BACKGROUND: Triage at the site of a major incident is key to effective scene management. A number of triage algorithms have been suggested to assist the triage officer to determine triage priorities. However, many advocated scores were not specifically developed for use in major incidents, nor are they designed for multiple age groups. Many of these algorithms have not been validated: those that have were validated against the Injury Severity Score, which is of little relevance in a major incident--it is the urgency of medical intervention that is of importance in this setting. OBJECTIVES: To develop a set of criteria against which major incident triage algorithms can be tested. METHODS: Sixteen experts from the UK and South Africa took part in a three round Delphi consensus method in order to develop clinical criteria against which major incident triage algorithms may be tested. RESULTS: Thirty nine statements were initially identified as possible determinants of triage priority: 29 statements reached consensus. These associate specific clinical interventions with triage priority. CONCLUSION: Delphi may be used to identify which clinical criteria define triage priority in a major incident setting. These criteria and the associated triage categories may be used as for the validation of specific major incident triage algorithms. This method may be used to develop specific criteria for other triage algorithms.
The first defence of the America's Cup outside of the USA took place off Fremantle, Western Australia, between October, 1986 and February, 1987. Fremantle Hospital, the nearest hospital to the marinas, was involved in the planning for disasters and for the provision of clinical services to the syndicates involved and the projected crowds of tourists and spectators. The impact of this unique sporting event on the hospital was measured by collecting data from all patients whose presence in Fremantle was related to the yachting. The America's Cup resulted in only 654 attendances and 31 admissions over the 5 months of the event. The figures fell far short of those predicted. A number of conclusions have been drawn.
The ideal requirements of a logbook for trainees in accident and emergency medicine in the United Kingdom were sought by means of a postal survey of 100 Senior Registrars and recently-appointed Consultants in the specialty. Sixty-two replies were received. An overwhelming majority supported ACLS and ATLS certification, as well as formal training in Management/Clinical Budgeting skills, the use of Information Technology, dealing with medicolegal issues and Disaster Planning. Secondments to General Practice, Regional Poisons Centres and the Emergency Services were similarly recommended. A total of 71% (42 responders) approved of a minimum of 3 years experience in an A&E Department before appointment as a Senior Registrar in the specialty. The majority of responders disapproved of acquiring laboratory-based skills. The results of the survey are discussed and suggestions for the possible role of a logbook are made.
