The fundoscopic features for differentiation between polypoidal choroidal vasculopathy and choroidal neovascularization from age-related macular degeneration.

OBJECTIVES: To evaluate the fundus features that can differentiate polypoidal choroidal vasculopathy (PCV) from choroidal neovascularization (CNV) due to age-related macular degeneration (AMD). DESIGN: Retrospective, case-control study. MATERIAL AND METHOD: The fundoscopic features of AMD-related CNV and PCV were compared for evaluation of relative risks for PCV. The odds ratio, sensitivity and specificity of each feature were also calculated. RESULTS: A total of 30 patients, 15 PCV and 15 age-matched non-PCV cases, approved by indocyanine green angiography (ICG), were enrolled. The odds ratio is highest for subretinal polyp-like structure and lowest in scar formation (28 and 0, respectively) and these ratios are statistically significant differences (p = 0.001 and 0.032 respectively). The ratio for either circinate exudates, or larger than 4-disc area of subretinal fluid, or retinal pigment epithelial detachment is 1.75. The ratio for peripapillary location is 1.63. Subretinal membrane has a ratio of 1 while subretinal hemorrhage and subretinal fibrin have less than 1; they are 0.31 and 0.23 respectively. All these ratios are not statistically significant differences (p > 0.05). CONCLUSION: The orange polyp-like structures beneath retina are hallmarks for diagnosis of PCV. The "serosanguineous retinopathy", described as serous retinal deatchment or retinal pigment epithelial detachment with circinate exudates with or without subretinal hemorrhage at the posterior pole or peripapillary region, may also suggest the condition.

Malignant melanoma of the choroid presenting as choroidal detachment following cataract surgery.

The progression of a monocular nuclear cataract was observed for thirty-two months in an eye which was apparently otherwise normal. Three weeks after uncomplicated cataract surgery the patient presented with an apparent detachment of the choroid which enlarged dramatically within a relatively short time and which was further complicated by a retinal detachment. The subsequent development of rubeosis iridis and increased intraocular pressure suggested some other etiology. Ultrasonography revealed a malignant melanoma which was confirmed following enucleation. Transillumination of all suspected postoperative choroidal detachments is recommended with more definitive examination when indicated.

The proteomic analysis of mouse choroid plexus secretome reveals a high protein secretion capacity of choroidal epithelial cells.

Choroid plexuses (CP) are involved in multiple functions related to their unique architecture and localization at the interface between the blood and cerebrospinal fluid compartments. These include the release by choroidal epithelial cells (CEC) of biologically active molecules, such as polypeptides, which are distributed globally to the brain. Here, we have used a proteomic approach to get an unbiased overview of the proteins that are secreted by primary cultures enriched in epithelial cells from mice CP. We identified a total of 43 proteins secreted through the classical vesicular pathway in CEC -conditioned medium. They include transport proteins, collagen subunits and other cell matrix proteins, proteases, protease inhibitors and neurotrophic factors. Treating CEC cultures with lipopolysaccharide, increased the secretion of four protein species and induced the release of two additional proteins. Our study also reveals a higher protein secretion capacity of CECs compared with other CP cells or cultured astrocytes. In conclusion, this study provides for the first time the characterization of the major proteins that are secreted by CECs. These proteins may play a critical role in neuronal growth, differentiation and function as well as in brain pathologies.

Lectin binding sites in the choroid plexus and choroid plexus papillomas.

The surface of the cells of the normal choroid plexus and of the plexus papillomas is coated by sialomucopolysaccharides, containing substances which are positively stained with the colloidal iron (Hale-) reaction. After pretreatment with neuraminidase sialic acid is removed rendering the membrane negative to the Hale reaction. Using FITC- or rhodamine-labelled PNA (Arachis hypogaea) and RCA (Rhicinus communis) lectins specific receptors are demonstrable. The identity of these distinct oligosaccharides containing receptors with the Thomsen-Friedenreich antigens suggests the possibility of an immunologic significance, e. g. in bacterial or viral infections of the brain. The application of histochemical techniques seem suitable to clarify the differential-diagnosis between choroid plexus papillomas on the one hand and metastasis of carcinoma and papillary ependymomas on the other hand.

Intracellular plasma proteins in human fetal choroid plexus during development. I. Developmental stages in relation to the number of epithelial cells which contain albumin in telencephalic, diencephalic and myelencephalic choroid plexus.

The developmental stages of telencephalic, diencephalic and myelencephalic choroid plexuses in the human fetus and the morphology of choroid plexus epithelial cells in the various plexuses in different development stages were described on basis of PAS- and toluidine blue-stained material. Six different cell types were identified in various combinations in 4 different stages (I-IV). The number and distribution of albumin-containing epithelial cells in various plexuses in the different stages of development were investigated by indirect immunoperoxidase technique. Albumin-containing cells did not belong to a single cell type. The telencephalic plexus exhibited a staining pattern for albumin which was different from that of the diencephalic and myelencephalic plexuses. In the telencephalic plexus positive epithelial cells were frequent in stage I, whereas only a few positive cells were present in stage II and III. In contrast, 30-40% of the epithelial cells in both diencephalic and myelencephalic plexuses in stage I, II and III showed a positive staining reaction. Later in gestation less than 10% were positive. It is suggested that a main function of the diencephalic and myelencephalic plexuses early in gestation is associated with protein transport rather than with glycogen synthesis and storage which might be a major function of the telencephalic plexus.

The structure of the choroid plexus and the physiology of the choroid plexus epithelium.

The choroid plexuses (CPs) are leaf-like highly vascular structures laying in the ventricles. The main function of choroid plexuses is the production of the cerebrospinal fluid (CSF). Although CPs have a unique distribution of ion transporters/channels, the mechanism of CSF production is similar to the production of fluids in other epithelia and is based on energy released from ATP hydrolysis, which drives unidirectional flux of ions accompanied by movement of water by osmosis. The CPs have an important role in the homeostasis of nutrients in the CSF since the kinetic parameters of glucose and amino acid (AA) transport across the CPs are the main reason for the low concentration of these molecules in the CSF. The CPs appear to be source of CSF-borne hormones and growth factors, including insulin-like growth factor II (IGF II), vasopressin (VP) and transforming growth factor beta1 (TGF-beta1). The CPs also synthesise the thyroid transporting protein transthyretin and transferrin and can chelate heavy metals.

[Malignant transformation of a choroidal nevus 6 years after brachytherapy of a non-contiguous choroidal melanoma - a case report and review of literature].

BACKGROUND: Brachytherapy is a well established procedure for treatment of malignant choroidal processes. Approximately 5 % of the cases, will show local recurrences within 6 years. We report a patient who has developed a malignant transformation of an uveal nevus following brachytherapy of a non-contiguous uveal melanoma. In addition, we reviewed the literature concerning similar cases. HISTORY AND SIGNS: In 1991 a Ruthenium application was performed on a 65-year old patient suffering of an equatorial malignant choroidal melanoma. At that time, a nevus at the posterior pole was present. From 1997 to 1998 the nevus continuously increased in size. Visual acuity decreased and a retinal detachment developed. Combining the clinical and the sonographic findings, the diagnosis of a malignant melanoma arising of a nevus was established. Enucleation was performed in 1998. THERAPY AND OUTCOME: Histologic examination revealed the diagnosis of an epithelioid-cell-type malignant uveal melanoma arising in a nevus. Till now, the patient is free of metastatic disease. CONCLUSION: Non-contiguous development of uveal melanomas in the same eye are extremely rare. We could identify in the literature 5 cases of non-contiguous melanomas following brachytherapy of an uveal melanoma. In none of these cases, the secondary melanoma has arisen in a nevus. Follow-up examination of patients with uveal melanoma treated with brachytherapy should include not only examination of the treated area, but also the whole fundus of the eye in order to recognize secondary tumours as early as possible.

[Clinical and histopathological aspects of 113 necrotizing malignant melanomas of the choroid. 2. Immunogenetic characterization of T-cell receptor-positive tumor-infiltrating lymphocytes and survival of patients with necrotizing melanomas of the choroid].

BACKGROUND: T-cell receptor (TCR) gene analysis of tumor infiltrating lymphocytes (TIL) has been recognized to play a pivotal role in the immunosurveillance of solid neoplasms. PATIENTS AND METHODS: Therefore, 113 necrotizing choroidal melanomas (NMM) were compared to 100 non-necrotizing melanomas (MM) histologically and immunohistochemically. RESULTS: NMM showed TIL more frequently (76.11% vs. 21%, p < 0.001). V alpha/V beta- (70.3% of NMM) and V gamma 1- (39.2% of NMM) and V delta 1- (52.7% of NMM) TCR+ cells were distributed focal or diffuse, and correlated with tumor volume, diameter, and scleral invasion. 74.33% of NMM patients had died after 240 months (5 year survival: 58.1%). Mortality was not significantly associated with TIL (p < 0.33) or V alpha/V beta-TCR+ cells (p < 0.2), however, survival was significantly increased with evidence of V gamma 1- and V delta 1-TCR+ cells (p < 0.026). CONCLUSIONS: V alpha/V beta-, V gamma 1- and V delta 1-TCR+ tumor infiltrating lymphocytes can be demonstrated in choroidal melanomas. This is a basis for functional studies providing a rationale for the evaluation of immunotherapeutic modalities.

The "Knapp-Ronne" type of malignant melanoma of the choroid. A haemangioma-like melanoma with a typical clinical picture. So-called "preretinal malignant choroidal melanoma".

Five cases of a rare type of malignant melanoma of the choroid with a typical clinical and histopathological picture are reported. This tumour is characterized by it location near the optic disk, its early growth through the retina, its structure with bloodless and blood-filled cavernous spaces, and its manifestation by a massive haemorrhage into the vitreous. "Historical" cases are mentioned, showing that this rare tumour, occurring in about 1:250 of malignant melanomas of the choroid, has a Scandinavian tradition. The clinical and histopathological details are discussed.

Multicentric primary malignant melanomas of the choroid: two separate malignant melanomas of the choroid and two uveal naevi in one eye.

Two completely separate malignant melanomas of the choroid occurred in 1 eye containing 2 additional uveal naevi. Review of the literature disclosed 7 further patients with unilateral multicentric malignant melanomas of the choroid and 11 bilateral multicentric malignant melanomas of the uvea. General aspects of multicentric primary neoplasia are discussed and an interpretation is attempted.

Choroid Neoplasms↗

Primary choroidal and cutaneous melanomas, bilateral choroidal melanomas, and familial occurrence of melanomas.

The medical history of a patient with a familial atypical multiple more melanoma syndrome revealed a combination of rare findings: occurrence of a melanoma in both eyes, development of 3 separate skin melanomas, and occurrence of other malignancies in the family. Another patient developed a choroidal and a skin melanoma, but there were no known malignancies in the family. A third patient with a choroidal melanoma had a mother with an ethmoidal melanoma.

[Scintigraphy of the choroid plexus (choroid plexography)].

An increase concentration of pertechnate in the choroid plexus was demonstrated in 21 patients pretreated with tin pyrophosphate injected intravenously 24 hours before the examination. Using this technique, the choroid plexus are seen quite clearly, giving indirectly a picture of the intracerebral ventricles. A dynamic study was carried out in 12 patients, 3 of whom had abnormalities of the cerebrospinal fluid circulation demonstrated by isotope cisternography. The results obtained were characterised by a typical diphasic curve with variations in the pathological cases.

[Surgical and pathological study of the approach to the superior choroid plexus of the 3 rd ventricle. One case of posterior choroid arteriovenous aneurysm (author's transl)].

The authors describe a case of an arterio venous malformation of the tela choroidea of the third ventricle which was feeding by the postero medial choroidal artery and draining by the internal cerebral vein. This A-V malformation was microsurgically excluded in sitting position by an occipital interhemispheric, transtentorial and transversal transcallosal approach without any postsurgical clinical signs. An anatomical study of this region shows an easy recognition of the choroidal postero medial artery by the regular presence of an arterial loop which concavity is crossed by the terminal segment of the basilar vein of Rosenthal. The knowledge of the occipital internal veins drained in the callosal posterior vein to form a common trunk, the cuneo limbic vein drained itself into the vein of Galen is important to know. This venous system is to preserve at the time of retraction of the occipital lobe to not give rise to infarction and therefore hemianopsia. At least the transverse section of the posterior part of corpus callosum permits a good approach of the roof of the third ventricle and is clinically very harmless if the protection of the cuneo limbic vein is assumed.

[Early lesion of choroidal neovascularization. 2. Early lesion of choroidal neovascularization in pathological myopia].

We describe early features of choroidal neovascularization in 50 pathologically myopic eyes. The eyes were classified into three categories on the basis of degree of myopic chorioretinal degeneration in the posterior pole: mild diffuse atrophy, severe diffuse atrophy and focal atrophy. Choroidal neovascular membranes (CNMs) were grouped into a minimally leaking type and a markedly leaking type in fluorescein angiographic patterns. CNMs were more frequently associated with the mild diffuse atrophy than with the other atrophies. Of the markedly leaking CNMs, 83% were seen in eyes with mild myopic chorioretinal degeneration and 78.5% of the minimal leaking CNMs were found in eyes with severe myopic chorioretinal degeneration. In association with CNMs, lacquer crack and the granular hypofluorescein lesions were observed in 72% and 40%, respectively. The visual acuity deteriorated in all of the 14 eyes which could be followed up more than 1 year.

[Experimental research on choroidal circulation. 1. Effect of sectioning or stimulation of the cervical sympathetic nerve upon choroidal blood flow].

Choroidal blood flow (CBF) was continuously measured with a laser doppler flowmeter (LDF) in 32 pentobarbital-anesthetized cats. A rise in intraocular pressure (IOP) caused a decrease in CBF, but when the IOP increase was slight, it induced a CBF increase. A sustained increase of CBF occurred following sectioning of the cervical sympathetic nerve (CSN) in 7 out of 18 eyes, but no noticeable blood flow change was observed in the remaining 11 eyes. Two different vasomotor responses in CBF were observed in response to electrical stimulation of the peripheral cut end of the CSN; one was a decrease and the other an increase. From the present experiments, decrease of CBF elicited by CSN stimulation is considered to be due to an increase in the activity in vasoconstrictor fibers of choroidal capillary blood vessels, but the exact mechanism of the CBF increase is unknown. Slight changes in position of the LDF probe sometimes alter the vasoresponses, suggesting that the site at which CBF was measured is crucial for investigating the effects of dissection or electrical stimulation of the CSN on CBF.

Transvitreal retino-choroidal biopsy of suspected malignant lesions of the choroid. Follow-up of cases over 7 years.

The cases of transvitreal retino-choroidal (TVRC) biopsy from 1987 to 1994 were assembled in order to examine the efficiency, complications and risks to patients. Of 92 biopsies 91 had sufficient material for diagnosis. Eighty (87%) contained malignant tissue, whereas 10 cases were negative and two unsuccessful. Tumours of appropriate size were treated by brachytherapy. Twenty-three large tumours were enucleated primarily. Eight irradiated eyes had to be removed. In 31 cases, therefore, it was possible to examine the whole tumour for changes caused by the biopsy and to compare the typing of the biopsy with that of the whole tumour. In the irradiated eyes histologically viable tumour cells remained mostly in the inner part of the tumour. Scleral necrosis was not found. The technique used appears to be safe. Histological verification is highly recommended before any conservative therapy, inter alia to obtain true statistics of therapeutic results.

Risk factors for choroidal neovascularization in the second eye of patients with juxtafoveal or subfoveal choroidal neovascularization secondary to age-related macular degeneration. Macular Photocoagulation Study Group.

OBJECTIVES: To verify and quantify previously reported risk factors for development of choroidal neovascularization (CNV) in the fellow eye of patients with 1 eye affected with CNV secondary to age-related macular degeneration, to examine the value of characteristics of the pericentral macula in the quantification of risk for developing CNV, and to explore whether the presence of occult CNV in the first eye affects the development of CNV in the fellow eye. DESIGN, PATIENTS, AND SETTING: Follow-up study of fellow eyes of 670 patients enrolled in multicenter, randomized clinical trials of laser photocoagulation of juxtafoveal or subfoveal CNV. MAIN OUTCOME MEASURE: Development of CNV. RESULTS: Three characteristics of the central macula of the fellow eye and 1 systemic factor were associated independently with an increased risk of developing CNV: the presence of 5 or more drusen (relative risk, 2.1; 95% confidence interval, 1.3-3.5), focal hyperpigmentation (relative risk, 2.0; 95% confidence interval, 1.4-2.9), 1 or more large drusen (relative risk, 1.5; 95% confidence interval 1.0-2.2), and definite systemic hypertension (relative risk, 1.7; 95% confidence interval, 1.2-2.4). Estimated 5-year incidence rates ranged from 7% for the subgroup with no risk factors to 87% for the subgroup with all 4 risk factors. Characteristics of the pericentral macula were not strongly associated with the development of CNV. The presence of occult CNV in the first eye affected had no influence on the development of CNV or on the type of CNV in the fellow eye. CONCLUSIONS: The prognosis of the fellow eye is affected strongly by characteristics of its central macula and by systemic hypertension. These factors should be considered when counseling patients with unilateral neovascular age-related macular degeneration and when targeting patients for preventive interventions.
