Studies on the human leukocyte antigens in patients with polycystic ovary syndrome in a Japanese population--possible susceptibility of HLA-A11 and -DRB1*0403 to patient population with polycystic ovary syndrome.

PROBLEM: The objective of this study was to identify the human leukocyte antigen (HLA) alleles that confer susceptibility or resistance to polycystic ovary syndrome (PCOS) in the Japanese population. METHOD OF STUDY: HLA-A, -B and -C antigens were determined in 56 patients with PCOS using conventional serological method. HLA-DRB1 genotypes were determined using a polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method in 68 patients with PCOS. As a control population, 237 healthy individuals (males and females) were examined concerning HLA-A, -B and -C antigens, and 292 individuals were examined concerning HLA-DRB1 genotypes. The rate of possession of each antigen was compared between the two populations. RESULTS: The rate of possession of the HLA-A11 and HLA-DRB1*0403 in the patients with PCOS was significantly higher compared with that in the control group. The rate of possession of HLA-B39 in the patients with PCOS was significantly lower compared with that in the general population group. CONCLUSION: Human leukocyte antigen systems appear to be linked to PCOS.

Histological examination of ovaries and uteri from cows with cystic ovaries.

Sections were cut from 486 cysts taken from 260 pairs of cystic ovaries which had been collected during an abattoir survey. The linine of the cysts was examined for the presence of granulosa, basement membrane, theca and luteinisation. Sections of the uteri were examined and the nature of the surface epithelium, the thickness of the endometrium and the activity of the uterine glands were recorded. In those cysts associated with a corpus luteum both granulosa and luteinisation were seen less often than in other cysts. Of the 486 cysts examined 111 (22-84 per cent) had luteinisation, the others had none. Luteinisation was most often encountered when granulosa was absent. The thickness of theca was greater in cysts than in normal follicles and was thickest in thick walled cysts associated with a corpus luteum. The majority of uteri (81-9 per cent) were normal and only in 4-78 per cent was "swiss chese" dilation of the glands seen. Thirteen cases of mucometra were seen and these are discussed in detail.

[The clinical and hormonal effects of laparoscopic multiple punch resection (MPR) of the ovary in polycystic ovary syndrome].

Eleven women with polycystic ovary syndrome (PCO) were treated with Laparoscopic Multiple Punch Resection (MPR) of the ovarian follicle and capsule and studied by hormone analysis before and after MPR. In 90.9% of the patients, ovulation appeared to occur within 9 weeks and 63.6% conceived within 26 weeks. No changes in serum LH and FSH levels were seen before and after MPR, but testosterone levels which were in the upper normal range or slightly elevated before treatment were reduced after MPR. Endocrine responses to MPR were similar to those described previously after wedge resection. Laparoscopic MPR is a simple and least invasive method and makes it unnecessary to worry about periovarian adhesion after operation. Furthermore, other causes of infertility can be examined by laparoscopy. MPR appeared to be a promising alternative treatment for patients with PCO.

Ovaries and adrenals in murine Schistosomiasis mansoni. I. Histopathological changes of the ovaries in acute and chronic infection.

Acute and chronic infections with schistosomiasis mansoni in mice were found to cause a reduction of the ovarian weight and atrophy of the corpus luteum cells, followed by lymphocytic and stroma cell infiltration. Finally, the corpora lutea disappeared completely. Acute schistosomiasis caused arrested development of the corpora lutea. Both acute and chronic schistosomiasis led to the formation of "wheel cells" in the interstitial tissue of the ovaries. A threshold level of intensity of disease was found to be necessary for these pathological changes. With less severe schistosomiasis, the morphology of the corpora lutea remained normal. The more intensive and long-lasting the infection, the greater became the atrophy of corpora lutea. The various factors which could have caused these pathological alterations are discussed in the light of available literature, and it is suggested that a pituitary hypofunction, and particularly a lack of luteinizing hormone effect, may play a role in the pathological transformation of the ovarian tissue.

Ovaries and adrenals in murine Schistosomiasis mansoni. II. Some observations on the function of the ovaries in acute infection.

In mice with acute schistosomiasis mansoni the mean serum level of progesterone, as determined by radioimmunoassay, was significantly lower than in non-infected control animals, probably due to the atrophy of corpora lutea and the discontinuance of their development described in an earlier paper. In spite of the histopathological changes in the ovaries and the decreased level of progesterone in the blood, estrus was present in some infected animals.

Gonadoblastoma with dysgerminoma in one ovary and gonadoblastoma with dysgerminoma and yolk sac tumor in the contralateral ovary in a girl with 46XX karyotype.

A case of gonadoblastoma with dysgerminoma in the left ovary and gonadoblastoma with dysgerminoma and yolk sac tumor in the right of a 10-year-old girl is reported. Her karyotype was 46XX, normal female type. A case of a gonadoblastoma without a Y chromosome is very rare. Furthermore, the prognosis for gonadoblastoma accompanied by germ cell tumors other than dysgerminoma was very poor in the past, but the patient remains well 25 months after the first operation.

Malignant stromal tumour of the ovary with virilizing effects in an XXX female with streak ovaries. Clinical and pathological studies.

In gonadal dysgenesis, the presence of the Y sex chromosome appears to have a strong influence on the development of germ cell tumours. The risk of malignancy associated with other sex chromosomal abnormalities is much lower. In the present report, the clinical manifestations--including primary amenorrhoea, tall stature, infertility and poor development of the genitalia and breasts--associated with a predominantly 47, XXX karyotype are described. Malignant change of the streak ovary to an unusual gonadal stromal tumour with lipid-containing cells occurred at a late stage in her life, and this was associated with progressive virilization and production of androgenic and oestrogenic steroids. In view of the risk of malignancy as well as the abnormal endocrine effects, especially in the presence of a Y chromosome, there is a place for the prophylactic removal of these dysgenetic gonads.

[Granulosa-stromal tumor of the ovary: a case of mixed germ cell-cord stromal tumor of the ovary with endocrinological considerations].

Granulosa-stromal tumors comprise 5 to 8% of all primary ovarian neoplasms. The first clinical manifestation is precocious puberty in most prepuberal patients. We report a case of mixed germ cell-cord stromal tumor of ovary in a 7.2 years old girl, who presented with isosexual pseudo-precocious puberty of progressive outcome. Serum testosterone, estradiol and 17alphaOH-progesterone levels were increased. Abdominal-pelvic ultrasound revealed a right ovarian mass. Unilateral salpingo-oophorectomy was performed with complete resection of the tumor. The patient is well 7 years after surgery with normal pubertal and growth development and no signs of tumor relapse. We review the clinical manifestations of ovarian tumors, classification and staging of sex cord-stromal tumors, follow-up, tumor markers, treatment and prognosis.
