Organization and administration of the United States Air Force Dental Service.

The United States Air Force was established as an independent department in 1947 and in 1949 became responsible for its own medical support. In 1951 the dental division was renamed the USAF Dental Service and 432 dental officers were transferred to it from the Army. The dental service now provides a world-wide oral health service from 286 fixed dental centers having a total of over 2200 treatment rooms. The backbone of the service is the general dental practitioner and great stress is laid on the preventive aspects of the service. Eight specialty areas are recognized and in-service training, supplemented by specialist training in civilian institutions provide the trained professional resources required. The facilities for the provision of dental services comprise clinics with multiple treatment rooms for each dentist and adequate space for oral hygienists and support and administrative services. Equipment follows the modern trend towards 4-handed seated practice by the dental team.

Distribution of tritiated cocaine in selected genital and nongenital organs following administration to male mice.

STUDY OBJECTIVE: To determine the distribution of cocaine administered to male mice in selected extragenital and genital organs and to investigate its possible binding to sperm. DESIGN: Twenty-seven sexually mature virus-free albino male mice were used in various experiments whereby following intravenous injection of tritiated cocaine hydrochloride, radioactivity was determined in several extragenital and genital organs, as well as sperm. RESULTS: Radioactivity was detected in all of the organs that were tested, and the highest concentrations per milligram of tissue were found in the kidney and epididymis. Removal of the sperm from the epididymis significantly reduced the radioactivity of the organ. The spermatozoa that were isolated on glass filters showed a linear correlation vs radioactivity (r = .93). CONCLUSIONS: Radioactivity is distributed to several organs, including the genital tract, and is found in association with sperm after in vivo administration of tritiated cocaine. These results may explain the mechanism underlying a male-mediated teratogenesis, which has been observed in animals that were exposed to cocaine, and they raise a possibility that the spermatozoa may carry cocaine into the oocyte during fertilization.

Health services utilization and cost in Ismailia, Egypt.

A pilot study has been undertaken to assess the utilization patterns of health services and to estimate the expenditure on health care in the Governorate of Ismailia, Egypt. A stratified random sample of 89 households (576 individuals), along with 5 pharmacies, 4 primary health care units (PHCUs) and 10 traditional health workers (THWs) has been selected and studied using personal interviews. PHCUs are facilities affiliated with the Governorate Health Directorate and provide free public primary health care services. They include rural health units, rural hospitals and urban health centres. THWs are individuals with informal training and experience in medical care, and officially unrecognized, but provide some primary health care services to the people. They include traditional midwives (TMs) and traditional healers (THs). The estimated average annual per capita expenditure on health care was L.E. 9.2 (Egyptian L.E. approximately or equal to US$ 1.25). The per capita cost of medication (L.E. 3.5) was double the cost of physicians' fees (L.E. 1.77). An annual rate of hospital admission of 24.3 per 1000 population at an average cost of L.E. 10.5 per case admitted was incurred by the sample. There is evidence that the PHCUs have not been utilized efficiently and there was some degree of dissatisfaction with their services. The households relied more on private physicians as their first contact for seeking help for health problems. The majority utilized TMs for care of delivery despite the shortcomings in their training and supervision. Eighty per cent of the mothers in the sample have had no ante-natal care during pregnancy despite the availability of such care in the PHCUs. Possible reasons for these underutilization patterns of the PHCUs have been discussed, and major approaches for increasing the efficiency and utilization of the health services have been proposed. THWs should be officially recognized by health authorities, should be properly trained and supervised, and their services should be fully integrated with the community health care programmes. Also, programmes for proper administration, organization and supervision of the PHCUs and programmes. Also, programmes for proper administration, organization and supervision of the PHCUs and programmes for developing health manpower responsive to community health needs should be planned and implemented. Finally, a flexible system to compensate fairly physicians and staff of the PHCUs and to allow reasonable chance for physicians for private practice must be established.(ABSTRACT TRUNCATED AT 400 WORDS)

Towards a learning organization: the introduction of a client-centered team-based organization in administrative surveying work.

Within administrative surveying work in Sweden, a transition to a client-centered team-based organization was made during 1998. The aim of this study was to describe the employees' perceptions and expectations of job and organizational practices when working as a generalist in a client-centered team-based organization; job and organizational practices and well-being and effectiveness measures were examined when introducing a team-based organization. Interventions such as courses in how to cope with the role of a generalist, how to increase service to clients, education in technology, law and economics, as well as computer information support, were ongoing at the time of the study. The Team Work Profile and QPS Nordic questionnaires were used. All the surveyors in five regions in Sweden participated, in total 640 surveyors. The transition to a client-centered team-based organization was expected to improve job control and job content but at the same time lead to impairments in job climate and group cohesion. Distress was associated with negative future expectations of the organization. High job control and group cohesion were the central contributors towards growth in personal competence and social effectiveness of teamwork. Both internal and external client-related activities of team and organization were in focus during the transition.
