Caring for survivors of torture in an urban, municipal hospital.

Survivors of torture can present with multiple health consequences, both physical and psychological, which can persist even years after the abuse. The authors developed a multidisciplinary program in the primary care medical clinic of an urban municipal hospital in New York City serving an ethnically diverse population to provide multidisciplinary care to survivors of torture and their families.

Evaluation of a deformable musculoskeletal model for estimating muscle-tendon lengths during crouch gait.

The hamstrings and psoas muscles are often lengthened surgically in an attempt to correct crouch gait in persons with cerebral palsy. The purpose of this study was to determine if, and under what conditions, medial hamstrings and psoas lengths estimated with a "deformable" musculoskeletal model accurately characterize the lengths of the muscles during walking in individuals with crouch gait. Computer models of four subjects with crouch gait were developed from magnetic resonance (MR) images. These models were used in conjunction with the subjects' measured gait kinematics to calculate the muscle-tendon lengths at the body positions corresponding to walking. The lengths calculated with the MR-based models were normalized and were compared to the lengths estimated using a deformable generic model. The deformable model was either left undeformed and unscaled, or was deformed or scaled to more closely approximate the femoral geometry or bone dimensions of each subject. In most cases, differences between the normalized lengths of the medial hamstrings computed with the deformable and MR-based models were less than 5 mm. Differences in the psoas lengths computed with the deformable and MR-based models were also small (<3 mm) when the deformable model was adjusted to represent the femoral geometry of each subject. This work demonstrates that a deformable musculoskeletal model, in combination with a few subject-specific parameters and simple normalization techniques, can provide rapid and accurate estimates of medial hamstrings and psoas lengths in persons with neuromuscular disorders.

Health problems following Campylobacter jejuni enteritis in a Lancashire population.

OBJECTIVES: Campylobacter jejuni enteritis can lead to musculoskeletal, neuropathic or other health sequelae. We investigated the coexistence, seasonal occurrence, strain-type associations and impact on work capacity of different health problems following C. jejuni enteritis in a Lancashire population during 1999 and 2001. METHODS: A semistructured questionnaire was used to characterize health problems that occurred in the community after laboratory-confirmed episodes of C. jejuni enteritis. The questionnaire was posted to all adults in the Preston and Chorley area who developed C. jejuni enteritis in 1999 or 2001. All Campylobacter isolates from this population were serotyped. RESULTS: Several types of sequelae occurred consistently in both years, including the coexistence of musculoskeletal and neuropathic problems. There was no evidence of C. jejuni strain-type associations or seasonal preponderance for any type of sequela. The overall health impact of C. jejuni enteritis, as measured by workdays lost, was high in this population. CONCLUSIONS: A variety of health problems occur consistently following C. jejuni enteritis and substantially increase morbidity due to campylobacteriosis in the community.

Do physical therapists in the United kingdom recognize psychosocial factors in patients with acute low back pain?

STUDY DESIGN: A cross-sectional, descriptive vignette survey of practicing, musculoskeletal physical therapists. OBJECTIVES: The aim of this study was to determine if musculoskeletal physical therapists, in the United Kingdom, recognize when patients with low back pain (LBP) are at risk of chronicity due to psychosocial factors. A secondary aim was to explore the advice they give to patients about work and activities. SUMMARY OF BACKGROUND DATA: Psychosocial factors have been shown to be important in the progression from acute LBP to chronic disability. Early identification of individuals at risk of developing chronic disability is important to enable targeted intervention. METHODS: Three vignettes were written based on acute LBP patients attending for physical therapy and incorporated into a self-completed postal questionnaire sent to a simple random sample of musculoskeletal physical therapists in the United Kingdom (n = 900). After one reminder, 20% of nonresponders were sent a further questionnaire (n = 80). Data were analyzed using the Statistical Package for the Social Sciences (SPSS version 11). RESULTS: The response rate was 57.7% (n = 518) with 453 meeting all inclusion criteria. The sample consisted of physical therapists working in the musculoskeletal field across all practice settings. Most correctly rated the chronicity risk of the low- and high-risk patient vignettes. Advice to restrict work and activity was common. CONCLUSIONS: Most physical therapists recognize when patients are at high risk of developing chronicity, yet many recommend the patient limit their activity levels and not work. Advice to "not work" is associated with more severe perceived spinal pathology, suggesting persistence of the biomedical model for LBP.

Assessing the relationship between the use of nonsteroidal antiinflammatory drugs and necrotizing fasciitis caused by group A streptococcus.

Group A streptococcal (GAS) necrotizing fasciitis is a rapidly progressive soft tissue infection. Elderly and chronically ill individuals are at greatest risk, particularly when skin breakdown is present. Reports suggest that nonsteroidal antiinflammatory drugs (NSAIDs) increase the risk of developing GAS necrotizing fasciitis, impede its timely recognition and management, and accelerate the course of infection. We present a literature review conducted to examine these hypotheses and present a case of GAS necrotizing fasciitis associated with rofecoxib use. Initial symptoms of fasciitis may be nondescript, mimicking more benign conditions such as cellulitis, arthritis, or musculoskeletal pain. Case reports and retrospective studies suggest that the application of NSAIDs to relieve these nonspecific symptoms can delay diagnosis and treatment of GAS necrotizing fasciitis. However, prospective studies do not support a risk of developing GAS necrotizing fasciitis as a result of NSAID therapy, or a worsening of established streptococcal infection. To avoid the application of NSAIDs in lieu of aggressive medical and surgical management of streptococcal fasciitis, clinicians must understand the early symptoms of GAS necrotizing fasciitis that precede visible soft tissue necrosis. A rational approach to patients presenting with localized soft tissue pain will enhance detection and therapy of this rapidly progressive, life-threatening disease.

Occupational health research in India.

India being a developing nation is faced with traditional public health problems like communicable diseases, malnutrition, poor environmental sanitation and inadequate medical care. However, globalization and rapid industrial growth in the last few years has resulted in emergence of occupational health related issues. Agriculture (cultivators i.e. land owners + agriculture labourers) is the main occupation in India giving employment to about 58% of the people. The major occupational diseases/morbidity of concern in India are silicosis, musculo-skeletal injuries, coal workers' pneumoconiosis, chronic obstructive lung diseases, asbestosis, byssinosis, pesticide poisoning and noise induced hearing loss. There are many agencies like National Institute of Occupational Health, Industrial Toxicology Research Centre, Central Labour Institute, etc. are working on researchable issues like Asbestos and asbestos related diseases, Pesticide poisoning, Silica related diseases other than silicosis and Musculoskeletal disorders. Still much more is to be done for improving the occupational health research. The measures such as creation of advanced research facilities, human resources development, creation of environmental and occupational health cells and development of database and information system should be taken.

Total tumor cell elimination with minimum damage to normal tissues in musculoskeletal sarcomas following photodynamic therapy with acridine orange.

Acridine orange (AO) has unique biological actions enabling tumor visualization (fluorovisualization) and a strong cytocidal effect (photodynamic therapy: AO-PDT) under illumination with blue light. Accordingly, in this study, we attempted to develop a new surgical technique for total tumor cell elimination using these photodynamic reactions with AO in a mouse osteosarcoma model. The results showed that local tumor recurrence was significantly inhibited (23%) in the group treated with curettage under fluorovisualization and AO-PDT, compared to that (80%) in the control group treated with curettage alone under ordinary light. Therefore, we concluded that the combination of curettage under fluorovisualization and AO-PDT may be useful for total tumor cell elimination with minimum damage to normal tissue in musculoskeletal sarcomas.

Septic arthritis secondary to vesicoureteral reflux into single ectopic ureter.

A 3-week-old male infant, born full term without complication, developed septic arthritis of his left shoulder. His joint fluid, blood, and bone marrow were all positive for Escherichia coli. Urinalysis demonstrated pyuria. Urine culture obtained after one dose of ceftriaxone and several doses of nafcillin was negative. Work-up revealed a refluxing, right single ectopic ureter with severe hydroureteronephrosis and a non-functioning ipsilateral kidney. After appropriate management of the musculoskeletal infection, he underwent a right nephroureterectomy. Coliform septic arthritis is exceedingly rare in children, with only a few cases reported. We report the first case of septic arthritis with anomalous genitourinary tract development as the source of bacterial seeding. This report re-emphasizes the need to screen the urinary tract in all cases of pediatric gram-negative sepsis.

The role of compensation in chronic pain: analysis using a new method of scoring the McGill Pain Questionnaire.

Patients who receive worker's compensation or are awaiting litigation after an accident have long been regarded as neurotics or malingerers who are exaggerating their pain for financial gain. However, there is a growing body of evidence that patients who receive worker's compensation are no different from patients who do not. In particular, a recent study found no differences between compensation and non-compensation patients based on pain scores obtained with the McGill Pain Questionnaire (MPQ). Since the MPQ is usually scored by using rank values rather than more complex scale values, the negative finding might be attributable to the loss of information by using rank values. Consequently, a simple technique was developed to convert rank values to weighted-rank values which are equivalent to scale values. A study of 145 patients suffering low-back and musculoskeletal pain revealed that compensation and non-compensation patients had virtually identical pain scores and pain descriptor patterns. They were also similar on the MMPI pain triad (depression, hysteria, hypochondriasis) and on several other personal that were examined. The only differences were significantly lower affective or evaluative MPQ scores and fewer visits to health professionals by compensation patients compared to non-compensation patients. These results suggest that the financial security provided by compensation decreases anxiety, which is reflected in the lower affective or evaluative ratings but not the sensory or total MPQ scores. Compensation patients, contrary to traditional opinion, appear not to differ from people who do not receive compensation. Accidents which produce injury and pain should be considered as potentially psychologically traumatic as well as conducive to the development of subtle physiological changes such as trigger points. Patients on compensation or awaiting litigation deserve the same concern and compassion as all other patients who suffer chronic pain.

Early identification and intervention in the prevention of musculoskeletal pain.

BACKGROUND: A large number of people suffer from upper extremity disorders, but a few apparently consume the majority of the resources. Early interventions are badly needed to prevent the development of persistent disability. Since psychological factors are central in the development of a chronic problem these might be utilized in this endeavor. METHODS: A series of studies are described where a screening procedure based on psychological risk factors was employed to help identify people at risk for developing long-term work disability. The utility of a cognitive-behavioral group intervention that focuses on coping strategies as prevention was assessed in three randomized-controlled studies where participants had low, medium, and high risk, respectively. RESULTS: The study with low risk showed no significant difference between the groups, while the studies with medium- and high-risk populations demonstrated significantly lower work disability than control groups receiving treatment as usual. CONCLUSIONS: It appears to be feasible to identify patients with high levels of risk and to subsequently lower the risk for work disability by administering a cognitive-behavioral intervention focusing on psychological aspects of the pain problem.

Serum markers of bone metabolism in dogs.

OBJECTIVE: To establish reference values for a panel of serum markers of bone turnover in dogs of various ages. ANIMALS: Dogs in 4 age groups (0 to 1 year; 1 to 2 years; 3 to 7 years; > 8 years). PROCEDURE: Serum concentrations of the carboxyterminal propeptide of type-I procollagen (PICP) and the aminoterminal propeptide of type-I procollagen (PINP), both markers of type-I collagen synthesis (hence, bone formation), were measured by use of commercial human radioimmunoassay kits. Serum concentrations of the carboxyterminal cross-linked telopeptide of type-I collagen (ICTP), a marker for type-I collagen breakdown (hence, bone resorption), also were measured by use of a commercial human radioimmunoassay kit. Serum osteocalcin (OC) concentrations and alkaline phosphatase (ALP) isoenzyme activities were measured by use of techniques developed specifically for dogs. RESULTS: As expected, the highest values for all of the markers were found in young dogs (< 12 months old). Concentrations of OC and ICTP decreased with age, and were lowest in dogs > 8 years old. Total ALP and bone-specific ALP activities initially decreased with age, then increased in dogs > 8 years old. CONCLUSIONS AND CLINICAL RELEVANCE: Serum markers of bone turnover may be useful diagnostic and prognostic tools for management of dogs with musculoskeletal disorders.

The use of Roentgenstereofluorogrammetry to predict the 3-D spatial coordinates of points in low speed events.

Imaging technologies such as cine-radiography, cine-MRI, and X-ray stereo photogrammetry have become popular diagnostic tools in biomechanical studies of musculoskeletal systems. However, their widespread use for research purposes has been restricted due to their high cost and somewhat limited availability. In an attempt to develop a reliable low cost system, a dual-fluoroscopic system capable of tracking the 3-D spatial motion of discrete landmark points in real time was developed. A simple methodology was developed to convert the analog fluoroscopic images to digital files for post-processing. A custom computer code based on the principles of X-ray stereo photogrammetry was also developed to predict 3-D coordinates from the 2-D images from the individual flouroscopes. The goal of the current study was to assess the accuracy and resolution of this system by using it to predict the motion of a test point following a known curvilinear trajectory. Our system predicted the time-varying motion and path of the test point within 0.25%. However, the current system is limited to studying low speed events only (max event frequency of 3Hz) due to the limited sampling frequency of the A/D conversion employed here.

Strain on the back in concrete reinforcement work.

To investigate the long term effect of heavy construction work on the back the occurrence of postures, lifting, carrying, and accidents in concrete reinforcement work and in maintenance house painting were measured. The 32 620 observations covering 272 work hours showed that reinforcement work necessitated stooped postures and heavy lifting more often than did painting. Reported minor back accidents were more than ten times as common in reinforcement work than in painting (1.3 compared with 0.11 accidents per man-year, p less than 0.001). Accidents of the musculoskeletal system, registered by the insurance companies, were also several times more common in reinforcement work (81 compared with 25 per 1000 man-years, p less than 0.001). The premature development of lumbar degeneration detected in the clinical study of the reinforcement workers was evidently due to the occupational strain on the back. The different types of hazardous back loads probably potentiate the effects of each other.
