[A comprehensive hygienic assessment of the prevalence of cerebrovascular diseases among the population of Kiev Province before and after the accident at the Chernobyl Atomic Electric Power Station].

The population of Kiev Province show higher level of primary morbidity of cerebrovascular disorders (CVD) and rate of its increment than on the average in Ukraine, as per medicostatistical analysis in space and time of CVD prevalence in the studied territories. Among regions in the highest ranks in respect of levels and rates of increase in morbidity over five post-Chernobyl years are Ivankov and Polesye regions which residents had become victims to ChNPP breakdown. It should be noted that in previous years the above regions were rated as being in the ranks below the average in the area.

[The effect of vitamin- and beta-carotene-enriched products on the vitamin A allowance and the concentration of different carotenoids of the blood serum in victims of the accident at the Chernobyl Atomic Electric Power Station].

The results of clinical trials of efficiency of foods enriched by vitamins and beta-carotene in people suffered from Chernobyl's accident are presented. The level of beta-carotene in clinical diets was the same during trial. Daily consumption of enriched food supplying ingestion of 4-5 mg of beta-carotene increased the level of beta-carotene in serum by 2-4 times. The concentration of total carotenoides in serum was increased by 1.6 times practically at the expense of beta-carotene.

[The status of the gastric and duodenal mucosa, biopsied from those who worked in the cleanup of the aftermath of the accident at the Chernobyl Atomic Electric Power Station, in an organotypic tissue culture].

Biopsies of stomach and duodenum different regions mucosa obtained from patients with gastroduodenitis (liquidators of consequences of accident in Chernobyl nuclear station--LCA CNS-- and control patients) were studied cultured in vitro. The following are the differences found. Mononuclear cells, differentiating into macrophages were exposed from control biopsies. Mononuclear cells exposed from LCA CNS biopsies formed rosette-like accumulations after mitotic dividing on a distance from the explant. Part of cells within the rosettes acquired processes and formed syncytial structure.

Main scientific results of the WHO International Programme on the Health Effects of the Chernobyl Accident (IPHECA).

Scientific results obtained within the WHO International Programme on the Health Effects of the Chernobyl Accident (IPHECA) have confirmed the increase of thyroid cancer cases in children who were exposed to radiation due to the accident in 1986. In the zones under surveillance in Belarus, the Russian Federation and Ukraine, the general morbidity for leukaemia and related diseases did not undergo a significant change after the accident. Accident recovery workers ("liquidators") are an especially high-risk group and need further medical follow-up.

[The morphometric and immunohistochemical indices of the gastric and duodenal mucosa in the dynamic treatment of erosive gastroduodenitis in those who worked in the cleanup of the aftermath of the accident at the Chernobyl Atomic Electric Power Station in the follow-up period].

62 Chernobyl wreckers with erosive gastroduodenitis (50 cases) and ulcer (12 cases) were admitted to hospital where they received standard drugs and local He-Ne laser treatment of erosions and ulcer located in the antral stomach and duodenal bulb. Histological, bacterioscopic, morphometric and immunohistochemical examinations have identified chronic inflammation and disturbance of local immunity. After treatment epithelialization of erosions was not accompanied by normalization of morphometric and immunohistochemical parameters. This suggests a high risk of recurrences.

[Oncoepidemiologic situation in the Kaluga region of the Russian Federation ten years after the Chernobyl accident].

Cancer morbidity and mortality in radionuclide-contaminated areas of Kaluga Region have been investigated. The study was mainly concerned with the extent of radiation pollution influence on the present-day cancer morbidity and mortality rates. The correlations between the latter factors, on the one hand, and relevant population risks, on the other, have been analysed. It was found that the present-day neoplasm morbidity rates recorded in Chernobyl-stricken areas are chiefly accounted for by a combination of factors which were there before the disaster. The trends of morbidity and mortality from neoplasms of the respiratory tract in females are still unfavorable. There is still no statistical verification of the radiation effect on cancer morbidity except that of thyroid cancer in women in contaminated areas. The morbidity and lethality rates in these areas basically conform to the general trends. The findings are in conformity with those reported worldwide on radiation-related cancer induction latency and biological effects observed in the inhabitants of contaminated regions.

[The clinico-hemodynamic and psychological characteristics of the course of hypertension in those who cleaned up the aftermath of the accident at the Chernobyl Atomic Electric Power Station].

A comparative evaluation of 53 liquidators of the Chernobyl NPP accident and 49 subjects who were not exposed to ionizing radiation, patients with stage II hypertensive disease (HD), revealed clinical features of the disease course, apparent changes in the psychological profile and vegetative supply of the reactions of the cardiovascular system in the former category of the patients. Based on the reactions to the functional tests and urinary excretion of catecholamines, low threshold was shown of resistance to psychoemotional stress in the liquidators, as was usefulness of changes in alpha- and beta-adrenoblockers.

[The clinico-hematological characteristics of the participants in the cleanup of the aftermath of the accident at the Chernobyl Atomic Electric Station with stable deviations in blood analyses].

On a screening examination of 6145 individuals who took part in the elimination of Chernobyl breakdown and were exposed to radiation in the range of 0.05-1 Gr, a group was identified of those subjects displaying stable deviations from the norm in their blood analyses, such as leukocytosis, leukopenia, thrombocytopenia, anemia, lymphocytosis, monocytosis, and some other abnormalities. A 3-yr follow-up revealed 17 cases of malignant diseases of the hematopoietic system, suggesting high risk for hemoblastoses morbidity. Comparative characterization is given of hematologic deviations in blood analyses together with the clinical status of persons who took part in the elimination of the effects of ChNPP accident in 1986 and those subjects working permanently within a 30-km radius.

[The nature of the changes in the immunity of the women who worked in the cleanup of the sequelae of the accident at the Chernobyl Atomic Electric Power Station and were evacuated from the radioactively contaminated area].

During examination of 37 women taking part in elimination of consequences of Chernobyl's disaster was revealed, that complex of disaster factors action resulted the formation of disbalance in the system of immunocompetence cells and caused the oppression of functional activity of neutrophils. In 110 women living on the radioactive contaminated zone (5-40 Ci/km) the phenomena of the secondary immunodeficiency in the form of defeat as cellular as humoral factors of immunity and nonspecific resistance system was diagnosed.

[The genetic effects in the somatic cells of persons working under chronic irradiation of different intensities after the accident at the Chernobyl Atomic Electric Power Station].

A complex genetic study of two groups from of Chernobyl NPP personnel (from "Shelter" unit and 3rd Block) has been carried out using classical cytogenetic and GPA methods. The first group was the most vulnerable from the viewpoint of accumulated dose (exceeding 25 cGy for the moment of study). Positive correlation between individual and group frequencies of cytogenetic markers of irradiation (stable and unstable chromosomes aberrations) and NO mutations in the GPA locus was found.

[Vitamin status of the population from regions suffering from the accident at the Chernobyl power plant, and its correction with multivitamins "Duovit" and "Undevit" and multivitamin premix 730/4 of the firm "Roche"].

There was no differences of principle in vitamin status of population living in 'zones of strict control' (Brjanskaja, Mogilevskaja, Gomel'skaja regions) and Moscow's population. Mean indices of vitamin C, A, E, B1 and carotene status in children and adults were not differed in two groups of population. But vitamin B2 and niacin status in Moscow's adults were worse. Among children from Moscow vitamin C deficiency was found more rare and vitamin E deficiency more often than those in population from regions suffering from Chernobyl. It was established that supplement of third dishes with vitamin C is not sufficient for elimination of vitamin C deficiency. Polyvitamin pills 'Duovit' or premix 'Roche' were more effective in removal vitamin deficiency.

A more comprehensive application of the micronucleus technique for biomonitoring of genetic damage rates in human populations--experiences from the Chernobyl catastrophe.

The current method for scoring micronuclei as a measure of genetic damage rate in peripheral blood cells is to enumerate this end point in cytokinesis-blocked binucleated cultured lymphocytes. However, one can expect that, due to chronic exposure to genotoxins or inherent genetic instability, micronuclei may be expressed continually in vivo in dividing cell populations such as the progenitor cell lineages leading to mature lymphocytes or erythrocytes. Consequently, micronuclei may already be expressed in peripheral blood lymphocytes prior to culture. In view of these considerations, we have performed a study in children living in regions of Belarus that are contaminated by radionuclides from the Chernobyl disaster and compared their micronucleus frequency in erythrocytes, nondivided lymphocytes, and cultured cytokinesis-blocked binucleated lymphocytes to that of controls living in noncontaminated areas. Preliminary data presented in this paper indicate a significant two- to fourfold increase in micronucleus expression (P < 0.05) in exposed children relative to controls in erythrocytes or peripheral blood lymphocytes in blood smears as well as in mononuclear and cytokinesis-blocked binucleated lymphocytes in cultures. The measurement of micronuclei in nondivided mononuclear lymphocytes represents chromosomal damage expressed during in vivo divisions. The micronuclei in binucleated cultured cells represent micronuclei expressed ex-vivo and may include micronuclei already present in a cell prior to tissue culture. These preliminary data suggest that a different spectrum and level of damage may be observed in nondivided mononuclear lymphocytes, binucleated lymphocytes, and erythrocytes and that a combination of these approaches may provide a more comprehensive assessment of the extent of genetic damage induced by chronic exposure to radionuclides or other genotoxins in haematopoietic tissue.

[Cardiac contractile function in the participants of the cleanup of the aftermath of the accident at the Chernobyl Atomic Electric Power Station who are ill with hypertension].

In a major proportion (67%) of those examined (persons with stage-II hypertensive disease who took part in the elimination of the aftermath of the Chernobyl breakdown) with the aid of echocardiography, hypertrophy of the left ventricular (LV myocardium was revealed as was augmentation of the mass of the LV myocardium together with decline in the contractile function, while in other examinees (33%) all above parameters showed no deviation from the norm. This permits predicting probability of cardial complications in such patient populations.

[One decade after Chernobyl: sequelae of the accident and urgent problems of radiation pathology].

Medico-biological sequelae of irradiation of humans as a result of the Chernobyl accident are considered. Acute radiation sickness, thyroid carcinoma in children, chromosomal aberrations in the peripheral blood lymphocytes have an etiological link with radiation. Three problems are important for future studies: 1) collection of reliable information about the radiation, 2) assessment of dose-effect relations, 3) organization of supervision.

[The late sequelae of the action of irradiation during the cleanup of the aftermath of the Chernobyl catastrophe on the concentration of macro- and microelements in human blood serum].

The content of Cu, Fe, Zn, Mg, P, and S in the blood serum and its ultrafiltrate was studied by the inductively coupled argon plasma atomic emission spectrometry in eliminators of the accident at the Chernobyl Nuclear Power Station who worked in the accident zone in 1986-1987. Changes in the mineral equilibrium of the blood serum was expressed in elevated concentrations of the ultrafiltered Cu and S, increased Mg content in the serum, and decreased content of ultrafiltered Zn. The Fe concentration in the serum and ultrafiltrate did not differ from the control indices, thus suggesting balanced Fe transport, accumulation, and mobilization at a remote time after the radiation injury.

[Health status of persons with acute radiation sickness caused by Chernobyl AES accident].

After Chernobyl accident 75 survivors with acute radiation syndrome (ARS) have been under observation in the Clinic of the Institute of Biophysics. Years after accident serious problems occurred in the individuals, who suffered from late effects from radiation injuries (late radiation ulcers and radiation cataracts). One patient suffered on hypernephroma, one-kidney cyst. Observed somatic diseases are gastrointestinal diseases, different neurological diseases and syndromes and upper pulmonary tract diseases. Hemopoiesis system condition was characterised by the development of transient moderate cytopenia (light and moderate degree of ARS patients) as well as the periodical increase of blood cell counts above standard indices (severe degree of ARS patients).

Detection of a novel type of RET rearrangement (PTC5) in thyroid carcinomas after Chernobyl and analysis of the involved RET-fused gene RFG5.

A novel type of RET rearrangement, PTC5, was detected in papillary thyroid carcinomas of two patients exposed to radioactive fallout after Chernobyl. Reverse transcription-PCR and rapid amplification of 5'-cDNA ends revealed a fusion of the ret tyrosine kinase (TK) domain with a sequence identical to that described previously as ret-II. Ret-II is a transfection artifact in NIH3T3 cells and has not yet been detected in any human tumor. Overlapping sequences found in the expressed sequence tag databases enabled us to sequence the COOH terminus of the ret-fused gene 5 (RFG5). The combined data made it possible to assemble a full-length rfg5 protein sequence. Computer-assisted analysis of this sequence reveals four putative coiled-coil structures, possibly involved in dimerization, but no membrane-binding sequences. Northern blots show a ubiquitous RFG5 expression in various normal tissues, including the thyroid gland. In addition to the RFG5/RET, we also detected the reciprocal RET/RFG5 transcript in both tumor samples, suggesting that the rearrangement is based on a balanced reciprocal translocation. In agreement with other rearranged TKs, it is concluded that the transforming action of the new fusion protein rfg5/ret in thyroid tumors may be due to an activation of the ret TK by constitutive expression and dimerization potential of the 5'-fused rfg5 protein. Ret immunohistochemistry indicates that the fusion protein is expressed in all cells of PTC5 tumors, suggesting that RFG5/RET rearrangement is an early event in thyroid carcinogenesis.
