Microradiographic study of amelogenesis imperfecta.

A material of 22 primary and 4 permanent teeth from 22 children with amelogenesis imperfecta (AI) were examined by microradiographic techniques. The children were part of a patient material earlier examined in genetical and clinical studies. The results were compared with corresponding data two non-affected control groups and correlated with the available clinical and genetical data. Teeth were examined from seven of the eight different variants of AI seen in the clinical study. In most cases both hypoplasias and areas of hypomineralization were observed in the same tooth, indicating that both the secretory and the maturation phases of the amelogenesis are affected in AI. In teeth from children with the same clinical variant but different inheritance patterns, no specific finding could be related to a specific inheritance pattern. The findings in the one boy with AI as an X-linked trait were unique in this material. In all control teeth except one, no hypoplasisas or areas of hypomineralization were found in the enamel. In conclusion, the subclassification of AI into different forms can be questioned. Variations in clinical and histologic characteristics connected with the same inheritance pattern suggest that the genetic defect, in conjuction with a large biological variation, could explain the multiplicity in clinical expressivity that characterizes AI.

The diagnosis of chronic simple conjunctivitis. Vital staining of tarsus with tetrazolium - alcian blue mixture.

Vital staining with a tetrazolium - alcian blue mixture gave a characteristic punctate red staining of the superior or the inferior tarsus in 69% of all examined patients with chronic simple conjuctivitis (69 patients subjected to 168 examinations). The superior tarsus was more frequently stained than the inferior (61% against 33%). The staining was most often concentrated centrally or in the middle anterior part of the superior tarsus. If located on the inferior tarsus, it was most often found medially, anteriorly. No corresponding staining was seen in normal eyes. The differential diagnosis involving keratoconjunctivitis sicca and pemphigoid is mentioned. The staining is due to enzymatic reduction of tetrazolium in the cytoplasm of the tarsal epithelial cells. The positive vital staining finding is consistent with a diagnosis of chronic simple conjunctivitis.

Transient and residual stresses and displacements in self-curing bone cement - Part I: characterization of relevant volumetric behavior of bone cement.

In this first part of a two-part report, some aspects of the volumetric behavior of bone cement during its curing process are examined as a prelude to an analysis for the transient and residual stresses and displacements in stem fixation systems. Experiments show that stress generation in the cement is associated with its temperature while curing and that during the cooling phase, the stresses are mainly due to thermal as opposed to bulk shrinkage. The appropriate coefficient of thermal expansion of bone cement has been evaluated from measurements in a simulated fixation system in conjuction with a thermoelastic analysis.

Virus isolated from inclusion conjunctivitis of newborn (inclusion blennorrhea).

An infant developed acute conjunctivitis 7 days after birth. Smears of the mucopurulent discharge contained many typical elementary-body inclusions. From conjunctival scrapings a virus resembling trachoma was grown in eggs. When instilled into monkey eyes, it produced an acute conjuctivitis resembling the human disease. Nine other patients with inclusion conjunctivitis of similar intensity failed to yield viruses.

Alarm, defense, and construction behavior relationships in termites (Isoptera).

Evidence indicates that building behavior in termites is a direct consequence of low-level alarm stimuli and that its immediate function is defense. As in other forms of termite defense behavior, recruitment of nymphs and workers is accomplished by trail laying in conjuction with transmission of the alarm. The number recruited is related to the intensity of the input stimulus. Primary construction ceases when the original causal stimulus is eliminated by the effects of the actual building.

Energy dispersion x-ray analysis by scanning electron microscopy for measuring cellular elemental composition in bacterial cells.

The use of scanning electron microscopy in conjuction with energy dispersion analysis of X rays is presented as a technique in a novel application to in situ studies of cellular elemental composition. The method has been applied, in the example of its use reported here, to examining the effects of copper-chromearsenic wood preservative on growth and element uptake of selected bacteria in agar cultures.

Interaction of inflammatory cells and oral microorganisms. III. Modulation of rabbit polymorphonuclear leukocyte hydrolase release response to Actinomyces viscosus and Streptococcus mutans by immunoglobulins and complement.

In the absence of antiserum, rabbit polymorphonuclear leukocytes (PMNs) released lysosomal enzymes in response to Actinomyces viscosus (19246) but not to Streptococcus mutans (6715). Antibodies had a marked modulating influence on these reactions. PMN hydrolase release was significantly enhanced to both organisms when specific rabbit antiserum and isolated immunoglobulin G (IgG) were included in the incubations. Immune complex F(ab')2 fragments of IgG directed against S. mutans agglutinated bacteria. Immune complexes consisting of S. mutans and F(ab')2 fragments of IgG directed against this organism were not effective as bacteria-IgG complexes in stimulating PMN release. The intensity of the release response to bacteria-IgG complexes was also diminished when PMNs were preincubated with isolated Fc fragments derived from IgG. Fresh serum as a source of complement components had no demonstrable effect on PMN release either alone or in conjuction with antiserum in these experiments. These data may be relevant to the mechanisms and consequences of the interaction of PMNs and plaque bacteria in the pathogenesis of periodontal disease.

Genetically manipulated virulence of Yersinia enterocolitica.

Mobilizable virulence plasmids of Yersinia enterocolitica of serotypes O:3 and O:9 were constructed by cointegration of a mobilizable vector into the virulence plasmids. The obtained cointegrates were mobilized into plasmidless Y. enterocolitica strains of serotypes O:3, O:5, O:8, and O:9. The transfer experiments revealed the existence of two different subgroups of plasmid-associated traits. (i) Animal virulence functions (mouse lethality and conjuctivitis provocation) were only transferable to plasmid-cured derivatives of virulent parent strains (serotypes O:3, O:8, and O:9), but they were not transferable to Y. enterocolitica antigen reference strains (serotypes O:3 and O:8) or to a plasmidless clinical isolate of serotype O:5. A further striking result was that a serotype O:8 strain regained the mouse lethality trait after receipt of a plasmid from a strain not lethal to mice. These results demonstrate that plasmid-mediated animal virulence functions are not uniformly expressed within Y. enterocolitica. (ii) The second subgroup of plasmid-mediated traits (calcium dependency, surface agglutinogens, HEp-2 cell adherence, and protein release) were transferable to all Y. enterocolitica recipient strains tested (serotypes O:3, O:5, O:8, and O:9 of different origin). For the first time HEp-2 cell adherence and temperature-induced release of five major protein species are described as transferable traits.

Assessing the prevalence of hand osteoarthritis in epidemiological studies. The reliability of a radiological hand scale.

OBJECTIVE: The hands are often involved in the osteoarthritic disease process. A radiological grading scale is presented, derived from a published atlas, to assess the prevalence of hand osteoarthritis (OA) involvement in clinical and epidemiological studies and its reproducibility is studied. METHODS: This hand scale is based on the radiological feature "joint space narrowing", which represents the macromorphological process of cartilage loss. Osteophytes and sclerosis are less important unless seen in conjuction with joint space narrowing. Nine individual joints per hand (four proximal interphalangeal joints (PIP), four distal interphalangeal joints (DIP), first carpometacarpal joint (CMC-1)) are scored dichotomously for the presence of OA. To save time and to increase reliability a severity grading of radiological features is not performed. To determine inter-rater and intra-rater reliability of the individual joints and the presence of OA in two separate joint groups (>/= 2 PIP or DIP and at least one CMC-1, used to define "generalised OA" in the ongoing Ulm Osteoarthritis Study) 50 pairs of anteroposterior hand radiographs were read by two investigators twice within one month. The kappa coefficient was calculated to quantify the strength of associations. RESULTS: On average five minutes were needed to score one hand radiograph. Both raters were able to reproduce their own readings in all individual joints and for the presence of OA in two separate joint groups after one month. Reliability was highest for the PIP joints (kappa: 0.56-1. 00) it was slightly lower for the DIP joints (0.38-0.87), for the CMC-1 joints (0.58-0.69) and for OA in two separate joint groups (0. 54). The values for inter-rater agreement were good as well, kappa coefficients ranged from 0.52 to 0.92. CONCLUSION: This grading scale was shown to be reliable within and between readers for all the individual joints as well as for the presence of OA in two separate joint groups. Scoring a limited number of joints dichotomously makes this scale efficient and therefore useful for clinical and epidemiological trials, when dealing with large patient samples.

Adenovirus type 21 keratoconjunctivitis.

A case of keratoconjunctivitis caused by adenovirus type 21 in London has been described. A 59-year-old woman attented hospital in August 1974 complaining of a 3-week history of redness, grittiness, watery discharge, and photophobia in her left eye and a slight upper respiratory infection. Clinical examination showed a moderate follicular conjuctivitis mainly in the lower and upper fornices, which lasted for 6 weeks. In the cornea a moderate amount of epithelial and subepithelial punctate keratitis was observed. The subepithelial opacities were coarse, discrete, and round and lasted for 4 months. The course of follicular conjunctivitis and the subepithelial punctate keratitis in this patient was similar to epidemic keratoconjunctivitis caused by adenovirus 8. A conjunctival swabbing collected from this patient was positive for adenovirus serotype 21.

Immunoassay of tear lysozyme in conjunctival diseases.

The tear lysozyme content in 111 normal subjects and in 159 patients with various conjunctival diseases was determined by a single radial immunodiffusion technique. Tear lysozyme level in normal people was 1.33/mg/ml. (SI conversion: mg/ml = g/l.) The mean tear lysozyme levels in patients with chronic irritative conjunctivitis (0.97 mg/ml) and nutritional deficiency with epithelial xerosis (0.76 mg/ml) were significantly lower than in the normal controls. The mean tear lysozyme levels in tears from patients with vernal conjuctivitis (1.20 mg/ml), phlyctenular conjunctivitis (1.10 mg/ml), and acute bacterial conjunctivitis (1.48 mg/ml) were not significantly different from those in the normal controls. Superimposition of acute bacterial conjunctivitis on trachoma did not alter the low tear lysozyme level that existed before in these patients.

Superior sagittal sinus thrombosis: an evaluation of the changes demonstrated on computed tomography.

Five female patients, all either on contraceptive medication or in the postpartum period, with angiographically confirmed superior sagittal sinus thrombosis have been scanned. CAT appearances are nonspecific, but when taken in conjuction with the symptoms should suggest the possible diagnosis. The most consistent appearance was of diffuse cerebral swelling with subsequent return to normal. Bilateral low attenuation lesions with haemorrhagic areas were noted in one patient.

Mutation of the sterol 27-hydroxylase gene (CYP27) results in truncation of mRNA expressed in leucocytes in a Japanese family with cerebrotendinous xanthomatosis.

OBJECTIVES: A Japanese family with cerebrotendinous xanthomatosis (CTX) was investigated for a sequence alteration in the sterol 27-hydroxylase gene (CYP27). The expression of CYP27 has been mostly explored using cultured fibroblasts, prompting the examination of the transcripts from blood leucocytes as a simple and rapid technique. METHODS: An alteration in CYP27 of the proband was searched for by polymerase chain reaction-single strand conformation polymorphism (PCR-SSCP) analysis and subsequent sequencing. Samples of RNA were subjected to reverse transcription PCR (RT-PCR) and the product of the proband was amplified with nested primers and sequenced. RESULTS: A homozygous G to A transition at the 5' end of intron 7 was detected in the patient. In RT-PCR analysis, only a truncated transcript was detected in the patient, whereas both normal and truncated transcripts were detected in the siblings. The sequencing of the patient's cDNA fragment disclosed a direct conjuction of exon 6 and exon 8. CONCLUSION: The mutation at splice donor site and the truncation of mRNA were identical with those of a recently reported Italian patient, although different in symptomatology. The application of blood leucocytes can be a simple technique on analysing a constructive abnormality of CYP27 mRNA.

A neurological evaluation of workers exposed to mixtures of organic solvents.

Workers with long term exposure to mixtures of organic solvents below regulatory limits have been reported to experience mild, but clinically detectable, sensory or sensorimotor polyneuropathies. In conjuction with a cross sectional study of behavioural performance a clinical neurological evaluation was conducted among printers and spray painters to examine dose response relations. All 240 subjects completed an occupational history and symptom questionnaire and underwent a clinical neurological examination. On average, subjects had been employed on their current job for six years. Classification of solvent exposure for each subject was based on exposed versus non-exposed job titles and observations during an industrial hygiene walk-through or on the measured concentration of solvents in full shift personal air samples. The average full shift solvent concentration was 302 ppm for printing plant workers and 6-13 ppm for workers at other plants. Isopropanol and hexane were the major constituents. Neurological abnormalities consistent with mild polyneuropathy were found in 16% of subjects; none was clinically significant. Exposed/non-exposed comparisons showed slightly higher frequency of symptoms in the exposed subjects which was not related to solvent level. Subjects categorised as exposed during the walk- through survey also had poorer vibratory sensation measured at the foot and diminished ankle reflexes. In multiple linear regression models, however, controlling for age, sex, alcohol intake, and examiner, no significant (p less than 0.05) relation was found between solvent concentration and poor neurological function except for two point discrimination measured at the foot. This investigation has not provided evidence for dose related adverse neurological effects from exposure to moderately low levels of solvent mixtures for a relatively short duration, although this may be due to the shortness of exposure duration, the type of solvent exposure, or to selection factors.

Investigations of moving-slit radiography.

The technique of moving-slit radiography for reduction of scattered radiation was investigated using a prototype moving-slit radiographic apparatus. Slit widths of 1-2 cm, when used in conjuction with an 8: 1 grid, were found to improve radiographic contrast by a factor of 2 in studies employing physical as well as anatomic phantoms. Use of the moving-slit technique at elevated kVp's was found to provide radiographic contrast as good as or better than that obtained in conventional low-kVp full-field techniques, but with a substantial reduction of patient exposure (factor of 2-4). Considerations in the design of an optimal multislit apparatus are discussed. Excessive tube loading is shown not to be a factor in such a system.

These measurements can be performed with accuracy and precision for any treatment field size and at any treatment beam angle. The system can be used in conjuction with several different tissue compensation methods.

Computed tomography in primary reticulum cell sarcoma of the brain.

The CT findings in 16 patients with histologically proved primary central nervous system (CNS) reticulum cell sarcoma (RCS) showed this malignancy to have predilection for the basal ganglia and thalamus, the periventricular white matter, the corpus callosum, and the vermis cerebelli. In 43% of patients with untreated tumor it presented as multifocal lesions; in each of these patients the basal ganglia were involved in conjuction with another site. In the other 57% of patients with untreated tumor presenting as solitary lesions the basal ganglia, the corpus callosum, and the frontal lobe were sites of predilection. Such solitary lesions may be indistinguishable from other tumors. Contrast enhancement of RCS was characteristically homogeneous with rare lucency, even in very large lesions.

Real-time ultrasound in abdominal examinations.

Three types of real-time ultrasonic equipment are discussed: sequenced linear arrays, mechancial sector scanners, and phased array systems. Their limitations, including those involving documentation, are briefly reviewed. Although real-time scanners are satisfactory for small, local examinations, they should be used in conjuction with static B-scanners for larger abdominal surveys.
