Severe migratory granulomatous reactions to silicone gel in 3 patients.

In humans implanted with silicone gel breast prostheses, a mild foreign body response results in the formation of a collagenous capsule around the prosthesis. Although many such patients may show evidence of a microscopic granulomatous foreign body reaction upon examination of capsular material at explantation of a prosthesis, it is unusual to have large, palpable granulomas, even in the presence of rupture or leakage. Rare patients have had severe local inflammation and complications resulting from silicone migration to the axilla, arm, or abdominal wall. We describe 3 patients who had deforming granulomas after implant rupture, along with other consequences of silicone gel migrating down the upper extremity. Silicone gel, once it leaves the implant, is not biologically inert and in some persons can elicit profound pathologic responses.

[Cancer of the male breast].

BACKGROUND: Male breast cancer is a rare type of neoplasm, being 1% of all breast tumors. An increasing relevance is given to genetic factors. Familiarity is present in 10% of cases reported in literature. Gynecomastia is frequently associated. Personal experience about this pathology is compared with the most recent data of the literature. METHODS: Nine cases of male breast cancer observed among 519 breast tumors operated between 1982 and 1997 are reported. Etiologic and prognostic aspects, surgical and complementary therapy of breast cancer in man are examined and the high rate of II-III stage patients, mostly ER+ and PR+ is underlined. Diagnosis is reached by ultrasonography and mammography, after an accurate clinical examination and confirmed by cytology after needle biopsy. The choice operation is total mastectomy with axillary lymphadenectomy according to Patey. RESULTS: According to our experience, 5 patients died for non neoplastic pathology, 1 patient lost at follow-up, 3 patients still alive after 66, 60 and 12 months respectively. CONCLUSIONS: Male breast cancer is similar to the female one, but characterized by a higher hormone receptors positivity. Our survival data have no statistic significance. It is still discussed if prognostic difference between men and women is present or not.

[Aspects of the development in dysphagia and its functional dynamics].

100 patients (88 women, 12 men) of 16-64 years of age with functional dysphagia were examined. In all the cases the disease develops under the influence of stress in individuals with high anxiety, easy formation of dysthymic, obsessive-phobic reactions in the situation of frustration. Functional dysphagia arises most frequently at the age of 20-40 years. 4 stages pass in the course of the disease: a stage of reactive (neurotic) depression; psychosomatic reactions; psychosomatic cycles (mono- and polysomic); psychopathization. Psychopathization manifests pathologic (psychosomatic) development of personality observed usually in long-term course of dysphagia.

[Psychiatric diseases and driving fitness (author's transl)].

Mental factors frequently play a decisive role in the conditional setup of many accidents. Analysis of the mental factors determining the cause of accidents show these to be by far often mental factors of a non-pathological nature, such as personality factors, disordered concentration and alertness in conflict situations, overstrain and fatigue, than factors conditioned by illness, e.g. as associated with depression or schizophrenia. Psychological alterations in subjects of advanced age constitute a border area of high significance in medical science concerned with traffic safety. In the consulting room, particular attention should be paid to the recognition of potential risk constellations in traffic precipitated by alcohol or drugs.

[Sudden death and physical activity and sports].

A growing number of people are involved in recreational physical activity. We examined the clinical and cardiac pathological patterns in 29 persons who died suddenly during a sport activity and were autopsied at the Institute of Forensic Medicine of Paris, 1990-1999. Twenty-seven males ranging in age from 13 to 57 years, mean 30.5, and two females, 8 and 60 years old, died suddenly during a sport activity. The sports included running, 11 cases; cycling, 4; soccer, 3; swimming, 2; basket-ball, 2; rugby, 1, gymnastic exercises, 2; tennis, 1; gockart, 1; skiing, 1; hand-ball, 1 case. The causes of deaths were as follows; coronary artery disease, 9 cases; congenital anatomic anomaly of a coronary artery, 1; hypertrophic cardiomyopathy, 5; right ventricular dysplasia, 3; myocarditis, 2; endomyocardial fibrosis, 1; bridging of the left anterior descending coronary artery, 2; stroke, 2; rupture of aortic aneurysm, 4. Despite severity of lesions, only 4 victims had a known cardiovascular disease. Efforts should be continued to improve sensitivity and specificity of diagnostic tools and screening strategies. Therefore, medico-legal autopsies should be systematically performed for a better knowledge of sport-related mortality.

[Analysis of mouse model exhibiting neurofibrillary changes].

Dysfunction and filamentous microtubule-binding tau protein are key markers of neurodegenerative pathologies, including the pathology and neural degeneration associated with Alzheimer's disease (AD). Immunocytochemical studies of NFT-bearing neurons showed that NFTs are composed of ubiquitin and phosphorylation-dependent tau. Congo-red birefringency and thioflavin-S reactivity in NFT-bearing neurons also demonstrated that the tau aggregation forms a beta-sheet structure. Discovery of the molecular mechanisms of NFT formation may lead to more insight about events occurring during neurodegeneration. In frontotemporal dementia parkinsonism 17 (FTDP17), genetic studies indicated that tau is a causative gene, and mutation is found in exons and introns of tau gene. A patient who possesses this mutation exhibits pathologically NFT and clinically personality change and cognitive dysfunction. Then, we produced the Tg mice expressing human longest tau with missense mutation V337M. In the present study, neurons of hippocampus and cerebral cortex in our Tg mice showed phosphorylated and ubiquitinated tau aggregations with a beta-sheet structure. This was demonstrated by Congo-red and thioflavin-S positive staining, a histological criterion used to identify NFTs observed in neurodegenerative disorders. The mice also displayed altered behaviors that were associated with NFT formation. Thus, V337M mice provide a first animal model exhibiting altered behavior due to NFTs.

Metabolic effects of weight reduction in obese diabetics and nondiabetics.

A weight loss of about 10 kg was acheived by means of individual periods of total fasting, hypocaloric diets and physical activity in three groups of patients: maturity-onset diabetics, patients with pathological OGTT and overweight persons with normal OGTT. Adipocyte volume decreased in all groups, glucose tolerance improved. IRI curves showed a significant overall lowering and FFA concentrations during OGTT were dimished.

The properties of perimetric thresholds in normal and glaucomatous eyes.

In perimetry, threshold is defined as the stimulus intensity at which the frequency of perception is 50%. Around the threshold, there is a transition zone in which the frequency of perception turns from 0% to 100%. We examined the frequency-of-seeing-function in 21 persons with and without pathological damage at various locations in the visual field. We used the classic method of constant stimuli: 12 different stimulus intensities were presented 25-50 times each. The results were: (1) The frequency-of-seeing curve showed the typical S-shaped transition zone as is known from other psychophysical thresholds. The slope of the curve and thereby the size of the transition zone varied by a factor of 17. The 'threshold coefficient,' which is the difference between the intensities that provoke a 50% and a 84% frequency-of-seeing, ranged from 1.13 to 19.83 dB. (2) The threshold coefficient was strongly correlated with the threshold level itself (r2 = 0.596). (3) Other influencing factors such as pathological damage or peripheral location appeared to be covariant, because they influenced the threshold level. A substantial influence of these factors on the threshold coefficient could not be found. Different fluctuations of the threshold are therefore mainly an effect of the threshold level. This may serve as a anticipatory knowledge that allows the optimization of staircase measurement procedures and enables the improvement of the statistical analysis of visual fields.

[Comprehensive prophylactic examinations of women for the detection of early forms of breast cancer].

Under examination were 2699 females over 30 years of age having no complaints of the mammary gland induration. The following diagnostic tests were used: 1) clinical; 2) thermography of mammary glands; 3) mammography in two projections. Breast cancer was revealed in 12 females (in 4.4 per 1,000 persons under examination). The clinical method solely revealed mammary gland cancer in 0.14%, thermography--in 0.28%, mammography-in 0.57%. A two-step examination, when persons suspected of breast pathology being preliminary identified, enabled the recognition of breast cancer in 2.97% (29.7 per 1,000 examined subjects). 41.6 per cent of the detected tumors proved to be lobar carcinoma in situ, 33 per cent--microfoci of scirrhous and solid cancer. The conclusion is drawn on somewhat limited opportunities of thermography as a screening method due to very frequent pseudopositive diagnoses.

Renal cell carcinoma metastatic to the ovary: a report of three cases emphasizing possible confusion with ovarian clear cell adenocarcinoma.

The clinical and pathological features of three personally observed and six previously reported cases of renal cell carcinoma metastatic to the ovary are reviewed. The patients' ages ranged from 39 to 64 (average, 52) years. In five patients the ovarian tumor was discovered first. In four of these patients renal tumors were detected during the initial clinical studies or in the early postoperative period, but in the fifth the renal primary tumor was not detected until 8 years later. The ovarian tumor in two cases was initially misdiagnosed as a primary ovarian clear cell carcinoma. In the remaining four patients the ovarian metastasis was detected 5 months, 12 months, 19 months, and 11 years after a renal tumor had been removed. In two patients the initial clinical manifestations were due to a metastasis of the renal tumor, to the thyroid gland in one and to the vagina in the other. The renal tumors in these nine patients typically were well-differentiated renal cell adenocarcinomas of clear cell type. The ovarian tumors measured from 7 to 18 (average, 12.5) cm in greatest dimension; two of them were bilateral. Grossly they were usually solid or solid and cystic; one was a unilocular cyst with a predominantly smooth lining and a 2.5-cm solid nodule in one area. The solid component of the tumors was typically either uniformly yellow or had focal yellow areas with hemorrhagic foci. Microscopic examination showed a relatively uniform picture in most cases: solid or alveolar nests of epithelial cells with abundant clear cytoplasm or tubules lined by clear cells and containing intraluminal eosinophilic material and extravasated blood.(ABSTRACT TRUNCATED AT 250 WORDS)
