[History of the beginnings of Czech stomatology].

The authors present a brief account of the history of dental surgery--stomatology--in Bohemia and Moravia from the very beginnings to the period after the Second World War with indicators of the brisk development of the discipline in the new democratic state which occurred in particular after the foundation of the stomatological branch of medical faculties in 1950/51. The authors mention in more detail only the main personalities and representatives of the discipline who contributed most to the development, being aware that they collaborated with several tens of teachers and research workers of more than four generations who also deserve admiration and gratitude.

[Training in social competence in undergraduate education in pediatric stomatology].

Hitherto practised training revealed that in the undergraduate training in paedostomatology there are disproportions between the shaping of the professional skill and the formation of professional social and psychological qualities of the student. The consequences of the one-side attention of teaching to professional training are manifested already during clinical practicals and during the implementation of systematic dental care in practice. The authors innovated the programme of undergraduate training in paedostomatology. This resulted in rationalization, not extension of the training at the expense of other subjects. As far as stomatological training is concerned, they adhered as regards scope and contents to the hitherto existing seven thematic entities of the uniform curriculum. A new subjects, psycho-social training was included where students are not taught only basic theoretical knowledge; methods of social training were applied and students are taught the necessary practical skills. The method was applied so far for five years in the training of paedostomatology at the First Chair of Stomatology. The acquired experience justifies the statement that the method was accepted by members of the department (of all categories), child patients and parents. The children become rapidly familiar with the technique of substitutional signals of communication. During subsequent visits they do not require treatment by the same doctor but adherence to the ritual. The change of the general style of training enables the teacher to make the students familiar in a more instructive manner with the basic principles of three-step preventive care, to teach the students not to treat but to "be a doctors". By simulation of field practice in the final part of the clinical course training was approximated to the actual procedure in practical work.

[Effects of a conservative stomatological treatment on central and peripheral stress parameters and the influence of a beta-adrenergic blocking agent].

Under conservative stomatological treatment the behaviour of peripheral stress parameters is investigated in connection with an estimation of the subjective state of health as well as for the first time in the ambulatory stomatology the STH secretion as an expression of a central stress reaction. The possibility of the central nervous as well as cardiovascular influence by an unusual premedication form of the beta-adrenergic blocking agent "Obsidan" is estimated in various stages of treatment in comparison to a placebo application.

[Inquiry data for stomatological health education of pregnant women in the town of Sumen and the region].

The purpose of the authors with the present study was to obtain data on the information of the pregnant women about problems associated with the stomatological health. Anonymous inquiry was carried out among 250 pregnant women from the town of Sumen and the region. The questionnaire has 7 questions. The answers give information on the knowledge about problems associated with nutrition, fluorine prophylaxis, formation of dental germs and cutting. The results reveal insufficient knowledge in all problems. Conclusions are drawn that the micro-courses during the last months of pregnancy in stomatological prophylaxis are ineffective. The individual work proved to be insufficient as well as the participation of the regional and workshop stomatologists.

[Desensitization during play situation for children with negative behaviour in stomatological environment].

A new form of desensitization during play situation for children, aged from to six, is applied. The method includes a fictitious treatment of the children with the participation of a figurative toy and a play-sketch. The children have shown fear reactions and negative behaviour in stomatological environment before the desensitization. The results from the study confirm the effectiveness of that form in influencing the fear and behaviour of the children in stomatological environment.

[Examination of the influence of selected drugs administered orally on amount of saliva excreted and pocket fluid as well as on patients' feeling during stomatological treatment].

The influence of pyralgin, acenol, hydroxizinum and scopolan administered orally on the amount of the saliva excreted and pocket fluid as well as one the patients' feeling during stomatological treatment has been examined by the dual blind test method. The examinations haven't proved any significant influence of these drugs on the excretion of the saliva and pocket fluid of a statistical importance; these drugs, however, have reduced the painfulness of the operations depending on hard dental tissues processing. Among the drugs applied Pyralgin has proved to be the most useful drug for patients premedication before stomatological treatment.

[Stomatological manifestations in Sjögren's disease and syndrome].

For diagnosis of SS and SD and the detection of early stages of disease one should necessarily take into account the symptom complex of "major" (salivary gland enlargement, xerostomia, exacerbation of parotitis) and "minor" stomatological signs (multiple cervical caries, dry lips, perlèche, mycotic and herpetic stomatitis, lymphadenopathy). The initial, marked and late stages were defined according to a degree of expression of stomatological manifestations. The initial stage prevailed in SS, the late stage in SD. The chief method of examination were sialometry, sialography and minor salivary gland biopsy. Sialography was widely used as a less traumatic diagnostic procedure. In addition to common signs with SD, salivary gland involvement was characterized by changes typical of SS combined with rheumatic disease (sclerosis in sclerodermia, vasculitis in RA and SLE, nuclear pathology in SLE).

[Pyogenic infections of the jaws and adjacent soft parts - retrospective analysis of 9,069 patients treated on an outpatient basis between 1969 and 1978 at the Clinic of Stomatology, University of Halle/Saale (author's transl)].

Of 162.338 patients who between 1969 and 1978 received treatment at the Clinic of Stomatology, University of Halle-an-der-Saale, 9.069 (or 5.6 percent) were treated on an outpatient basis after having been diagnosed as having actinomycosis, osteomyelitis, lymphadenitis, difficult dentition, and nonspecific pyogenic soft-part infections (abscesses or infiltrates). A retrospective analysis by electronic data processing of case histories provided valuable information about morbidity, disposition, cause, localization, and the tendency for dental surgeons to refer certain cases to specialty clinics. There are pathognomonic differences in indexical systems for the different clinical pictures. Cases of osteomyelitis and soft-part infection with suspected actinomycosis were found to increase in correlation with the proportion of antimicrobially pretreated patients. About 60 percent of all patients receiving treatment at this Clinic were not referred thereto by dentists. Of the documents presented by patients referred to the Clinic of Stomatology by other dental surgeons, some 42.5 percent were filled out incompletely.

Treatment of stomatological complications in 31 cases of acute leukemia with Chinese herbal drugs.

The authors treated 31 cases of stomatological complications of acute leukemia with Chinese herbal drugs. It was found that the prevalence of oral mucosal ulcers was the highest (80%) in acute lymphocytic leukemia, and the prevalence of gingival swelling was the highest (44%) in acute monocytic leukemia. In accordance with symptom differentiation, Qing Wei San (Powder for Clearing Stomach-heat) with modifications was administered in cases of exuberance and Yü Nü Jian (Gypsum Decoction) with modifications was administered in cases of deficiency, plus gargling solution and hemostatic powder for external use. As a result, 7 cases (22.6%) were markedly effective, and 19 cases (61.3%) effective, the total effective rate being 83.9%. The authors believed that stomatological complications of leukemia were due to insufficiency of yin and blood in the body or to the toxicity and heat of anti-neoplastic medication, and the Chinese herbal drugs were to replenish yin and clear the heat.
