[New method of dissection of hemorrhoids].

The technique of a new method of dissection of hemorrhoids is described. The nearest and remote results of the treatment of patients operated for hemorrhoids by the new method (82 patients) are compared with the results of the ligature method used in 212 patients. After hemorrhoidectomy by the new method good results were noted in 73.7% of cases, satisfactory results - in 21,0% and recidivations were observed in 5,3% of cases. The results of the comparison show the advantages of the new method of dissection of hemorrhoids.

[Use of the dia-contact method in ophthalmology. A new method of fluorescein angiography presentation].

An innovative presentation of fluorescein angiograms on radiographic film, called "dia-contact", is described. Photographs are produced by the classical procedure of contact-printing on subtraction film and are of very high quality resolution. Less expensive than conventional prints, the resultant document is easy to copy and file. The reading of this document with the help of two + 10 lenses, allows a sequential analysis of the angiogram and a stereoscopic interpretation alternatively. The image can be enlarged without loss of resolution by means of a microfilm reader device. This method requires a certain amount of self discipline, in that a new habit must be learned. However it rapidly proved itself invaluable in the author's daily practice.

A new method for objective evaluation of cardiac parametric images.

A new method for analyzing cardiac parametric images is introduced. Its original feature is the representation of local heart motion in a polar coordinate plane which makes the integration of the information contained in three parametric images possible. These images are calculated using first harmonic Fourier filtering and are the maximum volume image, the local ejection fraction image, and the phase image. The last two parametric images are considered as the complex ejection fraction parametric image which is described by a 2D distribution in the complex plane. This method is a generalized 2D version of the well-known phase histogram method. By segmenting the complex plane in which this representation is made, four indexes are defined called the index of hypokinesia, the index of asynergy, the mean phase shift in asynergic regions, and the mean ejection fraction in asynergic regions. The values of those indexes are used to assess the degree and extent of regional wall motion abnormalities. An automated classification as normal, hypokinetic, akinetic, slightly dyskinetic, and strongly dyskinetic made on the basis of the values of those indexes has been compared with a visual classification made by three observers in 219 studies. ROC curves show a good agreement between the automated and visual methods. This new method provides an efficient means of automatically classifying cardiac studies.

A new method for screening for hyperammonemia.

A new method for the detection of hyperammonemia, using a kit based on the principle of microdiffusion of ammonia, is described. The method requires only one drop of blood and takes only 15 min to complete. Experiments for recovery and reproducibility were satisfactory, and good correlation was obtained when compared with an enzymatic method for blood ammonia determination. The new method is considered to be useful for routine, low-cost mass-screening of newborn infants for hyperammonemia. It will also be useful for monitoring blood ammonia levels at the bedside in cases with hepatic disease or receiving parenteral nutrition.

Psoralen plus near-ultraviolet light: a possible new method for measuring DNA repair synthesis.

A new method is proposed to inhibit semiconservative DNA synthesis in cultured cells while DNA repair synthesis is being measured. The cells are treated with the DNA-crosslinking agent Trioxalen (4,5,8-trimethylpsoralen) plus near-ultraviolet light, and consequently 99.5% inhibition of replicative DNA synthesis is achieved. Additional DNA-damaging agents induce thymidine incorporation into the double-stranded regions of the DNA. The new method gave results very similar to those obtained with the benzoylated naphthoylated DEAE (BND) cellulose method using three human fibroblast strains, of which one had deficient capacity for DNA repair synthesis following treatment with gamma rays and methyl methanesulfonate. The advantages of the new method are simplicity and rapidity, as well as the high extent to which replicative DNA synthesis is inhibited.

A new method of screening for vertical atlantoaxial dislocation.

A new method for the screening and grading of vertical atlantoaxial dislocation is described. The Sakaguchi-Kauppi method is based on a pathologic process in the atlantoaxial joints and the grading system gives a reproducible (interobserver coefficient of variation = 0.062) tool for the screening of vertical dislocation and is also applicable to patients for whom earlier radiographs are not available. There were differences between the results obtained using Redlund's method and the new Sakaguchi-Kauppi method (chi 2 = 44.7, p less than 0.001). This difference stems from the variations in the height of the axis in the normal population and results in a sensitivity and specificity of 84.2% and 90%, respectively, for Redlund's method compared to the new method.

Automated three-dimensional finite element modelling of bone: a new method.

Three-dimensional finite element stress analysis of bone is a key to understanding bone remodelling, assessing fracture risk, and designing prostheses; however, the cost and complexity of predicting the stress field in bone with accuracy has precluded the routine use of this method. A new, automated method of generating patient-specific three-dimensional finite element models of bone is presented--it uses digital computed tomographic (CT) scan data to drive the geometry of the bone and to estimate its inhomogeneous material properties. Cubic elements of a user-specified size are automatically defined and then individually assigned the CT scan-derived material properties. The method is demonstrated by predicting the stress, stain, and strain energy in a human proximal femur in vivo. Three-dimensional loading conditions corresponding to the stance phase of gait were taken from the literature and applied to the model. Maximum principal compressive stresses of 8-23 MPa were computed for the medial femoral neck. Automated generation of additional finite element models with larger numbers of elements was used to verify convergence in strain energy.

Revascularized periosteal grafts--a new method to produce functional new bone without bone grafting.

Rib periosteum was transplanted to the groins of 9 dogs. In half of the periosteal grafts, no microvascular anastomoses were done (free grafts); at 6 weeks after grafting they had become resorbed. The other periosteal grafts were revascularized by microvascular anastomoses of the intercostal vessels to local muscular vessels; at 6 weeks those with confirmed vascular patency had all formed substantial amounts of new bone. Five cm, full-thickness defects were created in the tibias of 10 dogs. The control animals (without grafting) did not heal in two months. However, the experimental dogs, with vascularized periosteal grafts in the defects regenerated their tibias with healthy new bone by 6 weeks--and were walking on them then.

[From gene to behavior, a new method for elaboration of new psychotropic agents].

Traditionally, the screening of new neuropsychotropic agents started from the observation of behavioural effects resulting from the administration of a new chemical. Then, one tried to determine its mechanism of action. Since about a decade the way for discovering psychotropic agents tends to be inverse. It starts from the characterization of genes and investigates on their expression products which are new biological targets. Ligands for these targets are developed and then the effects resulting from their administration are considered. In this new strategy, we will consider the cloning of genes and their expression in cultured cells; the knock out of these genes by homologous recombinaison; the extinction of gene expression by antisense oligodeoxynucleotides; the concentration of behavioural phenotypes by selective breeding.

New method for the isolation of Listeria monocytogenes from contaminated samples.

A new method for the isolation of Listeria monocytogenes from samples contaminated with other bacteria is described. The method requires 10 to 11 days and is based on the motility of listeriae in semisolid selective medium. The new method was compared with isolation techniques described by Füzi and Pilis and by Ralovich et al. All 30 faecal samples containing 100 listeriae per g of faeces were shown positive by the new method. Using the method of Ralovich et al. the same positivity ratio could be obtained only with samples containing 1000 listeriae per g of faeces. At a contamination rate of 10000 listeriae per g of faeces 28 out of 30 faecal samples were positive by the method of Füzi and Pilis. The new method gave 10 to 86% positivity when 775 faecal samples from 10 animal species were tested.

New methods of hormonal contraception.

Several new hormonal contraceptive methods have recently been approved by the Food and Drug Administration. most new methods combine the high efficacy of oral contraceptives with the longer-term delivery methods that eliminate the need to take a pill each day. New methods also offer reversibility and resumption of fertility if desired. This article discusses these new methods and reviews the literature on efficacy and side effects.

Esters of cephalosporins part IX. New method of isolation of cefetamet pivoxil.

A new method of isolation of cefetamet pivoxil, which results in higher purity and higher yield than according to the known methods, was worked out. The new method depends on ability to the adsorption on acidic active carbon of total amount of this antibiotic together with only a small amount of impurities. During two-step elution all impurities were washed out in the first step and in the next step pure cefetamet pivoxil was eluated by another solvent.
