This case study reports on a suicide made to look like murder. The case highlights the issue of false claims of criminal victimization and suggests the need for a reexamination of the dynamics of factitious allegations of criminal victimization. Factitious complaints of mental or physical illness are typically self-reported to psychiatric or medical authorities. This paper illustrates a different kind of phenomenon: one in which an injury or disorder is brought to medical attention by way of the police, who report the death or injury as arising from criminal victimization.
The vast majority of prepubertal gynecologic problems fall into three categories. The most serious category includes those "things that bleed": sarcoma botryoides, trauma (including sexual abuse), vaginal foreign objects, condylomata, urethral prolapse, and single organism vaginitis. Very rarely, the clinician will see precocious menarche, metastatic Crohn's disease, vascular vulvar lesions, and factitious cases. The next category contains entities that have an abnormal appearance: ambiguous genitalia, periurethral cysts of the newborn, hymenal variants, and agglutination of the labia and vulva. Rarely, an underlying skin disorder such as lichen sclerosus, seborrhea, or atopic vulvitis will be seen. The last and most bothersome category, distinguished by its symptoms of pruritus and discharge, includes the most common types of vulvovaginitis.
BACKGROUND: The prothrombin time (PT) test, which is the most common coagulation test used in the outpatient setting, has not been recommended for office laboratory use because it has been technically difficult to perform. Consequently, serious errors in patient care could occur because of an erroneous result. The Coumatrak (E.I. Du Pont, Wilmington, Del) now allows nontechnically trained office staff to perform PT tests using a fresh capillary whole blood sample, individually packaged reagent cartridges, and a portable battery-operated instrument. METHODS: Coumatrak PT testing was compared with standard methods for both precision and accuracy, using protocols developed by the National Committee for Clinical Laboratory Standards (NCCLS). Reagent stability and operator variability were also studied. RESULTS: The results produced by a trained technologist and nontechnically trained staff were comparable. Test results obtained with the Coumatrak were approximately 10% higher than results obtained using standard laboratory equipment and methods using comparable blood samples from the same patients. It was found that the capillary blood specimen had to be rapidly transferred to the reagent cartridge in order to avoid factitiously low results. CONCLUSIONS: The Coumatrak can rapidly provide PT test results that are clinically useful for the office management of patients being treated with a warfarin anticoagulant and for the diagnosis of selected disorders. The system was found to be easy to operate, appropriate for use by individuals with little laboratory experience, and was subject to few operational problems during this study.
Food allergy in childhood is a popular subject which has attracted disapprobation from certain quarters because of overinflated claims based on flimsy evidence. In this article food intolerance and allergy are defined and the pathogenesis of food allergic reactions is considered. There is a description of the role that food allergy may play in urticaria, angioedema, anaphylaxis, eczema, asthma, rhinitis, cow's milk sensitive enteropathy, infantile colitis, inflammatory bowel disease, migraine and hyperactivity. Factitious food allergy is discussed and the general unhelpfulness of 'allergy testing' commented upon. Finally there is a description of the use of various exclusion diets in the diagnosis and treatment of food allergic disorders. It is concluded that food allergy is important in an increasing number of childhood diseases, but it is not clear what proportion of children with a given condition will respond to dietary measures. The importance of ensuring that exclusion diets are nutritionally adequate is stressed, and there is a plea to remember that an exclusion diet might be worse than the disease itself.

We report an uncommon case of a 38-year-old woman who admitted for bladder lithiasis undergoing repeated surgical management and being apparently recurrent. The intravenous pyelography and retrograde urethrocystography clear the urinary tract. Biologic evaluation eliminates any metabolic or endocrine cause. The confirmation of a factitious origin is given by fluorescence X analysis of the extracted stones which conclude to calcium carbonate structure and psychiatric care. The self-intromission of these calculi into the bladder is due to the personality disorder being in the frame of Munchausen's syndrome. Two years later, no recurrence is noted and an excellent psychiatric care is achieved.

Plasma cell cheilitis is a rare inflammatory disorder of the lip with a characteristic band-like infiltrate of plasma cells in the upper dermis. Differential diagnosis should consider allergic/irritant contact cheilitis, candidiasis, syphilis, Queyrat's erythroplasia, granulomatous cheilitis, plasmoacanthoma, plasmacytoma, squamous cell carcinoma and exfoliative or factitious cheilitis. We observed plasma cell cheilitis in a 60-year-old Caucasian female who had a partial response to topical steroids and oral griseofulvin.
Fifteen families are described in which one or more child incurred factitious illness abuse as a result of the father's false story and actions. The degree of direct physical harm, and the chance of death, was high in those families in which the father had Munchausen syndrome or marked somatising disorder. Eleven children died and another six survived repetitive smothering or poisoning. Although the extent of the risk to children living with a parent who has marked somatising disorder is unsure, there must be vigilance on behalf of those children.
A 22-year-old female with factitious sickle cell anemia and recurrent painful crises is described. Because she had sickle cell trait and iron deficiency anemia, she could successfully feign the symptoms of homozygous sickle cell anemia. The identification of this syndrome in patients with genetic disorders is presented.
Four cases of factitious crusting of the lips in women are reported. Two of the women had hemorrhagic crusts, and two had keratotic yellow crusts. All four patients had personality disturbances. Biting, picking, or unconscious licking of the lips may be the underlying mechanism for trauma and crust formation. This entity should be distinguished from contact cheilitis, actinic cheilitis, infectious cheilitis glandularis, and cheilitis granulomatosa. Some cases of exfoliative cheilitis may also be factitious. The presence of bizarre hemorrhagic or keratotic crusts on the lips should alert the clinician to a possible factitious origin, and a psychiatric evaluation should be done.
