Drug sensitivity and cell cycle parameters of L1210 cells in culture in exponential (10(5) to 4 x 10(5) cells/ml) and plateau phase (greater than 8 x 10(5) cells/ml) of growth were compared. The percentage of cells in G1, S, and G2 + M for exponential and plateau-phase cultures was 24.1, 70.0, and 5.9, and 42.5, 45.6, and 11.9, respectively. These values correspond to those reported previously for early and late L1210 mouse leukemia cells. Cell survival (measured by cloning) after drug exposure showed that actinomycin D, 1-(2-chloroethyl)-3-cyclohexyl-1-nitrosourea, chlorozotocin, and streptozocin were all equally lethal to cells in both phases, and 1-beta-D-arabinofuranosylcytosine, high-specific-activity (20Ci/mmole) [3H]thymidine, vincristine, daunomycin, adriamycin, and 5-fluorouracil were more lethal to exponential cells than to plateau-phase cells. The percentage kill by S-phase-specific agents such as 1-beta-D-arabinofuranosylcytosine and[3H]thymidine corresponded well with the percentage of cells in S. Finally, 1,3-bis(2-chloroethyl)-1-nitrosourea was more toxic to plateau cells than to exponential cells.
In the period from Jan 1 1984 to Dec 31 1990 clinical and radiological evidence of brain glioma was found in 120 cases treated previously surgically. The group comprised 55 women (46%) and 65 men (54%). At the time of tumour diagnosis their age was ranged 40 to 60 years. All patients received non-radical surgical treatment, supplemented with Co60 radiation in 104 cases, in 3 cases Co60 treatment was given together with chemotherapy (CCNU) and 12 patients received no complementary treatment. Thirty four patients (28%) had reoperations, in two cases even twice. In 86 cases (72%) treatment was palliative. Three types of secondary tumour regrowth were discerned. Reoperation prolonged survival and its effectiveness was greatest in regrowth type I. The shortest survival till the appearance of regrowth signs and the shortest survival after recurrence were in type II of regrowth.
The effectiveness of combined treatment of posterior fossa tumours was analysed using the clinical material of a neurosurgery department. The material comprised 242 patients treated in the years 1980-1987 with 211 suboccipital craniotomies performed. In cases of cerebellar astrocytoma one-year survival rate was 9% and five year survival in 89% of cases. In the group of medulloblastoma treated in the period 1980-1982 the one-year survival rate was 75%, three-year 39% and five-year 25%. In the group treated in the years 1985-1987 the one-year survival rate was 95% and three-year was 60%. In ependymoma cases the one-year rate was 90%, three-year was 40% and five-year--33%. The use of combined treatment in cases of ependymoma and medulloblastoma improved significantly the therapeutic results.

The EORTC Brain Tumor Group has conducted 5 randomized and prospective phase-III trials in adults with supratentorial malignant gliomas testing the following adjuvant treatments: 1) CCNU 130 mg/m2, q.6 weeks (trial 26741); 2) VM-26 100 mg/m2 on day 1, plus CCNU 130 mg/m2 on day 2, q.6 weeks, given either during radiation therapy (trial 26751) or during 12 weeks prior to irradiation (trial 26841); 3) Misonidazole 12 mg/m2 total dose in 9 fractions (trial 26801); 4) Cisplatin 60 mg/m2 on days 1, 8, 15 and 21 of radiation therapy (trial 26851). None of these treatments did prolong significantly the survival nor the time to recurrence. The most powerful prognostic factors found in every trial were tumor pathology, patients' age and neurological status. Objective remission lasting 6 to 9 months, defined as a clinical improvement persisting after discontinuation of corticosteroids, was observed with CCNU, VM26 plus CCNU or HeCNU in approximately 20% of the patients, while 30% stabilized on HeCNU. The activity of procarbazine, AZQ or DDMP used as single agents was much lower.
Forty-three ambulatory patients with locally advanced or metastatic bronchogenic adenocarcinoma were sequentially treated with two potentially mutually non-cross-resistant chemotherapy regimens. A new regimen, MVPF (mitomycin-c, vinblastine, procarbazine, and 5-fluorouracil), was given until progressive disease occurred. Then, a second regimen--MOCC (methotrexate, vincristine [Oncovin], cyclophosphamide, and CCNU)--was initiated. At further progression, regional disease patients received radiotherapy, whereas extensive disease patients received Phase II agents. Of the 43 patients entered on the study, 40 were evaluable. Three patients withdrew early due to poor tolerance of the regimen. The response rate for MVPF was 33% (12 of 40 PR, 1 of 40 CR) compared to a 4% (1 of 23 PR) response for MOCC (difference: p < or = .03), for a total response rate of 35%. Although there was an initial improvement in survival for responders (31.7 weeks) versus nonresponders (15.7 weeks) at the 75th percentile (p < or = .05), there was no significant difference in median survival. The hematologic toxicity was equivalent for both groups, whereas nonhematologic toxicity revealed a high incidence of nausea and vomiting in the MVPF group. It is concluded that this approach lent itself well to ambulatory care, and MVPF could be considered an alternative to cyclophosphamide-based regimens. However, the absence of a meaningful CR rate and lack of influence of response on median survival were factors limiting its effectiveness.
The treatment of acute myeloid leukemia (AML) permits in a population of 25 to 60 years, a complete remission (CR) about 60 to 85% with relapse free survival at 5 years from 45 to 60%. We report our therapeutic results during two years, from june 1996 to december 1998. 104 patients with the novo AML treated according to AML 06/96 protocol, the mean age was 32.5 years old, from 16 to 55 years old. The hyperleucocytar form (GB > 50,000 elts/mm3) represented the third of the cases, only 98 patients received the induction. 6 patients died before treatment. The whole rate of CR was 55%. The rate of failure was 16%, the deaths was about 15.5%, the relapse represented 30.6% with mean delay about 14.1 months, from 4 to 35 months. The CR has been maintained in 9 patients with mean recession of 53 months, from 36 months to 62 months. The overall survival at 5 years was 9%. Our results are still unsufficient in comparison with the literature and could be improved by recess of ARA-C and donorubicin doses in induction and consolidation, as well as a good knowledge about the cytogenetical aspect of the treated population.
The latest technologies, introduced into neurosurgery during the last 2 decades of the 20th century, ensured progress in the diagnostics and treatment of brainstem tumors--pathologies that remain until now a most complicated problem in neurooncology. The modern diagnostic tools comprising profound dynamic neurological and neurophysiological examinations (AVP, EEG) visualization techniques (CT, MRT with contrasting, MRT, spectroscopy, PET) provide for detecting not only the initial signs of stem tumors and their sizes but also their topographic interaction with the stem structures; it, in its turn, opens up a possibility to choose between the combined surgical treatment and the radiotherapy or chemotherapy alone. The clinical course, diagnosis and combined treatment of 333 children and adults with tumors bordering on the brainstem are analyzed within the case study.
