The midurethral polypropylene sling for stress incontinence: 22-month results of 81 patients.

OBJECTIVE: The objective was to review a cost-effective procedure called the midurethral polypropylene sling (MPS) in the treatment of urodynamic stress incontinence (USI). STUDY DESIGN: A self-fashioned sling (7.5 x 1 cm) was created from a polypropylene mesh with two lengthening polypropylene sutures at the ends. The sutures are carried through the rectus fascia by a special needle carrier. The sling is placed around the urethra and its sutures are tied on the fascia. The procedure was performed on 81 patients. Of these patients, 73 had USI. The remaining 8 patients with uterine prolapse, who did not have USI, underwent the MPS prophylactically. Of 81 patients, 35 underwent the MPS alone, 46 underwent the MPS and abdominal or vaginal procedures. RESULTS: Three patients had bleeding in excess of 300 ml necessitating a vaginal tamponade. One had hematoma and the other underwent a laparotomy. Urinary retention for more than 3 days was observed in 6 of the patients who had the MPS alone, in 7 of the patients who had both MPS and abdominal procedures, and in 13 of the patients who had MPS with vaginal procedures. The sling was cut in 3 patients due to persistent urinary retention. Of 73 patients with USI, 70 were completely cured (95.9%) at a mean of 22 months' follow-up. CONCLUSION: The cure rates and complications of the MPS were comparable to those of the tension-free vaginal tap (TVT) procedure for the treatment of USI. The procedure costs about US$ 9. We conclude that the MPS can be considered as an alternative to the TVT procedure.

Laparoscopic transabdominal preperitoneal approach to place a polypropylene mesh on the abdominal wall: an experimental swine model of a technique that can be used for incisional hernia repair.

BACKGROUND: The placement of intraabdominal polypropylene mesh entails risks of adhesions and fistulas that can be avoided by preperitoneal placement. METHODS: This comparative, open, experimental, prospective, randomized, and transversal study randomized pigs into two groups of 11 each for intraperitoneal (IPOM) or preperitoneal (TAPP) polypropylene mesh placement by laparoscopy. Diagnostic laparoscopy and tissue en-bloc resection was performed 28 days postoperatively for histopathologic analysis. RESULTS: The following data were observed for the two study groups: surgical time (IPOM: 35.73 +/- 4.22 min; TAPP: 58.09 +/- 6.28 min; p = or < 0.0001); adhesions (IPOM: 81.81%; TAPP: 27.27%; p = 0.032), grade III for IPOM and grade II for TAPP (p = 0.001); and interloop adhesions (IPOM: 81.81%; TAPP: 9.09%; p = 0.003). No fistulas were found in either group. The TAPP procedure showed better integration of mesh, without lesion to abdominal organs. Two complications, occurred with IPOM, and one with TAPP (p = 1.0, not significant). CONCLUSIONS: The perperitoneal technique requires more time, but has fewer adhesions and less intraabdominal inflammatory response. It is a feasible technique that may diminish risks in the laparoscopic treatment of incisional hernias with polypropylene mesh.

Clinical evaluation of the polypropylene-polyester knit used as a cranioplasty material.

UNLABELLED: The paper presents clinical evaluation of the polypropylene-polyester knit used as a cranioplasty material. MATERIAL: Between year 1980 and 2002 275 cranioplastic procedures using the polypropylene-polyester plates Codubix were carried out in the Department of Neurosurgery of the Medical University of Łódź. There were 146 patients who primarily sustained head injuries and 129 non-traumatic patients with craniectomies carried out for various other reasons. In the majority of cases, i.e. in 158 patients, cranioplasty was performed later than 6 months after the primary surgery. The largest implant measured 430 cm2. RESULTS: Excellent and good outcome was achieved in 92% of the patients whereas the rate of local infection was 8%. CONCLUSIONS: Codubix knitted polypropylene-polyester implant proved to be useful and a safe cranioplastic material.

Repair of large midline incisional hernias with polypropylene mesh: comparison of three operative techniques.

Polypropylene mesh is widely used for the reconstruction of incisional hernias that cannot be closed primarily. Several techniques have been advocated to implant the mesh. The objective of this study was to evaluate, retrospectively, early and late results of three different techniques, onlay, inlay, and underlay. The records of 53 consecutive patients with a large midline incisional hernia -- 25 women and 28 men, mean age 60.4 (range 28-94) -- were reviewed. Polypropylene mesh was implanted using the onlay technique in 13 patients, inlay in 23 patients, and underlay in 17 patients. Either the greater omentum or a polyglactin mesh was interponated between the mesh and the viscera. The records of these 53 patients were reviewed with respect to: size and cause of the hernia, pre- and postoperative mortality and morbidity, with special attention to wound complications. Patients were invited to attend the outpatient clinic at least 12 months after implantation of the mesh for physical examination of the abdominal wall. Postoperative complications occurred in 14 (26.4%) patients. The onlay technique had significantly more complications, as compared to both other techniques. Reherniation occurred in 15 (28.3%) patients. The reherniation rate of the inlay technique was significantly higher than after the underlay technique (44% vs 12%, P=0.03) and tended to be higher than the onlay technique (44% vs 23%, P=0.22). Repair of large midline incisional hernias with the use of a polypropylene mesh carries a high risk of complications and has a high reherniation rate. The underlay technique seems to be the better technique.

Polypropylene in the intra-abdominal position: influence of pore size and surface area.

BACKGROUND: Polypropylene is a material widely used in surgery. Because of its association with formation of enterocutaneous fistulae and adhesions, direct contact between mesh and intestine is avoided. The following study was designed to investigate the adhesive potential of different polypropylene meshes when placed in direct contact with intestine. MATERIAL AND METHODS: In an established experimental model, a total of 45 chinchilla rabbits underwent laparoscopic placement of meshes with different pore size (Group I: monofilament PP 0.6 mm, Group II: monofilament PP 2.5 mm, Group III: multifilament PP 4.0 mm) with the Intra-Peritoneal-Onlay-Mesh Technique (IPOM). The degree of adhesion formation was measured after 7, 21, and 90 days, evaluated by an adhesion score, quantified by computer-assisted planimetry, followed by histological and morphometric investigation of the perifilamental granuloma formation. RESULTS: The heavyweight, small porous polypropylene meshes (PP 0.6) showed significantly stronger adhesion formation at all intervals of investigation compared with the lightweight meshes with a pore size >2.5 mm. Between the two different lightweight mesh variations, there was no significant difference. Granuloma formation was lowest in large-pore-size monofilament meshes (PP 2.5). CONCLUSION: The IPOM rabbit model is suitable for investigation of biomaterials in the intra-abdominal position. Our results show that the adhesive potential is significantly influenced by the pore size. However, the extent of the foreign-body reaction seems also to be influenced by the filament structure, respectively, the surface area, favoring monofilament material.

Hernioplasty with bilayer polypropylene mesh: a new tension-free technique.

Different open-mesh techniques have been developed for inguinal hernia repair since the introduction of the tension-free technique. The present study reports a new self-designed tension-free technique for hernioplasty using a bilayer polypropylene mesh. Fifty-one patients with severe transverse fascia weakness were repaired by means of a self-designed, tension-free technique using bilayer polypropylene mesh. The postoperative complications, inpatient hospitalization time, and recurrence rate were studied. Patients were mobilized within 6 h after surgery and no complications were found. The length of hospital stay was 2-3 days. The follow-up period ranged from 3 months to 40 months, with a mean of 20.4 months, and no recurrence was found. Our own experience showed the self-designed, tension-free technique using bilayer polypropylene mesh to be a reliable approach for inguinal hernia repair with many advantages--such as a much lower expense, simplicity, rapid return to unrestricted activities, minor complications, and impressively no recurrence, a particularly superior option for those patients with severe transverse fascia weakness or large defect.

Prevention of parastomal hernia by placement of a polypropylene mesh at the primary operation.

INTRODUCTION: Parastomal hernias occur frequently after placement of a permanent colostomy. Preliminary reports have shown a beneficial effect of placing a mesh at the primary operation to prevent the formation of a parastomal hernia. We studied the safety and prophylactic effect of placing a newly designed polypropylene mesh in an onlay position at the primary operation. METHODS: This was a prospective study that included 25 patients scheduled for elective colorectal surgery. Risk factors for development of parastomal hernia were recorded before surgery. A prepared lasercut polypropylene mesh with six "arms" was placed in an onlay position. Immediate and long-term complications were evaluated by an experienced stoma nurse and a surgeon. Abdominal ultrasound was performed at 6 and 12 months follow-up. Parastomal hernia was defined as both clinical and ultrasonographic signs of protrusion in the vicinity of the stoma. RESULTS: The median follow-up time was 12 (range, 2-26) months. One patient died eight days after surgery. Of the 24 patients included, none had infections or immediate complications after surgery. Two patients had minor complications necessitating a local revision of one of the mesh arms. No other long-term complication was found. Two patients had signs of parastomal hernia at 6 and 12 months follow-up, respectively. CONCLUSIONS: Placement of a polypropylene mesh in an onlay position at the primary operation is a safe procedure and probably results in a low risk of parastomal hernia occurrence.

Enhanced albumin binding to polypropylene beads via anhydrous ammonia gaseous plasma.

Plasma surface modification technique was used to add amino groups onto the surfaces of polypropylene beads by exposing them to anhydrous ammonia plasma. Through these amino groups, albumin was attached to the polypropylene beads. Attached albumin was further stabilized by crosslinking with glutaraldehyde. The effect of washing albuminated polypropylene beads with saline and human plasma was investigated. It was found that after initial rapid removal of albumin, the concentration of attached albumin tended to reach a steady-state. After 52 h of washing, the amount of albumin retained on the beads varied between 125 and 171 micrograms/cm2.

Post bending of the polypropylene flexible stent in mitral Hancock bioprostheses.

The occurrence of stent post deformity was investigated in 38 consecutive, polypropylene flexible-stented Hancock porcine bioprostheses (PB) recovered from the mitral position because of early postoperative death or late tissue failure. The degree of deformity was assessed for each explant by calculating the triangular area obtained by projecting the apex of the stent posts on a plane and comparing it with the same area calculated for unimplanted PBs of comparable size. A significant stent deformity (reduction of the triangular area greater than 40%) was observed only in 6 PBs explanted because of tissue failure after a mean function time of 69.6 +/- 28 months. In these PBs, scanning electron microscopy of the bare surface of the stent at the point of maximal flexion showed cracks 0.7-1 microns large, which represent the morphological substrate of the so-called "creep" of polypropylene. Inward stent post bending may occur after implantation in mitral PBs and when severe, it corresponds to a definite ultrastructural plastic deformity of polypropylene.

Novel application of polypropylene sling: transvaginal cervicoisthmic cerclage in women with high risk of preterm delivery.

STUDY OBJECTIVE: To describe a novel technique of transvaginal cervicoisthmic cerclage with a polypropylene sling in prevention of preterm labor in pregnant women at high risk. DESIGN: Retrospective study (Canadian Task Force classification: III). SETTING: University hospital (department of obstetrics and gynecology). PATIENTS: Twenty-one women showing symptoms of high risk of preterm delivery: histories of pregnancy losses in the second trimester and prior failure of MacDonald's cerclage or absent portio vaginalis of the cervix. The median age of the patients was 32.8 years (range 22-39 years). INTERVENTIONS: Cerclage was performed between 12 and 16 weeks' gestation. A polypropylene tape was placed at the cervicoisthmic junction by vaginal route. Caesarean delivery was systematically performed in all patients because the cerclage was considered to be definitive. MEASUREMENTS AND MAIN RESULTS: No intraoperative complications occurred. The mean operating time was 36 +/- 6 minutes (range 30-45 minutes). The mean length of stay was 1.4 +/- 0.5 days. Mean gestational age and birth weight at delivery were respectively 37.1 +/- 1.8 weeks (CI 95%: 36.4-37.9) and 2850 +/- 745 g (CI 95%: 2531-3168). The preterm birth rate was 19% (4/21). One neonatal death occurred after amniotic fluid infection at 34 weeks. Birth at less than 32 weeks occurred in one patient (4%). CONCLUSION: Transvaginal cervicoisthmic cerclage with polypropylene sling may be considered as an effective and minimally invasive alternative to the transabdominal cervicoisthmic cerclage in women presenting with high risk of preterm delivery.

Gold and gold-palladium coated polypropylene grafts in a S. epidermidis wound infection model.

BACKGROUND: The use of non-absorbable mesh grafts in both abdominal wall defects and inguinal hernias are impossible in the presence of contamination. This study was conducted for evaluation of the efficiencies of polypropylene mesh grafts coated with gold and palladium-gold. MATERIALS AND METHODS: Ten piece of 1 x 2 cm of polypropylene mesh grafts were used in each group of naïve, gold-coated, and palladium-gold-coated. The grafts were incubated in physiological saline buffered and 0.5 McFarland slime positive Staphylococcus epidermidis for 24 h. At intervals of 6, 12, 24, 48, 72 h grafts were washed with saline and vortexed for 2 min in 2 ml of physiological saline. There were 100 microl of samples of vortexed material incubated in blood agar and 24 h later, colony numbers were assessed. In the second part of study, the grafts were implanted below the musculoaponeurotic layer at inguinal region of rats following the same procedure of incubation and washing. On the 8th day, the rats were examined for infection rate and their wound cultures were obtained. RESULTS: The least amount of bacterial growth was detected in the samples obtained from gold-palladium coated grafts; whereas the highest rate of growth was found in samples of naive grafts. The superficial surgical site infection rate was 0% in gold-palladium coated, 30% in gold-coated and 100% in naïve polypropylene group. The bacterial growth rate from wound cultures confirmed the superficial surgical site infection rates in all groups. CONCLUSION: Prosthetic graft infection with S. epidermidis can be prevented by coating the graft with gold-palladium or gold.

The role of TGF-beta1 as a determinant of foreign body reaction to alloplastic materials in rat fibroblast cultures: comparison of different commercially available polypropylene meshes for hernia repair.

BACKGROUND: Animal experiments on hernia repair demonstrated better biocompatibility of light-weight polypropylene meshes. However, implanted medical devices trigger a variety of adverse tissue responses, such as inflammation, fibrosis, infection and thrombosis, but the mechanisms involved in such responses remain largely unknown. This study aimed to determine the effect of transforming growth factor beta1 (TGF-beta1) on host tolerance by quantification of foreign body reaction in cultured fibroblasts depending on the amount and composition of implanted material used for hernia repair. MATERIALS AND METHODS: An NRK-49F fibroblast culture was incubated in the presence of 4 commercially available meshes approved for hernia repair. A mesh-free cell suspension served as a control group, in which the influence of TGF-beta1 on fibroblasts was evaluated. Levels of TGF-beta1 in the supernatant were dynamically measured in a time interval of 6 to 96 h and cell proliferation rates were assessed colorimetrically using MTT test. RESULTS: A dose-dependent suppression of fibroblasts proliferation by TGF-beta1 was observed. All meshes suppressed the secretion of TGF-beta1 and conversely increased significantly cell proliferation in comparison to the control group (p<0.01) in the first 24 to 48 h of incubation. That effect was more pronounced in meshes partially containing absorbable material when compared to samples of pure polypropylene meshes (p<0.05) and to the control group (p<0.001). CONCLUSION: Our experiment revealed that early biological reaction of connective tissue cells towards polypropylene meshes and their variants depended much more on the composition and type of the material than on its absolute amount. The assumption that material weight reduction alone might affect the foreign body reaction of mesh implants could not be confirmed by our in vitro study.

Failed mitral valve repair caused by polypropylene suture.

A 65-year-old man with degenerative mitral valve disease had mitral valve repair consisting of posterior leaflet resection and leaflet repair with polypropylene suture and annuloplasty. Six years later, he presented with recurrent mitral regurgitation. The cut tail of a polypropylene suture used at the initial repair had eroded the free edge of the anterior leaflet and its chordae. Chordal transfer was used to re-repair the valve. Soft, braided suture is preferred for leaflet approximation at mitral valve repair; if polypropylene suture is used, suture tails and knots should be positioned such that they do not come into contact with leaflet tissue.

Analysis of two sling procedures using polypropylene mesh for treatment of stress urinary incontinence.

OBJECTIVES: To evaluate and compare the surgical outcome between the innovative tension-free vaginal tape (TVT) and conventional pubovaginal sling (PVS) procedures using polypropylene mesh. METHODS: Eighty consecutive women with urodynamic stress urinary incontinence (SUI), who chose to undergo either a TVT (n=23) or a PVS (n=57) procedure using polypropylene mesh based on financial consideration, were recruited for this study. The surgical results were analyzed and compared subjectively and objectively. RESULTS: The mean follow-up interval was 23 months for the TVT and 20 months for the PVS procedure (P=0.062). Postoperatively, SUI (91.3% vs. 93.0%), concomitant urge symptoms (85.0% vs. 85.3%) and the negative impact of incontinence and urogenital distress on patients' quality of life (79.8% vs. 77.8%) (77.4% vs. 68.8%) had improved markedly. After a multivariable logistic regression analysis, the treatment outcome of SUI was found to be independent of the main effects of patient age, parity, concurrent gynecological surgeries, intrinsic sphincter deficiency, previous failed incontinence surgeries, and concomitant urge symptoms. However, it was significantly related to treatment procedures (TVT vs. PVS) and their interaction with patient body mass index (BMI). Based on the fitted logistic model, we see that TVT performs better than PVS when BMI is less than 27.27 kg/m2, and the advantage of TVT decreases as BMI increases. CONCLUSION: Both TVT and PVS procedures using polypropylene mesh are effective treatment modalities for female SUI. However, TVT was not as effective in treating overweight or obese women as PVS.

Long-term durability of the distal urethral polypropylene sling procedure for stress urinary incontinence: minimum 5-year followup of surgical outcome and satisfaction determined by patient reported questionnaires.

PURPOSE: We report on the long-term outcomes of the distal urethral polypropylene sling. MATERIALS AND METHODS: We performed a prospective study of all consecutive patients who underwent a distal urethral polypropylene sling procedure between November of 1999 and April of 2000 for treatment of SUI. Surgical outcome was determined by patient self-assessment, and included symptom, bother and quality of life questionnaires. Physicians were blinded to patient responses. All patients had a minimum 5-year followup. Every patient treated was included in reporting outcomes in the intent to treat analysis. RESULTS: There were 69 patients treated a minimum of 5 years before the analysis. Cases lost to followup were defined as treatment failures. At a minimum followup of 5 years patient determined subjective success rate was 88%. More than 5 years after surgery 72% of patients reported no symptoms of SUI and 74% reported never being bothered by SUI. Patients reported an overall mean improvement of symptoms of 81%, and quality of life due to urinary symptoms between pleased and mostly satisfied. CONCLUSIONS: The distal urethral polypropylene sling procedure has low morbidity and excellent durability in treating patients with SUI.

Evaluation of nonwoven polypropylene oil sorbents in marine oil-spill recovery.

Mechanical recovery of oil by oil sorbents is one of the most important countermeasures in marine oil-spill response. Polypropylene is the ideal material for marine oil-spill recovery due to its low density, low water uptake and excellent physical and chemical resistance. Different forms of polypropylene nonwoven sorbents were evaluated in this study in terms of initial oil-sorption capacities and oil-retention properties. The investigation revealed that the fibre diameter, sorbent porosity and oil property are the most important factors in the oil-sorption performance of polypropylene nonwoven sorbents.

Polypropylene mesh closure after emergency laparotomy: morbidity and outcome.

BACKGROUND: Alternative methods for abdominal wall closure may be necessary after emergency laparotomy. The purpose of this study was to determine the morbidity and outcome of emergency fascial closure with polypropylene mesh. METHODS: A retrospective review was performed of all patients undergoing emergency fascial closure with polypropylene mesh from January 1990 to March 1994. RESULTS: Seventy patients were identified. Indications for mesh placement included visceral edema (40), infected/necrotic fascia (21), and planned reexploration (7). Enteric fistulas developed in five patients (7.1%). When omentum was interposed between intestine and mesh, the incidence of fistula was significantly reduced (0 of 51 vs 5 of 19, p < 0.01). Forty-two patients (60%) survived with wound closure, accomplished by skin flaps in 19 (45%), skin grafting in 11 (26%), and secondary healing in 6 (14%). The mesh was removed in six patients (14%). Complications of mesh extrusion and hernia occurred less often after skin flap closure compared with skin grafting or secondary healing (1 of 19 vs 9 of 17, p < 0.01). No mesh infection occurred. CONCLUSIONS: Polypropylene mesh placement is an effective alternative for abdominal closure after emergency laparotomy, even when intraabdominal sepsis is present. Fistulas associated with its use may be effectively eliminated by the interposition of omentum between bowel and mesh. Wound closure with full-thickness skin flaps is the preferred method for soft tissue coverage when mesh is used.

Use of macroporous polypropylene filter to allow identification of bacteria by PCR in human fecal samples.

The detection of pathogenic bacteria directly in human fecal specimens by PCR, requires removal of PCR-inhibitory substances. To investigate whether five different macroporous filters (polypropylene, nylon, polyester, polyethylene, fluorocarbon) could retain polysaccharides, major PCR inhibitors, an in vitro model and human fecal samples were used. The in vitro model consisted of Xanthum gum solutions (3 mg/ml PBS), a bacterial polysaccharide, to which Helicobacter pylori cells were added. Fecal samples from healthy volunteers were spiked with H. pylori and Mycobacterium paratuberculosis cells. Polysaccharide concentrations were significantly reduced only by the polypropylene but not by the other filters. Accordingly, both Xanthum gum solutions and spiked fecal specimens became PCR positive only after filtration with the polypropylene filter. We conclude that this filter can be used to prepare a bacterial DNA template suitable for PCR analysis from human feces.
