly melted fresh frozen plasma (FFP) contains a large number of particles smaller than 20 microns in diameter. These particulate materials (microaggregates) consist of aggregated triglyceride-rich lipoproteins (LA) and precipitable aggregates closely resembling fibrin (FA). Both LA and FA injected into a rat's tail vein were trapped in the lung and formed emboli in the pulmonary arterioles. To prevent the passage of these microaggregates into the systemic circulation, a new circuit was devised for plasma exchange containing a filter with a pore size of 0.2 microns. In 17 patients with fulminant hepatitis who received plasma exchange transfusions and subsequently died and were autopsied, the incidence of pulmonary complications was significantly lower in the 12 patients who received filtered FFP. According to these results, the possibility of pulmonary complications due to micro-embolism developed by microaggregates in FFP should be taken into account when a large amount of FFP is repeatedly infused into critically ill patients.

Ultrastructural characteristics of the plasma cells of a patient with nonsecretory myeloma and plasma cell leukemia.

The clinical features and the ultrastructural findings of the bone marrow and peripheral blood plasma cells of a patient with nonsecretory myeloma and acute plasma cell leukemia are described. The internal ultrastructure of the cells was characterized by invaginated nuclei, cytoplasmic filaments and scanty endoplasmic reticulum. Scanning electron microscopy examination revealed the presence of numerous membranal buddings giving the cells a grape-like appearance. The relationship of the morphological findings to the function of the plasma cells is discussed.
