Bronchial provocation tests in atopic patients with allergen-specific IgG4 antibodies.

Atopic asthmatics who produce allergen-specific IgE antibodies develop an immediate reaction to bronchial provocation; those who produce allergen-specific IgG4 antibodies develop a delayed response; and those who produce both classes of antibody against the same allergen exhibit a dual response. Individual patients showed a variable immune response to different allergens, producing bot classes of antibody against one, but only IgE or IgG4 against others. These findings suggest that allergen-specific IgG4 antibodies are important in the production of severe asthma and probably atopic eczema as well.

[Specific bronchial provocation test with solid aerosols. Quantification of results].

OBJECTIVES: To determine 1) the level of specific bronchial reactivity by challenge with flour and 2) the criteria of positivity using a new method for the expression of the results. METHOD: Thirty-eight asthmatic subjects, occupationally exposed to wheat flour, performed a challenge with lactose then with flour. The instantaneous measurement of the concentration and of the inspiratory airflow were used to calculate the inhaled dose and to establish the dose-response relationships. The results were given 1) by the dose of flour provoking a 20% fall in FEV1 (PDf20), 2) by the comparison of the variation of the FEV1 during the challenge with flour to the distribution of the values observed during the challenge with lactose (inferior limit of the confidence interval at 99.7%). RESULTS: The variations of the FEV1 were not significantly related to the inhaled dose of lactose. The specific bronchial reactivity to flour was a continuous data and three groups were distinguished: 1) subjects (n = 15) with high bronchial reactivity had a fall of FEV1 of more than 20% 2) subjects (n = 13) without significant variation of the FEV1 for doses higher than 1,400 micrograms by comparison to the distribution of the values of the lactose test 3) subjects (n = 10) with a significant fall of FEV1 by comparison to the distribution of the values of the lactose test but lower than 20%. For this group with moderate reactivity, the flow of the inhaled dose may be determinant for the bronchial response. CONCLUSIONS: PDf 20 measures the specific bronchial reactivity. However, if the fall in FEV1 is lower than 20%, the specific challenge with flour may be compared to the challenge with lactose to detect the subjects with moderate reactivity. Our results confirmed the role of the inhaled dose and suggested the role of the dose rate in the outset of bronchial obstruction among asthmatic subjects.

Forced oscillation technique and maximum expiratory flows in bronchial provocation tests in children.

Variables obtained with the forced oscillation technique (total respiratory resistance and impedance at 2, 4 and 12 Hz) and with the forced vital capacity manoeuvre (FVC, FEV1, MMEF, MEF75, MEF50, MEF25) were used in the evaluation of the bronchial allergen challenge test in 2 groups of asthmatic children. Each method was applied in 15 provocation tests and the changes of the different variables, allowing for their respective variations, were compared with the clinical assessment. Agreement with the clinical evaluation was found in 15 of 15 occasions with the forced oscillation technique and in 12 of 15 occasions with the forced vital capacity manoeuvre. With the forced oscillation technique a bronchial reaction was detected in 6 of 10 patients at 1/10 of the allergen concentration that gave positive clinical signs and the single most sensitive variable was resistance determination at 4 Hz, with significant change in 5 of 10 cases. The forced vital capacity manoeuvre allowed detection of a bronchial reaction preceding clinical signs at a tenfold lower allergen concentration in 2 of 9 cases and the most sensitive variables were FEV1 and MEF25.

[Pharmaceutical contribution to the non specific challenge in bronchial provocation tests].

Aerosols of methacholine chloride are used in inhalation tests to measure non-specific bronchial responsiveness. Inhalation challenge solutions are not commercially available neither by pharmaceutical industry nor in France by the PCH-APHP. To reply to doctor's request, dilute concentrations of methacholine must be prepared properly in order to protect the patient and to assure doctor of test reproductibility. Osmolarity, pH and degradation of different dilute concentration of methacholine chloride have been compared with two diluents. This study allowed us to validate our preparation of solutions.

[Procedure of the bronchial provocation tests (author's transl)].

A procedure of bronchial inhalation challenge with acetylcholine accepted by the working group "Aerosols in Medicine" of the GDR on its session in autumn 1976 in Leipzig is reported. The decisions on substance, dose, kinds of application and interpretation are discussed and confirmed.

[Current use, methodology and evaluation of nonspecific bronchial provocation tests in our country].

With the aim of determining the current use of non-specific bronchial inhalation challenge (BIC) testing in Spain, 147 questionnaires were sent to hospitals with pneumology departments or units. The questionnaire covered general, technical and methodological aspects of this diagnostic procedure, as well as its assessment and interpretation. Of the 42 informants who returned questionnaires, mainly from large urban hospitals, 34 reported using BIC. The most widespread criterion applied in deciding to use BIC was the presence of atypical asthma symptoms (33/34). The stimulus most often used was methacholine inhalation (33/34). We found that technical preparation of the drug was highly standardized, but that administration and follow-up protocols differed greatly. There was also great variety in the PC20/PD20 value assigned to indicate bronchial hyperresponsiveness. This study contributes to our better understanding of the current use of BIC in Spain and highlights the need to work toward greater standardization for this diagnostic procedure, to enable us to assess and interpret the results more consistently.
