[Effect of ornid on the regulation of cerebrovascular circulation].

An investigation of ornid, using radioisotopic, electromagnetic and resistographic methods, has shown the drug to diminish cerebral circulation and to abolish completely the constriction of the brain vessels induced by stimulation of the sympathetic and somatic nerves. Ornid prevented the development of experimental disturbances of the cerebral circulation of adrenergic nature.

[Ornid but not atropine abolishes the insulin receptor reaction of fat and liver plasma membranes to fasting and hyperinsulinemia].

In rats, atropine and saline did not affect the response of insulin receptors increased by fasting but decreased it under condition of hyperinsulinemia in fat and hepatic plasma membranes Ornid abolished the response of the membrane insulin receptors both in fasting and hyperinsulinemia. The data obtained confirm the participation of the sympathetic system in the insulin receptors regulation.

Life threatening ventricular dysrhythmias: intervention with bretylium tosylate.

The impact of psychogenic and physical stressors has been shown to have a negative effect on cardiovascular homeostasis (Figure 2). As CRNAs, we have a responsibility to maintain the patient's equilibrium under the stress-provoking conditions of surgery. Experience in coronary care units over the last 16 years has shown that the majority of patients who develop primary ventricular fibrillation during AMI can be rapidly resuscitated by prompt defibrillation. Only in a small number of patients does ventricular tachycardia or ventricular fibrillation become recurrent or resistant to treatment. It is in this small number that bretylium has proven itself to be a life-saving treatment.

Modification of sympathetic nerve stimulation frequency-response relationship in rabbit ileum by guanethidine, bretylium and tetracaine.

Effects of low concentrations of guanethidine, bretylium and tetracaine were comparatively studied with regard to inhibitory responses of rabbit ileum to periarterial nerve stimulation at 5, 10 and 20 Hz. Guanethidine 2 X 10(-7) to 10(-6) M and bretylium 3 X 10(-6) to 3 X 10(-5) M dose-dependently and preferentially reduced responses to lower frequencies, whereas tetracaine 10(-5) to 3 X 10(-5) M reduced responses, in a dose- and frequency-dependent manner. In conclusion, the mode of adrenergic neuron blocking action of guanethidine and bretylium differs from that of local anesthetics.

Stability and visual compatibility of bretylium tosylate with selected large-volume parenterals and additives.

The stability of bretylium tosylate when mixed with large-volume parenteral (LVP) solutions was assessed over a four-week period, and the compatibility of bretylium tosylate when mixed with eight frequently used drugs was evaluated. Bretylium tosylate admixtures of approximately 1 mg/ml were prepared in both polyvinyl chloride (PVC) bags and glass bottles of 5% dextrose injection, 0.9% sodium chloride injection, and lactated Ringer's injection. The admixtures were examined visually and stored for four weeks at 25 +/- 0.5 degree C under fluorescent light. The concentrations of bretylium tosylate were determined spectrophotometrically at times 0.25, 0.5, 1, 3, 8, 24, and 48 hours and twice weekly thereafter for four weeks. Spectrophotometric assays were confirmed with high-pressure liquid chromatography. Admixtures of bretylium tosylate were prepared with aminophylline, calcium gluconate, digoxin, regular insulin, lidocaine hydrochloride, phenytoin sodium, procainamide hydrochloride, and quinidine gluconate in 5% dextrose injection and 0.9% sodium chloride injection. The admixtures were examined visually for 48 hours. The concentration of bretylium tosylate did not change appreciably during the four-week study period. There were no signs of haze, precipitation, color change, or evolution of gas. There were no apparent differences in stability when comparing the glass with the PVC containers. Bretylium tosylate was also found to be compatible with all the additives tested except phenytoin sodium; a precipitate formed immediately when the latter drug was added to the bretylium tosylate solution. Bretylium tosylate was stable for four weeks in the LVP solutions studied in both glass and PVC containers. The admixtures of bretylium sodium with the other drugs were all visually compatible except those containing phenytoin sodium.

Bretylium tosylate in the management of recurrent ventricular fibrillation complicating acute myocardial infarction.

Eighteen patients with recurrent ventricular fibrillation complicating acute myocardial infarction were treated with bretylium tosylate. All except one patient had been given therapeutic doses of lidocaine and some, in addition, had received other antiarrhythmic agents before bretylium. Bretylium therapy was initiated with intravenous administration of 5 mg/kg body weight and was usually maintained by administering the same dose every 6 to 8 hours by intramuscular route. Ten patients recovered while eight died. Only two of the survivors exhibited heart failure and none of them was in shock, while cardiogenic shock dominated the course of patients with fatal outcome. Postural hypotension invariably occurred but without any clinical deterioration of cardiac function. It is concluded that often bretylium tosylate is an effective therapeutic agent in suppression of recurrent or resistant ventricular fibrillation.
