[The use of titanium mesh plates for graft fixation in mandibular osteoplasty].

Opportunities of the use of mesh titanium plates as fixing devices are considered at reconstruction operations. Data of following up 8 patients with defects of the mandible of various location and extent are analysed. They were operated with reconstruction of the mandible with iliac graft and bone graft fixing with the mesh titanium plates. A technique of the operations is described, a case of successful use of mesh titanium implant modelled by the form of the mandible in repair of the mandible defect. The analysis of long-term results shows the potential efficiency of the use of mesh titanium plates as fixing adaptations in bone plastic operations.

Mandibular reconstruction with fibula osteoseptocutaneous free flap and osseointegrated dental implants.

The fibula osteoseptocutaneous free flap is ideal for reconstruction of composite mandible defects. Osseointegration is possible and advantageous. Primary osseointegration of dental implants is a safe and reliable procedure in selected groups of patients, promoting early total oral rehabilitation with restoration of both function and cosmesis. Further studies are necessary to assess the specific indications for osseointegration teeth in patients with malignant tumors and in those patients who have received radiation therapy.

Vascularized free bone grafts for maxillary and mandibular reconstruction.

Vascularized osteocutaneous free grafts have been successfully applied to maxillofacial reconstruction. Careful planning allows for implant prosthesis fabrication in these cases. The scapula, fibula, and ilium are the donor sites that result in the most favorable application of implant prosthetics. Intraoral soft tissues, although compromised, are manageable with meticulous home oral hygiene practices and frequent recall examinations with professional cleaning. Many patients severely impaired by trauma, neoplasm, or congenital deformity can have restoration of form and function with the amalgamation of microsurgical and osteointegration techniques.

[Reconstruction of mandibular defect by using Titanium mandibular prosthesis: a preliminary experimental study].

PURPOSE: The aim of this experiment was to investigate the possibility of application of mandibular reconstruction with titanium mandibular prosthesis. METHODS: Two healthy adult goats were used in this study. Spiral CT of the mandibular region was performed. The data collected was related to the computer. Then a 5 cm mandibular body defect and a mandibular angle defect were designed accordingly. Titanium mandibular prostheses were manufactured according to the size of the above defect. Surgical operation was performed and the prosthesis were implanted into the aimed defects and internal fixation was used. Autogenic bone graft was introduced into the holes of the angle prosthesis. Radiographic examination was used to evaluate the healing of prostheses every month after operation. Three months later, the animal was sacrificed and the mandible specimens were sectioned for histological and biomechanical evaluation. RESULTS: One of the body prosthesis was found to be exposed two weeks after operation. Four weeks later, fixation failed and the anterior part of the prosthesis separated from the mandible attached. The other body prosthesis also suffered from exposure, but the connection between the prosthesis and the mandible was still stable. Histological examination revealed that there was presence of osseointegration in the angle prosthesis as well as area that was not exposed in the body prosthesis model. Reattachment of the muscles to the bone graft area was found. CONCLUSIONS: CAD/CAM generated Titanium mandibular prosthesis is applicable in human model. The vitality of this experiment revealed that a tight closure plays an important role in the healing by osseointegration. If there was any exposure, the osseointegration will be failed. Reattachment of the muscle to the prosthesis can be achieved by "muscle-bone-prosthesis"model.

[Treatment of mandibular fractures in patients at the Department of Oral and Maxillofacial Surgery, University Hospital in Lublin in the years 1988-1997].

Between 1988-1997 in the Department of Dental and Maxillo-facial Surgery of Medical University in Lublin there were applied three types of fixation of mandible bone fracture. Depending on the kind of fracture and teeth condition with 692 (i.e. 60%) patients there was applied preservative treatment and 530 undergone surgery (i.e. 18.1%) and undergone surgical and orthopedic treatment (i.e. 21.9%). Upon analysis of treated population it was concluded, that during period of assessment there occurred significant change of implications for the application of fixation of mandible bone fracture and the leading method became mandible osteosynthesis with intraoral approach.

[Individual digital design and functional reconstruction of large mandibular defect with computer-aided design/computer aided manufacture technique].

OBJECTIVE: To build up a new contour and functional reconstruction technique of mandibular defects with rapid prototyping and reverse engineering technique. METHODS: From April 2002 to August 2004, 4 cases of mandibular defects due to resection of large mandible lesion were treated. Of 4 patients, there were 3 females and 1 male, with an age range of 21-42 years, which underwent secondary operation and presented a deviation as mandibular movement. The opening-mouth extent was 1.8-2.5 cm (2.2 cm on average). The data of defects area were renewed with Mimics and Geomagic Studio software; and the titanium reconstructive frame was designed and manufactured with rapid prototyping technique. Defect were reconstructed by using CT digital data of patients. RESULTS: The CT data could be used by image software directly. The implant design could be completed by computer-aimed design (CAD) / computer-aided manufacture (CAM). The resin model and titanium frame were manufactured accurately by RP technique. Four patients achieved one stage healing. After a follow-up of 3 months to 2 years, large mandibular defect was reconstructed satisfactorily and the opening-mouth extent was 3.0-3.4 cm (3.2 cm on average). The occluding relation was normal. The implant denture was put on and the mastication function was good in 1 case. CONCLUSION: Individual design and repair of large mandibular defect with CAD/CAM techniques is worth extending application clinically. It is a simple and accurate method.

[Cancellous mandibular autografts and titanium mesh].

Pure cancellous bone transplants constitute an alternative to the usual cortico-cancellous grafts used in certain cases for restoring interrupting substance loss (ISL) in the mandible. The greater number of surviving transplant cells, the rapid revascularization and more complete osteogenesis that are achieved cause indications for this type of graft to be particularly well adapted for cases where local repair conditions are precarious, such as bullet wound related sequelae. Indications for their utilization also extend to patient who have had benign tumors excised. The titanium-made receiving splint designed by Pr. Dumbach is particularly well adapted to this type of restoration; it provides for necessary functional stresses to be translated to the implant, allowing for the latter's incorporation, while controlling the direction of osteogenesis, thereby yielding excellent morphological and functional results.

[Reconstruction of extensive mandibular defects with the use of Osteo-Mesh and a bone graft. A preliminary report].

In the reconstruction of extensive bone defects of the facial skeleton, bone grafts are taken for granted even though the handling and stress such grafts are subjected to do not always guarantee the functional and aesthetic result. The use of a special kind of prosthesis (osteo-mesh) consisting of the synthesis of two biocompatible substances (polyurethane and dacron) with bone graft, preferably iliac spongiosa, is suggested by the preliminary experience of the Authors, as a good solution for the achievement of a satisfactory result.

[Experience with titanium mesh as a temporary foreign material implant for the reconstruction of mandibular defects].

Restoration of a loadable mandible after extensive resections, defects or comminuted fractures, serious osteomyelitis and pseudarthrosis will be successful only if sufficiently stable fixation of the fragments and immobilization beyond the physiological period of bone healing are guaranteed. By means of various types of plates and extroral appliances for osteosynthesis the conventional methods have been improved, but failures and complications are still frequent. The treatment itself is a great strain for the patient. The titanium mesh implant system developed by Ph. Boyne and his group for reconstruction in mandibular defects means, in our opinion, an important therapeutical step forward. Its technical advantages and our own clinical experience advocate application of this system.
