Gastrointestinal bezoars: a retrospective analysis of 34 cases.

AIM: Bezoars (BZ) are the most common foreign bodies of gastrointestinal tract. Clinical manifestations vary depending on the location of BZ from no symptoms to acute abdominal syndrome. When located in small bowel, they frequently cause small bowel obstruction (SBO). We aimed to present our experience by reviewing literature. METHODS: Thirty-four patients with gastrointestinal BZ were presented. The data were collected from hospital records and analyzed retrospectively. Morbidity and mortality rates were statistically analyzed between the subgroups according to SBO and endoscopic or surgical treatment modalities. RESULTS: The 34 patients had phytobezoars (PBZ). Two patients with mental retardation and trichotillomania had trichobezoars (TBZ). More than half of them (55.88%) had previous gastric surgery. Also most of them had small bowel bezoars resulting in obstruction. Surgical and endoscopic morbidity rates were 32.14% and 14.28% respectively. The total morbidity rate of this study was 29.41%. Four patients in surgically treated group died. There was no death in endoscopically treated group. The total and surgical mortality rates were 11.76% and 14.28% respectively. The differences in morbidity and mortality rates between the subgroups were not statistically significant. CONCLUSION: BZ are commonly seen in stomach and small intestine. SBO is the most common complication. When uncomplicated, endoscopic or surgical removal can be applied easily.

Compositional data on some Persian bezoars.

Fourteen Bezoars from Persia, used in past times and even to-day as a medicine or as a health-preserving precious tone, are examined. Among the concretions ellagic acid prevails. Some are bile pigmented or calcite stones. Other bezoars are mineral substances or artifacts.

Drug bezoars.

(1) Drug bezoars are rare and difficult to diagnose, meaning that they may be overlooked. (2) The severity of drug bezoars is linked to the risk of intestinal obstruction and delayed overdose due to the release of the active substance from compacted tablets.

Bezoars↗

[Gastric bezoar].

Bezoar consist of the accumulation of different substances in the digestive tract. We present the clinical case of a girl with gastric trichobezoar discovered in a postoperatory follow-up to an appendicectomy. Ultra-sound images and oesophagus-gastric transit are fundamental points in the diagnosis. Endoscopy was useful in confirming the diagnosis and in ruling out associated ulcus, frequent in these patients. Surgical treatment is the choice in big bezoar but laser, prokinetics and enzymatic dissolutions have also been used. Theses patients should undergo psychological control.

Postgastrectomy bezoars.

Sixteen cases of postgastrectomy bezoars were diagnosed and treated in the period of five years. Various mechanisms of their information, symptomatology and therapy are discussed. We believe postgastrectomy bezoars are relatively frequent but often missed entities. Considering the relative frequency of this entity and ease of therapy, this condition should not be missed if one is aware of its existence.

[Simultaneous bezoars in the stomach, ileum and cecum causing acute ileus in a 30-year-old imbecile--case report].

Bezoars are concretions of foreign materials in the stomach, small intestine or bowel in people or animals, which impair GIT motility or cause intestinal obstruction. The authors describe an interesting case report of a 30-year-old imbecile with manifestation of simultaneous bezoar of stomach, ileum and caecum and its operative treatment. They describe a short characteristics of this clinical unit, which is reported in the literature only in a few case reports.

Acute Disease↗

Bezoars: a special cause of ileal obstruction in mentally retarded patients.

Abdominal symptoms in mentally retarded patients may pose difficult diagnostic problems due to an unreliable history of disease and incooperation of the patients. The case histories are presented of three patients with bezoars in the ileum. Delayed diagnosis and signs of peritonitis necessitated emergency surgery. All patients could successfully be treated by small bowel resection and primary anastomosis. In mentally retarded patients with abdominal pain a bezoar must be considered.

[Gastroparesis diabeticorum and bezoar (author's transl)].

Gastroparesis diabeticorum has been isolated by Kassander in 1958. Since that time, 65 well documented cases have been published. Very often the patients are asymptomatic and the disorder is discovered by an occasional X-ray examination of the G-I tract. But, sometimes, the diabetes of these patients becomes brittle and they loose weight; bezoar, gastroplegia and hemorrhages may occur. We report two additional cases with severe undernutrition and bezoar. The gastroparesis may be related to a vagal neuropathie. The treatment is disappointing; metoclopramide gives the best improvement.

[The physician at wit's end; a bezoar].

The case is described of a three-year-old Moroccan girl with iron deficiency anaemia and a bezoar resulting from trichotillomania and trichophagy leading to a slowly developed obstruction ileus. Causes, symptoms and treatment of the various forms of bezoar are discussed.

[A case of high intestinal obstruction caused by mycotic bezoar of the duodenum].

A case of the mycotic bezoar in the female patient with gastric hypersecretion is reported. The symptoms of the high intestinal obstruction accompanied underlying disease. Bezoar formed of Geotrichum candidum was fragmentated with biopsical forceps of "alligator" type. Then, natamycin was administered for 5 weeks. The patients recovered completely.

Intestinal obstruction caused by bezoar: a rare complication after bone marrow transplantation.

Gastrointestinal complications are described frequently after preparation of patients for bone marrow transplantation (BMT). Two patients who underwent BMT developed complications due to gastrointestinal bezoars. One patient developed intestinal obstruction, which necessitated emergency surgery, and the other patient had a gastric bezoar, which caused vomiting. The diagnosis and treatment of this rare complication after BMT are described.

Polystyrene bezoar in a patient with polystyrenomania.

A 35-yr-old man with hematemesis, epigastric pain, and no predisposing factors to delayed gastric emptying was found to have a gastric bezoar. Surgical removal was required. Pathological and biochemical examination revealed a mass composed of polystyrene. This represents an unusual composition and a newly recognized predisposing factor to bezoar formation, "polystyrenomania" from ingestion of styrofoam cups.

Aluminum hydroxide gel bezoar--CT diagnosis.

An obstructing aluminum hydroxide gel bezoar was first recognized on a retrograde gastrografin study through an ileostomy. In retrospect, a computed tomographic (CT) scan had demonstrated the bezoar as a high density intraluminal mass surrounded by somewhat less dense contrast material distending the bowel. A CT image of a phantom made of aluminum hydroxide gel surrounded by gastrografin was remarkably similar.

[Bezoars in childhood].

Hospital Infantil de México reports 13 cases of bezoars out of which, 12 were trichobezoars and one fitobezoar. Bezoars are uncommon, but they must be known by the pediatrician as well as by the children surgeon in order to be able to reach a diagnosis and give the treatment. A complete review of the main points is given.

Bezoars↗

Prolonged bromide intoxication resulting from a gastric bezoar.

Bromide intoxication is now a relatively uncommon disease. We describe a critically ill patient who had an elevated chloride level and neurologic abnormalities. Bromide levels were also elevated, and remained so for several days. After more than 12 days of abnormally high serum bromide and chloride levels, endoscopy was performed. This procedure disclosed a gastric bezoar containing identifiable pill fragments. The gastric washings from these fragments contained high levels of bromide. We believe that the prolonged course of bromide intoxication secondary to a gastric bezoar makes this case unique.

Intestinal obstruction due to bezoars.

A patient with acute intestinal obstruction due to unrecognized gastric phytobezoars is presented. There was no history of prior gastric or other abdominal surgery. At laparotomy several fragments of rubbery material were discovered obstructing the ileocecal valve. Exploration of the upper gastrointestinal tract revealed two partially fragmented bezoars in the stomach which were removed through a gastrotomy. After surgery the patient recounted the ingestion of persimmons a few weeks before the onset of her symptoms. A review of the literature reveals bezoars to be an infrequent, although not an altogether rare, cause of small bowel obstruction in patients without antecedent gastric surgery. A careful dietary history should accompany all patients presenting with acute intestinal obstruction.

Gastric bezoars: treatment and prevention.

Gastric bezoars may occur in the normal stomach as a result of ingestion of various objects which do not pass through the pylorus. Most gastric bezoars occur as a complication of previous gastric surgery in which there is a loss of normal pyloric function, hypoperistalsis, and low gastric acidity. They may also occur as a complication of cimetidine therapy. Symptoms include epigastric fullness, regurgitation, nausea and vomiting, and epigastric pain. A simple treatment utilizing an ordinary Teledyne Water Pik jet stream through a gastroscope is described to break up a large phytobezoar. This method is probably the treatment of choice and should be used more widely.
