Acute left ventricular filling time and rate changes in hypertrophic cardiomyopathy after verapamil and propranolol evaluated by nuclear stethoscope.

We have undertaken the present study to evaluate the acute effects of propranolol and verapamil on the diastolic function in patients with hypertrophic cardiomyopathy. We used a non imaging isotope device, the nuclear stethoscope, that has been so far mainly employed for assessment of systolic function, although fitted for detecting diastolic filling rates and times as well. Relative cardiac output, ejection fraction, rapid and slow filling times and rates were measured in five patients suffering from hypertrophic cardiomyopathy in basal conditions and 10.20 and 30 minutes after i.v. administration for either propranolol or verapamil, with a time interval of at least 24 hours between the two acute drug studies. With respect to the baseline values only verapamil showed a significant improvement in rapid and maximum filling rates, suggesting that diastolic function is more beneficially affected by Ca-entry blockers rather than beta-blockers in hypertrophic cardiomyopathy.

A preliminary report of "subclinical varicocele": diagnosis by Doppler ultrasonic stethoscope. Examination and initial results of surgical therapy.

Two hundred sixteen consecutive infertility patients were examined with the Doppler ultrasonic stethoscope to detect incompetence of the gonadal vein valves, the precursor of frank varicocele. All 75 men with clinically palpable varicoceles had positive Doppler findings. In 33 cases varicocele could not be definitely excluded because of unsatisfactory physical examinations; the Doppler findings were negative in 13, positive in 13 and equivocal in 7 of these men. Eighty-nine men had no evidence of varicocele on physical examination and negative Doppler findings. However, 19, patients with no palpable varicocele had positive Doppler examinations. With respect to testicular size and semen quality, this subclinical varicocele group could not be clearly distinguished from patients with idiopathic semen quality depression. Nevertheless, five men with subclinical varicocele and oligoasthenospermia underwent internal spermatic vein ligation. Postoperatively, four had negative Doppler examinations. Two patients showed a significant improvement in semen quality, and both men fathered children after several months. We believe that these preliminary results justify further investigation of "subclinical varicocele".

[Clinical evaluation of a miniaturized two-dimensional echocardiograph. Ultrasonic stethoscope].

An autonomous, miniaturised, semi-automatic linear-array system of real time imaging is described. This device was used for over one year in the coronary care unit, in the out-patient clinic and in the ward at the patient's bedside. Comparison with echocardiographic measurements obtained at M-mode echocardiography showed a slight tendency towards over estimation, especially of the dimensions of the left ventricle. The possibility of obtaining a rapid dynamic scan of the cardiac structures with the aid of an ultrasonic stethoscope will undoubtedly have the same stimulating effect on the development of miniaturised systems of real time imaging as the perfectioning of linear array system by Bom in 1971 had on two dimensional echocardiography.

New vacuum stethoscope for immediate monitoring and recording of resuscitation of the newborn.

A new, low-cost, noninvasive system is described, which accurately monitors and records both the initial respirations and heart rate of the neonate during resuscitation and the first critical seconds of life. The apparatus consists of a vacuum stethoscope attached to an ordinary cassette tape recorder and a 12-inch loudspeaker. The system was found to be a more accurate tool for assessing neonatal heart rate than the empirical estimation as part of the Apgar scoring system. In 6 of 37 births monitored, recordings indicated discrepancy between the actual heart rate and that estimated in the Apgar scoring system.

[Use of the ultrasonic Doppler stethoscope in the diagnosis of varicocele].

In cases of varicocele the reverse venous blood flow can be detected by the use of the Doppler-stethoscope in a simple way. Applied to the basic examination of fertility patients this method has proved to be a valuable non-invasive screening in the evaluation of varicoceles including cases with minor clinical symptoms. According to our results, "subclinical varicocele" as a entity of venous diseases shows special clinical features. Varicocele-orchiopathy is caused by the reverse venous blood flow, when peritesticular venous anastomoses and crosses are less developed or insufficient in their compensatory function.
