Antibody-dependent cell-mediated cytotoxicity of mononuclear cells against Langerhans islets of Wistar rats in normal man and in patients at diabetes risk.

Antibody-dependent cell-mediated cytotoxicity against pancreatic islets was investigated in 13 newly diagnosed insulin-dependent diabetics, in 38 patients at high risk for the disease and in 20 age-matched healthy controls. For this purpose 51Cr-labeled neonatal rat pancreatic islets incubated with the specific anti-rat islet cell antiserum 339 or with serum of the lymphocyte donors were used as targets. The antibody-mediated cytotoxic activity of mononuclear cells was evaluated from the specific chromium release after 6 h exposure of pretreated islets to the effector cells. The specific cytotoxic effect of mononuclear cells from healthy controls on pancreatic islets pretreated with serum is weak. ADCC mediated by the specific antirat islet cell antiserum is significantly increased in 54% of newly diagnosed diabetics as well as in 32% of patients at high risk for insulin-dependent diabetes mellitus. 46% of the newly diagnosed diabetics were also ADCC-positive when their own serum was used for the pretreatment of islets, regardless of whether they were islet cell antibody- or islet cell surface antibody positive or not. Subsets of mononuclear cells (non E-rosette-forming cells, high affinity E-rosette-forming cells, low affinity E-rosette-forming cells) were prepared and their cytotoxic potential was analysed in patients with positive ADCC test results.(ABSTRACT TRUNCATED AT 250 WORDS)

Glucagon antibodies alter Langerhans' islets.

The passive transfer of rabbit anti-glucagon antibodies into mice induces periinsular and intrainsular inflammation and hyperplasia of alpha cells. Active immunization of rabbits with heterologous glucagon induces alpha cell hyperplasia.

Ultrastructure of B-cells of Langerhans islets in rats after the administration of L-leucine.

The results of studies with electron microscope supported the earlier biochemical findings on the stimulation of insulin secretion with the aid of L-leucine. The changes of the cell organelles of B-cells showed that both the synthesis and the release of insulin were activated, while the secretion by specific granules was unchanged. The stimulation of B-cells was accompanied by numerous membrane-like structures ("myelin-like figures") which were presumably formed from the endoplasmic reticulum.

Islet amyloid polypeptide (IAPP) and pro-IAPP immunoreactivity in human islets of Langerhans.

Islet amyloid polypeptide (IAPP) is a 37-amino-acid putative hormone which is expressed by islet B-cells and most probably is co-released with insulin. IAPP is synthesized as an 89-amino-acid prepropeptide in which IAPP is flanked by two short peptides. The two short peptides are ultimately cleaved off at basic residues. In the present study, we used antisera to three different synthetic peptides corresponding to positions 18-30, 40-50 and 53-62 of prepro-IAPP. The two latter peptides fall within the mature IAPP molecule while the first peptide corresponds to the N-terminal flanking peptide. We demonstrate that normal B-cells and islet amyloid both react immunohistochemically with all of these antisera. Using the immunogold labelling technique, we also demonstrate electron microscopically that both the IAPP immunoreactivity and the pro1-IAPP immunoreactivity in amyloid deposits are confined to the amyloid fibrils per se. These data indicate that not only mature IAPP but also the N-terminal flanking peptide is present in islet amyloid deposits. It remains to be shown if the propeptide segments are involved in the pathogenesis of these amyloid depositions.

[Functional characteristics of the Langerhans islets in type I gastric heterotopic pancreas].

In a retrospective five years study of gastric wall tumors, we selected the type 1 heterotopic pancreas. The assessment of fibrosis and inflammatory infiltrate were performed. Also the immunohistochemical evaluation with monoclonal antibodies for insulin, glucagon, p substance, somatostatin, gastrin and antichimotrypsin. None of the case showed remarkable fibrosis or inflammatory infiltrate. All the Langerhans's islets stained positive for 2 or 3 hormones, gastrin and antichimotrypsin were always negative. These findings are comparable with others previously reported, but no cases of evaluation of islet's gastrin exist in the literature reviewed.

[A modified method of isolation of Langerhans islets from the rat pancreas].

A simple procedure is developed for isolation of Langerhans islands (LI) from rat pancreas. Homogenization of pancreatic tissue was carried out using two "Record" syringes equipped with needles of various diameter. Collagenase was not used in the procedure. After homogenization LI were individually separated using stereomicroscope. The insulin-producing capacity of LI was estimated by assay of immunoreactive insulin content in LI and in the incubation media at 0 degrees and 37 degrees as well as by their reaction to various concentrations of glucose. The procedure enabled to isolate functionally active LI and to study biosynthesis and secretion of insulin in vitro.

[Focus on Islets of Langerhans transplantation].

INDICATIONS: There are two types of indications for Langerhans islets transplantation, patients with type 1 diabetes mellitus who have a functional kidney graft but who cannot be candidates for total pancreas transplantation, and soon, patients without renal failure who develop particularly uncontrollable diabetes mellitus. SEVERAL BASIC CONDITIONS: Several conditions are required for the success of Langerhans islet transplantation: a laboratory with demonstrated skill is isolating islets, a sufficiently large number of islets for injection (600,000 for a 60-kg patient), fresh islets harvested within less than 24 hours before transplantation, effective immunosuppressive treatment with no toxic effect on Langerhans islets. The injection is a simple procedure performed under local anesthesia via transhepatic catheterization of the portal vein. RESULTS: Through 2000, 10% to 20% of implanted grafts were functional at 1 year. Islets survival has exceeded 80% at 1 year. The gold standard for success is withdrawal of insulin therapy and normal glucose control. Islets grafts will undoubtedly become the next important step in the treatment of type 1 diabetes mellitus, but other techniques are also envisaged for the future.
