Eleven children with hemiplegic or an asymmetric diplegic cerebral palsy who had a preoperative leg length discrepancy of more than 2.5 cm underwent gait improvement surgery which included leg length equalisation. Sagittal plane kinematics and kinetics before and about 3 years after surgery for the lengthened limb and contralateral limb were evaluated. Preoperatively the unaffected limb had excessive stance phase flexion at the hip and knee, and dorsal flexion at the ankle joint. These changes could be partially reversed to produce a kinematic gait pattern comparable to age matched normal controls on the uninvolved side after equalisation of leg length.
Current concepts in limb lengthening in short stature are discussed together with management protocol and a review of 32 patients undergoing surgery using the three surgical techniques of lengthening. Limb lengthening in achondroplasia requires careful planning. Decisions must be reached regarding the suitability of the patient, timing of surgery, and the amount of lengthening.
Bilateral simultaneous elongation of the lower limbs was performed in 3 achondroplastic children. A total of 10 bones were elongated with Wagner's distraction apparatus, mainly by a modified Wagner technique. Maximal elongation of 34 to 44% of the original bone length resulted in relatively minor complications. Elongation of the lower limbs in achondroplastic children improves trunk-to-lower-limb proportions.

In part 2 of the article the authors evaluate a group of 45 completed cases of lengthening of long bones, using the method of consecutive distraction according to Ilizarev. On average a lengthening of 5.4 cm was achieved. The authors evaluate the procedure of lengthening, the period taken by therapy in relation to the patients' age. The mean period of treatment was 5.7 weeks per 1 cm lengthening and it depends distinctly on the patient's age. In a total of 16 patients at the same time also correction of the axis of the leg was made. A major part of the investigation is devoted to evaluation of complications which, consistent with the literature, are classified as major, requiring further surgery, and minor ones which can be resolved without surgery during the lengthening procedure. Major complications were recorded 9 times, minor ones 8 times. Only in one instance the lengthening was terminated before the planed distraction was achieved. The authors evaluate these complications in relation to the length of distraction, the patient's age and the time when the lengthening was performed. At the same time they give instructions how to prevent complications and how to deal with them, if they arise. The conclusions and evaluation of the group of patients are dealt with in a detailed discussion where the authors compare their results with extensive references in the literature.

The effect of lengthening on muscle metabolism was measured and correlated to the percent lengthening at early and late time points. Using the rabbit tibial lengthening model, the authors examined the effects of lengthening on the tibialis anterior muscle using phosphorus-31 magnetic resonance spectroscopy. Thirty-six rabbits were divided into five groups, four groups by percent lengthening (0%, 15%, 20%, and 25%), with each group divided into subgroups of early (end distraction) and late (12 weeks after end distraction), and the fifth group using the opposite untreated leg as control. Several parameters measuring metabolism of muscle using phosphorus-31 magnetic resonance spectroscopy analysis were compared. No changes occurred to 15% lengthening, but significant decreases were measured at 20% and 25% lengthening. After a 25% lengthening, the decreased metabolism persisted at 12 weeks after distraction, indicating the possibility of permanent damage. After 20% lengthening, the same parameters improved but never to normal levels. The authors conclude that lengthening to 15% is safe for muscle, but 20% to 25% lengthening may result in permanent metabolic damage. The current study also suggests that phosphorus-31 magnetic resonance spectroscopy may provide a viable clinical method for evaluating muscle damage during lengthening.
We reviewed our experience with a modified triple innominate osteotomy for hip instability and limb shortening due to poliomyelitis in 62 adolescent and adult patients, treated from 1973 to 1990. Their ages at surgery ranged from 12 years to 35 years (average 22.3). At a mean follow-up of 4 years (2 to 18) 59 of the patients (95.2%) had substantial improvement in hip stability, and all but one had radiological improvement as determined by the acetabular angle, centre-edge angle and acetabulum-head quotient. In 59 cases in which transiliac limb lengthening was attempted, the mean gain was 1.7 cm (0.6 to 3.0). When the abductor muscles had been partially paralysed, the operation produced an appreciable increase in power in 12 of the 39 hips examined.
