Perioperative anaphylaxis from locally applied rifamycin SV and latex.

A patient developed severe anaphylaxis during irrigation of a wound with rifamycin SV. The temporal relationship between application of rifamycin SV, the positive skin test and basophil activation test for rifamycin SV strongly supported diagnosis of anaphylaxis from the locally applied antibiotic. However, after operation the patient had two anaphylactic reactions with pruritus, urticaria and angio-oedema after routine care by a nurse, and these were probably caused by natural rubber latex. This case report has several messages. First, it is not widely appreciated that topically applied drugs and related compounds can elicit life-threatening anaphylaxis. Second, it illustrates patients can present with more than one allergy. Finally, it provides an opportunity to summarize the applications of flow cytometry-assisted quantification of in vitro activated basophils in diagnosing the cause of anaphylaxis during anaesthesia.

Bu and SV: two closely related B-locus specificities.

The B15/Bw21/Bw35 related specificities "Bu" and "SV" have been studied in Bristol and in Leiden, both independently and with mutually exchanged sera and cells. Evidence is presented that these specificities are likely to be discrete though very closely related antigens occurring with a combined phenotype frequency of about 1% in Dutch and English Caucasians. SV may be associated with Aw36, while no clear-cut haplotype or phenotypic association has yet emerged for Bu with any HLA--A sor C locus antigen. Bu and SV appear to be related to the variant of B15 reported to be common in Negroid populations.

Different gene defects in the salt-wasting (SW), simple virilizing (SV), and nonclassical (NC) types of congenital adrenal hyperplasia (CAH).

HLA (human leucocyte antigens) alleles and plasma 17-hydroxyprogesterone levels after ACTH stimulation were studied in 134 German families of patients with salt-wasting (SW), simple virilizing (SV), and nonclassical (NC) late-onset forms of CAH. HLA typing revealed a genetic difference between the two classical disease forms. SW-CAH was strongly associated with Bw47 and SV-CAH was closely linked to B5. The nearly complete connection of NC-CAH with B14 was confirmed. Bw47 and B14 were mostly components of the normally rare haplotypes A3, Bw47, DR7 and Aw33, B14, DR1, respectively. They did not occur in the families' disease-unaffected haplotypes. The HLA linkage data were consistent with those obtained from the ACTH stimulation test which showed a higher 17-hydroxyprogesterone increase in the group of genetically defined heterozygous relatives of SW patients than in the groups of heterozygous members of SV and NC families.

Sites of competition in the selective hepatic uptake of rifamycin-SV, flavaspidic acid, bilirubin, and bromsulphthalein.

In rats both rifamycin-SV and flavaspidic acid impaired the disappearance of bromsulphthalein (BSP) from plasma. The addition of rifamycin-SV to rat liver supernatant containing BSP did not displace BSP from ligandin or Z protein unless it was added in very high concentration. In similar studies in vitro with flavaspidic acid, BSP was displaced from Z protein even at low concentrations of flavaspidic acid, whereas there was only a minor effect on the binding to ligandin. The intravenous administration of rifamycin to Gunn rats raised the plasma bilirubin concentration slightly after 30 minutes whereas flavaspidic acid was without significant effect. At 30 minutes the amount of bilirubin in the liver was markedly reduced by rifamycin and moderately depressed by flavaspidic acid. Although both the rifamycin and flavaspidic acid lowered the bilirubin in cell sap, the main effect of flavaspidic acid appeared to be on binding to Z protein. Rifamycin reduced the amount bound to both ligandin and Z protein. It is suggested that flavaspidic acid acted on the hepatic uptake of bilirubin and BSP predominantly by competing for binding to Z protein. Rifamycin-SV acted at a different site, probably blocking uptake at the plasma membrane.

Herpes zoster treatments: results of a clinical trial relative to the use of rifamycin SV versus neuramide.

The authors describe a controlled clinical study in which rifamycin SV 250 mg intramuscularly and topical b.i.d. was compared to intramuscular neuramide b.i.d. plus, where necessary, other drugs (antibiotics, polivitamins, analgesics, etc.) for the treatment of two groups of thirty randomly selected patients suffering from herpes zoster. In all patients the symptoms were controlled by both treatments but statistical tests revealed that rifamycin SV was able to heal pain (p less than 0.05), vesicles, crusts and burning sensation (p less than 0.1) faster than neuramide. Furthermore, by the seventh day of therapy, the authors found that rifamycin SV reduced the intensity of both pain and erythema (p less than 0.01 for pain; p less than 0.05 for erythema) more than neuramide.

Aromatase expression in the human fetal osteoblastic cell line SV-HFO.

A number of clinical studies have highlighted the importance of estrogen in bone growth and maintenance in men and postmenopausal women. In these instances, estrogen is synthesized locally within bone tissue by aromatase, encoded by the CYP19 gene. The mechanisms regulating aromatase expression in bone, however, are unclear. In this work we characterized the expression of aromatase activity and gene transcripts in the human fetal osteoblastic cell line, SV-HFO. Aromatase activity and gene transcript expression were stimulated by dexamethasone. Oncostatin M strongly stimulated aromatase expression in synergy with dexamethasone. These factors induced CYP19 transcripts that included the sequence of exon I.4 in their 5'UTR. Consistent with this, a reporter construct harboring the genomic sequence of the promoter region of exon I.4 (promoter I.4) was also activated by dexamethasone and oncostatin M. 5' deletion and mutation analysis revealed important roles for a glucocorticoid response element, an interferon gamma activating sequence and a putative binding site for Sp1. Transfection of exogenous glucocorticoid receptor, STAT3 or Sp1 increased promoter activity, indicating a potential role for these transcription factors in regulating aromatase expression in SV-HFO cells. These data suggest that the SV-HFO cell line is a valuable model with which to elucidate the mechanisms regulating local estrogen synthesis in osteoblasts.

[Usefulness of continuous SV(-)O2 monitoring during hepatectomy].

The mixed venous oxygen saturation (SV(-)O2) was continuously monitored during hepatectomy including the resection and reconstruction of great vessels in five patients. During the artificial bypass, the inadequacy of venous return due to failed bypass and/or hemorrhage was detected by the monitoring of SV(-)O2. Low cardiac function and the effect of a vasodilator were also confirmed by this monitoring. The conventional monitoring, such as blood pressure and heart rate, did not always reflect the acute changes in patients' hemodynamics. The monitoring of SV(-)O2 is useful in the anesthetic management during the surgeries which are accompanied with acute hemodynamic changes.

Kinetics of G1 progression in 3T6 and SV-3T3 cells following treatment by 25-hydroxycholesterol.

The kinetics of cell cycle progression in continuously proliferating 3T3 fibroblasts and two tumor transformed derivatives (3T6 and SV 3T3 cells) following treatment by growth-factor deprivation (serum starvation) or 25-hydroxycholesterol were studied. Normal 3T3 cells were found to respond immediately (in the first cycle) to growth factor deprivation by leaving the cell cycle from G1, whereas the tumor transformed derivatives did not. However, all three cell types were forced to stop the progression through the beginning of G1 when treated by 25-hydroxycholesterol. It was ensured that the doses of 25-hydroxycholesterol used really induced substantial decrease of HMG CoA reductase activity. However, the effects of serum starvation on HMG CoA reductase activity varied considerably. In 3T3 cells HMG CoA reductase activity was substantially depressed, in 3T6 cells it was moderately depressed, and in SV-3T3 cells it was not depressed at all. This difference of HMG CoA reductase activity between 3T6 and SV-3T3 cells was related to the difference of growth activity in serum-free medium. The data indicate that a certain activity of HMG CoA reductase is required for the proliferation of normal as well as tumor transformed cells but also that impairment of the control of HMG CoA reductase, leading to increased enzyme activity, may result in uncontrolled growth in tumor transformed cells.

Rifamycin SV in local treatment of synovitis--a clinical, arthroscopic and pharmacologic evaluation.

Intraarticular injections of rifamycin SV were repeated weekly in patients with rheumatic disease with a chronic knee joint effusion. The clinical signs of synovitis were reduced in all, but disappeared in only one. A post injection reaction with transient local pain and effusion appeared at the third or a later injection in all the patients prompting withdrawal of 3. At these later injections local drug retention was shown by parallel determinations of rifamycin SV in synovial fluid and serum. The post treatment synovial fibrosis seen at arthroscopy could possibly explain the drug retention and the post injection inflammatory reaction. Thus, local treatment with rifamycin SV alleviated but rarely abolished synovitis.

[Effectiveness of cystolusis with rifamycin SV in patients with permanent catheters].

It has tested the effect of Rifamicina SV in the cystolusis employed on patients with permanent catheter: compared to the treatment with physiological solution this one, with Rifamicina SV, has showed, in most cases, the lower incidence of urine bacteria and their quick negative answer. This by an only local theatment. During the experiment has also been showed the possibility for the Rifamicina SV of selecting the Citrobacter as a resistant bacterial species.

Investigations on the presence of antibodies to papova viruses in patients with different forms of cancer and in other categories of patients or apparently healthy subjects. Note I. Neutralizing antibodies to SV-40 in the cerebrospinal fluid of patients with brain tumors and chronic neurological diseases.

Neutralizing antibodies to SV-40 were detected in cerebrospinal fluid samples from 16 out of 43 patients with brain tumors and in 6 out of 20 patients with different nonmalignant chronic neurological diseases. The patients' age excluded the correlation with a vaccine-associated or occupational contact with SV-40. The results ascertain the occurrence of SV-40 or of an antigenically related virus in the population of Romania.

Breast cancer formation in transgenic animals induced by the whey acidic protein SV40 T antigen (WAP-SV-T) hybrid gene.

After injection of the whey acidic protein (WAP)-SV-T hybrid gene into fertilized mouse eggs, eight independent transgenic mouse lines were obtained. Females from three lines developed mammary carcinomas with high frequency, coinciding mostly with lactation. In contrast to the endogenous WAP gene, expression of the hybrid gene continued after lactation. The tumor cells had a very invasive growth characteristic. Tumor regression in vivo was not observed. However, after transfer into tissue culture 25% of the cells ceased to express the hybrid gene and acquired the growth characteristic of normal cells. It was possible to retransform these cells by injection of wild-type SV40 DNA, but not after transfer of the hybrid WAP-SV-T gene. Inactivation of the endogenous WAP and of the WAP-SV-T transgene did not correlate with DNA methylation.

The SV stent study: a prospective, multicentre, angiographic evaluation of the BiodivYsio phosphorylcholine coated small vessel stent in small coronary vessels.

OBJECTIVE: To evaluate the use of the phosphorylcholine (PC) coated BiodivYsio small vessel (SV) stent in native coronary vessels of small calibre. DESIGN AND SETTING: Prospective, multi-centre, multi-national registry with 6-month clinical and core-lab angiographic follow-up. Adverse events were adjudicated by a Clinical Events Committee (CEC) and included peri-procedural analysis of cardiac enzymes. PATIENTS: Patients with signs or symptoms of ischaemia with an identified target lesion in an epicardial vessel with reference diameter 2.0-2.75 mm were enrolled. Intervention in other epicardial territories in the same patient was permitted. RESULTS: Recruitment of 150 consecutive lesions (in 143 patients) was completed in 19 centres in Europe and Israel. The stent was deployed successfully in all but one lesion. At 6 months, 1 patient (1%) had experienced sudden cardiac death, 4 further patients (3%) had a non-Q wave MI, and a further 24 patients (17%) had repeat revascularisation of a study target vessel. The mean reference vessel diameter prior to stenting was 2.2 mm (S.D. 0.4). Mean minimal luminal diameters at pre-procedure, post procedure and follow-up were 0.6 mm (S.D. 0.3), 2.0 mm (S.D. 0.4) and 1.2 mm (S.D. 0.6), respectively. The late lumen loss index was 0.55 (S.D. 0.53) with a binary restenosis rate of 32%. CONCLUSIONS: In stenting of selected lesions in small vessels, the BiodivYsio SV stent demonstrated high rates of implant success. The rates of major adverse cardiac events (MACE), angiographic restenosis and repeat revascularisation are similar to those reported in other small vessel bare metal stent studies.

SV-IV, a major protein secreted from rat seminal vesicle epithelium, promotes lymphocyte cytotoxic activity against the lymphoblastoid Raji cell line in human peripheral blood mononuclear cells.

The treatment of human peripheral blood mononuclear cells (PBMC) with micromolar concentrations of SV-IV, a major protein secreted from the rat seminal vesicle epithelium, promotes in this cell population a marked cytotoxic activity against the Raji lymphoblastoid cell line. This activity is apparently due to cell-to-cell contact interactions. The expression of HLA DR on Raji cells has a modulatory effect on the SV-IV-induced cytotoxic activity. The experimental evidence strongly suggests that the cytotoxic effector cells are functionally activated NK cells.

Regulation of amino acid and glucose transport activity expressed in isolated membranes from untransformed and SV 40-transformed mouse fibroblasts.

Membrane vesicles isolated from untransformed Balb/c and Swiss mouse fibroblasts and their SV 40-transformed derivatives were shown to catalyze carrier-mediated, intravesicular uptake of alpha-aminoisobutyric acid and D-glucose. Concentrative uptake of alpha-aminoisobutyric acid required the presence of a Na+-gradient (external greater than internal) and could occur independently of endogenous (Na+ + K+)ATPase activity. A K+ diffusion gradient (internal greater than external) in the presence of valinomycin, or the addition of the Na+ salt of a highly permeant anion, conditions expected to create an interior-negative membrane potential stimulated Na+-gradient-dependent uptake, suggesting this process is electrogenic. D-Glucose uptake was nonconcentrative and did not require ion gradients or metabolic conversion. Na+ gradient-dependent transport of alpha-aminoisobutyric acid was reduced both in initial rate and extent of uptake in vesicles from confluent untransformed cells and increased in those from SV 40-transformed cells, compared with activities observed in vesicles from proliferating untransformed cells. No changes in D-glucose carrier activity were observed when assayed at low glucose concentrations.

The control of human WI-38 cell proliferation by extracellular calcium and its elimination by SV-40 virus-induced proliferative transformation.

The proliferative activity of diploid human WI-38 cells in sparse cultures depends on the extracellular concentration of free (or physiologically available) calcium, and cultivation in a medium having a calcium concentration of 0.1 mM or less gradually, but reversibly, arrest their proliferative development in the prereplicative (G1) phase of the cell cycle. Calcium's proliferative control of this cell type is eliminated by proliferative and morphological transformation by the oncogenic SV-40 virus, and the proliferative activity of SV-WI-38 cells in sparse cultures is unaffected by variation of the extracellular free calcium concentration between 0.00 and 1.25 mM.

Midline brain tumors in MSV-SV 40-transgenic mice originate from the pineal organ.

Adult transgenic mice expressing the large T-antigen of the Simian virus 40 (SV 40) under the control of the Moloney murine sarcoma virus (MSV) enhancer and the SV 40 promoter develop inheritable uniform midline brain neoplasms showing features of primitive neuroectodermal tumors. The origin and histogenesis of these tumors were investigated in the present study. The brain and pineal organ of fetal and young transgenic mice less than 3 months old displayed normal macroscopic and microscopic features. In 3.5-month-old animals, the pineal organ was considerably enlarged due to hyperplasia, finally leading to tumor formation. Immunocytochemical demonstration of large T-antigen showed that this oncoprotein was already expressed in the nuclei of certain cells in the pineal organ of fetuses (16 and 18 days old) and newborn animals, but was absent from all other parts of the brain. The immunocytochemical demonstration of S-antigen (arrestin), a highly characteristic marker for pinealocytes, was used for further characterization of the large T-antigen-immunoreactive cells. The fetal pineal organ did not contain immunoreactive S-antigen. This first occurred in certain pinealocytes of newborn mice. Double immunostaining revealed that in newborn and older transgenic mice the immunoreactive large T-antigen was exclusively found in nuclei of cells containing S-antigen immunoreaction in their cytoplasm. Thus, transformed pinealocytes appear as stem cells of the experimental tumors. The results of this study suggest that primitive neuroectodermal tumors and the normal tissue from which they originate share certain molecular and immunocytochemical features.

SOS-type functions in mammalian cells. Enhanced reactivation of UV-irradiated SV 40 in UV-irradiated CV-1 cells.

The reactivation of UV-irradiated SV 40 was measured in UV-irradiated CV-1 cells as a function of time between irradiation of the cells and infection. To avoid the possible bias of multiplicity reactivation and/or virus propagation, infection was quantitated in terms of V-antigen-positive nuclei at the end of the first lytic cycle. In irradiated cell cultures enhanced reactivation of SV 40 was observed, which indicates induction of DNA repair enzymes. Maximal reactivation was obtained when the time interval between irradiation and infection was 72 h. The UV-inducible DNA repair enzymes might represent elements of an SOS-type response in mammalian cells.
