[Influential factors in mortality rate from congenital heart disease. Study of 1,216 children in the Autonomous Community of Murcia (1978-1990)].

INTRODUCTION: In the last few years, important progress has taken place in management of congenital heart disease. These changes have had an influence on diagnosis, preoperative management, surgery treatment and postoperative care, giving rise to better results in the treatment of children suffering from congenital heart disease. AIM: To assess the results of congenital heart diseases in a reference hospital by comparing two periods with reference to both diagnosis and therapeutical management. We also intend to investigate the influence that factors such as the existence of extracardiac congenital malformations and heart surgery have on mortality. PATIENTS AND METHODS: Our sample group was made up of 1,216 children suffering from congenital heart disease. Their ages ranged from 1 day to 7 years old. These children were born over a period of thirteen years and studied at the paediatric cardiology unit in a reference hospital in the Autonomous Community of Murcia, a region of Spain. We retrospectively analysed their development by individual heart diseases (and their associated factors), and the global results. Our research was divided into two periods: between 1978 and 1983, and between 1984 and 1990. Differences were found regarding diagnosis and treatment. RESULTS: a) Mortality rate from congenital heart disease decreased in the period between 1984 and 1990 in comparison to the period between 1978 and 1983, from 28 to 21,7% (p < 0.05); b) individually, the mortality rate decreased with statistical significance in two diseases: interventricular communication and patent ductus arteriosus, and c) there is a higher mortality rate of patients with no surgery treatment and/or extracardiac malformations. CONCLUSION: Progress in the management of congenital heart disease has led to a more favourable outcome in the last years.

[Clinicopathological study of influential factors on prognosis in patients with renal cell carcinoma].

Between January 1986 and August 1996, 120 patients were diagnosed with renal cell carcinoma (RCC) at Kanagawa Cancer Center. In this study, we analyzed these patients in view of the clinicopathological factors, which affect the prognosis of these patients with univariate and multivariate analysis using Cox's proportional hazard model. Of these 120 patients, 40 patients (33%) died of RCC during this period. The cause specific survival rate was 65% after 5 years and 57% after 10 years. Univariate analysis showed some factors, performance status, symptoms, tumor size, histological grade, local invasion (pT), venous involvement (pV), lung metastasis, lymph-node metastasis, and bone metastasis, affected the prognosis. Furthermore, multivariate analysis using Cox's proportional hazard model showed that performance status, pT, pV, lung metastasis, lymphnode metastasis, and bone metastasis independently affected the prognosis. In summary, TMN factors and performance status importantly affect the prognosis of RCC patients.

[Influential factors in the elevation of serum creatine phosphokinase for the patients with Kanemi Yusho--freeze fracture study of rat muscle plasma membrane].

We studied the factors of the elevation of serum creatine phosphokinase (CK) using the data from the routine medical checkup of Kanemi Yusho patients during 1995 and 1999. We also studied rat muscle plasma membrane by the freeze fracture method, which were given the polychlorinated biphenyls (PCB) and controls. The patients with elevation of serum CK showed significant elevation of PCB in their blood but not in polychlorinated quanterphenyls (PCQ). The rat muscle plasma membrane showed a slight increase of orthognal array density but it was not statistically significant. The densities of caveolae and particles were not changed. Accordingly, PCB were thought to be a factor in the elevation of CK in the serum.

[Marius Tausk (1902-1990), influential endocrinologist and producer of medicines; a retrospect to mark the centenary of his birth].

Marius Tausk, born in Sarajevo, studied medicine at Graz in Austria. In 1926, he attended a socialist youth rally in Amsterdam. Whilst there, he met Prof. Ernst Laqueur, a pharmacologist in Amsterdam, who offered him a position at Organon, a newly-founded pharmaceutical company in Oss, the Netherlands. He remained in the Netherlands and became the driving force behind this innovative company. Tausk made many contributions to new developments across the field of endocrinology, including the discovery of the adrenal steroids (together with T. Reichstein, Nobel Prize Laureate in 1950), and the development of oral contraceptive pills. He was astute enough to quickly patent the first corticosteroids. He could quickly extract the essential elements from an information source and disseminate this in five languages. His sharp judgment brought him friends and admirers, yet also those who feared him. In 1937 he was appointed as a private lecturer at the Medical Faculty of Utrecht University and in 1956 he was made Special Professor of Theoretical Endocrinology. He was awarded a number of scientific distinctions including two honorary doctorates. In 1949 he was honoured with the Knighthood of the Order of The Netherlands Lion.

[The influential factors on DNA cycle sequencing and data evaluation of practical example].

OBJECTIVE: To study some factors that may influence DNA sequencing by analyzing 2040 sequencing samples retrospectively. METHODS: The effects of DNA templates, primers, cycle sequencing reactions as well as purification methods were comparatively analyzed. RESULTS: 70 percent of sequencing failure is caused by DNA template. Characteristics of data from poor template preparation include top-heavy with a short read length, acceptable signal with some noise peaks (Ns), weak signal, overall noise and flat lines or no signal. The ineffective primers can cause 15.4 percent sequencing failure. Possible explanations include primer mismatches and low Tm value. CONCLUSIONS: The quality of template DNA can have a major impact on the quality of the DNA sequencing. Purification of the extension products is absolutely necessary in DNA sequencing, and isopropanol precipitation is highly recommended for this procedure.

[Research on mental health status of 726 adolescents and its influential factors].

Mental health status of 726 adolescents and their backgrounds were investigated with Symptom Check List(SCL-90) and self-designed questionnaire. The results showed that the prevalence rate of psychological problems ranged from 6.34% to 24.93%. Among these problems, obsession led all the others. Females had higher scores in interpersonal sensitivity, depression, anxiety, phobia than males. Logistic regression analysis indicated that the major factors contributed to mental health status of adolescents were the relationship between teacher and student, father's concern with his child, only child or not, parents' expectations, personality and sex. The study provides reference source for improving the mental health status of adolescents.

[The influential factors in using cadmium reduction method for measurement of the stable products of NO--nitrates and nitrites].

To establish an efficient assay method for detecting nitric oxide indirectly, we compared the effects of cadmium filing, 5 mmol/L and 80 mmol/L copperized cadmium filing on the measurement of nitrates and nitrites (the stable products of NO) using the cadmium reduction method. The results demonstrated that cadmium filing was the most efficient cadmium preparations (P < 0.001). It's mean reduction rate was 96%, and it showed stronger reduction effectiveness in deproteinization cell culture media. The results indicated that cadmium filing has strong anti-interference capacity in biological fluid. We recommend the cadmium filing reduction method because it is simple, practical, inexpensive, highly efficient and can be performed in ordinary laboratories.

Paving the way: training the influentials in population education.

A number of countries in Far East Asia are in the process of launching national population education programs. To get them off the ground and to keep them working needs the support of key personnel in the program. This means that basic training in population education should be given to key personnel to enable them to promote and efficiently run the population education programs. The Sub-Regional Group Training Course in Population Education organized by the Mobile Team of the Unesco Regional Office for Education in Asia and Oceania to be held in Bangkok from 18 August to 5 September is a response to this need. Training will be given to key personnel from Afghanistan, India, Nepal, and Pakistan. The specific objectives of the course are to provide the participants with: 1) opportunities for exchanges of information and experience; exposure to and involvement in developing population education curricula, teaching units and sample instructional materials; 2) an understanding of various strategies for curriculum development and methodologies used for designing and implementing programs; and 3) preparation as teams from each Member State able to contribute to the promotion of population education in school and out of school in their countries. The training course has 5 main thrusts, namely: 1) sharing of experiences and clarification of the concepts of population education; 2) development of population education curriculum; 3) development of teaching units; 4) development of sample alternative instructional materials; and 5) field visit to ongoing population education programs in Thailand. The content will cover: 1) the knowledge base which includes concepts, nature, issues and status of population education; demographic concepts, population and development; and 2) strategies and approaches such as those used in curriculum development and teaching units, methods of teaching, and instructional materials development. The training methodologies will include lecture and panel discussions, film shows, and practicum.
