Pathogenic amoebae in natural thermal waters of three resorts of Hidalgo, Mexico.

In a search for free-living amoebae, seven water samples from three thermal water bathing resorts in Tecozautla, Hidalgo, were analyzed during December 1984. The samples were concentrated by filtration and centrifugation, and inoculated later on monoxenic and axenic media. The identification of the isolates was performed by morphology and isoelectric focusing of isoenzymes and total proteins. Thirty-three strains of free-living amoebae belonging to the genera Naegleria, Acanthamoeba, and Willaertia were isolated. Twenty of these strains belonged to the Naegleria genus, 16 of them were classified as Naegleria spp., and 2 were classified as Naegleria lovaniensis. Noteworthy was the finding of two pathogenic strains of the species Naegleria australiensis. N. australiensis and N. lovaniensis may be considered good indicator organisms, since they live in the same environmental conditions as N. fowleri, the agent of primary amoebic encephalitis (PAM). On the other hand, amoebae other than Naegleria were isolated and identified as Acathamoeba castellanii (two strains), and Acanthamoeba lugdunensis (one strain), which proved to be pathogenic when tested in mice. Nine more pathogenic strains of the genus Acanthamoeba spp. were isolated together with one strain of Willaertia magna, a thermophilic nonpathogenic amoeba. The chlorination and periodical surveillance of water resorts like the one studied is recommended, in order to prevent the appearance of more cases of PAM or other human diseases associated with pathogenic Acanthamoeba spp.

Why resort to illegal abortion in Zambia? Findings of a community-based study in Western Province.

This article presents part of the findings of a community-based study on the causes and effects of unplanned pregnancies in four districts of Western Province, Zambia. The study broke the silence around abortion in Western Province and revealed that induced abortion poses a public health problem. Using innovative methodology of recording and analyzing histories of deaths from induced abortion, the abortion mortality ratio was calculated for the study districts. Findings reveal all extremely high induced abortion mortality ratio of 120 induced abortion-related deaths per 100,000 live births. More than half the deaths were of schoolgirls. Although abortion in Zambia is legal on medical and social grounds, most women in Western Province resort to illegal abortions because legal abortion services are inaccessible and unacceptable. The main reasons women resort to abortion is for fear of being expelled from school, their unwillingness to reveal a secret relationship, to protect the health of their previous baby and common knowledge that postpartum sexual taboos have been transgressed. An inventory was made of abortion methods, taboos and abortion-providers. The article describes how health staff were involved throughout the study, and shows how recommendations were made by involving all parties concerned.

Scene transport of pediatric patients injured at winter resorts.

OBJECTIVE: To examine the characteristics of pediatric patients (age =16 years) injured at winter resort scenes and transported by helicopter emergency medical services (HEMS) or ground EMS (GEMS) ambulance services to regional trauma centers. METHODS: Between 1997 and 2001, a total of 119 patients (GEMS = 69; HEMS = 50) were identified from trauma registries and HEMS transport records. Demographic data, initial vital signs, hospital interventions, and discharge status of the two groups were examined. RESULTS: The distributions of gender, initial vital signs, Injury Severity Score (ISS; either = or > 15), intensive care unit (ICU) length of stay (LOS), total hospital LOS, and home discharge status were similar between the two groups (p = 0.05). Patients transported by HEMS were older (14 +/- 2 vs. 10 +/- 4, p < 0.001), less likely to be admitted to the hospital (73% vs. 98.5%; p < 0.001), and more likely to have multiple injuries [13 (27%) vs. 8 (11.6%), p = 0.032]. The GEMS patients had a higher rate of isolated extremity [33 (80.5%) vs. 8 (19.5%)] and thoracoabdominal [11 (73.3%) vs. 4 (26.7%)] injuries. The high orthopedic injury rate in the GEMS patients contributed to a higher rate of surgery in this group (45% vs. 24%, p = 0.028). Regardless of transport mode, patients requiring immediate interventions (intubation, chest tube placement, or blood product administration) had either a depressed level of consciousness (GCS = 12) on emergency department arrival or thoracoabdominal injuries. No deaths were recorded. CONCLUSIONS: Patients transported by HEMS and GEMS had similar hospital characteristics but different injury patterns. A prospective study examining the initial triage of pediatric patients injured at winter resorts would help to determine which subset of patients are best served by HEMS transport.

Modified natural cycle for IVF does not offer a realistic chance of parenthood in poor responders with high day 3 FSH levels, as a last resort prior to oocyte donation.

BACKGROUND: The purpose of this study was to evaluate the use of the modified natural cycle (MNC) for IVF in poor responders as a last resort prior to oocyte donation. METHODS: Thirty-two patients with a regular menstrual cycle, FSH levels on day 3 of the cycle >12 IU/l and one or more failed IVF cycles with five or fewer cumulus-oocyte complexes (COCs) retrieved were included in this prospective study. Recombinant FSH 100 IU and GnRH antagonist 0.25 mg/day were started concomitantly when a follicle with a mean diameter of 14 mm was present at ultrasound. HCG 10 000 IU was administered as soon as the mean follicular diameter was > or =16 mm. RESULTS: Twenty-five out of 78 cycles performed (32.1%) did not result in oocyte retrieval. In nine out of 53 cycles (16.9%) in which oocyte retrieval was performed, no COCs were retrieved. Following fertilization, embryo transfer was performed in 19 out of 44 cycles in which COCs were retrieved (43.2%). No ongoing pregnancy was achieved in 78 MNCs (0.0%; 95% confidence interval 0.0-4.7). CONCLUSIONS: MNC does not offer a realistic chance of parenthood in patients with high levels of FSH on day 3 of the cycle and previous poor response to ovarian stimulation, when offered as a last resort prior to oocyte donation.

HIV infection and associated risks among young men who have sex with men in a Florida resort community.

OBJECTIVE: Several recent studies have reported high rates of sexual risk-taking and HIV infection among young men who have sex with men (MSM). Most of these studies used samples of convenience. The authors obtained population-based data on young MSM living in South Beach (Miami Beach, Florida), a resort community where some of the highest AIDS rates in the United States have been reported. METHODS: A household probability sample was drawn to survey unmarried 18- to 29-year-old MSM living in South Beach. Subjects were interviewed, completed self-administered questionnaires, and provided oral specimens for HIV antibody testing. RESULTS: From the 2,622 screened residential units, 100 mostly white and Hispanic MSM (92.6% of eligible participants) were enrolled in the study. Fifteen percent of the sample tested positive for antibodies to HIV. White and Hispanic MSM had similar rates. Forty-five percent of the sample reported engaging in unprotected anal intercourse (UAI) in the prior 12 months, and 31% reported UAI with a nonprimary partner. The estimated annual incidence of HIV infection was 6.3%. CONCLUSION: The high prevalences of UAI and HIV infection in South Beach attest to a previously undocumented public health concern. The extremely high estimated incidence for young MSM in South Beach highlights the urgent need for more effective risk-reduction interventions and further epidemiological research on resort areas.

Waterborne outbreak among Spanish tourists in a holiday resort in the Dominican Republic, August 2002.

On 3 September 2002, the Spanish national centre of epidemiology (Centro Nacional de Epidemiologia--CNE) was alerted to a high number of gastroenteritis cases in Spanish tourists who had travelled to a hotel in Punta Cana on different days during august 2002. Entamoeba hystolitica cysts have been visualised by microscopy in the stools of several patients that sought medical attention in the Dominican Republic. The CNE informed the health authorities in the Dominican Republic and conducted in conjunction an epidemiological investigation. A descriptive study of the 76 initial cases estimated a mean illness duration of 5.1+2.9 days and a exposure period of 3.6+2.2 days. Following a retrospective cohort study, the attack rate was found to be 32.4%. Consequently, 216 (95% CI=114.75-317.25) spanish tourists had probably developed the illness. Stool samples were collected in Spain from untreated patients who still felt unwell. None of the samples were positive for E.hystolitica. On 10 September, a hygiene inspection was undertaken at the hotel. Samples of the ice and meals served at the buffet that day, yielded coliform bacteria. Consumption of water from the resort water system was the only risk factor associated with the symptoms (RR= 3.55; 95% CI =1.13-10.99). To avoid similar outbreaks occurring again at the hotel, it is essential to regularly monitor the water quality and to improve food handling hygiene standards. Basic food hygiene training for food handlers should be mandatory. An international guideline for the management foodborne and waterborne outbreaks among tourists in holiday resorts should be drawn up, involving all competent authorities of both destination and tourist origin countries.

Waterborne outbreak of Norwalk-like virus gastroenteritis at a tourist resort, Italy.

In July 2000, an outbreak of gastroenteritis occurred at a tourist resort in the Gulf of Taranto in southern Italy. Illness in 344 people, 69 of whom were staff members, met the case definition. Norwalk-like virus (NLV) was found in 22 of 28 stool specimens tested. The source of illness was likely contaminated drinking water, as environmental inspection identified a breakdown in the resort water system and tap water samples were contaminated with fecal bacteria. Attack rates were increased (51.4%) in staff members involved in water sports. Relative risks were significant only for exposure to beach showers and consuming drinks with ice. Although Italy has no surveillance system for nonbacterial gastroenteritis, no outbreak caused by NLV has been described previously in the country.

[Environment capacity of eco-tourism resort].

The results of quantitative analysis on the amount of tourist, service-environment capacity, eco-environment capacity, and their relations in Five-finger Mountain eco-tourism resort indicate that the amount of tourist in common situation and in its extreme was 1918 and 2301 visitor-hour per day, and the service-environment capacity and eco-environment capacity were 6000 and 2400 visitor-hour per day, respectively. The eco-environment capacity was smaller than its service-environment capacity, and would become the first limiting factor to the increase of tourist amount, which was mainly due to the ecological fragility of resort, the lower resistance of biological communities to the disturbance, and the slower speed of ecosystem restoration after its being destroyed.

[Sport and kinesiotherapy within the framework of health resort clinics for prevention and medical rehabilitation (author's transl)].

In most treatment clinics a considerable portion of the treatment programme, is terms of space as well of time occupied, is dedicated to the kinesiotherapy field. Based on the register of the Verband Deutscher Rentenversicherungsträger (Federation of German Pension Insurance Institutes), 433 health resort clinics were surveyed by mail in 1978/79 in order to obtain information on their sport and kinesiotherapy situations. The present findings are based on the replies of 206 clinics, corresponding to 55 percent (i.e. 39,840 beds) of the initial total of beds. The majority of patients covered suffered from disorders of inner organ function (66 percent) or from musculo-sceletal impairments (22 percent). Actual delivery of the sport and kinesiotherapy programmes lies with massage and balneology personnel in 60 percent, physiotherapists and gymnastics teachers in 32 percent of cases, and only in some 2.5 percent are the measures implemented by qualified sport teachers. The question is discussed whether these percentages are compatible with the health educational goals of present-day concepts for preventive or rehabilitative medical treatment in the health resort clinics. Changes are needed both at the staffing and organisational levels (staffing schedules, space allotment) if the pension insurance institutes' approach of "health do it yourself" is to prove successful in this field.

Determination of 222Rn in natural water samples from health resorts in the Sudety mountains by the liquid scintillation technique.

A method involving a Wallac 1414 WinSpectral alpha/beta liquid scintillation counter for determining 222Rn in aqueous samples is described. Samples were collected from 23 springs and 4 taps in health resorts of the Klodzka valley in the Sudety mountains in Poland. Half of the investigated spring water samples were radon enriched with an activity concentration higher than 74 Bq/l. In the tap waters the radon concentration level is very low or below the lower limit of detection. Owing to the statistical nature of radioactive decay the uncertainty of the measurement was determined as the standard deviation of 222Rn activity. The method introduced is fast and simple and does not require a chemical sample preparation procedure.

[Effect of sulfur administration on lipid levels, antioxidant status and peroxide concentration in health resort patients].

The aim of the study was to test the effect of drinking sulphurous mineral water on lipid status and antioxidative balance in patients with degenerative arthroses during a 3-week stay in a health resort. Two groups of patients with and without drinking sulphurous water were investigated. Lipid levels, glutathione peroxidase, superoxidedismutase, antioxidative status, and peroxide concentration in plasma were measured. In both groups similar alterations of the activities of antioxidative enzymes as well as peroxide concentrations could be detected. Possible specific effects of drinking sulphurous mineral water have to be investigated in further studies.

[A study on the physical effects of short-time recreation activities at a hot spring resort on unhealthy middle-aged workers].

A pilot study to improve unhealthy life habits of thirty middle-aged male clerical workers (45 +/- 3.58 yr.) with mild health disorders such as hypertension, hypercholesterolemia, diabetes mellitus and fatty liver was carried out. Under prohibition of smoking and alcohol intake, they spent five nights and six days at a hot spring resort, taking part in planned health training programs which included aerobic training, hiking in forests, hot spring baths, cooking practice and lectures about healthy life, controlled by medical, dietary and physical training staffs. To evaluate the short-term effects of these activities, body weight, blood pressure, serum lipid (total cholesterol, high-density-lipoprotein cholesterol, triglyceride, total free fatty acid and phospholipid), blood sugar, uric acid, gamma-glutamyl transpeptidase (gamma-GTP) and glutamic-oxaloacetic transaminase (GOT) were examined early in the morning of the second (before) and the fifth (after) days, and then their impressions of these recreation activities were monitored by questionnaires on the sixth day. By t-tests of all before-and-after data, it was shown that mean values of body weight, systolic blood pressure, high-density-lipoprotein cholesterol, triglyceride, phospholipid and gamma-GTP were improved, but fasting blood sugar, uric acid and GOT were not improved. In comparison of blood pressure levels, the hypertensive group (n = 9) showed lowering in both systolic and diastolic blood pressure, though the normal group (n = 10) had slight elevation. In addition, in the hypercholesterolemic group (n = 11, greater than or equal to 220 mg/dl) mean total cholesterol values decreased, conversely in the hypocholesterolemic group (n = 6, less than 180 mg/dl) they increased. Moreover, the obese group (n = 15, obesity index greater than or equal to 120%) showed greater decreases of body weight, triglyceride and phospholipid than the non-obese group. From questionnaires, it was confirmed that through these recreation activities most participants found mental and spiritual satisfaction, in spite of heavy physical loads. The short-term recreation activities under a stressless environment seemed to maintain the function of homeostasis in the body, but further investigation is needed to examine the relation between the contents of diets and physical activities, and to follow the long-term effects on the participants.

[Marketing and the distribution of passes to health resorts of the Ministry of Defense of the Russian Federation].

Social and economic reforms in Russia have resulted in the appearance of the recreation services market. To coordinate its activities with orientation to consumers the Department of vouchers' distribution and realisation was set up in February 1998 in scientific methodology center in Marfino Central Military Clinical Sanatorium. The article deals with the Department's attempts to introduce principles of the market economy into the work practice of the military health resorts and sanatoria.

[Health resort factors in the regulation of sexual disorders in men who permanently live in a radionuclide-contaminated area].

The paper presents a clinical material on effects of health-resort factors, hyperbaric oxygenation, and psychotherapeutic correction on the gonadal function in patients having sexual problems. The multimodality approach permitted not only a significant alleviation of pathological symptomatology on the part of the autonomic nervous system, pain syndrome, improving the consistency and function of the prostate, but also had a beneficial effect on the copulative sphere, indices for microscopy of the prostatic secretum, spermatogenesis, and fertility of the ejaculate.

[Changes in lipoprotein metabolism during the rehabilitation of ischemic heart disease patients after aortocoronary bypass at a health resort in the foothills].

284 patients with IHD after coronary artery bypass operation have undergone rehabilitation at low-mountain health resort Kislovodsk. The study of lipoprotein metabolism has detected a positive trend in this metabolism as shown by lowering of blood cholesterol, low density lipoproteins, phospholipids and triglycerides, elevated levels of high density lipoproteins.

[Carbonate mineral baths in the combined treatment at a piedmont health resort of patients operated on for rheumatic heart defects].

The response of the cardiovascular system of 64 patients operated for rheumatic heart disease to mineral carbon dioxide baths was studied in the course of rehabilitation in low-mountain health resort. It was found that the above baths have a training effect on the cardiac muscle, improve myocardial metabolism. They are an essential factor in combined sanatorium treatment of patients after surgical correction of valvular defects. Baths-induced changes in the central and peripheral hemodynamics depend on cardiovascular function. Therefore, they require an individual approach. If the exposition increases to 15 minutes, mineral carbon dioxide baths have more potent training effect on the myocardium and raise effectiveness of the sanatorium treatment.
