Application of Bayesian estimation for the prediction of an appropriate dosage regimen of amikacin.

A Bayesian approach was developed to determine an amikacin dosage regimen to achieve the desired plasma concentrations for each patient. Statistical characteristics of pharmacokinetic parameters were first evaluated in a group of patients (reference population), which when combined with three individual plasma concentrations of drug led to a Bayesian estimation of individual pharmacokinetic parameters. By using these parameters, an individual dosage regimen was then established to avoid residual and peak amikacin concentrations of up to 3 and 25 micrograms/mL, respectively. In a test group of 33 patients, adapted amikacin dosage regimens ranged from 4 to 43 mg/kg/d, with schedules requiring up to four infusions per day. Infusion time varied from 40 min to 4 h. These differences in drug administration protocol result from the wide interindividual variability of amikacin pharmacokinetic parameters. Performance of the developed methodology was evaluated by computing bias and precision of the estimated total body clearance and of the trough and peak amikacin concentrations that were reached after dosage regimen determinations.

Automated detection of hereditary syndromes using data mining.

Computer-based data mining methodology applied to family history clinical data can algorithmically create highly accurate, clinically oriented hereditary disease pattern recognizers. For the example of hereditary colon cancer, the data mining's selection of relevant factors to assess for hereditary colon cancer was statistically significant (P < 0.05). All final recognizer-formulated patterns of hereditary colon cancer were independently confirmed by a clinical expert. Applied to previously analyzed family histories, the recognizer identified the definitive hereditary histories, correctly responded negatively to the putative hereditary histories, and correctly responded negatively to empirically elevated colon cancer risk situations. This capability facilitates patient selection for DNA studies in search of gene mutations. When genetic mutations are included as parameters in a patient database for a genetic disease, the process yields an expert system which characterizes variations in clinical disease presentations in terms of genetic mutations. Such information can greatly improve the efficiency of gene testing.

Barriers to sustainable water-quality management.

Due to the pressures of increasing population and developing economy all over the world, the present situation of water-quality management is far from satisfactory. To enhance sustainability of water-quality-management systems, in-depth research of the related barriers and the relevant mitigation approaches is desired. In this paper, recent developments, advancements, challenges, and barriers associated with practices of water-quality management were analyzed. A number of related methodologies, applications, and policy considerations were examined. Issues of information support, technology development, system integration, and policy implementation were discussed. Perspectives of sustainable water-quality management in the twenty-first century were investigated, demonstrating many demanding areas for enhanced research efforts, including issues of data availability and reliability, concerns in system complexity and methodology validity, limitations of computer techniques, usefulness of research outputs, difficulties in policy implementation, and necessity of training programs.

A computerized review of the prehospital care rendered to EMS cardiac patients: validation and application.

A computerized system of evaluating the prehospital EMS care being rendered to cardiac patients is reviewed. The primary component of the evaluation methodology presented is a computer language called PROTO. PROTO is used to evaluate the process of prehospital EMS care that was rendered to cardiac patients. Previously developed protocols of care are applied by means of PROTO to the case records of patients in three EMS districts in the southeastern part of the United States. Comparisons of PROTO's evaluations and those made by a review team consisting of two physicians and an RN are presented. Applications of PROTO's case evaluations are reviewed.

Methodological assessment of combined spiral CT angiography and CT arterial portography.

BACKGROUND: To assess an optimal methodology of combined spiral computed tomographic (CT) angiography (CTA) and CT arterial portography (CTAP) for detection and characterization of liver tumors. METHODS: We performed spiral CTAP only in five patients with 30-32% contrast (subset A), CTAP combined with preceding spiral CTA using 30-32% contrast in 19 (subset B), and CTAP combined with preceding spiral CTA with 60-64% contrast in seven (subset C). The CT numbers of the aorta immediately before preceding CTA and subsequent CTAP and the CT numbers of malignant tumor and liver parenchyma with CTAP were measured. RESULTS: The differences of the CT number between the malignant tumor and liver parenchyma on CTAP were 61.1-161.8 (mean +/- SD, 114.5 +/- 39.3) HU, 50.7-164.8 (104.2 +/- 31.2) HU, and 101.2-368.3 (219.5 +/- 90.5) HU in subsets A, B, and C, respectively. Two cavernous hemangiomas showed pathognomonic findings with preceding CTA. CONCLUSION: Combination of preceding spiral CTA and subsequent spiral CTAP using 30% contrast with a 5-min interval is an optimal method for detection and characterization of liver tumors.

A study of how socioeconomic status moderates the relationship between SNPs encompassing BDNF and ADHD symptom counts in ADHD families.

Recent animal research suggests that brain-derived neurotrophic factor (BDNF), may mediate response to different environmental stimuli. In this paper, we evaluated the possible role of BDNF as a moderator of attention deficit hyperactivity disorder (ADHD) in the context of different socioeconomic classes. We genotyped ten single nucleotide polymorphisms (SNPs) in and around BDNF in 229 families and evaluate whether there are SNP-by-socioeconomic status (SES) interactions for attention deficit hyperactivity. We developed three quantitative phenotypes for ADHD from nine inattentive and nine hyperactive-impulsive symptoms that were used in SNP-by-SES interaction analyses using a new methodology implemented in the computer program PBAT. Findings were adjusted for multiple comparisons using the false discovery rate. We found multiple significant SNP-by-SES interactions using the inattentive symptom count. This study suggests that different SES classes may modify the effect of the functional variant(s) in and around BDNF to have an impact on the number of ADHD symptom counts that are observed. The two exons within BDNF represent potential functional variants that may be causing the observed associations.

The structural basis of the genotoxicity of nitroarenofurans and related compounds.

The CASE (Computer-Automated Structure Evaluation) methodology has been applied to an investigation of the basis of the genotoxicity (sfiA induction) of 79 nitroarenofurans and related molecules examined with the E. coli PQ37 genotoxicity assay (SOS chromotest). CASE identified 9 major activating structural fragments (biophores) responsible for the probability of genotoxicity (SAR). With respect to quantitative features, CASE identified 8 major molecular subunits related to the genotoxic potency (QSAR). Both the SAR as well as the QSAR analysis indicate that a nitro group on position 2 of the furan ring is important for activity provided one or more aromatic rings are attached to the furan ring, i.e. 2-nitrobenzofuran, 2-nitronaphthofuran, 2-nitroanthrafuran and 8-nitropyrenofuran. Additionally, a small substituent at position 3 of the furan ring, i.e. the methyl group of R7371, the ethyl group of R7427 and the butyl group of R7429, enhance the activity of 2-nitronaphtho[2,1-b]furan (R6597), whereas longer aliphatic chains decrease activity. Moreover, the activity of the nitro group at position 2 of the furan ring was increased by substitution of a methoxy group at position 7 of the R6597 structure. Additionally the n-octanol/water partition coefficient (log P) was found to be an important descriptor for the genotoxic potency in E. coli PQ37. Using the identified descriptors CASE correctly predicted the probability of genotoxicity of all of the genotoxicants and non-genotoxicants in the data base. The calculated genotoxic potency was equally good: 94% of all predicted results were within plus/minus one order of magnitude of the experimental result. Using CASE in the predictive mode, the program correctly predicted the probability of sfiA induction in E. coli of 95.8% of 24 "unknown" nitroarenofurans which were not part of the learning set (QSAR with r = 0.88-0.97).

Assessing short-term recognition memory with forced-choice psychophysical methods.

The rationale and methodology for using computer-controlled forced-choice psychophysical methods to assess short-term recognition memory in human subjects are presented. Here, we use non-verbal computer-synthesized auditory and visual stimuli with an adaptive psychophysical procedure. Sequence-length thresholds (SLTs, span lengths) for randomly generated binary auditory and visual-sequential patterns and simultaneous visual-spatial patterns are determined to assess short-term memory capacity. The SLTs can also be used to equate for initial retention level for delayed matching-to-sample (DMS) or delayed matching-to-non-sample (DMNS) tasks which assess memory decay. The DMS/DMNS tasks have also been modified for use with the forced-choice paradigm. In contrast to many verbal paradigms requiring immediate ordered recall, non-verbal stimuli in a forced-choice paradigm provide a more direct measure of sensory memory because long-term memory, complex encoding/decoding processes, and motor-sequencing factors are minimized or avoided. Furthermore, the forced-choice recognition memory tasks are applicable over a broad age range, are less sensitive to socio-economic factors and educational level, and avoid complex instructions. Taken together, these factors enhance the applicability of these tasks in children and adults with CNS lesions, particularly where cognitive status may be compromised.

Structural basis of the mutagenicity of phenylazoaniline dyes.

The CASE (Computer-Automated Structure Evaluation) methodology was used to gain an understanding of the basis of mutagenicity of phenylazoanilines. It was found that the activity of these molecules is dependent upon an intact moiety that spans the azo linkage, i.e., the azo bond must remain intact for mutagenicity. The study also addressed the effect of sulfonation on the activity of these azo dyes. It was revealed that sulfonation at only certain sites resulted in loss of mutagenicity. Sulfonation of the structural moiety responsible for the activity of phenylazoaniline dyes did not necessarily result in complete elimination of activity as this substitution could generate new structural moieties which contribute to the activity of the molecules.

A program for oriented treatment of essential hypertension.

Treatment of essential hypertension is often a complex task and needs appropriate methodology. A newly developed computer program can help physicians to choose the best pharmacological approach to the treatment of essential hypertension. Based on an oriented clinical history, the program asks the user to give the physiological, pathological and pharmacological antecedents of the patient to be treated. Taking into account these characteristics, 19 antihypertensive drugs are ordered according to their indications and contraindications in the patient. Diet advice based on body weight and the patient's habits are considered as well. The program is designed to explain why decisions are made, so it can be useful in teaching medical students or postgraduates.

A SAS macro for stepwise correlated binary regression.

Several regression methods have been proposed for the analysis of correlated binary data, but none deals with the selection of covariates when there exist a large number of potentially relevant covariates. We present a SAS macro based on a stepwise selection procedure for the analysis of correlated binary data. Using regression methods based on generalized estimating equations originally proposed by Liang and Zeger and extended by Prentice, we describe a score test for forward selection, a Wald's test for backward elimination, and a test for model adequacy based on generalized scores. The methodology and the accompanying computer macro program written in SAS IML are illustrated with data from a prospective study of functional decline in the activities of daily living in a group of elderly patients.

Stability of ventricular size after the onset of psychosis in schizophrenia.

This study examined whether ventricular enlargement in schizophrenia is progressive by scanning 15 schizophrenic patients at the onset of their first psychotic episodes and again 1-3 years later. Sizes of the body of the lateral ventricles, the frontal horns, and the third ventricle were assessed. The results indicated no tendency for the ventricles to get larger in this sample, with lateral ventricular size actually showing a significant decrease across the rescanning interval when the ventricle-to-brain ratio was used as the dependent variable. Methodological issues related to computed tomography and the quantification of ventricular size were considered by developing an alternative method of calculating ventricle-to-brain ratios and examining the reliability of measurements made on a group of 11 medical patients who were scanned twice on the same day.

Salt effects on nucleic acids.

Salt-dependent electrostatic effects are a major factor in determining the stability, structure, reactivity, and binding behavior of nucleic acids. Increasingly detailed theoretical methods, especially those based on Monte Carlo and Poisson-Boltzmann methodologies, combined with powerful computational algorithms are being used to examine how the shape, charge distribution and dielectric properties of the molecules affect the ion distribution in the surrounding aqueous solution, and how they play a role in ligand binding, structural transitions and other biologically important reactions. These studies indicate that inclusion of detailed structural information about the nucleic acid and its ligands is crucial for improving models of nucleic acid electrostatics, and that better treatment of the ion atmosphere and dielectric effects is also of major importance.

A comparison of morphometric characteristics of sperm from fertile Bos taurus and Bos indicus bulls in Brazil.

The current work has as main objective the systematic investigation of sperm morphometric characteristics of fertile Bos taurus and Bos indicus bulls living in Brazil by using several traditional as well as more modern and advanced computer vision concepts and methodologies. Ten smears of B. taurus semen and ten smears of B. indicus semen have been evaluated. Sperm morphology was quantified in terms of the following morphological features: head area, perimeter, width, length, width:length ratio, ellipticity, shape factor, width of sperm basis, the three first Fourier values, symmetry and hydrodynamics. Morphometric differences have been observed between the sperm cell of B. taurus and B. indicus bulls. The sperm cells of Zebu bulls tend to be smaller and less elliptic, however without modifying hydrodynamic, side symmetry and width of sperm head base. These differences clearly indicate that the geometrical characterization of bull sperm cells should take into account morphological peculiarities that are specific to each subspecies. Another important contribution is the identification that morphological differences implied by bulls of different fertility, as characterized by other authors, were found to be less as compared with those obtained in the current study where highly fertile animals from the two subspecies were studied.

Automated detection of qualitative spatio-temporal features in electrocardiac activation maps.

OBJECTIVE: This paper describes a piece of work aiming at the realization of a tool for the automated interpretation of electrocardiac maps. Such maps can capture a number of electrical conduction pathologies, such as arrhytmia, that can be missed by the analysis of traditional electrocardiograms. But, their introduction into the clinical practice is still far away as their interpretation requires skills that belongs to very few experts. Then, an automated interpretation tool would bridge the gap between the established research outcome and clinical practice with a consequent great impact on health care. METHODS AND MATERIAL: Qualitative spatial reasoning can play a crucial role in the identification of spatio-temporal patterns and salient features that characterize the heart electrical activity. We adopted the spatial aggregation (SA) conceptual framework and an interplay of numerical and qualitative information to extract features from epicardial maps, and to make them available for reasoning tasks. RESULTS: Our focus is on epicardial activation isochrone maps as they are a synthetic representation of spatio-temporal aspects of the propagation of the electrical excitation. We provide a computational SA-based methodology to extract, from 3D epicardial data gathered over time, (1) the excitation wavefront structure, and (2) the salient features that characterize wavefront propagation and visually correspond to specific geometric objects. CONCLUSION: The proposed methodology provides a robust and efficient way to identify salient pieces of information in activation time maps. The hierarchical structure of the abstracted geometric objects, crucial in capturing the prominent information, facilitates the definition of general rules necessary to infer the correlation between pathophysiological patterns and wavefront structure and propagation.

Biomechanical simulation of manual lifting using spacetime optimization.

Previous optimization techniques for the prediction of lifting motion patterns often require a change in either the number of variables or the order of the mathematical functions used to express the angular displacement of selected joints in response to change in variant conditions. The resolution of predicted results can also be seriously constrained by the number of variables used. These restrictions may often limit the applicability of these methodologies. In this paper, we proposed a new methodology for generating the optimum motion patterns for para-sagittal lifting tasks. A detailed description of this methodology is introduced. An example of an analysis using this methodology is presented. The computer program generated lifting motion patterns with a reduction of the overall objective function values. The actual versus predicted lifting motion patterns are compared. Using this method, constraints can be added anywhere within the lifting cycle without the need of rewriting the whole program. These features provide for a more flexible and efficient prediction of the lifting motion.

Correlation of gas chromatographic properties of chlorobenzenes and polychlorinated biphenyls with the occurrence of reductive dechlorination by untamed microorganisms.

To understand the dechlorination ability of chlorobenzenes (CBs) and polychlorinated biphenyls (PCBs) by untamed microorganisms under anaerobic condition and to correlate gas chromatographic properties with the occurrence of reductive dechlorination, introduction of CBs and PCBs in the culture medium inoculated with microorganisms from sludge and sediment, respectively, were performed. Three kinds of culture media preparing from sludge, river water and a synthetic medium were used in the experiments. HCB was degraded to 1,3,5-trichlorobenzene (1,3,5-TCB) and 1,3-dichlorobenzene (1,3-DCB) in both sludge medium and synthetic medium with inoculated microorganisms. Three PCB congeners including 2,3,4-, 3,4,5- and 2,3,4,5-CBp (chlorinated biphenyl) were not found to be dechlorinated in the river water medium with inoculation culture but to be dechlorinated in the synthetic medium. MNDO methodology was used to compute theoretical dechlorination reaction heats and GC-ECD techniques were used to estimate chromatographic data of CB and PCB congeners. Both CB and PCB congeners showed that dechlorination by untamed microorganisms under anaerobic mixed cultures were more likely to occur when larger amounts of energy were released and greater deltaln RRT value between the parent congener and the daughter product was observed. Deltaln RRT provided a more precise information on the singularity of PCBs ortho-dechlorination in an aspect of thermodynamic favorable rule.

Influence of food and diabetes on pharmacokinetics of sodium tungstate in rat.

In this paper, the influence of food and diabetes on the pharmacokinetics of sodium tungstate in rat was investigated. The compound was administered intravenously (9 mg/kg) and orally in the form of solution (36 mg/kg). An empirical Bayes methodology was used to compute individual pharmacokinetic parameters. Sodium tungstate followed first-order kinetics, and plasma concentration versus time data were described by a two-compartment model. A significant relationship was found between the bioavailability and the status of the animals. Total plasma clearance and elimination half-life averaged 3.1 ml/min/kg and 1.6 h, respectively. Food had some effects on the extent of sodium tungstate absorption. After oral administration, the bioavailability (0.67 versus 0.85), C(max) (6.10 versus 15.2 microg/ml) and AUC (70.7 versus 105 mgh/l) were 20, 60 and 32% lower in fed than in fasted rats, respectively. The presence of cellulose and sulphate anions in rat chow could partially explain the fed state-associated reduction of tungstate bioavailability. In streptozotocin-induced diabetic fed rats, a 25% decrease occurred in AUC and F, and a 14% increase occurred in the elimination rate constant compared with healthy fed rats. These changes could be explain on the one hand, by the increase of liquid consumption and food intake, and on the other hand, by a gastroparesis in the early diabetic rats.
