Experimental canine arterial thrombus formation and thrombolysis: a fiberoptic study.

We produced arterial thrombi that simulated the clinical condition in canine iliac arteries by endothelial denudation and partial occlusion. The processes of thrombus formation and thrombolysis were examined by vascular fiberscope. Laminar mural thrombi developed in all 21 preparations 10 minutes after blood reperfusion, with stenosis formation. Over 50% luminal obstruction with thrombi occurred in 10 preparations 30 minutes after inducing stenosis. Total occlusion with thrombi occurred in four preparation (31%) at 1 hour, and in all nine at 3 hours after inducing stenosis. Usually, red thrombus at the denudated region grew in size distal to the sited of stenosis. Histologically, thrombi of less than 1 hour duration consisted of a loose fibrin network and those of over 3 hours' duration consisted of a dense fibrin network. Infusion of 144,000 IU of urokinase (UK) reduced the size of thrombi less than 1 hour old during fiberoptic observation. However, UK infusion of similar dose failed to recanalize two of four preparations with 3-hour-old thrombi. In conclusion, arterial thrombi that closely simulate those observed clinically can be made by endothelial denudation and partial occlusion, and the vascular fiberscope provides a useful method for analysis of thrombosis and thrombolysis.

Is the sleeve anastomosis a risky technique?

We report 15 free flap transfers using sleeve vascular anastomoses with minor modifications of the original method. There was partial necrosis of one flap and one other required reoperation due to kinking of the feeding artery and not to the method of anastomosis. We believe that the sleeve anastomosis is reliable in clinical microvascular surgery if proper cases are selected. Indications for the method are discussed.

Development of a novel SNV-based genotyping method for Prototheca bovis.

We developed a novel single nucleotide variant (SNV)-based genotyping method for Prototheca bovis based on mitochondrial and plastid genome polymorphisms. Sequencing of five PCR amplicons targeting six informative SNVs discriminated field isolates with a Simpson's D value of 0.927, providing a simple and robust epidemiological typing method.

[A healthy carrier of verotoxin-producing Escherichia coli, who was detected by periodic feces examinations].

Verotoxin-producing Escherichia coli (VTEC) was rapidly detected by the PCR method in one of the 9 feces samples. They were collected from the people who had been cooking meals for patients on a periodic feces examination on November 25, 1992. It was confirmed by PCR that the isolate had both VT1 and VT2vh toxic genes and showed cytotoxicity on Verocells. In this case, the isolate was highly susceptible to common antibiotic agents, it was removed by the administration of tosfulaxacin. Some isolates which had the same properties as those of the strain described in "VITEK GNI card" and the antimicrobial susceptibility and VT toxin types, were detected in 2 of the 3 members of her family, they were healthy carriers and did not have any subjective symptoms such as diarrhea or abdominal pain. In order to detect Verotoxic genes, the sample of feces preincubated for 3 h in trypticase broth was subjected to PCR. We recommend that this method is much more useful because of rapid detection and identification of VTEC compared with the classical culture method.

Clinical use of the measurement of airway resistance during spontaneous respiration.

Airway resistance was measured during spontaneous respiration in healthy smokers and patients with chronic obstructive pulmonary disease, and the relationship between the airway resistance and respiratory frequency was examined. No significant difference was observed between the airway resistance in smokers and nonsmokers. In the chronic obstructive pulmonary disease group, patients with pulmonary emphysema had highest airway resistance. The determination of airway resistance during spontaneous respiration appears to be a very useful, noninvasive method of pulmonary function testing.

[The change in cerebral blood flow during hypotensive anesthesia induced by prostaglandin E1].

We investigated the effect of prostaglandin E1 (PGE1)--induced hypotension during sevoflurane anesthesia on the cerebral blood flow (CBF), autoregulation and internal jugular venous O2 tension (PjO2) in 8 patients undergoing neurosurgery. Although the mean arterial pressure decreased from 95.3 +/- 3.8 mmHg (mean +/- SD) to 63.6 +/- 8.0 mmHg by continuous intravenous infusion of PGE1, CBF did not change significantly (97.2 +/- 10.6% of control value). During hypotensive anesthesia, autoregulation was well maintained in all patients, and the values of PjO2 did not suggest brain hypoxia at all. The results indicate that hypotensive anesthesia induced by PGE1 and sevoflurane is a safe and a reliable method for neuroanesthesia.

Efficacy of levocabastine hydrochloride ophthalmic suspension in the conjunctival allergen challenge test in Japanese subjects with seasonal allergic conjunctivitis.

BACKGROUND: This study was conducted to investigate the efficacy and safety of 0.025% levocabastine hydrochloride in Japanese subjects with seasonal allergic conjunctivitis and its duration of action using the conjunctival allergen challenge (CAC) test. METHODS: Twenty-four asymptomatic subjects were randomized to instill 0.025% levocabastine ophthalmic suspension in one eye and vehicle in the other eye 10 minutes before the CAC test. Signs and symptoms of allergic conjunctivitis were scored 10, 15, and 25 minutes after the CAC test. The duration of drug effects was also evaluated by allergen rechallenge 4 hours after levocabastine administration. The itching score for each eye as the primary efficacy endpoint was assessed 15 minutes after the CAC test using a 5-point scale. RESULTS: The mean itching score in the levocabastine-treated group was 0.08 +/- 0.06, which was significantly lower than the mean score of 1.98 +/- 0.16 in the vehicle group (P < 0.0001). The redness and chemosis of the conjunctiva were also improved significantly compared with the vehicle group. Levocabastine showed prolonged efficacy in inhibiting itching (0.42 +/- 0.12 vs 0.94 +/- 0.17, P < 0.0002) and redness (1.04 +/- 0.18 vs 1.42 +/- 0.22, P < 0.01) of the conjunctiva upon the rechallenge test. No significant topical or systemic adverse safety findings were observed in the levocabastine group. CONCLUSIONS: The results indicate that 0.025% levocabastine ophthalmic suspension is effective and safe in the treatment of allergic conjunctivitis with a duration of action of at least 4 h.

Introduction of macromolecules into hemopoietic stem cells with an erythrocyte-ghost-mediated system.

We have developed a method of introduction of macromolecules into normal human hemopoietic stem cells. The erythrocyte ghosts were loaded with diphtheria toxin fragment A (molecular weight = 22,000 daltons), which exerts cytotoxicity only in the intracellular space. Granulocyte-macrophage colonies of human bone marrow cells incubated with the above ghosts in the presence of Sendai virus decreased in number to about 10% of the control. This means that the cell fusion and the subsequent introduction of the fragment A into granulocyte-macrophage progenitors occurred at a high incidence (about 90%). This method will be useful to study intracellular events during the proliferation and differentiation of hemopoietic stem cells.

Immobilized-enzyme electrode for nicotinamide adenine dinucleotide (reduced form) (NADH) sensing and application to the kinetic studies of NADH dependent dehydrogenases.

Amperometric determination of nicotinamide adenine dinucleotide (reduced form) (NADH) at an immobilized-diaphorase (Dp) electrode is described. The measurement was conducted using ferrocenylmethanol as a mediator in a stirred solution at 0.20 V versus a saturated calomel electrode. A linear relationship between the steady-state current and the concentration of NADH was found over the range 0.005-0.125 mmol dm-3. The immobilized-Dp electrode showed outstanding stability and the current response reached a steady state within 2-3 seconds upon addition of NADH. The proposed electrode was used to follow the reactions of pig heart lactate dehydrogenase and horse liver alcohol dehydrogenase. The kinetic investigation using the immobilized-Dp electrode gave the kinetic parameters (Michaelis constants, Km values, and maximum velocities, Vm values), which were in satisfactory agreement with those determined by a conventional spectrophotometric method.

A radioimmunoassay of bile acids.

A radioimmunoassay for beta-muricholic acid was developed using an antiserum which was prepared by injecting beta-muricholic acid conjugated with bovine serum albumin into rabbits. The antiserum reacted with glyco-beta-muricholic, tauro-beta-muricholic and beta-muricholic acids, but not with other bile acids. The radioimmunoassay showed good reproducibility with inter- and intra-assay coefficients of variations of 6% to 15%. When the validity of the method was examined by comparing it with a gas-liquid chromatography method, a linear correlation was obtained.
