Comparison of the clinical efficacy of two commercial fatty acid supplements (EfaVet and DVM Derm Caps), evening primrose oil, and cold water marine fish oil in the management of allergic pruritus in dogs: a double-blinded study.

Twenty dogs with atopy or idiopathic pruritus were treated in a double-blinded clinical trial with computer-randomized and computer-generated sequences of 4 fatty acid-containing products: evening primrose oil, cold water marine fish oil, DVM Derm Caps, and EfaVet. Each dog received each product for a 2-week period. Five of 20 dogs (25%) had a good-to-excellent reduction in their level of pruritus with at least 1 of the products: evening primrose oil (2 dogs), DVM Derm Caps (1), EfaVet (1), DVM Derm Caps and cold water marine fish oil (1). Only 1 dog experienced a side effect (loose stools). Clinical response to fatty acid supplements appeared to be quite individualized, and independent of age, breed, sex, weight, duration of disease, specific diagnosis, or number of positive intradermal test reactions.

Intraoperative anaphylaxis: verba volant, scripta manent!

We describe a case of intraoperative gelatine-induced anaphylaxis whose diagnosis was delayed as the use of gelatine during surgical procedures was omitted for two times in patient's medical records. The subject is a 66-year old woman, with a negative medical history of atopy, food and drug allergy, with arterial hypertension on a course of beta-blockers and with bladder carcinoma requiring surgery. The patient had tolerated both general and local anaesthesia on several previous occasions. On the first occasion she experienced arterial fibrillation secondary to a severe episode of hypotension following local anaesthesia, while on a course of beta-blockers. On the second occasion she developed a very severe episode of hypotension followed by the outbreak of a generalised rash during general anaesthesia. The tryptase sera level was 109 mg/L one hour after the reaction had subsided, while the basal values were normal. On the third occasion the patient redeveloped severe hypotension and a generalised rash during general anaesthesia. The allergological work-up was negative, except for intradermal test with gelatine. A study of the intra-cellular cytokines in blood lymphocytes showed a production of IL4 from CD4+ lymphocytes after stimulation by gelatine. The patient underwent a successive surgical procedure without any adverse event.

[A case of pulmonary aspergillosis accompanied with eosinophilic pneumonia].

An infiltrative shadow was observed in the upper lobe of the left of a 60-year-old man lung during the course of paroxysmal tachycardia. Fever and coughing occurred one month later, and ground-glass opacities (GGOs) were found extensively in the lower left lung field. Although his condition strongly suggested allergic bronchopulmonary aspergillosis (ABPA) due to significantly increased eosinophil count in the peripheral blood and bronchoalveolar lavage fluid of the lower lobe of the left lung, anti-aspergillus antibody precipitation, and positive immediate intradermal test, the Rosenberg diagnostic criteria for ABPA were not satisfied. He was given a diagnosis of eosinophilic pneumonia, and GGO disappeared when itraconazole and prednisolone were administered. However, since the shadow in the upper lobe of the left lung later increased and percutaneous thoracocentesis revealed the presence of Aspergillus fumigatus, prednisolone treatment was discontinued and micafungin was administered. Nonetheless, the shadow spread into both lungs and regardless of treatment with amphotericin-B, the patient died. Although the present case is different from a typical ABPA, it indicates that allergic reaction can be induced by Aspergillus infection in the lung field, apart from the central airway. In addition, the development of an appropriate therapeutic strategy against both infection and allergy in such cases is essential.

Lepromatous leprosy: a review and case report.

Leprosy is a contagious and chronic systemic granulomatous disease caused by Mycobacterium leprae (Hansen s bacillus). It is transmitted from person to person and has a long incubation period (between two and six years). The disease presents polar clinical forms (the multibacillary lepromatous leprosy and the paucibacillary tuberculoid leprosy), as well as other intermediate forms with hybrid characteristics. Oral manifestations usually appear in lepromatous leprosy and occur in 20-60% of cases. They may take the form of multiple nodules (lepromas) that progress to necrosis and ulceration. The ulcers are slow to heal, and produce atrophic scarring or even tissue destruction. The lesions are usually located on the hard and soft palate, in the uvula, on the underside of the tongue, and on the lips and gums. There may also be destruction of the anterior maxilla and loss of teeth. The diagnosis, based on clinical suspicion, is confirmed through bacteriological and histopathological analyses, as well as by means of the lepromin test (intradermal reaction that is usually negative in lepromatous leprosy form and positive in the tuberculoid form). The differential diagnosis includes systemic lupus erythematosus, sarcoidosis, cutaneous leishmaniasis and other skin diseases, tertiary syphilis, lymphomas, systemic mycosis, traumatic lesions and malignant neoplasias, among other disorders. Treatment is difficult as it must be continued for long periods, requires several drugs with adverse effects and proves very expensive, particularly for less developed countries. The most commonly used drugs are dapsone, rifampicin and clofazimine. Quinolones, such as ofloxacin and pefloxacin, as well as some macrolides, such as clarithromycin and minocyclin, are also effective. The present case report describes a patient with lepromatous leprosy acquired within a contagious family setting during childhood and adolescence.

Humoral and cell mediated immune responses in scabietic patients.

Humoral and cell-mediated immune responses were studied in forty scabietic patients and twenty healthy control individuals. The parameter findings for quantitative assessment of humoral immune response by the single radial immune diffusion technique indicated that IgA was significantly decreased, IgG & IgM were significantly increased while there was no statistical difference in the level of IgE, C3 & C4 compared to the normal control group. The cell mediated immune response was estimated in vivo by the intradermal test. It showed insignificant decrease in the number of positive reactors versus the control group. The macrophage migration inhibition test as an in vitro index for determining the cell mediated immune response was significantly diminished. This study indicates that humoral and cell-mediated immune responses play an important role in the pathogenesis of scabies.

[Immunological study of inefficiency schizophrenics with deficiency syndrome treated with xin shen ling].

This paper reports 30 cases of chronic schizophrenics with deficiency syndrome who had chronically taken many sorts of neuroleptic medications and other therapies to be ineffective. The 7 immunological functioning markers were detected: phytohemagglutinin (PHA) intradermic test; circulating immune complex (CIC); T, B lymphocytes, null (N), double (D) cell; and large granular lymphocyte (LGL) and to be compared with a control group of 30 healthy individual. The result showed that 6 immunological markers (PHA, CIC and T, B, N and D cell) were significantly different as compared with the control group (P less than 0.01). In order to regulate proportion and function to immune cell, the 30 patients were given to take immuno-modulating herbs (xin shen ling, XSL) during 6 weeks, while their 7 immunological markers were detected before treatment (BT) and after treatment (AT). The results showed that 5 immunological markers (PHA, CIC, T, N, and D cell) of BT were significantly different as compared with that of AT (P less than 0.01). However, the 5 immunological markers (PHA, CIC, and T, N and D cell) of AT were not significantly different as compared with that of the control group (P less than 0.05). The brief psychosis rating scale (BPRS) and nurses observation scale for inpatient evaluation (NOSIE) were used as evaluating changes of clinical symptoms BT and AT. The results showed that BPRS and NOSIE of BT were significantly different as compared with that of AT (P less than 0.05). The clinical efficacious rate was 67%. We have followed up the results for near 3 years which had a relapse of 5 cases of 20 cases be discharged. It seemed that XSL may be one of the preventive relapse agent for these patients.

Iodinated soluble adult worm protein preparations of Schistosoma mansoni do not induce immediate hypersensitivity reactions but retain other immunogenic properties.

Samples of Schistosoma mansoni soluble adult worm proteins (SWAP) were iodinated with 4-15 mumol I/mg protein using iodine monochloride. The capacity to elicit immediate hypersensitivity reactions of the iodinated derivatives was compared to that of the native SWAP preparations. The degranulation of mast cells from infected mice decreased with increasing iodine incorporation and was absent in fully iodinated samples containing 15 mumol I/mg protein. The response of guinea pigs and humans to the intradermal test with iodinated SWAP also decreased in proportion to iodine incorporation, and no responses were obtained with fully iodinated samples. No false-positive tests were observed. Antibodies to the fully iodinated extracts generated in C57BL/10 mice reacted by ELISA with the unmodified proteins and by immunoprecipitation on agar gel. The immunoprecipitation pattern suggested that some epitopes were altered by iodination.

Studies on clinical manifestations, diagnosis and control of paragonimiasis in China.

Paragonimiasis is a common parasitic zoonosis in China. Its clinical manifestations may be classified into two clinical entities, eg, paragonimiasis westermani type and paragonimiasis szechuanensis type. The latter mainly manifests as trematode larva migrans. The immunodiagnostic methods frequently used in the diagnosis of paragonimiasis are intradermal test, CFT and ELISA. The detection of circulating antigens in patients has been used in China for diagnosis and bithionol and praziquantel are drugs of choice in the treatment of paragonimiasis. Combined measures including mass survey, mass treatment and health education have been found to be effective in the control of paragonimiasis in China.

Immunological changes in opportunistic parasitic infections.

This study was done on 110 patients with different types of malignant diseases and 20 healthy controls. Cases were divided into four groups according to parasitological and serological examination. The first group included 37 patients without any parasitic infection, the second 28 patients with no opportunistic parasitic infection, the third group 33 patients with opportunistic parasitic infection while the fourth included 12 patients with mixed opportunistic and non opportunistic parasites. Immunological status was estimated by carrying out leucocyte migration inhibition test and intradermal skin test for cell mediated immunity and quantitative determination of immunoglobulins IgG, IgM and IgA for humoral immunity testing. It was found that cell mediated immunity was decreased in cancer patients compared with the normal control group. Super added parasitic infections lead to greater decrease. Opportunistic parasitic infection aggravated the condition more. Marked suppression of the cell mediated immunity was found in the group of cancer patients with mixed opportunistic and non opportunistic parasitic infections. IgG and IgA were increased in the first three groups and decreased in the mixed one, IgM was increased in the first and second groups while decreased in groups with opportunistic infections.

[Insect venom allergy in childhood].

Hymenoptera venom allergy is relatively rare in childhood. In 59 children, aged 2-16 years, with a systemic allergic reaction we could detect the following findings: There was no difference between males and females. Relatives of these children had more atopic diseases than normals. The degree of clinical manifestation was not so pronounced as in adults. RAST was more sensitive in detecting bee venom allergy whereas intradermal test showed a higher sensitive in wasp venom allergy. The diagnostic value of prick test was low without any obvious reason.

[In vitro and in vivo efficacy of a toxin-neutralizing human staphylococcal immunoglobulin].

The pathogenic relevance of alpha-hemolysin of Staph. aureus in human infections is up to the present in discussion. Numerous therapeutic human trials to modify the outcome of a Staph. aureus infection by giving passively hyperimmune sera did not clarify the situation. First the work of Bhakdi22 has provided us with a rational approach for a systemic use of specific immunoglobulin preparations. A highly purified Staph. aureus alpha-hemolysin was carefully detoxified and injected into volunteers. From these sera were collected by plasmapheresis and a 5S immunoglobulin was prepared. In preclinical trials the antitoxin efficacy was evaluated. We examined in vitro cultures of human thrombocytes and monocytes, and of porcine pulmonary arterial endothelial cells. As in vivo models we used the intoxication of mice and monkeys and an intradermal test in rabbits. In all these models we were able to demonstrate the high antitoxic efficacy of a human anti-alpha-hemolysin-5S-immunoglobulin preparation.

[Chronic urticaria and allergy to Acari. Experience with a specific desensitization therapy].

Eighty-five patients affected by chronic urticaria (45 females and 40 males) were submitted to intradermal tests with house and storage mites in order to evaluate a possible sensitization to these allergens. The same allergological tests were performed in an 80-subject control group. The results showed immediate cutaneous reactivity to one or more mites in 40 patients (47%) while only 6 positivities (7.5%) were found in the control group. 27 patients, out of the 40 allergic to mites, were submitted to specific desensitizing therapy. 6 of them showed a complete recovery, 20 good results and only 1 slight improvement of clinical manifestations. These data allow us to hypothesize that the allergy to mite allergens is an important etiopathogenic factor in several chronic so called "idiopathic" urticarias.

[Nutritional status in stomach cancer--a criterion for evaluating operability?].

The nutritional status of 41 male patients with carcinoma of the stomach was recorded, right after admission to hospital, and was related to tumour-specific operability. The nutritional condition of most of the patients was moderately impaired. Significant differences between operable and non-operable patients were recorded from the following parameters: upper-arm circumference, haemoglobin concentration, serum albumin, pseudocholinesterase, intradermal testing, E-rosette count. The findings thus obtained provided some limited information on operability.

Clinical value of in vitro diagnostic in asthma.

The better comprehension of the dynamics involved in the etiopathogenesis of asthma in determined circumstances needs the complement of immunologic methods. The improved immunologic methods for the diagnosis of allergic diseases in the last twenty years has in general confirmed the results obtained by the intradermal test. However, it is important to bear in mind that in a reliable "in vivo" diagnostic, the technique has to be considered, and that there is a better correlation in the intracutaneous test than in the Prick one. Of the various immunologic tests carried out, the hemagglutination test offers a better reliability in the diagnosis of food-induced bronchial asthma, together with the histamine release test. In our experience, the RAST was less reliable when it came to the diagnosis of food allergy, though on the other hand it was applicable for inhalant allergens (pollens and dermatophagoides). In clinical practice and in daily routine work ups, all these techniques of course, impose an additional cost for diagnosis. Therefore, the use of sophisticated modern immunologic methods has to be reserved unless needed for clinical research, or as happens in some cases, for suspicious clinical histories with negative skin tests, in order to confirm the possible allergens responsible.

Anaphylactic hypersensitivity to mercurochrome (merbrominum).

By the use of intradermal tests, passive transfer (P.K.), and an in vitro histamine release test, we were able to demonstrate immediate hypersensitivity to Mercurochrome in a young man who had suffered anaphylactic shock after having contact with this antiseptic. Cross-reactions among different organic mercurial compounds were observed.

Immunological screening of welders.

74 healthy welders were subjected to immunological screening. Serum immunoglobulins (IgG, IgA, IgM) were measured as well as total and active E-RFC and cellular immunity was evaluated using the intradermal testing of PPD, candidin D-7, trichophytin D-5 and tetanus anatoxin. The obtained results suggest suppression of the T and B lymphocyte immune systems as expressed by decreased levels of serum IgG and total E-RFC. Cell-mediated immune deficiency was found in 21.6% welders. Suppression of the immune system may be accounted for by a complex effect of occupational factors, such as manganese compounds, vibration and noise.

[A case of anaphylactoid reactions to lactated Ringer's solution with 5% maltose].

A 27-years-old man was admitted for correction of nasal fracture. He presented with circulatory collapse and generalised erythema on induction of general anesthesia and during preparation of intradermal test for identifying the offending agent. As both of these responses occurred during intravenous infusion of lactated Ringer's solution with 5% maltose, anaphylactoid responses to this intravenous solution were strongly suspected.
