Studies on nodules and adult Onchocerca volvulus during a nodulectomy trial in hyperendemic villages in Liberia and Upper Volta. I. Palpable and impalpable onchocercomata.

In hyperendemic onchocerciasis areas of West Africa nodulectomy trials were performed. 163 nodule carriers from four villages in the Liberian rain-forest and 52 nodule carriers from a savanna village in Upper Volta were operated on. During the surveys in the villages in Liberia and Upper Volta, an average of three and five nodules respectively per patient were palpated. Before each operation a further palpation was done and two more nodules per patient from both countries were found. The nodulectomies revealed 75% more nodules in the Liberian patients and 53% more in the patients from Upper Volta than were palpated immediately before. Half of these additional subcutaneous nodules extirpated in Liberians were superficially located, the other half deeper and impalpable through the skin. In the patients from Upper Volta two thirds of the additional nodules were seated in the depth of the subcutis. In both countries an average of three deep subcutaneous nodules per patient were found and this was equivalent to 20% of all extirpated onchocercomata. Most of these deep subcutaneous nodules were at those sites where most of the superficial nodules were located.

Studies on Bancroftian filariasis in Liberia, West Africa. III. Efficacy of repeated treatment with diethylcarbamazine and vector control on the microfilarial reservoir in a rural population.

The population of four villages in the savanna area in Upper Lofa in the hinterland of Liberia was treated with diethylcarbamazine and the decline of the density of microfilariae of Wuchereria bancrofti was studied. It became obvious that even a repeated treatment with the drug did not result in a complete eradication of the parasites, but that about one third of microfilaria carriers who had received two courses of treatment remained infected, showing ultra low densities which could only be-discovered by employing the sensitive membrane filtration technique. Vector control by means of residual spraying with DDT carried out every six months for 2 1/2 years in two villages did not result in a decrease of the microfilaria carrier rates. However, the microfilarial density of the infected persons declined distinctly during the three years of control. Experiences made with the membrane filtration technique under field conditions are described.

An endemic disease survey of Bong County, Liberia.

An endemic disease survey was carried out in Bong County, Liberia by a health survey team of the American Public Health Association in 1977. The results of the survey and recommendations for control and/or elimination of the various diseases are presented.

Ocular onchocerciasis in a hyperendemic village in the rain forest of Liberia.

A sample of 76 persons in a hyperendemic village of onchocerciasis in the rain forest of Liberia was examined ophthalmologically. A reduced visual acuity of less than 6/18 was found in nearly 10%. Punctate keratitis was observed in 36%, microfilariae in the anterior chamber in 46%, proliferative chorioretinitis in 4% and optic atrophy in one person. The overall frequency of ocular onchocerciasis was 62%, but the severity of findings in most individuals was very minimal. The occurrence of microfilariae in the anterior chamber was positively correlated with microfilarial densities in the skin of the hip, outer canthus and blood and the burden of adult O. volvulus isolated from extirpated onchocercomata.

Distribution and prevalence of Wuchereria bancrofti in various parts of Liberia.

A nightblood survey has been carried out in 82 localities situated in various parts of Liberia. A total of 8072 people have been examined for the presence of microfilariae of Wuchereria bancrofti and 403 (5.0%) have been found positive. However, the distribution of the disease was quite uneven. The infection was common in the coastal zone showing an average microfilaria prevalence of 9%, while in the forest belt among 5383 people living in rural areas only 109 (2.0%) were found to harbour microfilariae. In the savannah area out of 2198 people examined 209 (11.4%) persons were carriers of microvilariae.

Cataract surgery in a leprosy population in Liberia.

In Liberia, 43 eyes of 30 patients with ocular leprosy underwent cataract extraction; 33 eyes had extracapsular cataract extraction (ECCE) and 10 eyes had intracapsular cataract extraction (ICCE). ICCE was performed in eyes with poor visualization of the anterior chamber. In 95% of the eyes, the postoperative vision improved by 2 Snellen lines or more, but functional visual acuity (better than 20/200) was achieved in only 65% (82% post-ECCE and 10% post-ICCE). Fewer postoperative complications were observed after ECCE. These findings may have been related to less ocular involvement by leprosy preoperatively. ECCE should be attempted when the visualization of the anterior chamber is fair.

[A survey of sexual psychology and attitude of Chinese peace-keepers in Liberia].

OBJECTIVE: To investigate the sexual psychology and sexual attitude of the Chinese peace-keepers in Liberia, attempting a correct guidance and psychological intervention in this aspect. METHODS: In the middle of the mission tour, a survey was conducted among 314 Chinese male peace-keeping soldiers with a self-designed questionnaire. RESULTS: More than 90% of the soldiers learned some sexual knowledge by themselves; quite a proportion of them were relatively deficient in sexual knowledge and lacked sexual education; 77% of them were afraid of AIDS even after sexual education. Married soldiers showed more maturity in sexual psychology than CONCLUSION: Appropriate psychological intervention the unmarried(P <0.05) , but with no differences in sexual attitude( P >0. 05). and guidance directed at the particulate population of peace-keepers may help them adopt a rational attitude towards their sexual drive and desire and convert, their sexual energy into further impetus to better creative performance in the peace-keeping mission.
